Communication between paramedic students and patients in clinical practice (임상현장실습에서 응급구조(학)과 학생들과 환자간의 의사소통능력에 관한 연구)

  • Purpose: This study aimed to identify the level of communication ability between paramedic students and patients in clinical practice. In addition, it was performed to produce essential data contributing to the qualitative improvement of communication ability. Methods: Data were collected from paramedic students with clinical experience after they provided online consent. Data collection was performed from June 3 to July 12, 2022. One hundred forty-seven questionnaires with 92 items were analyzed using SPSS statistics 28.0. Results: 57.1% of students took communication as a subject despite 92.5% of respondents saying that communication education is necessary for them. The average level of participant communication ability was 4.60 (±1.05) points (measured on a 7-point Likert scale). Among the subdivided communication ability levels, the highest level was demonstrated in the physical examination-related interview (5.36±0.96), and the lowest was shown in the completion part (3.93±1.51). Furthermore, a statistically significant difference was found in communication ability level according to school grade (F=4.709, p=.000). The highest correlation (r=.972, p<.001) was obtained between total communication ability and understanding the patient's point of view. Conclusion: Paramedic students should improve their communication skills to serve patients more effectively. Finally, more educational content should be developed on field-based situations in communication courses.

The Relationship Between Nurse-Nurse, Nurse-Doctor Cooperation and Professional Self-Concept: Nursing Students with Clinical Practice Experience (간호사-간호사, 간호사-의사 협력 및 전문직 자아개념 간의 관계: 임상실습 경험이 있는 간호대학생을 대상으로)

  • This study is a descriptive survey study to identify the relationship between nurse-nurse cooperation, nurse-doctor cooperation, and professional self-concept perceived by nursing students among nursing students with clinical practice experience. The subjects of the study were 140 nursing students, and the collected data were descriptive statistics, Pearson's correlation coefficient, and regression analysis of the factors influencing professional self-concept. As a result of the study, professional self-concept was positively correlated with nurse-nurse cooperation and nurse-doctor cooperation, and the influencing factor of professional self-concept was nurse-nurse cooperation, with explanatory power of 48.0%. Therefore, in order to improve the nursing professional self-concept of nursing students, it is necessary to develop an educational program to enhance cooperation between professionals to promote the cooperative relationship between nurses and nurses, and to develop a curriculum for the development and application of simulation scenarios for cooperation between professionals based on clinical field cases.

Hip and Finger Degenerative Arthritis Case Series Protocol for Applying Korean Medicine Clinical Pathway (퇴행성관절염(고관절, 수지관절염) 한의표준임상경로 적용연구 프로토콜)

  • Objectives : The aim of this study is to set out case series study protocol to complete clinical pathway (CP) of hip and finger degenerative arthritis by applying CP, based on Korean medicine clinical practice guideline developed by clinical experts, to clinical field. Methods : The treatments included Manual acupuncture, acupoint injection, electroacupuncture, laser acupuncture, cupping, moxibustion, chuna, and physiotherapy. They were conducted in the 2nd week of admission and 4th week of out patient department (OPD) days. We carried out 10-point Likert scale questionnaires on the clinical usefulness and the satisfaction of patients and staff after applying CP. Appropriateness and improvement on patients were conducted using a 10-point Likert scale. An open-ended questionnaire was also conducted to ask if there was any requirement to be added. Results : In past research studies, there were no related studies about Korean medicine CP on hip and finger degenerative arthritis. Final version of CP is going to be completed based on the questionnaire. Conclusions : This evidence-based case series study protocol is expected to contribute development of hip and finger degenerative arthritis.

Current insights into inherited bone marrow failure syndromes

  • Inherited bone marrow failure syndrome (IBMFS) encompasses a heterogeneous and complex group of genetic disorders characterized by physical malformations, insufficient blood cell production, and increased risk of malignancies. They often have substantial phenotype overlap, and therefore, genotyping is often a critical means of establishing a diagnosis. Current advances in the field of IBMFSs have identified multiple genes associated with IBMFSs and their pathways: genes involved in ribosome biogenesis, such as those associated with Diamond-Blackfan anemia and Shwachman-Diamond syndrome; genes involved in telomere maintenance, such as dyskeratosis congenita genes; genes encoding neutrophil elastase or neutrophil adhesion and mobility associated with severe congenital neutropenia; and genes involved in DNA recombination repair, such as those associated with Fanconi anemia. Early and adequate genetic diagnosis is required for proper management and follow-up in clinical practice. Recent advances using new molecular technologies, including next generation sequencing (NGS), have helped identify new candidate genes associated with the development of bone marrow failure. Targeted NGS using panels of large numbers of genes is rapidly gaining potential for use as a cost-effective diagnostic tool for the identification of mutations in newly diagnosed patients. In this review, we have described recent insights into IBMFS and how they are advancing our understanding of the disease's pathophysiology; we have also discussed the possible implications they will have in clinical practice for Korean patients.

Genetic tests by next-generation sequencing in children with developmental delay and/or intellectual disability

  • Developments in next-generation sequencing (NGS) techogies have assisted in clarifying the diagnosis and treatment of developmental delay/intellectual disability (DD/ID) via molecular genetic testing. Advances in DNA sequencing technology have not only allowed the evolution of targeted panels but also, and more currently enabled genome-wide analyses to progress from research era to clinical practice. Broad acceptance of accuracy-guided targeted gene panel, whole-exome sequencing (WES), and whole-genome sequencing (WGS) for DD/ID need prospective analyses of the increasing cost-effectiveness versus conventional genetic testing. Choosing the appropriate sequencing method requires individual planning. Data are required to guide best-practice recommendations for genomic testing, regarding various clinical phenotypes in an etiologic approach. Targeted panel testing may be recommended as a firsttier testing approach for children with DD/ID. Family-based trio testing by WES/WGS can be used as a second test for DD/ID in undiagnosed children who previously tested negative on a targeted panel. The role of NGS in molecular diagnostics, treatment, prediction of prognosis will continue to increase further in the coming years. Given the rapid pace of changes in the past 10 years, all medical providers should be aware of the changes in the transformative genetics field.

Future of Scientific Research on Preventive Medicine in Korea (예방의학의 발전을 위한 학술활동)

  • The Korean Society of Preventive Medicine has undergone continuous development, after overcoming the difficult early years, in the 59 years since its establishment in 1947. It has repeatedly upgraded its quality and quantity of research with the first journal edition in 1968 and the continuous increase in publication numbers, scientific articles, joint research projects, intra-field exchanges and participation in various international scientific activities. In the future, we should gather a more extensive collection of opinions regarding the introduction of clinical preventive medicine specialists and prepare for the establishment of a training program for clinical specialists into a preventive medicine residency course. Moreover, we should raise interest in the importance of protecting individual information and maintaining medicine ethics. It's impossible to develop academic activities without cooperation. We need such cooperation with basic medical approaches across a wide range of fields. Furthermore, we should strengthen our cooperation with aspects of clinical and drug epidemiology in many fields including public health, statistics, and dietetics. Finally, we should raise the level of international cooperation with many countries, including North Korea, to prevent diseases and promote health. Preventive medicine is a science in which practice is as important as theory. We must aim to nurture preventive medicine specialists who practice in many areas of society with the goal of preventing diseases, promoting health, improving fertility, and securing healthy elderly life for individuals and the entire population. To this end, we will endeavor to promote both theoretical and practical components of academic development.

Legal Study on the Explanatory Duty for Medical Practice in Korean Medicine by Judicial Precedent Analysis (판례분석을 통한 한의사의 설명의무에 관한 법학적 고찰)

  • Objectives : The purpose of this study is to set the explanatory duty on traditional Korean medical(TKM) treatment by analyzing the judicial precedents. Methods : The study was performed by analyzing nine cases of lawsuits related to Korean medicine doctor and explanatory duty among the medical dispute cases in Korea from 1968 through 2012. Results : Nine closed claims occurred regarding the violation of explanatory duties in the field of TKM practice. Two claims were decided by supreme court, three were decided by high court, and four were decided by district court. The causes of lawsuits were categorized as follows : bee venom pharmacopuncture, herb treatment, and an explanation for safety. Conclusions : To perform an explanatory duty has important legal implications for the protection of patients' rights and Korean Medicine doctors' autonomy on TKM treatment.

3D-Printed Disease Models for Neurosurgical Planning, Simulation, and Training

  • Spatial insight into intracranial pathology and structure is important for neurosurgeons to perform safe and successful surgeries. Three-dimensional (3D) printing technology in the medical field has made it possible to produce intuitive models that can help with spatial perception. Recent advances in 3D-printed disease models have removed barriers to entering the clinical field and medical market, such as precision and texture reality, speed of production, and cost. The 3D-printed disease model is now ready to be actively applied to daily clinical practice in neurosurgical planning, simulation, and training. In this review, the development of 3D-printed neurosurgical disease models and their application are summarized and discussed.

Application of Artificial Intelligence in Gastric Cancer (위암에서 인공지능의 응용)

  • Gastric cancer (GC) is one of the most common malignant tumors worldwide, with a 5-year survival rate of < 40%. The diagnosis and treatment decisions of GC rely on human experts' judgments on medical images; therefore, the accuracy can be hindered by image condition, objective criterion, limited experience, and interobserver discrepancy. In recent years, several applications of artificial intelligence (AI) have emerged in the GC field based on improvement of computational power and deep learning algorithms. AI can support various clinical practices in endoscopic examination, pathologic confirmation, radiologic staging, and prognosis prediction. This review has systematically summarized the current status of AI applications after a comprehensive literature search. Although the current approaches are challenged by data scarcity and poor interpretability, future directions of this field are likely to overcome the risk and enhance their accuracy and applicability in clinical practice.

Genomic Applications of Biochip Informatics (유전체 발현의 정보학적 분석과 응용)

  • Bioinformatics is a rapidly emerging field of biomedical research. A flood of large-scale genomic expression data transforms the challenges m biomedical research into ones in bioinformatics. Clinical informatics has long developed technologies to imp개ve biomedical research by integrating experimental and clinical information systems. Biomedical informatics, powered by high throughput techniques, genomic-scale databases and advanced clinical information system, is likely to transform our biomedical understanding forever much the same way that biochemistry did to biology a generation ago. The emergence of healthcare and biomedical informatics revolutionizing both bioinformatics and clinical informatics will eventually change the current practice of medicine, including diagnostics, therapeutics and prognostics.
