안면비대칭을 유발한 섬유성이형성증 : 증례보고 (FIBROUS DYSPLASIA CAUSING FACIAL ASYMMETRY : A CASE REPORT)

  • 본 증례는 연세대학교 치과대학병원 소아치과에 하악 좌측 구치부 잇몸의 팽융을 주소로 내원하여 파노라마, 컴퓨터 단층촬영 결과 두개 악안면 부위에 이환된 섬유성이형성증으로 진단되었으며, 하악 좌측골의 팽윤과 안모 비대칭 및 좌우 안와 비대칭 소견을 보이고 있었다. 섬유성이형성증의 두개 안면부 병소는 안면기형, 두개 비대칭 뿐만 아니라 안구돌출, 시각 손상, 안면 마비, 삼차 신경통, 청각 손실, 치아 변위 및 부정교합, 맹출지연 등을 유발할 수 있다. 따라서 환아에게 증상이 발생할 수 있음을 보호자에게 설명하고, 검진 및 치료를 위해 구강악안면외과와 협진을 시행하였다.

  • 저온 열처리법은 두개안면골 종양의 절제 후 재건에 있어 간단하고, 믿을만하며, 모양이 우수하고, 경제적이면서 또한 오랫동안 유지가 가능한 방법이다. 이 방법은 종양 재발의 걱정없이 생물학적으로 융합이 가능함과 동시에 기계적인 강도가 높은 장점을 가지고 있다. 따라서 두개안면골의 양성종양뿐만 아니라 절제가 가능한 악성종양의 경우에도 재건 시 우선적으로 고려해볼 만한 방법으로 사료된다.

  • 본 연구는 유기계 보온단열재의 장점인 경량성과 연질특성을 갖는 무기계 보온단열재의 제조를 위한 새로운 개념의 무기질 저온 발포 공정에 관한 것이다. 새로운 무기질 발포 공정은 섬유상인 해포석 및 규산알루미늄으로 하여금 발포체의 골격을 형성토록 하고, 저온에서 기체 발생이 가능한 발포제를 사용하여 무기질 섬유상 골격체가 팽창되어 공동을 형성하게 하며, 이 형성된 공동 속에 낮은 열전도도를 갖는 무기질 다포체인 팽창진주암을 채우는 것이다. 총괄적으로 무기질 재료를 고온 용융함이 없이 저온에서 무기질 발포체의 제조가 가능하게 된다. 이를 위해서 섬유상인 해포석의 해섬처리과정, 발포를 위한 섬유상 슬러리의 열처리공정 등 다양한 준비공정이 필요하며, 열처리 전 슬러리의 최적 조성물 조건이 요구된다. 제조된 발포체는 경량, 연질의 보온단열재로서의 겉보기 밀도, 내력 강도, 굽힘강도, 고내열성 등의 물성을 보여주었다.

Urokinase 와 Dextran 40 을 이용한 심막유착 방지에 관한 실험적 연구 (Experimental Studies for the Prevention of Pericardial Adhesion with Urokinase and Dextran 40)

  • Pericardial adhesions following open heart surgery pose a special problems, increasing the risk of cardiac reoperation because of the danger of damaging the heart, coronary artery and veins, or grafts and also the fibrous tissue may obliterate the pericardial space and eventually constrict the heart. This study was undertaken to evaluate the effect of intrapericardial urokinase and dextran 40 on the formation of pericardial adhesions in an animal model. latrogenic traumas on the pericardium were surgically induced in 30 rabbits, simulating injuries possible during actual surgery. In all rabbits, blood [1 ml] was obtained from an ear vessel and injected into the pericardium. Control group of ten rabbits did not receive any further medication, urokinase group of ten received 15, 000-20, 000 IU of urokinase, and remained ten received 1 ml of 10% dextran 40. All rabbits were sacrificed at 4 weeks. At autopsy, the development of adhesions were graded as none [Grade I], minimal [Grade II], moderate [Grade III], and severe [Grade IV]. Histological studies of the parietal pericardium and epicardium were performed. The results were as follows: 1. Group 1[Control group] showed minimal adhesion in 40%, moderate in 50%, and severe in 10% of the group. Sharp dissections were necessary in 60% of adhesions. 2. Group II [Dextran group] showed no adhesions in 20%, minimal in 60%, and moderate in 20% of the group. 3. Group III [Urokinase group] showed no adhesions in 40%, minimal in 40%, and moderate in 20% of the group. Considering in this group, the adhesion activity was significantly suppressed [60% adhesions] compared to the control group [100% adhesions] [P < 0.05]. 4. Histological findings revealed mild serosal fibrosis in none adherent group, loose fibrous connections between two layers of pericardium in minimal adhesion group, tight fibrous connections in moderate adhesion group, and marked fibrous thickening and close attachment of two surfaces were noted in severe adhesion group. These data have revealed the decreased incidence of pericardial adhesions with urokinase and dextran 40.

폐 악성 섬유성 조직구종 -1예 보고 - (Malignant Fibrous Histiocytoma of the Lung - A case report -)

  • 원발성 폐 악성 섬유성 조직구종은 매우 드문 종양이다. 12세 남아가 우연히 발견된 폐 종괴를 주소로 입원하였다. 흉부 X-선, 흉부 전산화 단층촬영, 양전자 방출 단층촬영에서 우하엽에 위치하는 $2.5{\times}2.5 cm$크기의 종괴가 관찰되었다. 우측 측방 개흉술을 통해 종괴를 절제하여 냉동 절편 조직검사를 시행한 결과 악성 방추형세포 종양으로 진단되었다. 따라서 우하엽 절제술 및 종격동 림프절 청소술을 시행하였고, 최종 조직학적 진단은 악성 섬유성 조직구종이었다. 환자는 수술 후 7일째에 퇴원하였으며, 보조적 항암 약물치료는 시행하지 않았다.

어린 Siberian Husky 견의 섬유성 골형성장애의 세포학적 고찰 증례 (Cytologic Aspect of Fibrous Osteodystrophy in a Juvenile Siberian Husky)

  • 5개월령의 거세하지 않은 암컷 시베리안 허스키가 상악 병변의 평가를 위해 내원하였다. CBC, 혈청 화학, 요검사 소견에서 염증과 신부전이 지시되었다. 병변의 세포학 검사에서는 거대 다핵 세포들과 원형$\sim$방추형의 다향한 모양의 세포들이 개별적으로 탈락되거나 군집을 형성하였다. 두 종류의 세포들 모두 중등도의 핵 대소부동증과 세포부동증을 보였고, 뚜렷한 한 개의 핵소체 또는 여러 개의 작은 핵소체를 갖고 있었으며 성긴 염색질이었다. 조직병리학적 검사 결과, 상악과 비갑개는 전반적으로 확장되어 있었으며 결합조직에 의해 대체되어 있었으며 신장에서는 형성 이상이 관찰되었다. 이러한 결과를 바탕으로, 신장형성이상과 섬유화로 인한 섬유성 골형성장애로 진단되었다. 대증적인 처치에도 불구하고 환자의 상태는 악화되어 안락사 되었다.

개에서 광범위한 추궁절제술의 임상적 평가 (Clinical evaluation of extensive laminectomy in dogs)

  • The purpose of the present study was to investigate structural stability of extensive laminectomy and the effect of subcutaneous fat autograft on restricting formation of postlaminectomy membrane (fibrous tissue). Eighteen healthy dogs of both sexes and of mixed breeding were divided into 6 groups : (1) unilateral hemilaminectomy (group H) on 3rd, 4th and 7th vertebrae ; (2) modified dorsal laminectomy (group D) on 3rd, 4th and 7th vertebrae. Hemilaminectomies were carried out incontinuously at 16 sites in 4 dogs, then subcutaneous fat autografts (group F) were applied to 8 laminectomy sites and no treatment (group C) was assigned to 8 laminectomy sites, too. Operating time of group H ($30.9{\pm}10.4$ minutes) was significantly shorter (p<0.05) than that of group D ($43.1{\pm}12.2$ minutes), but surgical hemorrhage of group H is severer than that of group D. General states, such as standing, gait, defecation and urination, were normal. Upper motor neuron/lower motor neuron signs were not found and superficial/deep pain, proprioception and anal sphincter tone were normal. Gross postmortem findings were similar in all groups. The laminectomy sites of groups H and D were filled with fibous connective tissue at 4 months after operation and histopathological abnormalities of spinal cord were not found. One of eight laminectomy sites in group F was filled with fibrous tissue at 2 months after operation, but all operating sites of group C were covered with fibrous tissue. The present study indicated that extensive laminectomy on 7 vertebrae, using unilateral hemilaminectomy and modified dorsal laminectomy technique, maintained structural stability and subcutaneous fat autograft was effective on reducing the formation of fibrous membrane in laminectomy sites.

판막간 섬유체 재건을 시행한 후 발생한 인공판막 심내막염의 수술적 치유 -1예 보고- (Surgical Treatment of Prosthetic Valve Endocarditis after Reconstruction of the Intervalvular Fibrous Trigon -A case report-)

  • 저자들은 8년 전 감염성 심내막염으로 대동맥판막과 승모판 치환 및 섬유체 재건을 시행한 65세 남자 환자에서 발생한 인공판막 심내막염에 대하여 재수술을 시행한 예를 보고한다. 환자는 술 전 심초음파 소견상 1cm크기의 판막하 농양을 동반한 대동맥 판막륜 농양과 승모판 Vegetation 및 섬유체 결손을 보였다. 심부전 소견이 심하여 응급수술을 시행하였다. 두개의 소 심낭 패취를 이용하여 절재된 대동맥판과 승모판의 판륜 및 섬유체를 재건한 다음 기계판막을 대치하였다. 술 후 심초음파 소견상 판막주위 누출은 없었다. 현재 특별한 합병증 없이 통원 치료 중이다.

섬유성이형성증 유래세포의 특성연구 (CHARACTERISTICS OF FIBROUS DYSPLASIA DERIVED CELLS)

  • Purpose: Fibrous dysplasia (FD) is a fibro-osseous disease associated with activating missense mutations of the gene encoding the $\alpha$-subunit of stimulatory G protein. FD may affect a single bone (called monostotic form) or multiple bones (called polyostotic form). The extent of lesions reflects the onset time of mutation. In this study, cells from monostotic FD in maxilla of a patient were isolated and cultured in vitro for characterization. Materials and Methods: The single cells were released from FD lesion which was surgical specimen from 15 years-old boy. These isolated cells were cultured in vitro and tested their proliferation activity with MTT assay. In osteogenic media, these cells underwent differentiation process comparing with its normal counterpart i.e. bone marrow stromal cells. The proliferated FD cells were detached and transplanted into the dordsal pocket of nude mouse and harvested in 6 weeks and 12 weeks. Results and Summary: FD cells have an increased proliferation rate and poor differentiation. As a result, cells isolated from FD lesion decreased differentiation into osteoblast and increased proliferation capacity. MTT assay presented that proliferation rate of FD cells were higher than control. However, the mineral induction capacity of FD was lesser than that of control. Monostotic FD cells make fewer amounts of bone ossicles and most of them are woven bone rather than lamellar bone in vivo transplantation. In transplanted FD cells, hematopoietic marrow were not seen in the marrow space and filled with the organized fibrous tissue. Therefore, they were recapitulated to the original histological features of FD lesion. Collectively, these results indicated that the FD cells were shown that the increased proliferation and decreased differentiation potential. These in vitro and in vivo system can be useful to test FD cell's fate and possible.

Reinforced fibrous recycled aggregate concrete element subjected to uniaxial tensile loading

  • In this study, effect of recycled aggregates and polypropylene fibers on the response of conventionally reinforced concrete element subjected to tensile loading in terms of tension stiffening and strain development was experimentally investigated. For this purpose, concrete prisms of 100 × 100 mm cross section and 500 mm length having one central deformed steel re-bar were cast using fibrous and non-fibrous Recycled Aggregate Concrete (RAC) with varying percentages of recycled aggregates (0%, 25%, 50%, 75% and 100%) and tested under uniaxial tensile load. For all fibrous RAC mixes, polypropylene fibers were used at constant dosage of 3.15 kg/㎥. Effect of recycled aggregates and fibers on the compressive strength of concrete was also explored in this study. Through studying tensile load versus global axial deformation of composite and strain development in concrete and steel, it was found that replacement of natural aggregates with recycled aggregates in concrete negatively affected the cracking load, tension stiffening and strain development, and this negative effect was observed to be increased with increasing contents of recycled aggregates in concrete. The results of this study showed that it was possible to minimize the negative effect of recycled aggregates in concrete by the addition of polypropylene fibers. Reinforced concrete element constructed using concrete containing 50% recycled aggregates and polypropylene fibers exhibited cracking behavior, tension stiffening and strain development response almost similar to that of concrete element constructed using natural aggregate concrete without fiber.
