  • 1. 연부 조직과 골 조직사이에서의 악성 섬유성 조직구종의 근본적인 조직학적 차이는 없었다. 2. 악성 섬유성 조직구종은 조직학적 소견이 다양하므로, 확진을 위해서는 다른 종양을 반드시 배제하여야한다. 3. 재발시 다른 형태의 조직 소견을 보여줄 수 있기 때문에 정확한 진단을 위해 환자의 과거력 및 과 거의 조직학적 검토가 필수적이라 사료된다.

  • 섬유성 골 이형성증이 골육종, 연골육종, 섬유육종 등으로 악성 변환을 하는 것은 매우 드무나 잘 알려져 있다. 악성 변화가 흔히 일어나는 부위는 두경부, 근위 대퇴골, 상완골, 골반골, 경골, 및 견갑골 순이다. 41세 다발성 섬유성 이형성증 환자가 상완골의 동통을 주소로 내원하였다. 방사선 상 상완골 간부의 골 팽윤 및 파괴를 보였다. 생검 상 저악성도의 연골육종으로 진단하여 변연부 절제를 하였으나 7개월 만에 근위부에서 국소 재발 후 재 절제한 표본은 연골아세포형 골육종으로 진단하였다. 다발성 섬유성 이형성증에서 속발한 연골아세포형 골육종을 저악성도 연골육종으로 오진하여 치료방침에 오류가 있었던 1례를 문헌고찰과 함께 보고하는 바이다.

Molecular cloning of peroxidase cDNAs from dehydration-treated fibrous roots of sweetpotato and their differential expression in response to stress

  • Three peroxidase (POD) cDNAs were isolated from dehydration-treated fibrous roots of sweetpotato (Ipomoea batatas) plant via the screening of a cDNA library, and their expressions were assessed to characterize functions of each POD in relation to environmental stress. Three PODs were divided into two groups, designated the basic PODs (swpb4, swpb5) and the anionic PODs (swpa7), on the basis of the pI values of mature proteins. Fluorescence microscope analysis indicated that three PODs are secreted into the extracellular space. RT-PCR analysis revealed that POD genes have diverse expression patterns in a variety of plant tissues. Swpb4 was abundantly expressed in stem tissues, whereas the expression levels of swpb5 and swpa7 transcripts were high in fibrous and thick pigmented roots. Swpb4 and swpa7 showed abundant expression levels in suspension cultured cells. Three POD genes responded differently in the leaf and fibrous roots in response to a variety of stresses including dehydration, temperature stress, stress-associated chemicals, and pathogenic bacteria.

슬관절 관절내 발생한 양성 섬유성 조직구종: 증례 보고 (Intra-articular Benign Fibrous Histiocytoma of the Knee: A Case Report)

  • 슬관절에서의 양성 섬유성 조직구증은 매우 드문 질환으로 대부분의 경우 통증 등의 특별한 증상을 유발하지 않고 심부에 위치하고 있어 진단을 놓치기 쉬우며 우연히 발견되는 경우가 많다. 저자들은 슬관절의 운동제한과 동통을 주소로 내원한 53세 여자 환자에 대해서 외관상 촉지 되지 않았다가 MRI 검사에서 슬개건 하부의 종물을 발견하였고, 진단적 관절경 검사 후 절제 및 조직 생검 결과 양성 섬유성 조직구종으로 밝혀진 1예를 경험하게 되었다. 심부 양성 섬유성 조직구증은 모든 양성 섬유성 조직구증의 5% 이하로 흔하지 않은 것으로 알려져 있는 가운데 슬관절에서 발생한 1예에 대해서 문헌고찰과 함께 보고하고자 한다.

흉벽의 오래된 화상 흉터에서 발행한 악성 섬유성 조직구종 (Malignant Fibrous Histiocytoma Arising in Old Burn Scar on the Anterior Chest)

  • Purpose: Malignant changes of Marjolin's ulcer arising from chronic burn scar are rare. The majority of them are squamous cell carcinoma and basal cell carcinoma. Malignant fibrous histiocytoma is a deep seated pleomorphic sarcoma, which occurs principally as a mass of the extremities, abdominal cavity, or retroperitoneum in adults. Methods: We report a 58-year-old male patient who was admitted due to $3.5{\times}5cm$ chronic ulceration of anterior chest wall on the center of old burn scar. His scar had been occurred by boiling oil and treated with conservative treatment 45 years ago. Preoperative punch biopsy showed suspicious malignant changes and contrast enhanced chest CT showed well-defined, irregular shape enhancing lesion on anterior chest wall without intrathoracic metastasis. Results: The tumor was widely excised and defect was covered with skin graft without infection, necrosis and any other complication. The pathologic findings are compatible with malignant fibrous histiocytoma(storiform - pleomorphic type). The patient underwent 3 cycles of chemotheraphy. Although distant metastasis to the lung developed 6 months later and the patient died 9 month later, there was no local reoccurrence. Conclusion: Aggressive and early excision is needed because malignant fibrous histiocytoma has characteristics of high malignancy with a propensity for early and distant spread. Furthermore, the patient's education about disease entity and postoperative regular follow-up for local recurrence or metastasis is very important. To prevent malignancy from secondly healing burn scar, early skin graft is recommended for patients with deep second degree burn.

횡격흉막에서 발생한 고립성 섬유성 종양 (Solitary Fibrous Tumor of the Diaphragmatic Pleura)

  • 우리는 단순 흉부 방사선 촬영에서 우연히 발견한 47세 여자의 횡격흉막에서 발생한 양성 고립성 섬유성 종양 1례를 보고한다. 단순 흉부 방사선 촬영, 복부 초음파 검사, 전산화 단층 촬영과 자기공명영상으로 주변과 경계가 분명한 다엽성의 흉막 섬유성 종양으로 진단하였다. 우측 개흉 수술시 종괴는 대부분 유착이 없었으나 $5{\times}4$cm 정도의 횡격막 중앙부와 유경으로 붙어 있었다. 붙어 있는 횡격막과 함께 종괴를 완전히 절제하였다. 종양은 $23.5{\times}13.5{\times}8.0$cm 크기였으며, 병리조직학 진단은 양성 섬유성 종양이었고 함께 절제된 횡격막에는 종양세포가 없었다.

  • 악성 고립성 섬유종은 비교적 드문 종양으로 흉막, 횡격막, 후복막 및 심낭 등에서 발생할 수 있으며, 남녀 발생 빈도는 비슷하고 60~70세 사이에 호발하나 10대의 어린 나이에도 발생하는 경우가 있다. 17세 남자에서 악성 고립성 섬유종에 의해 인접한 좌측 9번째 늑골의 형태학적 변화가 있어 섬유종의 제거 및 늑골 근위부의 부분적 절제술을 시행하였다. 술 후 뇌척수액의 흉강 내로의 누출을 CT myelography로 확인 후 뇌척수액의 배액 및 부분적 반측 추궁 절제술 및 경질막 복원술을 통한 교정을 치험하여 발표하고자 한다.

양이온 계면활성제 Cetyltrimethylammonium Chloride를 이용한 한지 슬러지의 제거 (Application of Cationic Cetyltrimethylammonium Chloride Surfactant to Collecting of Hanji Sludge)

  • 전통한지 섬유질 슬러지 및 기계한지 섬유질 슬러지를 양이온 계면활성제 cetyltrimethylammonium chloride(CTAC)를 이용하여 효과적으로 제거하는 방법에 대해 연구하였다. 전통한지 슬러지 시료일 경우 고형분의 양이 약 0.08 g/L 일 때 CTAC의 농도가 $1{\mu}g/mL$ 되면 제거율이 95% 이상 되었으며, 이 때 걸리는 시간은 30초 이내이었다. 기계한지 슬러지에 대해 CTAC만을 이용하여 슬러지를 제거하였을 때 $15{\mu}g/mL$ 이상에서 90% 정도의 제거효율을 보였으나 슬러지 층이 안정하지 못하고, 부상시간이 비교적 오래 걸렸다. $5{\mu}g/mL$의 CTAC에 PAMID$^{(R)}$를 첨가하여 부상시켰을 때는 CTAC만을 이용하였을 때 보다 제거율이 증가하였으며 슬러지 층도 또한 안정하였다. 기계한지 슬러지보다 전통한지 슬러지를 제거할 때 더 효율적이었다.

감압증류법의 고구마소주 제조를 위한 식이섬유 분해효소의 선별 (Screening of Dietary Fiber Degradation Enzyme for Making Sweet Potato Soju by Vacuum Distillation)

  • Sweet potato soju(SPS) has been made by vacuum distillation because sweet potato contains much fibrous materials which give high density to sweet potato mash. Generally, the SPS made by vacuum distillation has soft flavors and tastes. If the viscosity of sweet potato mash could be decreased by degradation enzyme, the process and production of SPS making by the method of vacuum distillation may be simplified and easier to distil the fermented sweet potato. Because the fibrous materials of sweet potato contains pectin with methoxyl group, methanol can be produced by fibrous degradation enzyme. For appling the fiber degradation enzymes to sweet potato mash for making SPS, the enzyme should be needed to degrade fibrous material without producing methanol. Special two fibrolytic enzymes are selected from 26 kind of commercial enzymes for the simplified and easier production of sweet potato soju by vacuum distillation, The selected enzyme A and X can degrade the fibrous material pectin of sweet potato without producing methanol. Although the different companies have produced the enzymes, same cellulase has been prepared from Trichoderma. reesei. The viscosity of sweet potato mash treated by the enzymes is decreased by 3 times with comparison to the viscosity of sweet potato mash of control group. The methanol concentration in the vacuum distilled SPS treated with the enzymes is 0.16%. The concentration is similar to that of commercially distilled SPS(0.15%). The result may suggest that the selected cellulases, A and X, can be used to make SPS by vacuum distillation.

성견에서 하악골 신장술 후 하악과두 연골의 조직학적 변화와 Matrix Metalloproteinase-2 (MMP-2)와 Tissue Inhibitor of Matrix Metalloproteinase-2 (TIMP-2)의 발현 (HISTOLOGICAL CHANGES AND EXPRESSION OF MATRIX METALLOPROTEINASE-2 AND TISSUE INHIBITOR OF MATRIX METALLOPROTEINASE-2 IN THE CANINE MANDIBULAR CONDYLE AFTER DISTRACTION OSTEOGENESIS)

  • Purpose : This study was to clarify the changes in mandibular condyle after unilateral mandibular distraction osteogenesis throughout histological changes and expression of matrix metalloproteinase-2 (MMP-2) and tissue inhibitor of matrix metalloproteinase-2 (TIMP-2). Materials & Methods : Intraoral distractors were placed via submandibular incision in 8 dogs. Two unoperated animals served as controls. Distraction was performed five days after osteotomy as a rate of 0.5 mm twice per day for 10 days. Two animals were sacrificed on 7, 14, 28, and 56 days after completion of distraction, respectively. Ipsilateral condyles were harvested and processed for histological and immunohistochemical examinations. Results : The condyle cartilage is separated into four layers: fibrous layer, proliferative layer, hypertrophic layer, and calcified layer. At 7 days and 14 days after distraction, the condylar cartilage showed the decreased thickness of the articular cartilage and reduced cellularity. At 28 days after distraction, there was an increase in cellularity of fibrous, proliferative, and hypertrophic layer. However, it demonstrated reduced cellularity compared to the control. At 56 days of after distraction, the articular cartilage was an almost normal histologic structure. Positive Safranin-O staining, indicative of sulfated proteoglycans, was examined in the condylar cartilge of nonloaded control. At 7 days and 14 days after distraction, the sulfated proteoglycans is almost completely depleted from the noncalcified part of the condylar cartilage. At 28 days after distraction, there was an increase in Safranin-O staining intensity. However, the staining intensity of the experimental condyle was weaker than that of the control. At 56 days of after distraction, the condylar cartilage showed almost normal Safranin-O staining pattern. In control condyle, MMP-2 immunostaining was seen in fibrous, proliferative, and hypertrophic layer of condylar cartilage, however, it demonstrated lack of staining in fibrous and proliferative layer. At 7 days and 14 days after distraction, strong MMP-2 immunoreactivity was seen in the fibrous, proliferative and hypertrophic layer of the condylar cartilage. At 28 days after distraction, MMP-2 immunostaining was seen in the fibrous and hypertrophic layer of condylar cartilage, however, their immunoactivity was reduced. At 56 days after distraction, MMP-2 immunoreactivity showed almost normal immunostaining pattern. In control condyle, TIMP-2 immunostaining was primarily seen in fibrous and hypertrophic layer of condylar cartilage, however, it demonstrated lack of staining in proliferative layer. At 7 days after distraction, very weak TIMP-2 immunoreactivity appeared in fibrous, proliferative and hypertrophic layer of the condylar cartilage. At 14 days after distraction, weak TIMP-2 immunoreactivity was seen in the fibrous, proliferative and hypertrophic layer of the condylar cartilage. At 28 days after distraction, TIMP-2 immunoreactivity was increased in the fibrous and hypertrophic layer of condylar cartilage. At 56 days after completion of distraction, TIMP-2 immunoreactivity showed almost normal immunostaining pattern. Conclusions : The results show that short-term outcome of physiologic distraction osteogenesis may lead to degenerative changes in the condylar cartilage. These alterations in the condylar cartilage may be considered as a pressure-related degeneration of the cartilage tissue. However, the long-term results suggest that the condylar cartilage display repair activity after mandibular distraction osteogenesis.
