  • Calcifying aponeurotic fibroma (CAF) is an uncommon benign tumor and a locally aggressive fibroblastic lesion. It commonly affects the palmar side of the hand and fingers, and the soles of the feet. The typical clinical manifestations are known as a poorly circumscribed, slow-growing, and asymptomatic firm mass. Most CAFs usually reveal low to intermediate or isointensity on T1-weighted images, and strong heterogeneous enhancement. However, various signal intensities on T2-weighted images have been reported depending on the degree of hypocellularity or the amount of calcification or collagen within the tumor. This report provides the radiographic and MRI finding of CAF arising in uncommon site, the knee region, of a 19-year-old male who presented with a painful and palpable mass.

오제스키병 바이러스 항원검출을 위한 면역조직화학적 연구 -조직절편내 immunogold-silver법- (Immunohistochemistry for detection of Aujeszky's disease virus antigens : Immunogold-silver method in tissue sections)

  • 본 연구는 감염돼지와 조직배양세포의 동결 및 파라핀 절편에서 immunogold-silver(IGS)법을 이용하여 오제스키병 바이러스를 검출코자 하였으며, 이를 위해 자돈 5두와 돼지뇌유래 섬유 아세포 및 BHK 세포를 공시하였고, IGS법과 immunoperoxidase법의 1차 항원으로는 2종의 단크론성 항체와 고도면역혈청을 사용하였다. IGS법은 양성부위에서 흑색의 미세한 과립의 침착을 볼 수 있었고, immunoperoxidase법에 비해 탁월한 효과를 얻었으며, IGS법은 조직내에 존재하는 각종 바이러스 항원을 검출하는데 이용할 수 있을 것으로 생각된다.

Expression of the Multidrug Resistance Gene and its Product in Osteosarcomas of the Bone - Immunohistochemistry and In Situ Hybridization -

  • Resistance to combination chemotherapy remains challenge in the treatment of osteosarcoma. One of the mechanisms of multiple drug resistance is an increased expression of the multidrug resistance gene(mdr1). Expression of the P-glycoprotein(mdr-1 gene product) was studied immunohistochemically and the mdr-1 gene by in situ hybridization in 33 osteosarcomas relating to various prognostic factors. Thirty cases out of 33 osteosarcomas(90.9%) showed positive cytoplasmic reactions with P-glycoprotein and nineteen instances(57.6%) were strong positive(2+). The older(>20 years) and female patients revealed more intense immunohistochemical reactions rather than those of the younger and male patients. Osteoblastic and chondroblastic osteosarcomas revealed more strong immunohistochemical reactions compared to fibroblastic types. There were no significant staining differences between the type of bony involvement, Broder's grade and the presence of necrosis. On follow-up, the mean survival rate was decreased in the strong positive group, however, this was not statistically significant. In situ hybridization for mdr-1 gene revealed positive signals in 22 cases out of 29 osteosarcomas(75.9%). Chemotherapy was done in 15 cases out of 28 patients(53.6%). The results of immunohistochemistry and in situ hybridization were not correlated with the protocols for chemotherapy. However, this result should be confirmed by a larger scale study about mdr1 mRNA expression.

  • The peripheral odontogenic fibroma (WHO type) is a relatively rare and characteristically benign and unencapsulated, exophytic gingival mass of fibrous connective. Odontogenic epithelium is found within the gingival mass, but usually appears to playa minor role when compared to the fibrous component. The connective tissue is ranged from markedly cellular to relatively acellular and well collagenized. but the connective tissue in this case report appears less cellular. Peripheral odontogenic fibroma must be differentiated histologically from peripheral ossifying fibroma, Peripheral cemental epithelial odontogenic tumor and Peripheral ameloblastoma. The author reports the following conclusions after clinical and radiological examination, excisional biopsy and reviewing literatures. 1. Peripheral odontogenic fibroma is rare lesion and frequently occurs in interdental papila as a form of fibroblastic connective tissue including odontogenic epithelium within the lesion. 2. Peripheral odontogenic fibroma must be differentially diagnosed with Peripheral ossifying fibroma by including less cellular connective tissue, odontogenic epithelium and dysplastic dentin 3. Treatment consists of surgical excision including removal of alveolar bone which is eroded under the lesion

  • To investigate ultrastructural characteristics of cancer cells of the nervous system, 25 cases; i.e. astrocytoma(9), oligodendroglioma(1), medulloblastoma(1), meningioma(5), pinealoma(2) and pituitary adenomas(7). The common findings were marked irregularity of nuclear membrane with pronounced infoldings, clumping of heterochromatin along inner nuclear membrane, enlargement of nucleolus, and frequent observations of nuclear bodies and nuclear inclusions. But these findings are also the signs that can be observed in hyperactive cells. Thus, ultrastructural characteristics of cancerous nucleus are the great variability of nuclear size, shape and composition. but none of them appear to be specific. Among cytoplasmic organelles, massive fibrils are characteristic of astrocytoma and meningiomas, cytoplasmic protofibrils such as glial process and microvesicles in oligodendroglioma, secretory granules are characteristic in pituitary adenomas, and fine filamentous fibrils and desmosomes are characteristic of fibroblastic type of meningioma. Intercellular relationships and cell membrane specialization are important features in the differential diagnosis of various undifferentiated tumors. The frequent resolution of difficult diagnosis problems by electron microscopy outweighs the disadvantages of this technique, such as the expense and time required.

지피비대증 환자의 수술적 치료 1예 (A Case Report: Surgical Treatment of Pachydermodactyly)

  • Pachydermodactyly is a rare distinct form of fibromatosis characterized by acquired, asymptomatic dense fibrosis and fibroblastic proliferation around one or more proximal interphalangeal joints. Intralesional triamcinolone injection may be helpful in improving the clinical appearance. A 20-year-old unemployed female patient gave a four years history of progressive swelling of the fingers affecting predominantly the proximal interphalangeal joints of index fingers of left hand and index, middle fingers of right hand. There was no pain and tenderness. Physical examination, complete blood count, antinuclear antibody, rheumatoid factor were unremarkable and radiograph of both hands showed soft tissue swelling only without bony abnormality. We excised the redundant soft tissue from the both sides of proximal interphalangeal joint with longitudinal elliptical fashion. After operation, the fingers of the patient showed marked improvement cosmetically. There were no motor or sensory changes. Pachydermodactyly was first reported in 1996 in Korea. Histologic features include an increased dermal accumulation of collagen and occasionally an increased number of fibroblast. We report for one patient with pachydermodactyly treated with longitudinal elliptical excision and obtained an improvement of clinical appearance.

하악에 발생된 결체조직성 섬유종 (Desmoplastic Fibroma of the Mandible)

  • Desmoplastic fibroma is a rare. benign intraosseous fibroblastic tumor. which is locally aggressive. It is osseous counterpart of soft tissue fibromatosis. The authors experienced the patient who complained persistent mouth opening limitation with mild swelling on the left mandibular angle area. After careful analysis of clinical. radiological and histopathological findings. we diagnosed as desmoplastic fibroma of the mandible. The results were as follows: 1. Main clinical symptoms were mouth opening limitation which had been persistent for 9 months and mild swelling on the left mandibular angle area. 2. Radiographs showed the radiolucent lesion and expansion of lingual cortex. CT finding is homogeneous soft tissue mass with expansion of left mandibular ramus. Destruction of medial wall of ramus and invasion to adjacent soft tissue is also seen. 3. Histopathologically, plump spindle shaped fibroblasts arranged in bundles or fascicles are observed. The cells of tumor are infiltrating into muscle fiber with destruction of bony trabeculae and merged with surrounding salivary gland.

  • To evaluate the tissue response of demineralized and undimineralized xenogeneic bone-martrix graft in extraskeletal site, we prepared human bone as a implant matrix, and outbred mouse as a recipient. Before clinical application of bank bone of human in Wonkwang university, we should confirm the allogeneic bone grafts us a biologically useful bone graft substitutes, obtanined from the patients receiving oral and maxillofacial surgery. The clinical evaluation and histologic studies showed that both (demineralized and undemineralized) xenogeneic bone matrix grafts were not rejected and that they seemed to stimulate new bone formation at the transplanation site. Undemineralized xenogeneic bone marb6 grafts showed minimal bone induction and gradual demineralization with slow resorption and showed that the differentiation of cells showing fibroblastic activity adjacent to the sop tissue were slowly and less frequently than demineralized bone. Characteristical differences between the demineralized and undemineralized matrix were the appearance of foreign body giant cells (multinucleated giant cells) and the evidence of sloe resorption in undemineralized bone matrix.

  • Benign fibrous histiocytoma(BFH) is a mesenchymal cell-originated tumor composed of cells with fibroblastic and histiocytic differentiation. BFH occurs predominantly on sun-exposed skin of extremities. Oral BFH lesions are uncommon. The majority of oral lesions includes the soft tissue but not the jaw bones. The lesion appears as well-defined multilocular radiolucencies associated with bony swelling when it occurs on the jaw. The lesion induces the thinning and expansion of the cortex and shows many thin, indistinct septa in the lesion. Surgical excision is the choice of treatment. The recurrence rate is low and metastasis has not been reported. We report the clinical, radiographic and microscopic findings of a BFH case occurred in the mandible with literature reviews.

Triamcinolone acetonide가 설 창상치유에 미치는 영향에 관한 실험적 연구 (An Experimental Study on the Effects of Triamcinolone Acetonide on the Wound Healing of Rat Tongue)

  • In order to study the effects of traimcinolone acetonide on the wound healing of tongue, eight two healthy albino rats were asesthetized with pentothal. The wound, approximately 1mm in depth and 2mm in diameter, was created on the midlateral aspect of tongue with the tip of small rongeur fourcep. Forty one animals were employed in one of the following groups:(I) Control group: Received 0.5ml/100gm B.W. of saline suspension intralesionally at weekly interval until sacrificed.(II) Experimental group: Received $100\mu\textrm{g}/100gm$ B.W. of trimacinolone acet- onide suspension intralesionally at weekly interval until sacrificed. Biopsy specimens of wounds were taken at 6,18,24 hours, 2,3,5,7,14 and 28 days after wounding. The specimens were sectioned, and observed with light microscope and electron microscope. The results were as follows : 1. In the triamcinolone acetonide treated wound, the epithelial and fibroblastic regeneration were dealayed when compared with controls. 2. After triamcinolone acetonide treated, edema was decreased at the early stage of wound healing. 3. In the triamcinolone acetonide treated wound, the fibroblasts exhibited fine morphological changes and reduction of collagen formation. It seems that triamcinolone acetonide inhibits fibroblast function.
