Effects of an Experience-focused Prenatal Program on Stress, Anxiety, Childbirth Confidence, and Maternal-Fetal Attachment on Women in Their First Pregnancy (체험중심 산전 프로그램이 초임부의 스트레스, 상태불안, 분만자신감 및 태아애착에 미치는 효과)

  • Purpose: To evaluate the effectiveness of an experience-focused prenatal program on stress, anxiety, childbirth confidence, and maternal-fetal attachment for women in their first pregnancy. Methods: The participants were 57 pregnant women at 32 weeks or more of a first pregnancy who agreed to participate in this study. The data were analyzed with descriptive statistics, t-test, $x^2$ test, and Fisher's exact test using the SPSS 21.0 program. Results: The experimental group showed significant differences in stress, anxiety, childbirth confidence, and maternal-fetal attachment from the control group. Conclusion: The four-week experience-focused prenatal program can be used for women in their first pregnancy to reduce their stress and anxiety and to increase their childbirth confidence and maternal-fetal attachment.

The study on the reproductive rate of retained fetal membrane in Korean native cattles (한우의 후산정체가 수태율에 미치는 영향에 관한 연구)

  • The 3211 Korean native cows raised in Muan, Milyang were investigated for the purpose of finding reproductive rate of cows that retained fetal membrane for two years from April, 1981 to March, 1983. The results obtained were summarized as follows ; 1. The incidence of retained fetal membrane was found in 64 cows(1.98%). 2. The day to first estrus was similar to normal postpartum period unless concurrent genital infection occured. 3. Conception rate of cows that retained fetal membrane was 32.7%, 10.9%, 5.2% and 3.6% at the first, second, third and forth service, respectively and non pregnant cow was 47.6%. 4. Reproductive disorder of non pregnant cows was the highest in repeat breeder and the effect of treatment was very low.

A Study on the Fetal Amino Acids Nutrition at Term of Human Pregnancy (임신말 태아의 아미노산영양에 대한 연구)

  • This study was performed to investigate the placental transfer and the human fetal utilization of amino acids at term of pregnancy. The plasma levels of 23 free amino acids in both the umbilical circulation ( umbilical vein and artery ) and the uterine circulation ( uterine vein and iliac artery ) of 34 pregnant women were measured at delivery by the cesarean section. In the umbilical circulation, 9 amino acids ( alanine, lysine, valine, leucine, arginine, isoleucine, ornithine, cystine, ${\alpha}-aminobutyrate$ ) were significantly higher and 2 amino acids ( glutamate, aspartate ) were significantly lower in the umbilical vein than in the umbilical artery. In the uterine circulation, alanine, tyrosine and methionine were significantly lower in the uterine vein than in the iliac artery. Glutamate was significantly lower in the uterine vein than in the iliac artery. According to these results, the origin of fetal plasma amino acids was discussed in terms of the metabolic conversions which would occur in the Placenta and the fetal utilization of amino acids was estimated.

High Resolution Pitch Determination Algorithm for Fetal Heart Rate Extraction (태아심음주기의 검출을 위한 고해상 피치 검출 알고리즘)

  • Fetal monitoring is a routine procedure to obtain a record of physiologic functions during pregnancy and labor. It is required to determine fetal heart frequency accurately. There are various types of fetal heart rate(FHR) determination and the most frequently applied method is transabdominal Doppler ultrasound. However, in the case of weak or noise corrupted Doppler ultrasound signals, conventional peak detections and the autocorrelation function method have many difficulties to determine FHR precisely. Also the autocorrelation function is effected by threshold level and window size. To solve these problems, the high resolution pitch determination algorinthm is introduced to detect FHR from Doppler ultrasound signals. This scheme digitally processes Doppler ultrasound signal for digital rectification, envelope detection, decimation and correlation calculation of two interconnected segments and then FHR is determined by its maximal value. Even in the case of a greatly smeared noise signal, this algorithm is able to search FHR more accurately than autocorrelation function by means of compensating FHR with a constant correlation threshold. This algorithm is simulated by 386-MATLAB on PC 486/DX and verified that it is superior to the autocorrelation function method.

  • To produce reconstituted rabbit embryos with fetal fibroblasts, the present study was evaluated the efficiencies of the activation conditions as assessments of subsequent development and chromosome in the embryos. New Zealand White rabbits were used throughout the study. Fetal fibroblasts collected from 22-d of fetuses were cultured in DMEM+10% FBS in 5% CO₂ in air. The culture was maintained for 10 passages. In every passage half of cell suspension were kept in frozen. (omitted)

  • The purpose of this study was to evaluate the relationship between fetus-related pregnancy stress and temperament, parenting attitude, parent-fetal attachment, and marital quality. Respondents were 285 pregnant women and their spouses who participated in Pregnant Education Programs. Data was collected by means of questionnaire. The results are summarized as follows: The total fetus-related pregnancy stress level of pregnant couples demonstrated a higher relationship while parent-fetal attachment level demonstrated a very significant relationship. There were significant differences between spouses with regard to fetus-related pregnancy stress, parenting attitude, parent-fetal attachment, and marital quality.

  • Chorionic villus sampling has gained importance as a tool for early cytogenetic diagnosis with a shift toward first trimester screening. First trimester screening using nuchal translucency and biomarkers is effective for screening. Chorionic villus sampling generally is performed at 10-12 weeks by either the transcervical or transabdominal approach. There are two methods of analysis; the direct method and the culture method. While the direct method may prevent maternal cell contamination, the culture method may be more representative of the true fetal karyotype. There is a concern for mosaicism which occurs in approximately 1% of cases, and mosaic results require genetic counseling and follow-up amniocentesis or fetal blood sampling. In terms of complications, procedure-related pregnancy loss rates may be the same as those for amniocentesis when undertaken in experienced centers. When the procedure is performed after 9 weeks gestation, the risk of limb reduction is not greater than the risk in the general population. At present, chorionic villus sampling is the gold standard method for early fetal karyotyping; however, we anticipate that improvements in noninvasive prenatal testing methods, such as cell free fetal DNA testing, will reduce the need for invasive procedures in the near future.

Management of Persistent Pulmonary Hypertension in Preterm Infants

  • Persistent pulmonary hypertension of the newborn (PPHN) is a consequence of the failure of a decrease in the elevated pulmonary vascular resistance after birth. Pulmonary vasodilators, including inhaled nitric oxide (iNO), have been the mainstream of targeted therapy for PPHN, but no drugs have been proven to be effective in preterm infants with PPHN. The fetus remains hemodynamically stable despite lower arterial oxygen tension and pulmonary blood flow as compared to full-term newborns. This adaptation is due to the lower oxygen requirement and high oxygen-carrying capacity of fetal circulation. The immature lungs of preterm infants are more vulnerable to reactive oxygen species, and the response of pulmonary vascular dilatation to blood oxygen tension is blunted in preterm infants. Recently, iNO has been reported to be effective in a selected group of preterm infants, such as those with prolonged preterm rupture of membrane-oligohydramnios-pulmonary hypoplasia sequence. PPHN in preterm infants, along with maximum supportive treatment based on fetal physiology and meticulous assessment of cardiovascular function, is in dire need of new treatment guidelines, including optimal dosing strategies for pulmonary vasodilators.

Multi-Layered Mechanisms of Immunological Tolerance at the Maternal-Fetal Interface

  • Pregnancy represents an immunological paradox where the maternal immune system must tolerate the semi-allogeneic fetus expressing paternally-derived Ags. Accumulating evidence over decades has revealed that successful pregnancy requires the active development of robust immune tolerance mechanisms. This review outlines the multi-layered processes that establish fetomaternal tolerance, including the physical barrier of the placenta, restricted chemokine-mediated leukocyte trafficking, lack of sufficient alloantigen presentation, the presence of immunosuppressive regulatory T cells and tolerogenic decidual natural killer cells, expression of immune checkpoint molecules, specific glycosylation patterns conferring immune evasion, and unique metabolic/hormonal modulations. Interestingly, many of the strategies that enable fetal tolerance parallel those employed by cancer cells to promote angiogenesis, invasion, and immune escape. As such, further elucidating the mechanistic underpinnings of fetal-maternal tolerance may reciprocally provide insights into developing novel cancer immunotherapies as well as understanding the pathogenesis of gestational complications linked to dysregulated tolerance processes.

Effect of Maternal Undernutrition during Late Pregnancy on Growth and Development of Ovine Fetal Visceral Organs

  • This study investigated the effect of maternal undernutrition during late pregnancy on the growth and development of ovine fetal visceral organs. One hundred Mongolian ewes were mated at a synchronized oestrus and divided into three groups and offered 0.175 MJ ME $kgw^{-0.75}\;d^{-1}$ (Restricted Group1; RG1), 0.33 MJ ME $kgw^{-0.75}\;d^{-1}$ (Restricted Group2; RG2) and ad libitum access to feed (Control Group; CG) during late pregnancy (90 days). Selected animals in each group were slaughtered immediately at d 90 of pregnancy and after parturition (neonatal lambs), and major visceral organs were removed and weighed separately. The results indicated that the weights of lung (p<0.01), spleen (p<0.01), heart (p<0.05), liver (p<0.05) and abomasum (p<0.01) in RG1 were significantly lighter than those of CG. For RG2, only the weights of the lung (p<0.05) and spleen (p<0.01) were significantly lighter than those of CG; when expressed as a percentage of body weight, significance was retained in the spleen (p<0.01) for both restricted groups, but the percentage of brain in RG1 was significantly higher than that in CG (p<0.01). For lung and spleen, the amount of DNA was significantly lower (p<0.01) in both groups of restricted neonatal lambs compared to CG; however, there was a significant difference only between RG1 and CG for protein: DNA ratio (p<0.01). The DNA content of kidney, abomasum and jejunum were decreased (p<0.05) in RG1 neonatal lambs, but protein: DNA ratio in the liver was decreased compared with that of CG (p<0.05). The plane of maternal undernutrition during late pregnancy had a significant effect on the growth and development of fetal visceral organs, which altered ontogeny of fetal organ growth and development. These perturbations in fetal visceral development may have significant implications on postnatal growth and adult health.
