CRAMP was identified from a cDNA clone derived from a mouse femoral marrow cells as a member of cathelicidin-derived antimicrobial peptide. Tertiary structure of CRAMP in TFE/$H_2O$ (1 : 1, v/v) solution has been determined by NMR spectroscopy previously and consists of two amphipathic $\alpha-helices$ from Leu4 to Lys10 and from Gly16 to Leu33. These two helices are connected by a flexible region from Gly11 to Gly16. Analysis of series of fragments composed of various portion of CRAMP revealed that an 18-residue fragment with the sequence from Gly16 to Leu33 (CRAMP-18) was found to retain antibacterial activity without cytotoxicity. The effects of two Phe residues at positions 14 and 15 of CRAMP-18 on structure, antibacterial activity, and interaction with lipid membranes were investigated by $Phe^{14,15}$${\rightarrow}$ Ala substitution (CRAMP-18-A) in the present study. Substitution of Phe with Ala in CRAMP-18 caused a significant reduction on antibacterial and membrane-disrupting activities. Tertiary structures of CRAMP-18 in 50% TFE/$H_2O$ (1 : 1, v : v) solution shows amphipathic ${\alpha}$-helix, from $Glu^2{\;}to{\;}Leu^{18}$, while CRAMP-18-A has relatively short amphipathic ${\alpha}$-helix from $Leu^4{\;}to{\;}Ala^{15}$. These results suggest that the hydrophobic property of $Phe^{14}{\;}and{\;}Phe^15$ in CRAMP-18 is essential for its antibacterial activity, ${\alpha}$-helical structure, and interactions with phospholipid membranes.
Objective : The effects of SP6(Sanyinjiao, 三陰交) acupuncture stimulation on bladder parasympathetic nerve activity (PNA), tibial nerve activity (TNA) as well as blood pressure were examined in anesthetized rats and the results are as follows. Methods : The parasympathetic nerve leading to the bladder was exposed retroperitoneally, and the tibial nerve in the hindlimb was exposed through inguinal area. PNA and TNA were recorded using a bipolar platinum-iridium wire electrode and it was connected to preamplifier. Blood pressure was measured using cannula that was inserted into femoral artery and was recorded with blood pressure sensor that is connected with transducer amplifier. The mean value of all parameters measured for 30 seconds before the stimulation was expressed as 100%. An acupuncture needle was inserted into the SP6 region to a depth of 4 mm. Results : Under intact state (IS), acupuncture stimulation for 120 seconds induced significant increase of PNA which was accompanied with decrease of blood pressure. At the same time, TNA showed temporary increase only with acupuncture stimulation. In 4 mg/kg of naloxone-administrated state (NAS), significant decrease of PNA was seen and it was accompanied with increase of blood pressure. TNA also showed temporary increase only with acupuncture stimulation under the NAS just like the IS. Conclusion : consider that SP6(Sanyinjiao, 三陰交) acupuncture stimulation activated parasympathetic nerve and caused vasodilation to lead into the diuresis, but naloxone reversed the effect of acupuncture for antidiuresis.
Purpose: The basic vascular anatomy and versatility of the anterolateral thigh flap was reported firstly by Song in 1984 and then by Zhang who introduced the reverse flow pattern of this flap. In this case, the authors reviewed various articles and their experiences with the distally based anterolateral thigh flap and applied it for coverage of bone-exposed wound occurred at the distal of the disarticulated knee stump. We consequently reported the reliability and resourcefulness of this flap in the difficult and limited situation. Methods: A 67-year-old-man who had suffered from arteriosclerotic obliterans inevitably underwent the disarticulation at knee joint due to clinical deterioration. He presented to our clinic with soft tissue necrosis and bone exposure at the stump. We debrided the wound and conducted the distally based anterolateral thigh island flap by transecting proximal portion of descending branch of the lateral circumflex femoral artery and the $14{\times}10cm$ sized flap was transferred to cover the defect. The pedicle measured 14 cm in length with pivot point 7 cm above the patella. Results: The postoperative course was mainly uneventful except early venous congestion for 4 days and subsequent partial skin loss. The wound was healed by secondary intension and no other sequelae had been observed during follow-up period of 12 months. Conclusion: Despite the presence of various reconstructive choices, the distally based anterolateral thigh island flap can be designed to repair soft tissue defects around the knee region, providing its reliable blood supply and long pedicle length, especially in the challenging cases.
BACKGROUND/OBJECFTIVES: The effect of St. John's Wort extract (SJW) on MG-63 cell proliferation and trabecular bone loss induced by ovariectomy was examined. MATERIALS/METHODS: Proliferation, expression of estrogen receptor (ER) ${\alpha}$ and ER ${\beta}$, and gene expressions of osteoprotegerin (OPG), osteocalcin (OC) and alkaline phosphatase (ALP) were examined in MG-63 cells treated with or without SJW. Ovariectomized rats were treated with SJW at the dose of 100 or 200 mg/kg/day, ${\beta}$-estradiol-3-benzoate (E2), or vehicle only (OVX-C), and sham operated rats were treated with vehicle only (Sham-C). Serum ALP and C-telopeptide (CTX), and femoral trabecular bone loss were examined. RESULTS: SJW increased MG-63 cell proliferation and expression of ER ${\alpha}$ and ER ${\beta}$, and positive effect was shown on gene expressions of ALP, OC and OPG. SJW also showed estrogen like effect on bone associated with slowing down in trabecular bone loss. Histopathology by H&E showed rats treated with SJW displayed denser structure in metaphyseal region of distal femur compared with rats in OVX-C. SJW was shown to reduce serum CTX in OVX rats. CONCLUSION: The present study provides new insight in preventing estrogen deficiency induced bone loss of SJW and possibility for its application in bone health supplement.
This study developed the pants pattern, that improved appearance and gesture functions in clothes to resolve inconveniences in case of activities with being adequate in fitness of slim-fit pants for obese adolescents. The slim-fit pants pattern was developed through the fitting test after designing and making the pattern of the experimental clothing A(c/pu; 99/1%) with the primary commercial clothing as basic prototype, through the fitting test and implementation of the experimental clothing, modified and supplemented secondarily, and through the fitting test of the second experimental clothing, modified and supplemented tertiary. In terms of findings, first, as a result of analyzing the commercial slim-fit pants, it is interpreted to have projected an optical illusion, that appears to be slim due to being narrow in width of the front panel when observed from the front of the pants. Second, in the fitting test of the first experimental clothing, the prototype of commercial clothing was understood to have improved butt, crotch, waist and femoral regions. Third, pattern design of the second experimental clothing was allowed to decline abdominal pressure, in case of the sitting position, by making the crotch line in the front panel short and by handling it with a yoke belt, and was processed a difference between waist and hip circumference in the back panel, with a rubber band in the whole waist part. Fourth, in the gesture function test of the third experimental clothing, high evaluation was received in every item excluding the knee region.
Giant cell tumor is most frequently found in juxtaarticular region, and difficult to treat because of local recurrence. Although primary resections reduce recurrence, the joint function will be markedly impaired. Techniques involving physical adjuncts(high speed burr and electric cauterization), acrylic cement or en bloc resection with VFG(vascularized fibular graft) have been employed to reduce local recurrence. From October 1984 to April 1994, twenty-nine patients diagnosed as giant cell tumor were treated at department of Orthopaedic Surgery, School of Medicine, Kyung Hee University. There were eleven men and 18 women, ranging in age from 17 to 52 years(mean: 34 years). The average follow-up period was four years and five months. The location of the lesion was around the knee in 15, distal radius in three, femoral head in three, and others in eight patients. Fifteen patients around the knee joint were treated with several modalities; curettage with bone graft in five, curettage with cement filling in three, curettage with bone graft and physical adjuncts in five, en bloc resection with VFG in one and en bloc resection with arthroplasty in one patient. The functional results, according to the Marshall's knee score, were excellent in one, good in two, and fair in two after the curettage with bone graft, good in three after the curettage with bone cement filling, excellent in one, good in four after the curettage with bone graft and physical adjuncts, and good in two after the en bloc resection with VFG or arthroplasty. Three patients had local recurrence among 15 patients with giant cell tumor around knee. Vascularized fibular graft around wrist joint provided good functional restoration without local recurrence in all three patients who had giant cell tumor in distal radius. Although there is no statistical significance, it seems that curettage with bone graft using physical adjuncts or acrylic cement reveals better results than simple curettage with bone graft. Excellent functional result were obtained without local recurrence by using vascularized fibular graft after en bloc resection.

Free flap transplantation with microvascular anastomosis has been successfully performed by development of surgical technique, materials and postoperative monitoring equipments of flap. But success rate of microvascular anastomosis is influenced by various factors, and failure rate is about 5-10%. The most influential factor for success rate is surgical technique and other factors that influence failure of microvascular anastomosis are ischemic time of free flap, thrombus formation of anastomosis region and vascular spasm. In this study, vascular patency and thrombus formation in experimental micro-venous anastomosis, and endothelial repair were observed with histologic analysis, scanning electron microscopy, transmission electron microscopic examination. The results were obtained as follows: 1. In vascular patency test in 30 minute and 7 days after micro-venous anastomosis with heparin irrigation, all of 12 anastomosis site were good vascular patency. 2. In thrombus formation in 2 weeks group(Experimental I), 2 site of 6 cases were observed thrombus, and in 4 weeks group(Experimental II), 1 site of 6 cases were observed thrombus. 3. In histologic examination, normal vein(Control Group) showed continued internal elastic lamina, well formed thick smooth muscle layer and connective tissue. The group of 2 weeks after microvenous anastomosis(Experimental I) showd locally recovered internal lamina, discontinued internal lamina, disorganized smooth muscle cells and granulation tissue around suture silk. In the group of 4 weeks after micro-venous anastomosis(Experimental II), anastomosis site showed almostly continued internal lamina, disorganized smooth muscle cells and cicartrized tissue around suture silk. 4. In scanning electron microscope examination in 2 weeks(Experimental I) after micro-venous anastomosis, mesh fibrin formation showed near to endothelial cells, and in 4 weeks after micro-venous anastomosis(EXperimental II), numerous blood cells and fibrin mesh formation was seen associated with irregular endothelial cell arrangement. 5. In transmission electron microscope examination in 2 weeks after micro-venous anastomosis(Experimental I), irregular arrangement of smooth muscle cells was seen adjacent to collagenized tissue around suture silk. In 4 weeks after micro-venous anastomosis(Experimental II), denuded venous wall composed of relatively well arranged smooth muscle cells was covered by endothelial cells, but fibroblast cells and foreign body giant cells near to suture silk was remained. From the results obtained in this study, results of good vascular patiency and anti-thrombotic effect of heparin were obtained as a local irrigation solution, and repair of venous endothelial cell was observed in 2 weeks after micro-venous anastomosis.
Complete denervation after severe brachial plexus injury make significant muscle atrophy with loss of proper function. It is much helpful to reconstruct the essential function of the elbow flexion movement in patient with total loss of elbow flexion motion after brachial plexus lesion which was not recovered with nerve surgery or long term conservative treatment from onset. In whole arm type brachial plexus injury, if there were no response to neurotization or neglected from injury, the volume of the denervated muscle is significantely reduced month by month. About 18 months most of the muscle fibers change to fibrous tissues and markedly atrophied irreversibly, further waiting is no more meaningful from that period. Authors performed 14 cases of functioning gracilis muscle transfer from 1981 to 1995 with microneurovascular technique, neuromusculocutaneous free flaps were performed for reconstruction of lost elbow flexion function. Average follow-up period was 5 years and 6 months. We used couple of intercostal nerves as a recipient nerve which were anastomosed to muscular nerve from obturator nerve in all cases. Recipient vessels were three deep brachial artery and eleven brachial artery which were anastomosed to medial femoral circumflex artery with end to end or end to side fashion. Average resting length of the transplanted gracilis were 24 cm. We can get average 54 degree flexion range of elbow with fair muscle power from flail elbow. There were one case of muscle necrosis with lately developed thrombosis of microvascular anastomosed site which comes from insufficient recipient arterial condition, 3 cases of partial marginal necrosis of distal skin of the transplanted part which were not significant problem with spontaneously solved with time goes by gracilis muscle has constant neurovascular pattern with relatively easy harvesting donor with minimal donor morbidity. Especially it has similar length and shape with biceps brachii muscle of upper arm and longer nerve pedicle which can neurorrhaphy with intercostal nerve without nerve graft if sufficient mobilization of the nerves from both sides of gracilis and intercostal region. Authors can propose gracilis muscle transplantation with intercostal nerves neurotization is helpful method with minimal donor morbidity for neglected brachial plexus palsy patients.
동적 PET 데이터는 구획모델과 Nonlinear Least Squares(NLS)방법을 사용하여 분석함으로서 각종 생화학적 물질의 생체 대사율 등을 정량화 할 수 있다. 하지만 NLS방법은 변수의 초기 값이 적절하지 않을 경우 지역적인 최소점에 빠지거나 계산시간이 길어 동역학 변수들을 각 화소마다 구해야 하는 파라메터 영상(parametric image) 구성에는 효과적이지 않다. Patlak 도표분석법(PGA)은 비선형 방정식을 선형화하여 값을 추정함으로서 간단하면서 적은 계산량으로 인해 파라메터 영상을 구성하는데 많이 사용되고 있으나 잡음성분과 선형구간 선정에 따라 값이 영향을 받는 단점이 있다. 따라서 이 연구에서는 3구획 비가역 모델에 적합한 다중선형분석법(MLAIR)을 고안하였으며 3구획 비가역 모델의 대표적 예인 $[^{18}F]Fluoride$ PET을 이용하여 미니돼지에서의 뼈 섭취률을 계산하여 PGA방법과 비교 분석해 보았다. 대상 및 방법: 3마리의 미니돼지를 대상으로 100MBq의 $[^{18}F]Fluoride$를 대퇴부 정맥에 주사하면서 ECAT EXAET 47 PET 스캐너를 이용하여 60분간 PET 영상을 얻었다. 케타민과 자일라진을 이용하여 30분 간격으로 마취하였으며 실험동물을 진공 쿠션을 이용하여 반드시 누운 자세로 위치하도록 고정 시켰다. 입력함수인 혈장 내 농도곡선은 대퇴동맥으로부터 스캔 시작과 함께 혈액 채취를 통해 얻었다. ROI분석을 위해 대퇴골두, 척추 뼈, 근육에 ROI를 그려 조직 내 시간-방사능 곡선을 얻었다. $k_4$가 0인 3구획 비가역 모델로부터 MLAIR와 PGA방법을 사용하여 관심영역에서의 뼈 섭취률 $K_i$와 파라메터 영상을 구성하였다. PGA방법은 선형구간의 시작점인 $t^*$선택에 따른 영향을 보기 위해 분석구간을 변화시켜가며 분석하였다. 결과: ROI 분석결과 추정된 $K_i$값은 NLS방법에 비하여 MLAIR방법과 PGA방법 모두에서 과대 추정되었으나 두 분석방법 끼리는 비슷한결과를 보였다. Patlak 기울기는 $t^*$선택에 따라 값이 변하였으며 Patlak 상수는 Fluoride 섭취가 높은 대퇴골두나 척추뼈에서 30분이 지나서야 일정한 값으로 나타났고 섭취가 낮은 근육에서는 10분만에 일정해졌다. 파라메터 영상에서는 제안한 MLAIR방법이 PGA방법에 비해 영상의 질을 향상시킴을 알 수 있었다. 또한 PGA방법을 이용하여 구성한 파라메터 영상은 $t^*$값이 커질수록 급격히 영상의 질이 저하됨을 볼 수 있다. 특히 Fluoride 섭취가 높은 영역에서 Patlak 상수가 일정해지는 시간인 $t^*$값이 30분일 때 파라메터 영상은 MLAIR와 크게 차이가 났다. 결론 결론적으로 제안한 MLAIR방법은 선형구간을 정할 필요 없이 모든 데이터를 사용하는 이점이 있으며 선형적인 방법을 통해 $K_i$값을 얻을 수 있어 계산시간을 단축 시켜 줄 뿐 아니라 잡음성분에 강해 파라메터 영상의 질을 크게 향상 시켜 줌으로 비가역 3구획모델에서의 PGA방법을 대체할 새로운 파라메터 영상구성방법으로 적합할 것이다.
하지 정맥류 질환 환자를 대상으로 하지 정맥조영술 MDCT 검사에서 환자의 피폭선량을 최소화 하면서 최적의 진단영상을 묘출하기 위해 고정 관전류 기법과 Z-축 자동 관전류 변동 제어를 이용하여 CTDIvol(CT dose index volume), DLP(dose length product) 산출을 통한 영상의 노이즈를 측정하여 비교하였다. Monte Carlo simulation으로 200, 250, 300 mA에서 CTDIw, CTDIvol, DLP를 계산하여 고정 관전류 기법과 비교하였다. 고정 관전류 기법의 대상 환자는 50명(평균 나이, 46세; 연령 범위, 35-61세)으로 남성30명, 여성 20명 이었고, 평균 체중은 62.4 kg 이었다. Z-축 자동 관전류 변동 제어 대상 환자는 50명(평균 나이, 43세; 범위, 37-63세)으로 남성25명, 여성 25명 이었고 평균 체중은 60.1 kg 이었다. 고정 관전류 기법은 200, 250, 300 mA를 기준으로 하고, Z-축 자동 관전류 변동 제어는 노이즈 지수 10, 11, 12 HU에서 관전류 $70{\sim}450\;mA$ 범위 내에서 자동으로 선택하였다. 고정 관전류 기법과 Monte Carlo simulation 비교에서 200 mA에서의 CTDIvol은 차이가 없었으나, 250 mA, 300 mA 에서의 Monte Carlo simulation는 높았고, DLP는 모든 관전류에서 Monte Carlo simulation이 높게 측정 되었다. 노이즈는 고정 관전류에서 최소 $9.8{\pm}0.9\;HU$, 최대 $12.5{\pm}0.7\;HU$ 이었고, Z-축 자동 관전류 변동 제어에서는 최소 $11.3{\pm}0.8\;HU$, 최대 $12.9{\pm}0.7\;HU$이었다. Z-축 자동 관전류 변동 제어에서 노이즈 지수가 증가하면 CTDIvol과 DLP가 감소하였으나 노이즈는 증가하였다. 생식부위를 포함하는 하지 정맥조영술에서 Z-축 자동 관전류 변동 제어 방법이 고정 관전류 기법에 비해 선량을 감소하는 효과가 있었다.

① NDSL에서 제공하는 모든 저작물의 저작권은 원저작자에게 있으며, KISTI는 복제/배포/전송권을 확보하고
있습니다.
② NDSL에서 제공하는 콘텐츠를 상업적 및 기타 영리목적으로 복제/배포/전송할 경우 사전에 KISTI의 허락을
받아야 합니다.
③ NDSL에서 제공하는 콘텐츠를 보도, 비평, 교육, 연구 등을 위하여 정당한 범위 안에서 공정한 관행에
합치되게 인용할 수 있습니다.
④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
