A Case Study on Traditional Korean Medicine Therapy in an Elderly Patient with a Traumatic Intracerebral Hemorrhage (의식장애를 동반한 외상성 뇌내출혈 노인 환자의 한방치료 치험 1례)

  • Few studies have reported the effects of traditional Korean medical (TKM) therapy in patients with traumatic brain hemorrhage. The patient was a 93-year-old female who was unconscious after a traumatic intracerebral hemorrhage (ICH) and was not a candidate for any surgical treatment because of her age. We treated her with acupuncture and herbal medicine. After 28 days of treatment, we observed an improvement in consciousness on the Glasgow Coma Scale (GCS) and in other symptoms (aphasia, weakness in both legs, and headache).

  • Cutaneous leiomyosarcoma is an uncommon superficial soft tissue sarcoma and mainly found in the middle aged to elderly males. It can occur in any part of the body, mostly affecting the extremities and rarely affecting the face. It grows relatively slowly, can be diagnosed by biopsy and is treated by surgical excision. It needs to be distinguished from other spindle cell neoplasms, and immunohistochemical markers are usually required to attain an accurate diagnosis. We report a case of cutaneous leiomyosarcoma appeared on the left cheek within 6 month of a 73-year-old female patient suspected with malignant melanoma before surgery

Health Literacy and Health Behaviors among Older Adults with Cardio-cerebrovascular Disease Residing in Rural Areas (농촌 지역 심뇌혈관질환 노인의 건강정보이해능력과 건강행위)

  • Purpose: The aim of this study was to identify levels of health literacy and reported health behaviors among older adults with cardio-cerebrovascular disease residing in rural areas. Methods: A cross-sectional survey was conducted with a convenience sample of 134 older people (mean age=75.5 years, 71.6% Female) registered at seven health centers. The structured questionnaires were used to measure levels of health literacy and health behaviors. Results: The average functional health literacy score was $6.08{\pm}3.04$, and the health behavior score was $62.92{\pm}6.45$. Respectively health behavior was positively associated with health literacy. Education, economic status, number of family members, number of social activities and health literacy were contribution factors explaining 40.64% of the variance in health behavior. Health literacy specifically explained 12.5% of health behavior. Conclusion: These findings suggest that strategies for improving health behaviors and reducing health inequalities may benefit from adopting a stronger focus on health literacy within prevention, patient education, and other public health interventions. Further longitudinal studies are needed to help confirm that improving health literacy in the elderly may be effective in changing health behaviors.

The Effect of Footwear on Gait in Older Adults (신발의 형태가 노인의 보행에 미치는 영향)

  • Purpose : The purpose of this study was to evaluate the effect of footwear on gait in older adults. Methods : The footwear consists of loafer with fixation and mule and slipper without fixation. Twenty one female older adults voluntarily participated in this study. Each participant walked on the gait system GAITRite and measured temporal-spatial gait parameters. And the participants wore loafer, mule, slipper randomly. We measured stance time, swing time, heel-to-heel BOS, double support time, velocity, cadence, FAP as temporal-spatial gait parameters. Results : Three gait parameters showed significant difference statistically among 7 gait parameters. The stance time increased as loafer, mule, barefoot, slipper orderly. And there was a significant difference statistically (p<.05). The swing time increased as slipper, barefoot, mule, loafer orderly. And there was a significant difference statistically(p<.05). And the heel-to-heel BOS increased as barefoot, loafer, mule, slipper orderly. And there was a significant difference statistically(p<.05) according to footwear type. Conclusion : The footwear type with high stability like loafer is considered better than footwear type with low stability like mule and slipper for the elderly.

  • Summary: This study evaluated the relationship between voice complaint and deviant vocal fold status with special regard to presbylarynx, in patients aged more than 60 years with pharyngeal-laryngeal complaint. The material consisted of clinical histories and images obtained by laryngoscopies of 75 patients aged more than 60 years, who had sought otorhinolaryngologic treatment. Indicative glottic characteristics of the presbylarynx, such as vocal fold bowing(VFB), prominence of vocal processes (PVP), and membranous spindle shaped glottic chink(MSC) and the presence or absence of voice complaint were analyzed Also, acoustic parameters such as fundamental frequency(Fo), jitter percent and shimmer percent were analyzed. VFB showed a strong correlation with voice complaint in male. Jitter and shimmer were correlated with VFB, PVP, MSC in female.

Radiographic study of dermal fillers in the facial area: A series of 3 cases

  • In recent years, as interest in maintaining beauty and a youthful appearance has grown, filler procedures such as soft tissue augmentation have become more popular. These fillers are sometimes seen as radiopaque shadows on radiographic images, either due to the fillers themselves or because of secondary reactions; such findings may present a diagnostic challenge to dentists. The present report describes 3 cases of dermal fillers observed in panoramic and cone-beam computed tomographic (CBCT) images. All 3 elderly female patients had filler injected into their cheeks and chin area for cosmetic purposes decades ago. On panoramic images, multiple symmetric radiopacities were observed in the facial area; on CBCT, these calcifications were seen in the subcutaneous tissue in various shapes and with varying density. In conclusion, dentists should be aware of the imaging characteristics of dermal filler, and should be able to differentiate dermal filler from other pathological findings.

Factors Influencing the Quality of Life of Caregiver using Day Care Service (노인주간보호서비스 이용 부양자의 삶의 질과 영향요인)

  • Purpose: The purpose of this study was to identify factors influencing quality of life of caregivers using day care services. Methods: Data were collected from 14 different day care services in D city, with a total of 146 female caregivers as participants. All participants were at home caregivers whose patients were senior citizens older than 65. Data were collected from September 1 to 30 of 2008 and analyzed by t-test, ANOVA, Pearson's correlation coefficient, and stepwise multiple regression. Results: Quality of life among caregivers of day care services differed significantly according to state of health, other caregivers, job, and family income. Quality of life among caregivers showed correlation with caregiving burden. The significant predictors affecting quality of life were state of health, other caregivers, job, and family income and caregiving burden. A combination of these predictors accounted for 43.5% of the variance in quality of life. Conclusion: These findings show that quality of life of family caregivers using day care services is influenced by caregiving burden. Development of day care service programs is necessary in order to reduce caregiving burden of family caregivers for the elderly.

A Study on the Demand for Ubiquitous Computing Services (유비쿼터스 서비스 수요에 관한 연구)

  • Ubiquitous computing is a vogue word, and its service is very popular in Korea. However, present ubiquitous service(u-Service) is thought to be more oriented to the provider's point of view, not the consumer's. To complement this problem, an online survey was performed for internet users, which consists of 24 single choice questions on six fields such as transportation, healthcare, environment/city-management, culture/education, administration, and industry. 2463 people joined the survey, who were grouped according to their age, resident region, level of education, and sex. Each group's demand for specific field of u-Service was examined. General level of demand was higher in case of the elderly and male than the younger and female for overall u-Service.

Determinants of the Demand for Public Ambulance Calls in a Metropolitan Area (서울시 소방구급차(消防救急車)서비스 수요(需要) 결정요인(決定要因))

  • Purpose : This study was to analyze the demand for emergency ambulance service and to characterize the factors associated with the demand. Method : The basis for the model was from the actual demand for public emergency ambulance and socioeconomic and geographic characteristics. Multiple regression analyses were done for the related characteristics of public ambulance service. Result : The model explained total demand with a high degree of accuracy : the coefficient of determination($R^2=0.96$). For the regression, the set of variables indicative of low socioeconomic status were all significant. It showed the inappropriate use of public ambulance system. Public ambulance demand increased in higher housing density, low income, male unemployment and female labor force. Conclusion : The demand for public ambulances appeared to be highly predictable, using a simple linear model employing socioeconomic variables, quality of service variables, and land use variables. Low-income families tended, to use the public ambulance system more often than higher income. Area having elderly people or children also made many calls. Estimated demand calls were stable and had a tendency to be similar incident types.

Numerical Analysis on the Effect of Improved Fractional Effective Dose(FED) for Evacuation by FDS_EVAC (FDS_EVAC에 적용된 독성가스모델(FED)이 피난에 미치는 영향 분석 및 개선에 대한 연구)

  • When fire occurs in complex or building, toxic gas is important factor for evacuation because it cause death of evacuees. The effect of toxic gas which effects human motion is calculated using Purser's Fractional Effective Dose(FED). The FDS_EVAC is used for evacuation. However, FED of FDS_EVAC has some problem of application because it dose not considers evacuees's gender and age groups but using single subject. In this study, numerical analysis using modified FED which considers gender and age group of evacuees was performed. We investigate the effect of improved FED on the evacuation. We applied evacuation analysis to exhibition centre where the evacuees was set one thousand five hundred people. The FED is increasing with children 2%, male 17%, female 4%, adult 13% and elderly people 22%. Fatality is increasing due to increased FED.
