The Effect of Breast-feeding Education on Method, Attitude, and Duration of Breast-feeding by Primipara (모유수유 교육이 초산모의 모유수유 방법, 태도 및 지속정도에 미치는 효과)

  • The purpose of this study is to investigate the effect of breast-feeding education on skills, attitude, and duration of breast-feeding among primipara mothers. The educational session was offered during the period of June through September at the T medical center in T city. One hundred thirteen primiparas were selected for this study, 61 who received breast-feeding education(lecture, demonstration, and additional telephone counselling) served as the experimental group and 52 who did not as the control. Data collection was done by self-reporting method. the reliability of the instrument was from .77 to .87. Duration of breast-feeding was classified into I through V according to the WHO classification at the time point of 4 weeks of postpartum. The results were as follows : 1. The score for the experimental group on the skills of breast-feeding techniques was significantly higher than those of the control(t=9.12, p=.000). 2. The score for the experimental group in the attitude toward breast-feeding was significantly higher than those of the control(t=6.12, p=.000). 3. Duration of breast-feeding for the experimental group was significantly longer than that of the control at the time of 4 weeks of postpartum($x^2$=20.79, p=.001). In conclusion, the breast-feeding practice can be promoted effectively through the breast-feeding education.

A Study on Infant Feeding and Weaning Practices in Multicultural Families (다문화가정 영유아의 수유 및 이유 실태에 관한 연구)

  • The purpose of this study is to investigate the infant feeding and weaning practices in multicultural families. We surveyed 159 married female immigrants in Jeonbuk province from April 2010 to April 2011. They are from Vietnam (49.7%), China (25.8%), Cambodia (8.8%), Japan (8.2%), and the Philippines (7.5%). The average rate of colostrum feeding of all the subjects was 91% and there were significant differences between nationalities (p<.05), family types (p<.01), and delivery methods (p<.001). 41.5% of those answered did breast feeding, while 49.1% combined breast and bottle feeding, and 9.4% did bottle feeding only. The reason for bottle feeding is either because they are unable to produce enough breast milk or because they think the formula is better than breast milk. Average period of breast feeding was 10.3 months. There were significant differences of breast feeding duration between maternal age (p<.05) and economic level (p<.001). The mean onset time of weaning was 7.8 months, and there were significant differences between nationalities (p<.05), family types (p<.05), and feeding methods (p<.05). There was no significant difference in methods of supplementary food preparation between nationalities, family types, jobs, and education levels. The mean onset time of commercial milk was 12.8 months, and there were significant differences between nationalities (p<.05), the duration of marriages (p<.05), education levels (p<.05) and feeding methods (p<.001).

  • This was a pre-experimental study to confirm the breast-feeding ability and effect of follow up care on the breast-feeding rate and the breast-feeding method by the mother-infant breast-feeding assessment tool for the mothers who got the breast-feeding education. The subjects were 46 mothers who experienced normal delivery of infants at a college hospital in K-city, Kyungsangbuk-do, from July 1 to October 21, 2000. The instrument for data obtainment were The Mother-Infant Breast-Feeding Assessment Tool of Johnson et al. (1999), and The Breast- Feeding Method Measurement Tool of Jeong, Geum-hee(1997). This instrument was reliable, showing Cronbach $\alpha$.751. This study classified them into 3 groups: at high risk for breast-feeding failure, at risk for breast-feeding problems, and at low risk for breast-feeding failure by the mother-infant breast-feeding assessment tool on the day of discharge from the hospital after delivering individual breast-feeding education to the subjects. This study investigated the breast-feeding rate and the breast-feeding method through mail questionnaire at the four week after childbirth, and through the phone counselling and the home visit for follow up care at the first week and the second week after childbirth. The sixth week after childbirth, this study investigated the breast-feeding rate by phone. The data analyzed the hypothesizes by $x^2$-test, paired t-test, ANOVA, Wilcoxon signed rank test, Wilcoxon rank sum test and trend analysis using SPSS/PC+ WIN 10.0 program. The results were as follows : 1) Hypothesis 1-1, "there won't be any difference the breast-feeding rate of a group at risk for breast-feeding failure by the time elapsed" was supported through constant the breast-feeding rate, because changes in the breast-feeding rate by the time elapsed after childbirth wasn't statistically significant(t= -1.501, p=.270). Hypothesis 1-2, "there won't be any difference the breast-feeding rate of group at low risk for breast-feeding failure by the time elapsed" was supported through constant the breast-feeding rate, because changes in the breast-feeding rate by the time elapsed after childbirth wasn't statistically significant(t=-1.732, p=.225). 2) Hypothesis 2-1, "there won't be any difference between the breast-feeding method of group at risk for breast-feeding failure for four weeks after childbirth and just after childbirth” was rejected, because the mean point of post test appeared to be higher than that of pre test(t=-7.267, p=.000). Hypothesis 2-2, "there won't be any difference between the breast-feeding method of the group at low risk for breast-feeding failure for four weeks after childbirth and just after childbirth" was rejected, because the mean point of post test appeared to be higher than that of pre test(t=-2.501, p=.012). 3)The 3rd hypothesis, "there won't be any difference between breast-feeding method of groups at risk for breast-feeding problems and at low risk for breast-feeding failure at the 4th week after childbirth and just after childbirth" didn't show any difference between the breast-feeding method of groups at risk for breast-feeding problems and at low risk for breast-feeding failure in the advance test(t=-1.521, p=.130) but there was difference between them in post test (t=-2.012, p=.044). As a result, the 3rd hypothesis was supported by pre test, but it was rejected by post test. In conclusion, this study confirmed breast- feeding education and follow up care just after childbirth were effective for the breast-feeding rate and method. Accordingly, it is proposed that successful nursing intervention of breast-feeding to be necessary by continuously providing follow up care through the mother-infant breast-feeding assessment tool as well as to execute individual breast-feeding education to mothers just after childbirth.

Effects of Body Position and Time after Feeding on Gastric Residuals in LBW Infants

  • Purpose. To examine the effects of body position and time after feeding on gastric residuals in low birth weight infants(LBW). Methods. A repeated measures design was conducted. Twenty LBW infants being fed via indwelling nasogastric tubes were randomly assigned to one of 5 different position orderings. In each position, gastric residuals were measured at 30, 60, 90, 120, 150, and 180 minutes after feeding. Results. In comparisons of gastric residuals with position and time, the main effects of position and time were statistically significant (F=5.038, p=0.001; F=429.763, p < 0.001, respectively), but the interaction between position and time was found not to be significant. In pairwise comparisons, the gastric residuals were more reduced in the right anterior oblique and prone positions compared with the left lateral position (p < 0.05) and the differences across time periods were significant (p < 0.001). Conclusions. The body position after feeding has a significant affect on gastric residuals over time in LBW infants. The right anterior oblique or prone position is recommended rather than left lateral position after feeding. Knowledge of the proper position and the pattern of gastric emptying over time after feeding may lead to the development of evidence-based nursing care.

A Comparative Study on the Perception of the Impediment Factors between Mothers and Nurses in Nursery (모유수유 저해요인에 대한 산모와 신생아실 간호사간의 인식정도 비교 연구)

  • The condition of nutrition during infancy will greatly affect infants' physical, emotional growth, especially breast feeding is important in their growth and development, and emotional stability, too. Despite such advantages of breast-feeding, its rate has continued to fall year after year in Korea : the 95% rate in 1960's has fallen to 25.4% in 1990. It is known that such a downfall of breast-feeding rate is associated with various factors. The purpose of the study is to examine mother's and nurses' in nursery perception of the impediment factors of breast-feeding : to compare those between of breast-feeding : to compare those between two groups : to provide fundamental data for developing strategies for increasing breast-feeding. The subjects were 45 new mothers from one hospital and the same number of nurses sampled from 3 university hospitals, in Seoul. The data were collected for 11 days from April 12 to April 23, 1994 and a questionnaire was developed based on the interview with 14 nurses and 10 mothers and the literature reviews. Liker's Five-Point Scale was used as measurement. The Data were analyzed using SPSS / PC and descriptive statistics, t-test, ANOVA. The results of study are as follows : 1) Most new mothers get the information about breast-feeding from their mothers, books or newspaper, and relatives in rank than professionals(nurses or doctors). 2) The impediment factors the breast-feeding are categorized as mothers, hospital system, medical personnels and social factors. The most frequently mentioned impediment factors from mothers are 'Difficulty by operational wounds'(3.13), 'Lack of will for breast-feeding'(3.47), 'Insufficient rest and sleep'(3.52) and 'Opposition from husband or his family members'(4.77)in rank. On the other hand those factors from nurses are 'Inadequate nipple condition'(2.37), 'Decreasing milk secretion given medicine after operation'(2.63), 'Mothers knowledge deficit by poorly prepared education'(2.79) and 'Mothers abhorance of breast-feeing'(3.87) in rank. 3) As for the hospital system, the highly perceived impediment factors by mothers are 'Seperation of baby from mother after birth'(2.78), 'Lack of space for breast-feeding in the hospital'(2.93), 'Lack of facility for informing the time for breast-feeding'(3.18) and 'Because of babys' preference artificial nipple by being accustomed to it in hospital(3.97), in rank. Meanwhile, those from nurses are "Seperation of baby from mother after birth",(1.92), "Inconsistency between hospital's nursing time and mother's breast secretion time" scretion time(2.97), "Lack of space for breast-feeding in the hospital"(3.39), and "Lack of facility for informing the time for breast-feeding"(3.74) 4) As for the medical personnels, the highestly perceived Impediment factor from mothers in "Lack of professional nurses for breast-feeding"(2.96), and the lowestly perceived one is "Doctors' reluctance"(4.75). Nurses perceived same as mothers, too. 5) As for the social factors, the highestly perceived impediment factor by mothers and nurses is "Inconvience of social activities"(2.83) and the lowestly ones are "The sense of self-sacrifice"(4.22) by mothers, and "The sense of old fashioned"(4.13) by nurses. 6) The difference of the perception of impediment factors between mothers and nurses is statistically significant only in mother factor.

A study on the Sleep/Activity pattern of normal neonate in Kangwon area (강원도 지역 신생아 수면/활동 양상에 관한 연구)

  • The purpose of this research is to investigate Sleep/Activity pattern of normal neonate in Kangwon area. The subjects of this research were 23 normal neonate who visited pediatric clinic of kangneung general hospital from Jun 15, 1999 to August 10, 2000. The instrument of this research was the NCASA. The results of this research were as follows: 1. The mean time of daytime sleep was 8.21 hours. The mean time of nighttime sleep was 5.70 hours. The mean time of total daily sleep was 13.92 hours. The mean time of longest sleep period was 5.02 hours. The mean time of regularity of sleep were daytime 46.59%, nighttime 74.43%, total daily sleep 55.88%. The mean frequency of nighttime wakening was 2.03 times. 2. The each mean time of activity were daytime 7.72 hours, nighttime 2.28 hours, daily total activity 9.99 hours. The mean time of the longest activity period was 6.16 hours. The mean times of feeding frequency were daytime feeding 4.53, nighttime feeding 1.92, total daily feeding 6.45. The mean of regularity of feeding frequency was 65.71%.

Postnatal Breast-Feeding Knowledge, Techniques and Rates of First-Time Mothers Depending on a Prenatal Breast-Feeding Education Method (산전 모유수유 교육 방법에 따른 초산모의 산후 모유수유 지식, 방법, 실천율)

  • Purpose: To investigate the impacts of individualized versus group prenatal breast-feeding education on postnatal breast-feeding knowledge, this study addressed techniques and rates of first-time mothers as part of data collection for developing breast-feeding education programs. Methods: A quasi-experimental nonequivalent control group design was applied to compare the effects of prenatal breast-feeding individual vs. group education among first-time mothers. Collected data was analyzed using $x^2$-test, t-test, paired t-test, and ANOVA. Results: There was no difference on the knowledge of prenatal breast-feeding between individual and group education. However, individual education was a more effective method than group education on post-natal knowledge. In the 4th and 8th weeks of breast-feeding, individual education was more effective than group education. However, there was no difference between the two education methods at Week 12. Conclusion: This study indicates that many outpatient mothers should be given various individual approaches and prenatal breast-feeding education programs to provide them with opportunities for education to improve breast-feeding rates. However, there was no statistically significant difference in the breast-feeding rates at Week 12, which may be attributed not to education methods but to changes in external environmental factors. Further research is needed to address the changes.

Effect of Two-step Time-restricted Feeding on the Fattening Traits in Geese

  • The present study was conducted to determine whether the two-step time-restricted feeding improves the fattening traits of one-step time-restricted feeding in geese. Thirty-six 8-wk-old geese were allotted into one of three groups. Group R1 (the 1-step restricted feeding group) was allowed access to feed for 2 h in the morning from 8 wk to 14 wk of age. Group R2 (the 2-step restricted feeding group) was treated as Group R1, but was additionally fed for 2 h in the afternoon from 12 wk to 14 wk of age. Group C (the control group) was fed ad libitum from 8 wk to 14 wk of age. Feed intake and body weight (BW) were recorded daily and weekly, respectively. At 14 wk of age, the blood samples were collected to determine the fasting plasma levels of glucose, triacylglycerols and uric acid before sacrifice. The results showed that daily feed intake (DFI) was lower, feed efficiency (FE) was higher in both Groups R1 and R2 than in Group C, and daily gain (DG) in Group R2 was higher than in Group R1 during the whole experimental period (p<0.05). Group R1 exhibited lower abdominal and visceral fat weights in carcass than did Group C (p<0.05), and Group R2 was in intermediate. The fasting plasma glucose levels in Group C were higher, and triacylglycerol levels in Group R1 were higher, compared with the other groups (p<0.05). It is concluded that time-restricted feeding in the fattening period not only increases FE but reduces DFI, and the additional meal during the late fattening period improves the DG without the expense of FE in geese.

Perinatal Nutrition, Post-Hatch Holding Time and In Ovo Feeding (부화 전후 영양, 부화 후 유지시간, 그리고 In Ovo Feeding)

  • The most important times during the development of young chicks are the days immediately prior to hatching and the days immediately after hatching, known as the perinatal period. A sufficient supply of nutrients during the perinatal period is a crucial during the late stage of embryonic development and the starvation period of the young chicks. The delayed post-hatch holding time can restrict the development of the gastrointestinal tract, reduce final body weight, impair muscle development, and change immunological capacities. These symptoms are deleterious to the development of young chicks. Therefore, the post-hatch holding time and its influence on the fitness of young chicks are major concerns to the poultry industry. The in ovo feeding is a practical technology for perinatal nutrition to optimize poultry production and for attenuating the stress experienced by fasting young chicks. This study will discuss in ovo feeding and its effect on the development of the chick embryo, the establishment of a healthy microbiota, and the improving immune response.

Usefulness of Ad Lib Feeding for Hypertrophic Pyloric Stenosis (비후성 유문 협착증에서 수술 후 무제한 임의 식이법 (Ad Lib feeding)의 유용성)

  • Infantile hypertrophic pyloric stenosis (HPS) is a relatively common entity. A number of studies for the postoperative feeding schedule has been studied to allow for earlier hospital discharge and improve cost-effectiveness in the treatment of HPS. The purpose of this study was to compare 3 feeding-methods and to evaluate the usefulness of ad lib feeding for HPS. The authors retrospectively reviewed the records of 116 patients who underwent pyloromyotomy for HPS from 1995 to 2004. Three cases were excluded because of the duodenal perforation during pyloromyotomy. Three feeding-methods were defined as: Conventional feeding (>10 hours nothing by mouth and incremental feeding every 2 hours, C), Early feeding(for 4 to 8 hours nothing by mouth and incremental feeding every 2 hours, E), and Ad lib feeding (for 4 hours nothing by mouth and ad lib feeding, A). Time to normal feeing in C, E and A were $51{\pm}24$, $34{\pm}12$ and $24{\pm}6$ hours, respectively. Hospital-stay in C, E and A were $72{\pm}17$, $55{\pm}13$ and $43{\pm}12$ hours, respectively. There were statistically significant differences according to the method of feeding. Frequency of postoperative emesis in C, E and A were 38 %, 47 % and 53 %, but was not significant statistically. Ad lib feeding decreased time to normal feeding and hospital stay, and did not increase postoperative emesis. We conclude that ad lib feeding is recommended for patient with pyloromyotomy in HPS.
