  • Magnetic Resonance Imaging(MRI) has chemical shift phenomenon between fat and water, and the phenomenon has influence on structure enclosed by fat. Strong signals emitted from fat often generate false artefact, which reflects the importance of fat suppression techniques. There have been a number of researches on fat suppression techniques, but using fat suppression method alone in MRI can cause difficultproblems in diagnosis. This paper aims to study a fat suppression method by Chemical Shift Selective saturation(CHESS). This research describes the theoretical background and the experiment on water and fat phantom with MR instruments. In the experiment, CHESS pulse was designed by utilising Matlap program, and the pulse diagram was generated for the Pre-saturation process. The experiment using water and fat phantom was applied to C-spine, L-spine and Breast, and produced successful fat suppression results. This experiment has proved that the CHESSpulse fat suppression is a very helpful technique in diagnosing medical imaging. A Study on MR Imaging Method for The Patient with Inserting Shoulder Joint Suture Anchor (견관절 삽입술을 시행한 환자의 자기 공명 영상법에 관한 연구)

  • Metallic suture anchors are very useful and common fixation devices that are inserted into the target bone to sustain the tendon of a patient with musculus supraspinatus tendon ruptures. On the other hand, the presence of a metallic material prosthesis, such as a metal suture anchor, causes severe MR imaging artifacts, including field distortion, signal loss, and failure of fat suppression. The difference in magnetic susceptibility between metal and other organic materials causes magnetic field distortion surrounding the prosthesis. The resulting magnetic field inhomogeneity makes the images with a lower signal-to-noise ratio and distortion. For a patient with a suture anchor implanted, MR imaging is the golden standard for determining the postoperative prognosis, and a fat-saturation sequence is one of the imaging methods most affected by metal-induced artifacts. In this study, three fat-saturation sequences were compared. Artifact quantification and contrast comparison between the supraspinatus tendon and the surrounding muscle were presented. The images obtained using the STIR pulse sequence showed fewer susceptibility artifacts and better visibility in the supraspinatus tendon and the tissue area. Therefore, the STIR sequence is the most appropriate fat-saturation imaging method for patients with a metallic prosthesis.

  • There are several methods to achieve selective NMR image of differing chemical species with the three most popular methods of Dixon's, CHESS, and SECSI. A major problem common to all chemical shift imaging methods is the uniformity of the static magnetic field and distortions introduced when RF coils are loaded with a conducting specimen. Without magnetic field shimming, these methods cannot be used to acquire selectively image protons in fat and water which are separated by approximately 3.0ppm. Experiments with a phantom, with linewidths of 2.5 to 3.5ppm, were quantitatively evaluated for the three methods and a new chemical shift imaging method. In this study the new chemical shift imaging method (modified CHESS+SECSI technique) which included a selective saturation and refocusing pulse, was developed to determine the ratios of water and fat in different samples.

Magnetic Resonance Imaging Evaluation of the Prostate in Normal Dogs

  • The aims of this study were to describe the appearance and size of the normal canine prostate using magnetic resonance (MR) imaging and to calculate the apparent diffusion coefficient (ADC) values. MR images were obtained from seven intact male beagle dogs using a 1.5 T MR unit. The sequences included pre- and post-contrast T1- and T2-weighted imaging with and without fat saturation. The signal intensity of the prostate was compared with the adjacent musculature, fat, and urine in the urinary bladder. We recorded the mean prostatic length, width, and height and the length of the sixth lumbar vertebral body (L6). In addition, the prostatic length (rL), width (rW), and height (rH) ratios to L6 were calculated. Diffusion-weighted images of the prostate were obtained and ADC values were calculated. The prostate was bilobed and oval-shaped, homogenous on T1-weighted images, and heterogeneous with radiating lines on T2-weighted images. Post-contrast T1-weighted sequences showed contrast enhancement of the central and radiating striations. The prostatic capsule was clearly identified on post-contrast T1-weighted images with fat saturation. The ADC values were 1.72-2.04 × 10-3mm2/sec (mean, 1.88 × 10-3mm2/sec). Knowledge of the normal appearance of the prostate on MR images is essential to assess prostatic diseases in dogs.

A Study on Optimized MRI Fat-Saturation Technique for Brachial Plexus Patients : Focused on SPAIR and STIR Fat-Saturation (상완신경총 환자에서 최적의 자기공명영상 지방소거 기법에 관한 연구 : SPAIR, STIR 지방소거 중심으로)

  • The purpose of this study is to know optimized fat suppression techniques for brachial plexus compared with STIR and SPAIR T1, T2 techniques. A total of 30 normal volunteers without brachial plexus disease were studied on a 3.0 T MRI scanner. As an analytical method, SNR, CNR, 4-point grading scale were evaluated by using three pulse sequences. As a quantitative analysis, the SNR, CNR for SPAIR T1 technique provided high value in branchial plexus roots (03.07, -2.25), branchial plexus trunks(06.70, 36.31)(p<0.05). As a qualitative Analysis, The visibility for delineation of brachial plexus, fat suppression, artifact was significantly better on SPAIR T1(3.2, 3.6, 3.4) technique(p<0.05).

A Comparison of Anthropometry and Iron Status in Children Provided with and without Natinoal School Lunch Program (급식교와 비급식교 아동의 성장발달 및 철분영양상태 비교)

  • This study was designed to assess the iron nutritional status and growth development of children provided with and without the national school -lunch program(NLSP). The subjects consisted of 590 elementary school children (313 boys, 277 girls) in the 2nd, 4th and 6th grades provided with (n=390) and without (n=200) NSLP. anthropometric measurements were taken for body weight, height, triceps skinfold thickness and body fat percentage. Nutrient intakes were assessed by a modified 24 -hour recall method. Fastinig blood samples were obtained and analyzed for hemoglobin concentration, hematocrit, serum iron and total iron binding capacity. The results obtained are summarized as follows. No significant differences between children provided with and without NSLP were found in height and body weight, but triceps skinfold thickness and body fat(%) were significantly higher in children without NSLP than in those with NSLP. There was no significant difference in hemoglobin concentration and hematocrit between children provided with and without NSLP. However, serum iron concentration and transferrin saturation were higher in the children provided with NSLP(81.9$\mu\textrm{g}$/dl and 22.8%) than in the children without NSLP(73.1$\mu\textrm{g}$/dl and 20.9%). When comparing iron and vitamin C intakes , iron intake was significantly higher in children provided with school-lunch, but vitamin C intake was significantly higher in children provided without school-lunch. Percentages of iron -deficient anemia in underweight, normal and obese children when judged by total iron binding capacity were 14.9%, 12.5% and 25.8% respectively.

Field Map Estimation for Effective Fat Quantification at High Field MRI (고자장 자기공명영상에서 효율적인 지방 정량화를 위한 필드 맵 측정 기술)

  • The number of fatty liver patients is sharply growing due to the rapid increase in the incidence of metabolic syndrome, which can lead to diseases such as abdominal obesity, hypertension, diabetes, and hyperlipidemia. Early diagnosis requires examinations using magnetic resonance imaging (MRI), wherein quantitative analyses are implemented through a professional water-fat separation method in many cases, as the intensity values of the areas of interest and non-interest are considerably similar or the same. However, such separation method generates inaccurate results in high magnetic fields, where the inhomogeneity of the fields increases. To overcome the limits of such conventional fat quantification methods, this paper proposes a field map estimation method that is effective in high magnetic fields. This method generates field maps through echo images that are obtained using the existing IDEAL sequences, and considers the wrapping degree of the field maps. Then clustering is performed to separate calibration areas, the least square fits based on the region growing method schema of the separated calibration areas, and the histograms are adjusted to separate the water from the fats. In experiment results, our proposed method had a superior fat detection rate of an average of 86.4%, compared to the ideal method with an average of 61.5% and Yu's method with an average of 62.6%. In addition, it was confirmed that the proposed method had a more accurate water detection rate of 98.4% on the average than the 88.6% average of the fat saturation method.

Rare Imaging of Fat Embolism Seen on Computed Tomography in the Common Iliac Vein after Polytrauma

  • Fat embolism refers to the presence of fat droplets within the peripheral and lung microcirculation with or without clinical sequelae. However, early diagnosis of fat embolism is very difficult because the embolism usually does not show at the computed tomography as a large fat complex within vessels. Forty-eight-year-old male with pedestrian traffic accident ransferred from a local hospital by helicopter to the regional trauma center by two flight surgeons on board. At the rendezvous point, he had suffered with dyspnea without any airway obstruction sign with 90% of oxygen saturation from pulse oximetry with giving 15 L of oxygen by a reserve bag mask. The patient was intubated at the rendezvous point. The secondary survey of the patient revealed multiple pelvic bone fracture with sacrum fracture, right femur shaft fracture and right tibia head fracture. Abdominal computed tomography was performed in 191 minutes after the injury and fat embolism with Hounsfield unit of -86 in his right common iliac vein was identified. Here is a very rare case that mass of fat embolism was shown within common iliac vein detected in computed tomography. Early detection of the fat embolus and early stabilization of the fractures are essential to the prevention of sequelae such as cerebral fat embolism.

Energy Utilization of Growing Chicks in Various Nutritional Conditions

  • For the last two decades, energy utilization of growing chicks has been studied more and more. This paper focuses on the energy utilization estimated by the metabolizable energy (ME) values and the efficiency at which ME is used for growth of chicks under various nutritional environment. Degree of saturation of dietary fats is responsible for nitrogen-corrected apparent metabolizable energy (AMEn) of fats. The effect of dietary fat sources on heat production depends on the kind of unsaturated fatty acids as well as the degree of saturation. Medium chain triglyceride shows lower AME and net energy than long chain triglyceride. Phytase as feed additives increases the AME values of the diet along with improvement of the phosphorous utilization. Ostriches have higher ability to metabolize the energy of fiber-rich foodstuffs than fowls. Their higher ability seems to be associated with fermentation of fiber in the hindgut. Proportions of macronutrients in the diets have influenced not only the gain of body protein and energy but also the oxidative phosphorylation of the chicken liver. Essential amino acids deficiency reduces ME/GE (energy metabolizability) little, if any. Growing chicks respond to a deficiency of single essential amino acids with the reduction of energy retained as protein and increased energy retained as fat. Thus, energy retention is proportional to ME intake despite deficiency, and efficiency of ME utilization is not affected by deficiency of amino acids. Effect of oral administration of clenbuterol, a beta-adrenergic agonist, on the utilization of ME varies with the dose of the agents. Although the heat production related to eating behavior has been estimated less than 5% of ME, tube-feeding diets decreases HI by about 30%.
