Renin Activity, Habitual Ca, Na Intake and Hormonal Effect on Hypertension (혈압조절에 있어서 Renin 활성도의 차이와 Ca, Na 섭취습관 그리고 호르몬 간의 관련성)

  • Twenty two hypertensive and thirty normotensive in-patients were participated in this study to investigate the relationship between plasma renin activity and metabolism of Ca and Na, Prior to pharmacological treatments, renin activity, aldosterone and parathyroid hormone(PTH) levels were measured from the fasting blood samles. Twenty four hour urine samples were collected to analyze urinary levels of creatinine, Ca, Na and K. Habitual intake of Na and Ca were also measured for hypertensive and normotensive patients. Hypertensive subjects were classified into higher reinin hypertensive (HH), medium renin hypertensive(MH) and low renin hypertensive (LH) group according to their renin activities. PTH level of LH group was the highest among three hypertensive groups. It appeared that aldosterone levels of HH group were significantly higher than LH or MH groups(p<0.05). However there were no significan시 differences in aldosterone level between LH group and normotensive group. Habitual intake of Na and Ca were highest in LH group but lowest in HH group, however, they were not statistically different. Positive correlations of systolic blood pressure with PTH(r=0.2597) and aldosterone(r=0.26480existed(p<0.05). Urinary Ca level was positively correlated with urinary Na(r=0.5619), K(r=0.4533) and habitual Na intake(r=0.3253). Above results suggested the possible relationships among renin activity, habitual Ca intake and Na intake and suggested a further study on the interrelationship between the hormonal control of Ca and Na metabolism and blood pressure in hypertension.

Effects of Combined Exercise on Irisin, Body Composition and Glucose Metabolism in Obese Elderly Women with Type 2 Diabetes Mellitus (복합운동이 제2형 당뇨병 비만 여성노인의 Irisin, 신체조성 및 당 대사에 미치는 영향)

  • The purpose of this study was to investigate the effects of combined exercise on irisin, body composition and glucose metabolism in obese elderly women with type 2 diabetes mellitus. The subjects were thirty-six obese elderly women with type 2 diabetes volunteers, aged 65 to 85 years, composed of the combined exercise type 2 diabetes mellitus group (n=20) and non-exercise type 2 diabetes mellitus group (n=16). The 60 minute combined exercise program (outdoor walking exercise & elastic-band exercise) was performed 3 times per week for 12 weeks. Exercise intensity of outdoor walking exercise was performed as medium intensity (RPE 5~6) and elastic-band exercise was progressively increased every four weeks (1-4 weeks: OMNI-RES 3~4, 5-8 weeks: OMNI-RES 5~6, 9-12 weeks: OMNI-RES 7~8). The results of the study in the combined exercise type 2 diabetes mellitus group were as follows; Irisin and skeletal muscle mass had significantly increased (p<.001), percentage of body fat had significantly decreased (p<.001). Further, HbA1c (p=.020) and fasting glucose (p<.001) was significantly decreased, and HOMA-β was significantly increased (p<.001). Correlation results showed that change of irisin had a significant negative correlation between percentage of body fat mass (r=-.423, p=.010), HbA1c (r=-.351, p=.036) and fasting glucose (r=-.424, p=.010). Also, irisin changes showed a positive correlation with aerobic endurance (r=.355, p=.034) and HOMA-β (r=.411, p=.013). In conclusion, the practice of regular combined exercise was found to increase the level of irisin in elderly women with type 2 diabetes and have a positive effect on body composition changes. In addition, HbA1c, fasting glucose and insulin secretion was improved, which helped to regulate glucose metabolism. Walking exercise and elastic band exercise are recommended as effective exercise for the prevention and management of diabetes in obese elderly women with type 2 diabetes mellitus.

Effect of Water Temperature on Ammonia Excretion of Juvenile Dark-banded Rockfish Sebastes inermis (볼락 Sebastes inermis 치어의 암모니아 배설에 미치는 수온의 영향)

  • A study was carried out to investigate the effect of water temperature on daily pattern and rate of total ammonia nitrogen (TAN) excretion in juvenile dark-banded rockfish Sebastes inermis (mean body weight: $14.8{\pm}0.3g$) under fasting and feeding conditions. Fish were acclimated over 10 days under three different water temperatures (15, 20 and $25^{\circ}C$). After 72 hours of starvation, fasting TAN excretion was measured at each temperature. To investigate post-prandial TAN excretion, fish were hand-fed with a commercial diet containing 47.7% crude protein for 7 days, two times daily at 09:00 and 17:00 hr. Water was sampled from both the inlet and outlet of each chamber every 2 hrs over a 24 hr period. Both fasting and post-prandial TAN excretion increased with increase in water temperature (P<0.05). Mean fasting TAN excretion rates at 15, 20 and $25^{\circ}C$ were 8.1, 9.0 and 9.2 mg TAN kg $fish^{-1}h^{-1}$, respectively. The value of $15^{\circ}C$ was lower than those of 20 and $25^{\circ}C$ (P<0.05), but there was no significant difference between $20^{\circ}C$ and $25^{\circ}C$ (P>0.05). Mean post-prandial TAN excretion rates at 15, 20 and $25^{\circ}C$ were 20.1, 22.9 and 23.4 mg TAN kg $fish^{-1}h^{-1}$, respectively. A peak post-prandial TAN excretion rate occurred after 12 hrs from the first feeding at $15^{\circ}C$ (mean 28.7 mg TAN kg $fish^{-1}h^{-1}$), $20^{\circ}C$ (33.7 mg TAN kg $fish^{-1}h{-1}$) and $25^{\circ}C$ (36.8 mg TAN kg $fish^{-1}h{-1}$), respectively. The TAN loss for ingested nitrogen at $15^{\circ}C$ (36.2%) was lower than that of $20^{\circ}C$ (40.8%) and $25^{\circ}C$ (41.7%). Based on overall results, water temperature exerts a profound influence on the nitrogen metabolism of juvenile dark-banded rockfish.

  • Objective: This study was performed to investigate the effects of pioglitazone, an insulin sensitizing agent, on insulin resistance, ovarian function and intraovarian stromal blood flow in patients with polycystic ovarian syndrome (PCOS). Material and Methods: Thirty patients with PCOS, aged 18~34 years, were recruited. Criteria for diagnosis of PCOS were as defined in 2003 Rotterdam consensus. They were treated for 6 months with pioglitazone at a dose of 30 mg/day orally. The hormonal blood profile, fasting serum glucose levels, a glycemic response to 75 g oral glucose tolerance test (OGTT), and an ovarian stromal artery (OSA) blood flow were assessed at baseline and after 6 months of treatment. Results: Eighteen (60.0%) of 30 patients treated with pioglitazone demonstrated a spontaneous ovulation After pioglitazone treatment, fasting insulin concentrations, serum glucose levels after 75 g OGTT significantly decreased (p=0.001, p=0.04, respectively), and fasting glucose to insulin (G/I) ratio significantly increased (p<0.001). The pioglitazone treatment induced a significant reduction in serum LH, testosterone (T) and free T levels (p<0.001, p=0.02, p=0.002, respectively). The resistance index (RI) values of OSA significantly increased after treatment (p<0.001). In analyzing pioglitazone-treated patients according to their body mass index (BMI), nonobese group as well as obese group showed a significant improvement in fasting G/I ratio (p<0.01). The pioglitazone treatment induced a significant reduction in serum LH and free T levels in nonobese group (p<0.001, p<0.05, respectively) as well as obese group (p=0.001, p<0.05, respectively). The RI values of OSA significantly increased in both nonobese and obese groups after pioglitazone treatment (p<0.001, p=0.003, respectively). Conclusions: Pioglitazone could ameliorate the glycoinsulinemic metabolism, and this beneficial effects of this drug could improve the endocrine-reproductive condition associated with the decrease of ovarian stromal artery blood flow, in both nonobese and obese patients with PCOS.

The Plasma and Urinary Carnitine System in Korean Diabetic Patients

  • The goal of this study was to investigate abnotmalities in camitine metabolism present by determining blood camitine and lipid concentrations in Korean diabetic patients. The study subjects included 108 Korean diabetic patients (64 males and 44 females) who were hospitalized in Chonbuk National University Hospital and 27 subjects were also hospitalized as non-diabetic controls (10 males and 17 females). Glucose, total cholesterol, triglyceride (TG) and HDL-cholesterol in plasma were enzymatically assayed and insulin was measured by immunoradiometric assay. Nonesterified camitine (NEC), acid-soluble acylcarnitine (ASAC), and acid-insoluble acylcarnitine (AIAC) were determined by a modified radioisotopic method Glucose and insulin levels were significantly elevated in diabetic patients compared with controls. Total cholesterol was elevated in female but not male diabetic patients and triglycerides were elevated both in male and female diabetics. Plasma and urinary total carnitine (TCNE) were significantly elevated in diabetics as compared with normal controls. In male diabetics, NEC concentrations were significantly elevated above controls, but not in female subjects. Plasma NEC and TCNE concentrations were significantly increased in male diabetics, but significantly decreased in female diabetics. All urinary carnitine concentrations were significantly increased in diabetics as compared with controls. Urinary NEC concentrations were four times higher in male diabetics and three times higher in female diabetics than in controls. The ratios of serum and urinary acylcarnitine/NEC were also significantly higher in diabetics than in controls. This study suggested that there was a remarkable abnormality in lipid and carnitine metabolism in Korean diabetic patients, and the further study on carnitine metabolism and the effects of carnitine supplementation for Korean diabetic patients are needed.

Differential Effects of Cod Proteins and Tuna Proteins on Serum and Liver Lipid Profiles in Rats Fed Non-Cholesterol- and Cholesterol-Containing Diets

  • Fish muscles are classified into white and red muscles, and the chemical composition of the two fish muscles have many differences. Few reports have assessed the health-promoting functions of white fish muscle proteins (WFP) and red fish muscle proteins (RFP). We therefore evaluated the mechanisms underlying the alteration of lipid profiles and cholesterol metabolism following the intake of WFP prepared from cod and RFP prepared from light muscles of tuna. Male Wistar rats were divided into six dietary groups: casein (23%), WFP (23%), and RFP (23%), with or without 0.5% cholesterol and 0.1% sodium cholate. Compared to the WFP-containing diet, the RFP-containing diet supplemented with cholesterol and sodium cholate significantly increased serum and liver cholesterol contents. However, in the RFP groups, an alteration in cholesterol metabolism including an increased tendency to excrete fecal sterols and hepatic cholesterol $7{\alpha}$-hydroxylase was related to the reduction of hepatic cholesterol contents. This phenomenon might be related to the tendency of an increased food intake in RFP-containing diets. These results highlight the differential effects of WFP and RFP on serum and liver lipid profiles of Wistar rats fed non-cholesterol- or cholesterol-containing diets under no fasting condition.

Relationship between Nutrient Intake and Biochemical Markers of Bone Metabolism in Korean Postmenopausal Women (폐경 후 여성의 영양소섭취수준에 따른 골대사 지표물질의 관련성 분석)

  • To delineate the relationship between the nutrient intake from diet and the serum biochemical markers of bone metabolism, 56 postmenopausal women of 50 to 77 years of age were recruited. The biochemical markers including osteocalcin, calcium, phosphorus, estradiol and free testosterone were measured in fasting blood. Bone mineral density(BMD) was measured also by dual energy X-ray absorptiometry, and the nutrient intake of earth individual subject was estimated by 24-hour recall of 3 days. The age of the subjects was 64.8 $\pm$ 7.7 years, and the BMDs of the subject were 0.86 $\pm$ 0.26g/$cm^2$(Lumbar spine), 0.60 $\pm$ 0.10g/$cm^2$ (Femoral neck), 0.49 $\pm$ 0.10g/$cm^2$(Trochanter), and 0.41 $\pm$ 0.14g/$cm^2$(Ward's triangle). There were no significant differences among age and nutrient intake level groups due to the small sample size. The biochemical markers showed certain degree of relationship with nutrient intake levels. The results were compared among 3 groups with different nutrient intake level classified by the percentage of Recommended Daily Allowances(RDA) for Koreans as follows low < 75% RDA, 75% RDA $\leq$ adequate < 125% RDA, high $\geq$ 125% RDA. The low energy and low riboflavin groups showed significantly higher serum osteocalcin levels than those of the high intake groups(p < 0.05). On the other hand, there was a trend for serum Ca level to be higher with high nutrient intake. In this case, protein and thiamin were the only nutrients that reached a statistical significance(p < 0.05). And the groups with low intake for protein and Ca showed significantly lower serum free testosterone levels than that of other intake groups(p < 0.05). This study suggests an important role of nutrient intake levels on blood biochemical markers of bone metabolism.

Characterization of Plasma Carnitine Level in Obese Adolescent Korean Women

  • Carnitine is known to be involved in lipid metabolism and affects body composition as well as energy metabolism of the whole body. Improvement of obesity by L-carnitine supplement suggests that obesity can be related with the abnormality of carnitine metabolism and therefore, plasma carnitine level in normal and obesity groups was investigated. For the characterization of plasma carnitine level in obese people, 60 plasma samples collected from Korean women subjects were analyzed using LC/MS and plasma fatty acid level was also determined using GC/MS. Additionally, several clinical chemical parameters including fasting glucose, cholesterol, AST, and ALT level were measured. All the data obtained were combined and pattern recognition analysis was carried out with the dataset. Obese group showed a different metabolic pattern compared with normal group. Plasma acylcarnitine level of the obese group was found to be $11.7{\mu}g/ml$, which was higher than that of normal group ($8.0{\mu}g/ml$). Statistically significant differences in plasma fatty acid level were not observed between the two groups. Other clinical parameters for the obese group were within normal ranges but AST and ALT levels were slightly elevated compared to normal group. The obese group showed elevated plasma acylcarnitine level.

Association of Thigh Muscle Mass with Insulin Resistance and Incident Type 2 Diabetes Mellitus in Japanese Americans

  • Background: Skeletal muscle plays a major role in glucose metabolism. We investigated the association between thigh muscle mass, insulin resistance, and incident type 2 diabetes mellitus (T2DM) risk. In addition, we examined the role of body mass index (BMI) as a potential effect modifier in this association. Methods: This prospective study included 399 Japanese Americans without diabetes (mean age 51.6 years) who at baseline had an estimation of thigh muscle mass by computed tomography and at baseline and after 10 years of follow-up a 75-g oral glucose tolerance test and determination of homeostasis model assessment of insulin resistance (HOMA-IR). We fit regression models to examine the association between thigh muscle area and incidence of T2DM and change in HOMA-IR, both measured over 10 years. Results: Thigh muscle area was inversely associated with future HOMA-IR after adjustment for age, sex, BMI, HOMA-IR, fasting plasma glucose, total abdominal fat area, and thigh subcutaneous fat area at baseline (P=0.033). The 10-year cumulative incidence of T2DM was 22.1%. A statistically significant interaction between thigh muscle area and BMI was observed, i.e., greater thigh muscle area was associated with lower risk of incident T2DM for subjects at lower levels of BMI, but this association diminished at higher BMI levels. Conclusion: Thigh muscle mass area was inversely associated with future insulin resistance. Greater thigh muscle area predicts a lower risk of incident T2DM for leaner Japanese Americans.

Chronic Alcohol Consumption Results in Greater Damage to the Pancreas Than to the Liver in the Rats

  • Alcohol consumption increases the risk of type 2 diabetes. However, its effects on prediabetes or early diabetes have not been studied. We investigated endoplasmic reticulum (ER) stress in the pancreas and liver resulting from chronic alcohol consumption in the prediabetes and early stages of diabetes. We separated Otsuka Long-Evans Tokushima Fatty (OLETF) rats, a type-2 diabetic animal model, into two groups based on diabetic stage: prediabetes and early diabetes were defined as occurrence between the ages of 11 to 16 weeks and 17 to 22 weeks, respectively. The experimental group received an ethanol-containing liquid diet for 6 weeks. An intraperitoneal glucose tolerance test was conducted after 16 and 22 weeks for the prediabetic and early diabetes groups, respectively. There were no significant differences in body weight between the control and ethanol groups. Fasting and 120-min glucose levels were lower and higher, respectively, in the ethanol group than in the control group. In prediabetes rats, alcohol induced significant expression of ER stress markers in the pancreas; however, alcohol did not affect the liver. In early diabetes rats, alcohol significantly increased most ER stress-marker levels in both the pancreas and liver. These results indicate that chronic alcohol consumption increased the risk of diabetes in prediabetic and early diabetic OLETF rats; the pancreas was more susceptible to damage than was the liver in the early diabetic stages, and the adaptive and proapoptotic pathway of ER stress may play key roles in the development and progression of diabetes affected by chronic alcohol ingestion.
