Purpose: Involvement of families in rounds is one strategy to implement patient- and family-centered care to help families get clear information about their child, and be actively involved in decision making. The purpose of this paper was to identify the major concepts of family-centered rounds for hospitalized children. Methods: We searched five electronic databases for relevant articles and used Whittemore and Knafl's integrative review methods to synthesize the literature. Articles published between June 2003 and January 2016 were reviewed and through full text screening 24 peer-reviewed articles were found that met the selection criteria for this review. Results: Through in-depth discussion and investigation of the relevant literature, four overarching components emerged: (a) cognition of parents and medical staff, (b) effective communication, (c) collaboration of family and medical staff, (d) coaching of medical staff. Conclusion: For successful family-centered rounds positive cognition is important. Appropriate communication skills and consideration of multi-cultural family can lead to effective communication. Offering consistent and transparent information is important for collaboration between family and medical staff. Prior education on family-centered rounds is also important. Four major components have been identified as basic standards for implementing family-centered rounds for hospitalized children.
Purpose:The purpose of this study was to examine the effects of video-centered information among family members intensive care unit (ICU). Methods: A quasi-experimental, nonequivalent control group, pretest-posttest design was used. Participants (n=86) were family members who were the main caregivers for the patient in ICU. An experimental group (n=43) watched a video while the control group (n=43) was provided a leaflet. Levels of environmental stress, anxiety and nursing need satisfaction were measured by questionnaires before and after the interventions. Data were analyzed with ${\chi}^2$ test, paired t-test, independent t-test, Fisher's exact test and ANCOVA. Results: There were no differences in environmental stress (F=1.88, $p$=.065), and anxiety (t=0.37, $p$=.711) between 2 groups, but there was a significant difference in nursing need satisfaction (t=3.01, $p$=.004). Conclusion: Providing video-centered information would be an effective nursing intervention by improving nursing need satisfaction among family, the main caregivers members of patients in ICU.
Due to the development of technology and medical care following the 4th industrial revolution, the medical paradigm is shifting towards patient-centered medical services. Based on the development of smart home technology, the residential environment is changing into a residential space that cares for and heals the lifestyles and the healthcare of families. As lifestyle changes, the concept of supporting smart home care based on the residential environment is making it possible to build a smart home IoT service design with enhanced accessibility and convenience for medical appointments and well-being lifestyle care. This paper is a study on user-centered health care smart home IoT service design suitable for family members based on the health care, beauty care, exercise care, and customized diet care beyond the conventional concept of health care monitoring. Based on the analysis, this paper proposes a personal care coordinate smart home service design in a human-centered wellness clinic care smart home service design environment. Human-centered wellness clinic smart home IoT service design is meaningful in presenting a vision for research on smart home service design that links hospital-linked and care-linked service industries, which should be considered from the smart home construction planning stage.
본 연구에서는 소비자 중심 의료 관점에 기초해 미국·캐나다 재외국민들을 대상으로 진행된 의료 만족도 비교 조사를 바탕으로 가정의 도입과 관련된 탐색적 연구를 수행하였다. '16년 -'18년 사이에 설문조사지 배포 및 회수 방식으로 수집된 493개의 실증자료에 기초한 통계적 분석 결과 본 연구의 주요 결과는 다음과 같다. 첫째, 미국·캐나다 가정의에 대한 만족도가 한국의 1차 의료 의사에 비해 높다는 증거를 발견할 수 없었다. 둘째, 치료기술 및 수준, 진료예약 신속성 등에서 한국 1 차 의료 의사 만족도가 높았으며 가정의 제도에 비해 약점으로 언급되는 충분한 상담 및 설명 측면에서도 가정의 제도가 더 우수하다는 증거를 발견할 수 없었다. 셋째, 미국 재외국민은 캐나다 재외국민에 비해 가정의 제도에 대한 만족도가 더 낮게 나타났다. 넷째, 미국·캐나다 재외국민은 가정의를 거쳐 병원에 가도록 강제하는 제도에 대한 부정적인 평가가 높았고 질병 발생 시 가정의를 통하지 않고 병원을 직접 방문하는 것을 선호하였다. 결론적으로 미국·캐나다 가정의 만족도가 한국 1차 의사에 비해 높다는 충분한 증거가 없으므로 서구식 가정의 제도 도입 전 이 제도로 인한 효익과 비용에 대한 보다 심층적인 추가적인 분석이 수행될 필요가 있다.
본 연구의 목적은 인간중심돌봄에 대한 요양병원 간호사의 인식을 파악하는 것이다. 연구대상은 전라도와 경상도 내 위치한 요양병원 4곳에서 근무하고 있는 간호사 21명이며, 각 병원에 1개씩 포커스 그룹을 구성하여 총 4개의 포커스 그룹에서 면담을 수행하였다. 자료 수집 기간은 2018년 7월 19일부터 7월 30일까지였고 질적 주제분석을 이용해 자료를 분석하였다. 연구결과 인간중심돌봄에 대한 요양병원 간호사의 인식은 6개의 주제와 11개의 하위주제로 도출되었으며, 6개의 주제는 "개별 요구 존중하기", "끝까지 동행하기", "남은 꿈 지지하기", "가족과 동반자되기", "집처럼 생활하도록 돕기", "문화 변화시키기"로 나타났다. 간호사가 인식하는 인간중심돌봄은 노인의 선호도에 따라 개별화된 간호를 수행하고 다양한 활동프로그램을 통해 노인이 삶의 가치와 의미를 발견할 수 있도록 도와주는 것이었다. 또한 노인뿐만 아니라 이들의 가족 구성원과 협력적인 관계를 유지하고 의사결정을 공유하며 노인의 권리와 자율성이 존중받을 수 있는 물리적 환경 및 조직 문화를 형성하는 것을 인간중심돌봄이라고 인식하고 있었다. 본 연구결과를 토대로 향후 인간중심돌봄 문화 형성에 필요한 다각적 요구를 확인하고 인간중심돌봄을 기반으로 한 간호중재 프로그램을 개발하여 그 효과를 검증할 필요가 있다.
The purpose of this study was to investigate the caregiving and adaptation in families who awarded on filial piety. Using the qualitative methods-in depth interview genogram ecomap participation observation- 8 families were analyzed The major findings can be summarized as follows (1) These families showed very high family solidarity with strong collectivity. Their family role was interchangable and their familiarity was passed down from generation to generation. (2) Family members showed similar coping patterns on the basis of their own adaptationl Their coping was inclined toward acceptance especially religioous rather than emotional-focused ad hardly avoidance coping. (4) Although their economic emotional and service cares were family-centered with helps from their extended family household equipment for elder care was not prepared. (5) These families preferred to maintain this condition rather than to be well adapted. It was concluded that to help failed elder's family care iving social support program should be complemented in family life enrichment program for their children medical care program equipment rental program for elder in social network religion program and so on, Social norms forparent-children relation has be changed to more flexible care pattern. Also more social-emotional support has to be given to these families.
Purpose: This ethnograpy was done to explore patterns of health care behavior in patients with chronic health problems. Methods: The participants were 15 patients with chronic hepatitis B and 2 family members. Among the patients 4 had progressed to liver cirrhosis and liver cancer. Data were collected from iterative fieldwork in a department of internal medicine of I hospital. Data were analyzed using text analysis and taxonomic methods. Results: Illness and disease, relationship between health care givers and clients, and communication patterns between health professions and clients were discussed as the context of health care behavior. Health care behavior of the participants was categorized by its focus: every day work centered, body centered, organ centered, and pathology centered. Conclusion: Participants' health care behavior was guided by folk health concept and constructed in the sociocultural context. Folk etiology, pathology, and interpretation of one's symptoms were influencing factors in illness behavior. These findings must be a cornerstone of culture specific care for the chronic diseases.
Purpose : The aim of this study was to explore the attributes, antecedents, and consequences of patient-centered care (PCC) for older adults with multimorbidity in acute care hospitals. Methods : The concept analysis performed by Walker and Avant was used to analyze PCC. Fifteen studies from the literature related to PCC appear in systematic literature reviews in the fields of theology, medicine, psychology, and nursing. Results : PCC in acute care hospitals was defined according to the five attributes of 'maintaining patient autonomy', 'empowering self-care', 'individualized and relationship-based care', 'shared decision-making', and 'creating a homelike environment'. Antecedents of PCC were found to be a respect for patients' preferences, qualifications of the nursing staff, care coordination and integration, and organizational support. Consequences of effective PCC were a functional status; health-related quality of life; satisfaction with care, mortality, and medical costs from the perspective of the patient and family; and quality of care and therapeutic relationships from nurses' viewpoints. Conclusion : PCC as defined by the results of this study will contribute to the foundation of institutionalization and the creation of a safe and healthy acute care hospital culture focused on patients' preferences and values.
Purpose: This study aimed to identify the influence of marital conflict and family function on fatigue in mothers of young children hospitalized for acute diseases. Method: Via a cross-sectional design utilizing structured questionnaires, fatigue, marital conflict, and family function data of 120 South Korea mothers of such hospitalized children were collected and surveyed. The data were analyzed via t-test, ANOVA, Pearson's correlation coefficients, and multiple regression using SPSS Statistics. Results: Fatigue level was found to positively correlate with marital conflict and negatively correlate with family function. Overall, marital conflict, was significant predictor of maternal fatigue, explaining 33.0% of the model, with physical fatigue level being the highest type of fatigue. Conclusion: Therefore, a family-centered care program for families of hospitalized children needs to be developed. Comprehensive family nursing interventions that reduce fatigue levels and help reinforce marital relationships in families of hospitalized children are essential.
Although a family-centered approach to health care for developmentally disabled children has been advocated, existing systems of care have not adequately addressed the support needs of the family system and the essential role that parents play in the daily care of these children. The overall purpose of this research is to examine family system adaptation to the care of a developmetally disabled child using the Resiliency Model of Family Stress, Adjustement, and Adaptation Framework. Relationships among family system demands (stressors, strains, transitions, child's illness factors) and family system strengths and capabilities(resource of social support) were examined to determine their separate and combined contribution to explanining the variability in family system outcomes (family system coping ). The subject for this study was 46 families who have a child with developmental disabilities(mental retardation and / or autism) from three special educational programs in Seoul, Korea. Results from correlation and hierachial regression analysis revealed that perceived social support operated as a resiliency factor between family stress and coping. Child and family characteristics appeared to be important predictors of perceived social support and coping. In summary, there is evidence that the resource of social support as a family strength and capability was found to improve the family coping. These findings also must be viewed within the context that sample of families of children with disabilities was relatively small and eligible families from support group of special educational program.
