Study on Influencing Factors on Subjective Quality of Life and Family Burden of Caretaking Family Members who have a Patient with a Psychiatric Disorder - Comparison between Schizophrenia and Alcohol Dependence - (정신과 환자 주 보호자의 삶의 질과 부담 영향 요인 - 정신분열병과 알콜 의존 환자의 보호자 비교 -)

  • Objectives: The aim of this study was to investigate the quality of life of caretaking family members who have a patient with psychiatric disorders. The results will be served as a basic data of ameliorating the quality of life of caregivers. Methods: 78 caretaking family members who have a patient with schizophrenia and 54 caretaking family members who have a patient with alcohol dependence, a total of 132 persons completed the questionnaire, and analyzed. The Korean version of the SmithKline Beecham Quality of Life Scale and the Family Burden Scale were applied. Results: There was no statistically significant correlation between burden and sex, age, income, and duration of living with patients before onset. The male caregiver showed higher quality of life than that of female. It showed statistically significant correlation between age and factor physical well-being and factor activity. 41% of variance of quality of life of caregivers were explained by the stress response, burden, and overall merits of the field of psychiatry, and the tension had the most explanatory power. Conclusion: The chronic illness may give a burden on caregivers, and that decrease the quality of life of caregivers. The longer duration of illness of patients, the lower quality of life of caregivers on competence factor. Therefore, the authors recommend the therapeutic modality must be offered to the caregivers who may experience the stress and burden.

Burden and Needs of the Family Members of the Intensive Care Unit Patients (중환자실 입원환자 가족의 부담감과 요구도)

  • This study set out to investigate the burden and needs of family members of ICU(Intensive Care Unit) patients facing a crisis situation of a family member being hospitalized in ICU and check their connections. Data were collected from May to October 2013. The subjects include 93 main family caregivers of patients hospitalized in ICU at four general hospitals in the B area for three days or longer. Collected data were put to t-test, ANOVA, Scheff$\acute{e}$ test, and Pearson's correlation coefficient analysis with the SPSS WIN 20.0 program. As a result, the family members of ICU patients scored mean 3.56 points(out of five) on burden and mean 3.58 points(out of four) on needs, recording a moderate or higher level in both the areas. There was significant difference(F=3.463, p=.036) in burden according to the general characteristics with the number of days in the hospital. There was significant positive correlation(r=.332, p<.001) between their burden and needs. Those findings indicate that an active nursing intervention to reduce the burden of the families of ICU patients in a crisis situation and check their needs will be able to mitigate the family crisis and further have positive influences on the recovery of ICU patients.

Factors Associated with Care Burden among Family Caregivers of Terminally Ill Cancer Patients (말기암환자 가족 간병인의 간병 부담과 관련된 요인)

  • Purpose: It is important to alleviate care burden for terminal cancer patients and their families. This study investigated the factors associated with care burden among family caregivers (FCs) of terminally ill cancer patients. Methods: We analyzed data from 289 FCs of terminal cancer patients who were admitted to palliative care units of seven medical centers in Korea. Care burden was assessed using the Korean version of Caregiver Reaction Assessment (CRA) scale which comprises five domains. A multivariate logistic regression model with stepwise variable selection was used to identify factors associated with care burden. Results: Diverse associating factors were identified in each CRA domain. Emotional factors had broad influence on care burden. FCs with emotional distress were more likely to experience changes to their daily routine (adjusted odds ratio (aOR), 2.54; 95% confidence interval (CI), 1.29~5.02), lack of family support (aOR, 2.27; 95% CI, 1.04~4.97) and health issues (aOR, 5.44; 2.50~11.88). Family functionality clearly reflected a lack of support, and severe family dysfunction was linked to financial issues as well. FCs without religion or comorbid conditions felt more burdened. The caregiving duration and daily caregiving hours significantly predicted FCs' lifestyle changes and physical burden. FCs who were employed, had weak social support or could not visit frequently, had a low self-esteem. Conclusion: This study indicates that it is helpful to understand FCs' emotional status and family functions to assess their care burden. Thus, efforts are needed to lessen their financial burden through social support systems.

Study of the Positive and Negative Caregiving Experiences in the family members who care for the psychiatric mentally ill relatives (정신장애인 가족의 긍정적, 부정적 돌봄의 경험에 관한 연구)

  • The caregiving experiences of 100 family menbers of outpatients with schizophrenia and schizoaffective disorders were investigated for the presence of positive(positive family-patient relationship. patient' contribution to the family) and negative caregiving experience(objective and subjective burden) and their predictors. This study attempts to make the analysis of caregiving experience more useful by expanding the focus to incoporate these positive aspects of the experience of family caregiver. Objective burden consists of two elements: 'disruption of family life', 'care'(amount of caregiving related to activity of daily living). Subjective burden is defined as emotional reactions to the care giving and it comprised of 6 emotional subdimensions such as 'stigma', 'grief'. 'worry', 'pity', 'fear', 'despair'. Also we investigate the severity of patients' disturbing behaviors into two categories, positive and negative disturbing behaviors and patient' contribution to the family as a predictors of positive and negative caregiving experiences. This study use Pearson's correlation coefficient, Hierardhical regressions in the SAS Program. The results are as follows: 1. Respondents reported moderate level of objective burden 'disruption of family life' (mean = 2.48, range = 1-4), and 'care' (mean=2.54, range = 1-4), and slightly high level of total subjective burden(mean = 2.19, range = 1-4). Mean scores for the measure of the severity of behavioral disturbance indicated that the caregiver experienced negative disturbing behaviors around almost 'somtimes'(mean=2.28, range = 1-4), and positive disturbing behaviors 'almost not frequent'(mean=2.78. range=1-4). So they reported that they perceived patient's negative disturbing behaviors more than positive disturbing behaviors. Mean scores for the measure of the patient' contributions (mean = 1.99. range = 1-4) indicated that caregivers experienced these contributions a little. It means that there should be a positive aspect of possibilities of patient' family roles that can be developed in the daily life. Mean scores for the measure of the positive family-patient relationship indicated that caregivers experienced moderate level of positive family-patient relationship(mean=2.52, range = 1-4). 2. Hierardhical regression analysis 1) Hierardhical regression of 'disruption of family life' showed that the interaction between positive disturbing behaviors and patient' contributions (B = .20. p = .022) and caregiver's educational level(B=.06. p=.000) were 'significant and Hierardhical regression of 'care' showed that 'negative disturbing behaviors'(B= .35. p= .007). 'patient' contributions'(B= .28, p= .019). 'family income'(B=-.l1. p=.096) were significant. 2) Hierardhical regression of 'total subjective burden', 'stigma', 'grief', 'worry', 'pity'. 'fear', 'dispair' showed that 'positive disturbing behaviors'(B=.51. p=.000). 'negative disturbing behaviors' (B = .17, p = .026), 'caregiver's educational level'(B = .03. p=.036), 'family income'(B=.08. p=.041) were significant predictors of 'total subjective burden': 'positive disturbing behaviors'(B=.32. p=.066). 'negative disturbing behaviors'(B=.24, p=.096) 'durations of illness'(B= .03. p= .079) were significant predictors of 'stigma' 'negative disturbing behaviors'(B=.28. p=.005). 'patient sex'(B=-.32. p=.022). 'positive disturbing behaviors'(B=.28. p=.020), 'patient age'(B=.02. p=.010), 'caregiver age'(B=-01, p= .002) were significant predictors of 'grief' 'negative disturbing behaviors'(B= .28, p= .005). 'patient sex'(B= -.32. p=.039), 'caregiver age'(B=-.02, p= .023). 'caregiver's educational level'(B= .04, p = .044) were significant predictors of 'worry' 'patient sex'(B=-.46. p=.005). Role and Role Burden of Caregivers with Hospitalized Children (입원아동 보호자의 역할과 역할부담감)

  • Purpose: This study was conducted to identify the role and role burden of caregivers with hospitalized children. Methods: The participants included 149 main caregivers from two pediatric hospitals whose children have been hospitalized for at least 2 days at children's hospital. The data were collected from August 1 to september 30, 2013 using self-report questionnaires of checklist type for caregivers' role and Likert scale for role burden. Results: Role for hospitalized children showed that personal hygiene was the highest, and consultation with nurses and doctors was the lowest in total frequency per day. Of the role for the caregivers themselves and family, personal hygiene was the highest, and home management was the lowest. The degree of role burden perceived by hospitalized children's caregivers was the slightly high. Of the categories, emotional burden was the highest and dependent burden was the lowest. There were differences by caregivers' age, children's age, and children's health status. Conclusion: The caregivers performed various roles, especially more frequently on personal hygiene and nutrition for their hospitalized children and themselves and family, and had slightly high role burden. These results may contribute to develop nursing interventions that can help adapt on hospitalization of children and their caregivers.

Caregiver Burden and Health-related Quality of Life Among Stroke Caregivers (뇌졸중 환자가족의 가족부담감과 건강관련 삶의 질)

  • Purpose: This study was performed to examine the relationship between caregiver burden and health-related quality of life (HRQOL) in family caregivers of stroke patients in the community. Methods: Subjects were 33 stroke patients who had a experience of hospitalization during last 1 year and their family. Data were obtained from personal interview by nursing students in October, 2003. We assessed the functional status of stroke patients, caregiver burden and the HRQOL of caregivers using SF-8. Results: A higher subjective burden was related to dependent functional status of patients than independent(P<.01). The independent cognitive function of stroke patients and good health of caregivers were related to high HRQOL of caregivers. PCS and MCS of HRQOL were negatively correlated to subjective burden, -.34(p<.01) and -.37(p<.05). Conclusion: We founded functional status of stroke patients. caregiver burden and HRQOL of caregivers were related to each other. To improve the HRQOL of caregiver, decreasing burden of caregivers and the functional status improvement of stroke patients must be considered.

A Prediction Model for Unmet Needs of Elders with Dementia and Caregiving Experiences of Family Caregivers (재가치매 환자의 미충족요구와 가족부양자의 돌봄경험 예측모형)

  • Purpose: The purposes of this study were to develop and test a prediction model for caregiving experiences including caregiving satisfaction and burden in dementia family caregivers. Methods: The stress process model and a two factor model were used as the conceptual frameworks. Secondary data analysis was done with 320 family caregivers who were selected from the Seoul Dementia Management Survey (2014) data set. In the hypothesis model, the exogenous variable was patient symptomatology which included cognitive impairment, behavioral problems, dependency in activity of daily living and in instrumental activity of daily living. Endogenous variables were caregiver's perception of dementia patient's unmet needs, caregiving satisfaction and caregiving burden. Data were analysed using SPSS/WINdows and AMOS program. Results: Caregiving burden was explained by patient symptomatology and caregiving satisfaction indicating significant direct effects and significant indirect effect from unmet needs. The proposed model explained 37.8% of the variance. Caregiving satisfaction was explained by patient symptomatology and unmet needs. Mediating effect of unmet needs was significant in the relationship between patient symptomatology and caregiving satisfaction. Conclusion: Results indicate that interventions focusing on relieving caregiving burden and enhancing caregiver satisfaction should be provided to caregivers with high levels of dementia patients' unmet needs and low level of caregiving satisfaction.

Study on Family Caregiving Burden Scale of Dementia-Korea(FCBSD-K) (치매환자 가족부담감의 한국형 도구개발)

  • The purpose of this study was to develop and validate the scale to measure dementia patient's caregiver burden of Korea. In the first phase of the study, 15 caregivers of dementia patients were interviewed to provide narrative data from which items were developed. Initially 65 items were generated from the interview data of 15 caregivers. Content validity was judged by two separate panels of experts with 27 professionals and 30 family caregivers. These items were analyzed through the Index of Content Validity and 33 items were selected which met .80 or more of the CVI. This preliminary FCBSD-K was tested with 207 adult caregivers for reliability and construct validity including item analysis and orthogonal(Varimax) factor analysis. Eight items were deleted because of high or low item-item correlation. The result of the second factor analysis produced six factors that coincided with the conceptual framework posed for the scale developed. The six factors were labeled as 'physio social factor' 'emotional factor' 'family cultural factor' 'role obligation' 'guilt feeling' and 'financial & supportive system factor'. The alpha coefficient relating to internal consistency was .9264 for reliability. In conclusion, cultural factor is related to dementia patient's caregiver burden and FCBSD-K was useful in assessing the dementia patient's caregiver burden in Korea.

Factors Affecting Perceived Financial Burden of Medical Expenditures (건강보험 입원환자의 주관적 의료비 부담에 영향을 미치는 요인)

  • Purpose: This study was done to investigate factors affecting perceived financial burden of medical expenditures. Method: The participants were 2,024 inpatients who were enrolled in a survey on the benefit coverage rate of the National Health Insurance in 2006. The collected data were analyzed using t-test, ANOVA-test, Mann-Whitney-test, Kruskal-Wallis-test, Chi-square test and logistic regression. Results: The crucial factors for perceived financial burden were age, job, equivalence scale, ratio of annual family income vs medical expenditure, and private health insurance. Perceived financial burden was higher for people who were older, who were unemployed, whose medical expenditures were high compared to annual family income, whose index of family equalization was low and for those who had no private health insurance. Conclusion: The results of the study indicate a demand for system reform that will enable management of no-pay hospital bills in the National Health Insurance to decrease the medical expense of people in the low-income bracket.

Relationship between the Patients' Activities Daily Living and Family Burden, Social Support under (가정 간호 환자의 일상생활 수행정도와 가족의 부담감 및 사회적 지지와의 관계)

  • The purpose of this study is to clarify the correlation between the degree of patients daily activities and that of a sense and social support of families. This study covers families of 252 patients under home health nursing care at 6 University Hospitals during the period of March to May. 1999. The data were collected by using three different questionnaires. Data were analysed by utilizing SAS program such as frequency. average. percentage. t-test. ANOVA. Scheffe test and Pearson Correlation Coefficient. The results were as follows: 1. The average score of patients' daily living activity was $2.36\pm0.67$. $97.2\%$ patients turned out to be. in part. dependent on their family's care. $66.5\%$ of the patients were still dependent on their families in walking: $66.3\%$ In bathing: and $61.0\%$ in using toilet. 2. The average score burdened family caregivers had felt was 2.25 in 4 full point which can be interpreted as average. Among the six burden dimensions. time was said to be the No. 1 burden-yielding factor. When it comes to the characteristics of patients. there were tendencies for patients with lower incomes residing in communal housing. with cerebrovasculal diseases and with higher score of daily living activity to show a higher burden scores. 3. The average score of social support was 2.49 in 5 point. which represents a average score. Among other items. the opportunity of social integration was 2.66. topping the list whereas a chance of upbringing was starkly low at 2.42. The higher scores of social support were shown in cases where the caregiver was male spouse. total care giving duration exceeded 25 months and malignant patients daily living activity scores were low. 4. The score of patients daily activity was positively correlated to the degree of the family burden (r=0.1942). Data indicates there was direct correlation between daily living activity and burden of time consuming, self-improving and physical impairment. 5. The score of the patients daily living was negatively correlated to the social support (r=0.3414), As a whole. there was negative correlation between social support and the formation of intimacy and self-confidence.
