본 연구에서 회귀분석을 통한 중소형 건설 현장 추락재해에 대한 안전사고 위험요소를 추출하기 위해 첫째, 10년간의 중소형 건설 현장에서 발생한 추락재해에 대한 데이터를 수집하였으며 둘째, t-test를 통해 중소형 건설 현장과 대형 건설 현장 두 그룹 사이의 추락재해 발생 시 진료일수의 차이가 있다는 것을 증명하였다. 마지막으로 회귀분석을 실시하여 안전사고 위험 요소에 대한 회귀 모델을 도출하였다. 추락재해에 영향을 미치는 요소는 사고 시간, 사고 요일, 직업 분류 순으로 크게 작용하는 것으로 나타났다. 본 연구 결과는 건설 현장 규모에 따른 정책 개선 및 통계분석을 위한 기초자료와 중소형 건설 현장 위험 정량화 연구의 기초자료로 활용될 것으로 사료된다.
본 논문에서는 다수의 체압 센서가 내장된 의료용 침대의 원격 케어에 관해 기술한다. 낙상은 환자의 안전을 심각하게 위협하고 건강을 해치는 요인중의 하나이다. 본 연구에서는 이를 극복하기 위해 새로운 침대가 개발하였다. 침대 시스템은 동작이 가능한 건반, 건반의 동작을 관리하는 건반 제어기, 체압을 측정하는 센서, 센서의 값을 송수신하는 센서 제어기, 이를 확인하여 알고리즘에 따라 자동 혹은 수동으로 동작하게 하는 메인 제어기, 이 모든 정보를 총괄하는 서버로 이루어져 있다. 침대 시스템은 센서를 통해 환자의 위치를 확인하여 환자가 낙상의 위험이 있다고 판단하게 되면 메인 제어기를 통해 무선으로 서버에 경고를 보내 간호인 혹은 간호사가 환자의 위험한 상태를 인지할 수 있도록 한다. 서버는 유무선 단말에서 송신된 상태 데이터를 전송받아 침대 시스템이 정상적으로 동작하고 있는지 모니터링할 수 있다. 건반의 제어기는 건반형 메커니즘을 구동하고 체압 센서가 연결된 욕창 예방 제어를 자동으로 행하여 환자의 압력이 가해지는 부위를 물리적으로 분리해 욕창을 예방한다. 메인 제어기는 환자의 침상 존재를 확인하여 서버에 전송한다. 결론적으로 제안된 시스템은 사용자의 상태를 스마트 모니터링하고 원격 케어를 행할 수 있게 된다.
This study was a quasi-experimental study of non-equivalent control group pretest and posttest design. The purpose of this study was to determine the effect of Tai Chi exercise program on physical functions, psychological functions, and fall among the fall-prone elderly. The data were collected from September 19, 2001 to January 31, 2002. The study, conducted at two facilities located in Kwang-ju, was targeted to the ambulatory aged 60 years or older who had at least one of the key fall risk factors. Experimental group participated in Tai Chi exercise for 40 minutes per one time and three times a week for 12weeks at an auditorium. Fifty nine fall-prone elderly were assigned to 12-week Tai Chi exercise program (n=29) and control group (n=30). They underwent tests of lower muscle strength, time for chair stand, balance, flexibility, depression, falls efficacy, fear of falling, and numbers of fall at the baseline and at the 12th week. Numbers of fall and fall injuries were monitored for 16 weeks(12-weeks intervention plus 4-week follow-up periods) using fall calendar. Each participant was given a calendar to record the numbers of fall per day for a month. The calendars were collected at the last week of each month. 1. Tai Chi exercisers showed significant improvement in the strength of knee flexors, and ankle dorsiflexors and plantarflexors compared to the control group. The experimental group had improvement in the strength of knee extensors while the control group did not, with no statistical significance. 2. Tai Chi exercisers reported positive change in the average time of chair stand as compared to the control group. 3. Tai Chi exercisers had significant improvement in flexibility as compared to the control group. 4. There was no significant difference in the depression between the two groups, even though the Tai Chi exercisers maintained depression score in the same level while the others were increased. 5. Tai Chi exercisers showed significant improvements in the falls efficacy as compared to the control group. The falls efficacy was significantly improved among the experimental group while the opposite was identified among the control group. 6. The experimental group reported the significant reduction of the fear of fall, whereas control group reported the opposite. 7. Of the 59 subjects for 16weeks(12weeks intervention period and 4weeks follow up), 9 (31.0%) of the 29 in exercise group and 15 (50%) of the 30 in the control group fell (relative risk=0.62. 95% CI 0.32-1.19), even with no statistical difference. The results suggest that the Tai Chi exercise program can improve the strength of knee flexors, ankle dorsiflexors and plantarflexors, chair stand. flexibility, falls efficacy, and fear of falling for the fall-prone elderly.
본 논문은 뇌졸중 환자를 대상으로 그들의 균형능력을 평가하여 낙상의 위험을 예견하기 위하여 임상에서 가장 보편적으로 사용되고 있는 버그균형척도와 균형 및 평형능력을 평가하기 위하여 사용되는 Smart Balance Master System의 평형지수 사이의 상관관계를 알아보기 위함이었다. 22명의 만성 뇌졸중 환자가 본 연구에 참여하였다. 조용하고 잘 정돈된 치료실에서 대상자에게 버그균형척도와 Smart Balance Master System을 실시하였다. 연구결과 버그균형척도와 Smart Balance Master System의 평형지수는 중등도 이상의 유의한 상관관계를 보여주었다. 또한 Smart Balance Master System의 6가지 조건에서 모든 '눈 감고 지지면 동요 조건'에서 유의한 상관성이 없었으며, 다른 5가지 조건과는 중등도 이상의 유의한 상관성을 보여주었다. 뇌졸중 환자는 균형능력을 유지하기 위하여 시각에 대한 의존도가 높으며, 버그균형척도가 이런 부분을 적절히 평가하지 못하기 때문에 '눈 감고 지지면 동요 조건'과 유의한 상관성을 보여주지 못한 것으로 사료된다. 따라서 임상에서 뇌졸중 환자를 대상으로 그들의 낙상을 예견하기 위해서는 한 가지 이상의 임상 평가방법을 사용하여 감각기관의 모든 영향을 고려하는 것이 그들의 낙상 위험을 예견하는데 보다 적절하고 효율적인 방법으로 사료된다.
본 연구의 목적은 국외에서 중추청각처리장애의 선별도구로 널리 사용되고 있는 Fisher's 청각행동문제 체크리스트(Fisher's auditory problems checklist, FAPC)와 청각행동특성척도(Scale of Auditory Behaviors, SAB)를 한국어로 번역하여 학령기 아동을 대상으로 실시하므로 국내의 학령기 아동의 청각행동특성을 파악하고, 두 검사 간의 특성을 비교하므로 국내에서 중추청각처리장애 선별을 위한 기초 자료를 제공하는데 있다. 한국어로 번역된 FAPC와 SAB 검사도구를 1~6학년 223명의 학생의 부모나 양육자에게 실시한 결과, FAPC와 SAB 검사 도구는 모두 학년별로 유의미한 점수 차이를 보이지 않았고, 학년 증감에 따른 수행 점수 간에 특정 유형이 관찰되지 않았으므로 본 설문지들은 학년에 상관없이 사용하여도 무방하다고 여겨진다. 그러나 두 검사의 유의미한 상관관계에도 불구하고 공통위험군 범위의 아동 수가 적어 서로 다른 특성을 포함하고 있으므로 선별검사 및 청각행동특성을 기초로 한 진단검사의 선정 및 재활 계획 수립시 서로 보완적으로 사용하는 것이 바람직하다고 여겨진다. 더하여 두 검사의 중추청각처리장애와의 관련성 검증과 표본 집단을 확대한 규준 제시를 통한 후속 표준화 연구가 요구된다.
특수구조물 중에서 특히 높은 독립적 구조물은 낙뢰에 의한 뇌격을 받을 가능성이 높다. 특수 구조물은 대체적으로 국가 산업시설물에 속하므로 낙뢰로 인한 피해는 사회적 경제적 손실이 막대하다. 이러한 낙뢰 피해 방지를 위해 낙뢰 보호시설을 설치하였으나 2015년도에 서해 대교의 지지케이블에 낙뢰로 인한 화재가 발생하여 많은 경제적 손실을 초래하였다. 이에 낙뢰 보호 시스템의 설계는 구조물에 낙뢰 위험을 분석 한 후 보호 조치를 수립해야한다. 뇌격에 대한 위험도 평가에 대한 국제적 규격은 국제전기기술위원회(IEC) 규격이 전 세계적인 규격의 근간을 이루고 있으며, 특히 해외 국가규격이나 기준은 IEC 기준으로 낙뢰 위험도 평가를 실시하여 낙뢰 보호대책시 적용하고 있다. 국내에서도 KS C IEC 62305 규격을 적용하여 위험성평가와 피뢰보강을 위한 기초자료로 활용하고, 낙뢰 보호 시스템을 설계한다. KSC IEC 측정방법으로 국내 특수교 리스크 평가에 대한 결과로는 물리적 손상 및 전자계시스템 위험 특수교는 평가대상 전체 교량 77개 교량 중 7개 교량에서 허용위험도가 높은 것으로 보고되었다. 본 논문에서는 높고 독립적 구조물의 위치에 따라 낙뢰 수에 대한 실제 정보 수집 시스템을 운영하는 모델링 시스템을 제안하기 위해 실제적인 낙뢰 정보를 분석하고 기상 관측소에서 제공 한 낙뢰수와 높은 구조물에 직접 도달하는 위험한 낙뢰 수를 비교하여 검증하는 방법을 검토하였으며, 지상에 직접적으로 도달한 낙뢰수에 따른 낙뢰위험도 산출시 확률적 보정률을 모니터링하여 적용함으로써 실효적인 낙뢰위험도 평가를 할 수 있도록 제안하고자 한다.
The use of a gym ball is becoming more popular for dynamic balance exercises. However, little is known about the effects of the dynamic ball exercises in the elderly. The purpose of this study was to compare balance and functional mobility after dynamic balance exercises using a gym ball to reduce the risk factorfor falls. All of the 15 subjects were women between the ages of 68 and 91 (mean age=79.9 yrs, SD=5.87) at anursing home in Wonju. Seven of fifteen subjects were placed in the experimental group and the others in the control group. Three clinical tests were used to determine the degree of balance and functional mobility before beginning the exercise program, after 4 weeks of gym ball exercise, and after 8 weeks of gym ball exercise. These three tests included Timed Up & Go (TUG), Berg Balance Scale (BBS) and the Functional Independence Measure (FIM). Dynamic ball exercises training for 8 weeks (5 days per week) included side stretching, prone walking, bridging, marching and opposite arm and leg lifting. There were significant differences found before the gym ball exercise program began and after 4 weeks and then 8 weeks in the experimental group (p<.05). Significant differences between the groups were shown for TUG and BBS (p<.05). No significant difference was noted between the groups for FIM. Therefore, gym ball exercises can improve dynamic balance and may be recommended to include in a therapeutic program to help the elderly reduce their risk of a fall.
In this paper, COP (center of pressure) during quiet standing and squat-and-stand movement was analyzed to compare the postural control of young and elderly subjects with special interest in the elderly females who were reported to have higher fall rate than the elderly males. Subjects include the young subjects (10 males: $21.8{\pm}2.6yrs$, 10 females: $20.4{\pm}0.3yrs$) and the elderly subjects (8 males: $75.5{\pm}4yrs$, 8 females: $72.3{\pm}3.5yrs$). Analysis parameters were the mean of the distance between the instantaneous COP and the average COP (COP distance) and the mean of the COP movement velocity (COP velocity) in both AP (anterio-posterior) and ML (media-lateral) directions. During quiet standing, the COP distance in ML direction of elderly females was significantly greater than that of elderly males and the COP velocity of elderly females in both ML and AP direction were significantly greater than those of all the other groups. During squat and stand movement, the COP distance of elderly females was not significantly different with that of the elderly males. However, the COP velocity of elderly females was significantly greater than that of all the other groups. The large lateral weight shift (COP distance) of elderly females during quiet standing may explain their greater fall rate. However, this does not apply to squat-and stand movement. In contrast, COP velocity results show that the elderly females' COP is rapidly trembling compared to that of elderly males during both quiet standing and squat and-stand movement. This results suggest that rapid trembling or postural sway may reflect the reduced postural control ability and the risk of falling.
Slips and falls are associated with many occupational injuries in Korea. It is also estimated that slipping are major contributors to slip, trip and fall injury burden. In order to fulfill our objective, the another regulation in Korea and foreign countries were investigated and reviewed. Many kind of standard, mandatory documents and guideline were also reviewed. And then, regulations, standard, guideline etc. reviewed were compared with each others. The article 3 was revised as below. 1. The floors of the traffic route in workplace shall have no hole or slope, or be uneven or slippery so as, in each case, to expose employees to slip, trip and fall risk, except if adequate measures have been taken to prevent a employees falling. 2. The employer shall design, install and fix the drain for effective drainage if fluid contaminants were frequently occurred. So far as is reasonably practicable, An employer shall keep the workplace clean, sanitary, and dry so that employees won't have any risk to tripping or slipping at the workplace. 3. To facilitate cleaning, every floor, workplace, and passageway shall be, so far as is reasonably practicable, kept free from protruding objects, splinters, holes, etc. Also, some criteria was developed in this study. Standard and criteria developed in this study will help to prevent slip, trip, and fall injuries.
Background: The purpose of this study was to examine associations between classification of the Geriatric Screening for Care-10 (GSC-10) and the Morse Fall Scale (MFS) among elderly inpatients. Methods: Among elderly inpatients aged over 65 admitted to hospital (from November 1, 2016 to July 31, 2017), the data for 5,780 patients (who were evaluated using the Morse Fall Scale and the Geriatric Screening for Care-10) were analyzed using x2-tests and t-tests to examine differences between the GSC-10 and MFS, according to general characteristics of elderly inpatients (i.e., gender) using IBM SPSS Statistics 24. Results: : Scores for the GSC-10 were significantly higher in women than men for depression (p<.001), delirium (p=.048), functional decline (p<.001), incontinence (p<.001), and pain (p<.001). Statistically significant differences in all domains of the GSC-10 for elderly hospitalized patients were found for the classification of fall risk. Conclusion: The findings of this study, as supported by the GSC-10, indicate that the most common problems experienced by the elderly are related to the risk of falling. In order to reduce the incidence of falls in elderly inpatients, customized fall prevention based on the GSC-10 results is necessary.
