낙상 예방 프로그램이 농촌지역주민의 낙상 효능감과 활동 특이적 균형 자신감에 미치는 효과 (Effects of Fall Prevention Program on Fall Efficacy Scale and Activities-Specific of Balance Confidence Scale in Rural Residents)

  • Purpose: The study evaluated the effects of a fall prevention program in a Korean rural area. Methods: A single pretest-posttest design group was studied. Thirty nine residents in the same rural area participated in a program comprised of education and exercise. The 5-week fall prevention program was performed for 60 minutes, once a week. Results: Fall Efficacy Scale (FES) score and Activities-specific of Balance Confidence Scale (ABC) score were significantly increased after the program, but the pre-and posttest FES and ABS scores were not significantly different between the fall experience group and non-fall experience group. Conclusion: The fall prevention program can improve rural residents' FES and ABS, and is recommended as a fall prevention intervention.

  • Purpose : This study assessed the effects of transcranial direct current stimulation (tDCS) on balance, fall efficacy, and fall-related fitness in stroke patients, using a cohort of 30 stroke patients divided into two groups. Methods : The experimental group (was given transcranial direct current stimulation in a virtual reality program) and the control group was given false tDCS in virtual reality. there were 15 patients in each group, receiving appropriate treatment over 30 sessions (30 minutes per session per week over a six-week period). In order to assess the change in balance before and after the intervention, the Berg Balance scale was utilized. Fall efficacy was evaluated using the Korean Falls Efficacy Scale for the Elderly (FES-K), The following exercises were performed by patients to assess fall-related fitness : sitting down in a chair and standing up : walking a 244 cm round= trip, and standing on one foot. Results : After the intervention, the experimental group exhibited significantly increased fall efficacy and fall-related fitness, while the control group exhibited no change. These findings suggest that tDCS has positive effects on balance, fall efficacy, and fall-related fitness in stroke patients. Conclusion : Using tDCS as an intervention would bring positive effects on balance, fall efficacy, and fall-related fitness in stroke patients undergoing rehabilitation.

상상연습이 노인의 균형 능력과 낙상효능감에 미치는 영향 : 단일대상연구 (Effects of Mental Practice on Balance Ability and Fall Efficacy in Dwelling Elderly Population: Single Subject Design)

  • 목적 : 상상연습(mental practice)이 노인의 균형 능력에 미치는 효과를 규명하고 노인 낙상 예방을 위한 효과적인 중재 방법으로써 적용 가능성에 대해 알아보는 것이었다. 연구방법 : 본 연구는 2013년 7월부터 10월 까지 인지에 문제가 없고 낙상에 경험이 있지만 보조 도구 없이 자립보행이 가능한 65세 이상 노인 3명에게 상상연습을 제공하였다. 개별 사례 실험 연구 중 ABA' 설계를 사용하여 기초선 과정 4회기, 중재 과정 5회기, 재 기초선 과정 4회, 총 13회기로 진행되었다. 독립변수는 노인의 균형 능력 및 낙상 효능감 증진을 위해 상상연습을 사용하였고, 종속변수로는 전반적 균형능력의 변화를 알아보기 위해 버그 균형 척도(Berg Blance Scale; BBS), 낙상효능감에 변화를 알아보기 위해 낙상효능감척도(Fall Efficacy Scale: FES), 정적, 동적 균형에 변화를 측정하기 위해 각각 외다리기립검사(One-Leg Standing test; OLS), 일어나 걸어가기 검사(Timed Up & Go test : TUG)를 실시하였다. 결과 분석은 평균값과 시각적 그래프를 통해 제시하였다. 결과 : 상상연습 기법 적용 후 모든 대상자의 낙상효능감이 기초선 과정에서 측정된 수치 보다 증가된 결과를 보였으며 또한 전반적 균형 능력과 정적 동적 균형 능력 또한 증진 또는 유지 되었다. 결론 : 본 연구는 상상연습이 노인의 낙상효능감과 균형능력의 향상과 유지에 효과가 있는 것으로 나타났음으로 노인의 낙상 예방 활동으로 상상연습의 적용을 추천한다.

Effect of Fall Prevention Exercise Program on Fall Efficacy, Depression, and Quality of Life of Elderly in the Community

  • Objective: This study aimed to investigate the effects of fall-prevention exercise programs on fall efficacy, depression and health-related quality of life in elderly. Design: a randomized controlled trial Methods: A total of 57 participants over 65-years-old have been allocated to control and experimental groups. The experimental group received 8 weeks of fall-prevention exercises that included strengthening of lower extremities, balance and gait training. They received the exercises twice a week for 50 minutes as a group.Both groups were assessed using Modified Falls Efficacy Scale (MFES), Geriatric Depression Scale (GDS), and 36-Item Short-Form Health Survey (SF-36) prior and post to the intervention. The control group also received the same exercise program after the post evaluation. Results: The experimental group showed overall improvement in MFES, GDS, and physical components of SF-36 (p<0.05). The MFES was significantly increased in the experimental group after the intervention (p<0.05). The GDS was significantly decreased in the experimental group after the intervention (p<0.05). The SF-36, only physical Function, Role limitations due to physical health, general health, and energy and fatigue categories were improved in the experimental group after the intervention (p<0.05). Conclusions: The results showed 8 weeks of fall-prevention exercise programs can increase fall efficacy and physical related quality of life while reducing depression of elderly over 65.

입원 노인과 재가 노인의 신체능력과 낙상효능감 및 우울수준 비교 (Comparison of Physical Ability and Fall Efficacy and Depression between Elderly and Hospitalized Elderly)

  • Background: The purpose of this study is to provide basic data on physical therapy intervention by comparing the physical ability and fall efficacy and depression of elderly and hospitalized elderly. Method: This study was carried out with 22 hospitalized elderly who can live in Y-Hospital and 22 elderly people in Busan. We measured body mas index, time up & go test, function reach test, 10m walking test, bracket lifting test, fall efficacy and geriatric depression scale. The data collection period was from November to December 2017. The collected data was analyzed using SPSS Win 18.00 program. Results: There was a significant difference between the elderly and hospitalized elderly form physical ability and depression(p<.05). But Fall efficacy was no significant difference(p>.05). 10m walking with TUG, heel with fall efficacy showed quantitative correlation and 10m walking with fall efficacy, depression with fall efficacy, heel with TUG, 10m walking showed amulet correlation. There was a statistically significant difference. Conclusion: Elderly showed better results in physical ability and depression than hospitalized patients.

상상연습이 뇌졸중 환자의 균형능력과 낙상 위험도 및 낙상 효능감에 미치는 영향: 개별대상자 실험연구 (The Effect of Mental Practice on Increasing Balance and Fall Index & Fall Efficacy Scale of Stroke Patients: Single Subject Experimental Research)

  • 목적 : 본 연구의 목적은 상상연습이 뇌졸중 환자의 균형능력과 낙상 위험도 및 낙상 효능감에 미치는 영향을 알아보기 위한 것이다. 연구방법 : 뇌졸중 진단을 받은 4명의 성인을 대상으로 개별대상자 실험연구 방법 중 대상자간 다중기초선(multiple baseline across individuals)으로 설계되었다. 기초선 과정과 중재 과정을 포함해 총 20회를 4주 동안 진행하며, 기능적 손 뻗기 검사를 이용해 균형능력의 변화를 측정했다. 그리고 낙상 위험도(Tetrax) 프로그램과 낙상 효능감으로 중재 전 후를 비교했다. 시각적 분석과 2 표준편차 밴드 분석방법을 통해 나타냈다. 결과 : 상상연습 중재기간 동안, 기능적 손 뻗기 검사를 사용한 결과 대상자들의 균형능력의 향상이 나타났다. 이러한 변화로 낙상 위험도가 감소하였고, 낙상 효능감이 증가했다. 결론 : 본 연구 결과, 상상연습이 뇌졸중 환자의 균형능력 향상에 효과적임을 알 수 있었다. 향후 연구에서 더 많은 연구 대상을 통해 다양한 방법으로 균형능력을 측정하여 확인이 이루어졌으면 한다.

  • Purpose: The purpose of this study was to evaluate the effects of BeHaS exercise program on pain, balance, and falling efficacy in elderly with osteoarthritis. Method: A convenience sample of 52 elderly people with osteoarthritis who lived in C city were recruited through the public health center. For this quasi-experimental study design, 31 of the elders were assigned to the experimental group which received the BeHaS exercise program twice week for eight weeks, and 24 were assigned for the control group (no intervention). Outcome measures were numeric pain scale, standing on one leg for balance, fall efficacy scale. Analysis was performed with SPSS version 12.0 program. Results: Pain was decreased significantly in the experimental group as compared to the control group, Balance and fall efficacy were improved significantly in the experimental group as compared to the control group. Conclusion: The BeHaS exercise program can be effective in improving pain, balance, and fall efficacy in elders with osteoarthritis.

가상현실 프로그램이 뇌졸중 환자의 낙상 효능감과 일상생활활동에 미치는 영향 (The Effects of Virtual Reality Program on Fall Efficacy and Activities of Daily Living for Patients with Stroke)

  • 목적 : 본 연구는 가상현실 프로그램을 통해 뇌졸중 환자의 낙상 효능감과 일상생활활동에 미치는 영향을 알아보고자 하였다. 연구방법 : 뇌졸중 진단을 받은 환자를 대상으로 총 4주 동안 가상현실 프로그램 중재를 받는 실험군 10명과 작업치료 중재를 받는 대조군 10명으로 나누어 실시하였다. 치료 중재 전 후 낙상 효능감을 비교하기 위하여 낙상 효능감 척도(Fall Efficacy Scale: FES)를 시행하였고, 일상생활활동 수행능력을 비교하기 위하여 기능적 독립척도(Functional Independence Measure: FIM)를 시행하였다. 결과 : 실험군과 대조군의 치료 중재 전 후 뇌졸중 환자의 낙상 효능감과 일상생활활동 수행능력은 통계학적으로 유의한 차이를 보였고(p<.05), 실험군이 대조군에 비해 통계학적으로 유의하게 낙상 효능감과 일상생활활동 수행능력이 향상되었다(p<.05). 결론 : 작업치료와 더불어 가상현실 프로그램을 적용할 때 뇌졸중 환자의 낙상 효능감과 일상생활활동 수행능력을 향상시킨다는 것을 알 수 있었다. 따라서 가상현실 프로그램이 뇌졸중 환자의 낙상 효능감과 일상생활활동 수행능력을 위한 치료방법으로 작업치료와 더불어 임상에서 유용하게 적용될 수 있을 것이다.

  • Background: This study analysed the effect of the fall prevention education activities on balance, fall efficacy, activities of daily living (ADL) of sub-acute stroke patients. Design: Randomized controlled trial. Methods: The subjects of the study were 24 subacute patients, who have onset period of less than 6 months. They are randomly allocated in the conventional therapy group (n=12) and fall prevention education group (n=12) and observed with frequency for 5 times a week for four weeks. Measuring took place before and after the experiment leading to following results of the Fall efficacy scale (FES), Berg balance scale (BBS) and Functional Independence Measure (FIM). Results: FES, BBS and FIM score was significantly greater in the post-test than in the pre-test in both groups (p<0.01). Independent t-test confirmed that the fall prevention education group showed much greater improvement change in the FES, BBS and FIM score than conventional therapy group (p<0.05). Conclusion: The present study demonstrated the importance of clinical contribution of the fall prevention education in the individuals with subacute stroke patients to the balance, fall efficacy and ADL following the stroke rehabilitation.

  • Purpose: The aim of this study was to determine the effect of side walking on balance confidence, falls efficacy and fall risk in acute stroke patients. Methods: The study included 14 patients with acute stroke who were randomly allocated to a side walking group (Experimental group, N=7) and a forward walking group (Control group, N=7). Both groups performed the exercise 5 times a week for 2 weeks. Outcomes were assessed using Korean-Activities-specific Balance Confidence Scale (K-ABC), Korean-Fall Efficacy Scale (K-FES), Korean-Fullerton Advanced Balance Scale (K-FAB). Results: After 2 weeks of training, both groups showed significantly improved ABC, FES, FAB (p<.05 in both groups). However, the ABC, FES, FAB in the experimental group was significantly better than in the control group (p<.05). Conclusion: These findings indicate that side walking training may be effective at improving balance confidence and decreasing fall down risk in early stroke patients. Therefore, side walking training may be recommended as an intervention in reducing the incidence of falls in acute stroke patient.
