전완부 후골간 동맥 유리피판술을 이용한 안면부 조직 결손 재건 치험례 (Reconstruction of the Face Defects Using Posterior Interosseous Artery Forearm Free Flap)

  • With esthetic concern in the reconstruction of skin and soft tissue defects of face, the use of local flap has been the method of choice. However, when there is extensive tissue loss in the face, local flaps do not provide satisfactory results. The amazing development of microsurgical technique has decreased the percentage of free flap failure, thus making free flap use in reconstruction of facial soft tissue defects. Many free flaps has been applied for reconstruction of face defects. Especially, the radial forearm flap has numerous advantages with which facial reconstruction is made possible. But, its disadvantages are ; the sacrifice of one major artery supplying the hand and donor site complications. In order to circumvent these disadvantages, we employed posterior interosseous artery(PIA) forearm free flap for the reconstruction of the face defects. The posterior interosseous forearm island flap was first described by Zancolli and Angrigiani(1985). Currently, the PIA island flap and free flap have been used for hand reconstructions. The disadvantages of the PIA flap are ; the small caliber of the pedicle, different locations of the perforating branches, and the proximity of the motor branch of the radial nerve. But, its advantages lies in preserving the major artery of the hand, minimal donor site morbidity, and fairly well matched skin texture and color, and that the flap volume is sufficient, not too bulky with convenient handling. By using this flap, we performed 1 case of tumor resection and 1 case of traumatic defect. From our experiences we conclude that it is one of many useful methods in the reconstruction of the skin and soft tissue defects of the face. We also have discussed advantages and some limitations of various free flaps for reconstruction of the face.

위상최적설계를 통한 트레일러 제5차륜 연결구조물의 경량화 및 내구성 (New Weight-reduction Design of the Fifth Wheel Coupler with a Trailer by Using Topology Optimization and Durability Tests)

  • The fifth wheel coupler is a heavy automotive coupling structure which connects a tractor and a trailer used for heavy-duty trucks widely. It is subjected to various loads simultaneously such as rolling, pitching and yawing loads as well as coupling frictional and impact loadings. Most of existing couplers have been overdesigned and, therefore, it is necessary to reduce the dead weight to increase the fuel efficiency. The topology optimization was applied in order to find conceptual layout designs which could show major load paths and ribs locations, and then the size structural optimization was performed in order to determine the heights and thicknesses of coupler ribs with the predetermined various loading conditions for the development of a new slim coupler with a minimum weight and high enough strength and stiffness. As the results of the topology optimum design, an efficient new coupling structure for truck trailers was designed. The weight of the new fifth wheel coupler was reduced by 4.9 %, compared with the existing one, even though all strength requirements were satisfied. The fatigue test of the new coupler was performed with cyclic vertical loads (+78.4 to +235.2 kN) and horizontal loads (-91.2 to +91.2 kN) simultaneously at 1 Hz and the life of 2,000,000 cycles were achieved without failure.

A Web-based Information System for Plant Disease Forecast Based on Weather Data at High Spatial Resolution

  • This paper describes a web-based information system for plant disease forecast that was developed for crop growers in Gyeonggi-do, Korea. The system generates hourly or daily warnings at the spatial resolution of $240\;m{\times}240\;m$ based on weather data. The system consists of four components including weather data acquisition system, job process system, data storage system, and web service system. The spatial resolution of disease forecast is high enough to estimate daily or hourly infection risks of individual farms, so that farmers can use the forecast information practically in determining if and when fungicides are to be sprayed to control diseases. Currently, forecasting models for blast, sheath blight, and grain rot of rice, and scab and rust of pear are available for the system. As for the spatial interpolation of weather data, the interpolated temperature and relative humidity showed high accuracy as compared with the observed data at the same locations. However, the spatial interpolation of rainfall and leaf wetness events needs to be improved. For rice blast forecasting, 44.5% of infection warnings based on the observed weather data were correctly estimated when the disease forecast was made based on the interpolated weather data. The low accuracy in disease forecast based on the interpolated weather data was mainly due to the failure in estimating leaf wetness events.

  • 시추공에서 관찰된 절리면의 방향성과 위치자료를 이용하여 개착면에서의 trace 분포를 예측하는 대수학적 기법을 개발하였다. 절리 trace 예측은 절리면과 투영면의 3차원 평면식을 활용하여 대수학적으로 수행되며, 개별 불연속면의 영속성을 고려하여 투영 영역 내에서 trace가 표출되는 범위가 산정된다. 절리 예측기법을 활용하여 슬럼핑 현상에 의해 구조적으로 불안정한 김해 내삼사면의 안정성 및 보강 계획의 적정성을 분석하였다. DOM 시추작업을 수행하여 암반구조 특성을 조사하였으며, 사면 파괴를 유발시킬 수 있는 절리들을 추출하여 심도별 지반거동 특성을 분석하였다. 또한, 사면보강을 위하여 계획된 앵커 정착부에서의 절리분포 및 거동양상을 분석하고 앵커 보강의 효율성을 고찰하였다.

  • 심장의 원발성 종양은 소아에서는 매우 드문 질환이다. 횡문근종은 태아, 영아 및 소아 연령에서 가장 흔한 원발성 심장 종양으로 결절성 경화증이 동반되는 것으로 보고되고 있다. 결절성 경화증은 뇌, 심장, 피부 및 기타 장기들을 침범하는 신경피부 증후군으로, 결절성 경화증을 가진 영아의 50-64%에서 동반된다. 심장 횡문근종은 종괴는 심방을 비롯하여 심실 중격과 심실벽에 다발성으로 존재하며 종양의 위치와 크기, 침범 정도에 따라 심장 유출로 협착 및 심부전과 같은 여러 가지 임상 양상을 보일 수 있다. 드물지만 부정맥을 주증상으로 발현하는 경우도 보고되고 있다. 하지만 대부분 심장의 횡문근종이 특별한 문제를 일으키지 않고 성장과 함께 쇠퇴되는 경향을 보이므로 극히 일부분에서만 장기적인 항부정맥제 사용이 요구된다. 환자의 최종적인 예후는 동반되는 뇌병변에 의해 결정되는 경향이 있다. 저자들은 상심실성 빈맥으로 입원치료 중 진단된 심장 횡문근종을 가진 결절성 경화증 1예를 경험하였기에 보고하는 바이다.

한계상태 설계법을 이용한 지역난방 열배관의 국부좌굴 연구 (Study on Local Buckling of District Heating Pipes Using Limit State Design)

  • 지역난방은 열병합 발전소에서 생산된 열을 온수의 형태로 열배관 네트워크를 통해 넓은 지역에 일괄적으로 공급하는 난방시스템이다. 일반적으로 지역난방 열배관은 지중에 매설되며 내부를 흐르는 온수의 압력과 온도변화, 매설 토사와의 마찰 및 반력에 의한 하중을 받게 되므로 열공급 효율과 함께 기계적인 안전성에 대한 검토가 반드시 필요하다. 본 논문에서는 강관의 한계상태 설계법을 이용하여 한계판폭두께비와 한계변형률, 두 가지 관점에서 열배관의 국부좌굴 발생여부를 평가하였으며 그 결과 현재 국내에서 사용되고 있는 모든 열배관은 국부좌굴이 발생하지 않음을 확인하였다. 마지막으로 국부좌굴을 피하기 위한 최소 예열온도를 산출함으로써 예열온도에 대한 시공 기준을 제시하였다.

총폐정맥 환류 이상증에 대한 술후 장기성적 검토 (Long-term Results Following Surgical Repair of Total Anomalous Pulmonary Venous Return)

  • Seventy-three patients with isolated total anomalous pulmonary venous connection the patients associated with other major cardiac anomalies such as single ventricle, DORV[Double Outlet Right Ventricle and large VSD[Ventricular Septal Defect were excluded were underwent surgical repair from January 1980 through October 1993. There were 45 boys and 28 girls. The mean age at operation was 19.9 months[range 6 days to 24 years and mean body weight was 7.1kg[range 2.6kg to 45kg . The anomalous locations of connection were supracardiac in 38, cardiac in 21, infracardiac in 5, and mixed in 9. In 38 patients[52% , the venous drainage was obstructed. The obstruction ratios according to the connection type were as follows: 53%[28/38 in supracardiac, 52%[11/21 in cardiac, 100%[5/5 in infracardiac, 22%[2/9 in mixed type. The associated cardiac anomalies were persistent left SVC[2 , tricuspid valve regurgitation[3 , cor triatriatum[1 , and mitral cleft[1 . And associated noncardiac anomalies were imperforate anus[1 and Neil Weightman syndrome[1 . The operative mortality was 23%. The causes of death were pulmonary hypertensive crisis, perioperative myocardial failure, pneumonia with sepsis, arrhythmia and etc. The statistically significant factors in postoperative mortality were the pulmonary venous obstruction and age [p<0.01 . The operative mortality was high in groups of age under 1 month and pulmonary venous obstruction. The mean follow-up was 27.1 months. There were two late deaths. The first patient was three months old boy with supracardiac type and severe obstructive symptoms. The postoperative echocardiography was showed anastomotic stenosis and reoperations were performed twice but the patients expired due to pneumonia and sepsis. The second patient was three month old boy with supracardiac type and total correction was done and was doing well postoperatively. Eight years later, he expired suddenly due to arrhythmia. But all the other patients were in NYHA Fc I and received no medications. The 5-year survival rate excluding early expired patients is 97.1 $\pm$ 0.03 %. In conclusion, although the operative mortality of total anomalous pulmonary venous connection was relatively high compared to other major cardiac anomalies, we could expect excellent long-term results by early surgical correction.

The impact of frenulum height on strains in maxillary denture bases

  • PURPOSE. The midline fracture of maxillary complete dentures is a frequently encountered complication. The purpose of this study was to assess the effect of frenulum height on midline strains of maxillary complete dentures. MATERIALS AND METHODS. A removable maxillary complete denture was fabricated and duplicated seven times. Four different labial frenulum heights were tested for stresses occurring on the palatal cameo surface. The strains were measured with strain gauges placed on 5 different locations and the stresses were calculated. To mimic occlusal forces bilaterally 100 N of load was applied from the premolar and molar region. RESULTS. A statistically significant association between the height of the labial frenulum and the calculated stresses and strains was shown (P<.05) predominantly on the midline and especially on the incisive papilla. The results showed that stress on the anterior midline of the maxillary complete denture increases with a higher labial frenulum. CONCLUSION. Within the limitations of this in vitro study, it can be concluded that the stress on the anterior midline of the maxillary complete denture increases with a higher labial frenulum. Surgical or mechanical precautions should be taken to prevent short-term failure of maxillary complete dentures due to stress concentration and low cycle fatigue tendency at the labial frenulum region.

응급 유리 피판에 의한 사지 외상의 수복 - 증례 보고 - (Coverage of the Severely Traumatized Extremity by Emergency Free Flap - Case Report -)

  • Emergency free flap has been advocated to cover the severely injured extremity for more than two decades, due to its numerous advantages such as low incidence of flap failure and infection rate and early recovery of function. But there are very few reports about these. The authors report their experience in using the emergency free flap for reconstruction of extremities. For last 10 years, 4 patients ranging from 3 to 27 years old with severely traumatized extremities were treated with emergency free flap transfers. Three were males and the other was a female. Flap size ranged from $2{\times}5\;cm^2$ to $7{\times}22\;cm^2$. The locations of the recipient site were the dorsum of the foot, the cubital fossa, the popliteal fossa and the upper arm. The number of the donor sites used was as follows: one scapular flap, two parascapular flaps, and one radial forearm flap with the radial bone. All of the flaps survived without need of re-exploration. There was no infection or flap loss. Involved joints have recovered a normal range of motion. Therefore, we consider that the emergency free flap is a very safe and reliable method to cover the severely injured extremities.

Experimental modal analysis of railway concrete sleepers with cracks

  • Concrete sleepers are essential components of the conventional railway. As support elements, sleepers are always subjective to a variety of time-dependent loads attributable to the train operations, either wheel or rail abnormalities. It has been observed that the sleepers may deteriorate due to these loads, inducing the formation of hairline cracks. There are two areas along the sleepers that are more prone to crack: the central and the rail seat sections. Several non-destructive methods have been developed to identify failures in structures. Health monitoring techniques are based on vibration responses measurements, which help engineers to identify the vibration-based damage or remotely monitor the sleeper health. In the present paper, the dynamic effects of the cracks in the vibration signatures of the railway pre-stressed concrete sleepers are investigated. The experimental modal analysis has been used to evaluate the modal bending changes in the vibration characteristics of the sleepers, differentiating between the central and the rail seat locations of the cracks. Modal parameters changes of the 'healthy' and cracked sleepers have been highlighted in terms of natural frequencies and modal damping. The paper concludes with a discussion of the most suitable failure indicator and it defines the vibration signatures of intact, central cracked and rail seat cracked sleepers.
