더마스탬프(Derma stamp)를 이용한 노화 피부 치험례 (A clinical report on the rejuvenate treatment with derma stamp)

  • Background & Objective : Rejuvenate treatment with oriental medicine is popular these days. However, there are few reports about it. This study was performed to evaluate the effect of derma stamp treatment. Derma stamp is a new clinical equipment for Microneedle System. Methods : Restrospective study was conducted. Ten patients were treated for 5 weeks. Follow-up was performed after the treatment with Janus facial analysis system. Satisfaction degree was asked from the patients. Results and Conclusions : Derma stamp treatment was good for skin aging problems like pore, wrinkle, spot, sebum and porphyrin. Especially oily type had better effect than dry type.

Surgery-first Approach for Facial Asymmetry with Transverse Discrepancy Using Hyrax-type Palatal Expansion Appliance

  • This report presents a case of successful treatment of skeletal Class III malocclusion with transverse discrepancy in adult by surgery-first approach. Traditionally dental decompensation is necessary prior to surgery in 2-jaw surgery to correct transverse and rotational discrepancy. However, the hyrax-type palatal expansion appliance was used to improve maxillary expansion accuracy and postoperative stability in this surgery-first approach (segmental Le Fort I osteotomy and mandibular setback surgery). It was established to be an effective means of precisely predicting postoperative occlusion and achieving stable retention after surgery of skeletal Class III malocclusion with maxillary transverse discrepancy.

얼굴매력의 변화추세 -미스코리아 얼굴감성모형을 중심으로- (The trend of facial attractiveness: on affective facial model for a Miss Korea's face)

  • 한 시대의 얼굴매력에 대한 관점을 미인대회 당선자들의 얼굴분석을 통해 알 수 있다는 가정 하에 물리적 특징에 기반 한 미스코리아 당선자들 얼굴의 변화추세를 알아보았다. 얼굴인상감성모형을 이용하여 각 미스코리아 얼굴의 물리적 특징을 측정하고 얼굴유형을 분류하였다. 분석대상은 미스코리아 171명이었다. 얼굴매력의 변화추세를 알아보기 위해 1950년부터 2005년까지 미스코리아의 얼굴 변화를 살폈다. 분석결과 미스코리아는 지난 50년간 성숙한 인상에서 앳된 인상으로 바뀐 것으로 나타났다.

  • Advances in immunosuppressive treatments and microsurgical techniques have rendered composite tissues allotransplantation (CTA), such as heteregeneous or non-organ tissues, possible in humans. CTA has evolved dramatically since the first successful rat hind limb allotransplantation. Numerous clinical applications including face, hand, trachea, larynx, and vascularized joint have been performed. Although composite tissue allografts are still in their infancy, they have opened a new era in the field of transplantation surgery and pathology, so that maxillofacial reconstructive surgeons may occasionally be faced with the challenge of diagnosing skin refection of a composite tissue allograft. Facial allotransplantation (FAT) is a new surgical technique that could be considered as a new paradigm in facial reconstruction. Since the first human FAT had been achieved in 2005, 17 cases have been reported in the world up to date. However, many problems such as life-long immunosuppression, immune rejection, ethical problems and psychological problems are remained, so facial CTA is new reconstructive option with no general acceptance. The authors reviewed the indications, the results of 17 cases and their complications, and additional consideration factors in this article, and intended to raise the awareness of oral and maxillofacial surgeons in this type of facial transplantation.

Face Reconstruction Using Lateral Intercostal Artery Perforator-Based Adipofascial Free Flap

  • Background The aim of this study was to determine the efficacy of lateral intercostal artery perforator-based adipofascial free flaps for facial reconstruction in patients with facial soft tissue deficiency. Methods We conducted a retrospective study of five consecutive patients diagnosed with facial soft tissue deficiency who underwent operations between July 2006 and November 2011. Flap design included the area containing the perforators. A linear incision was made along the rib, which had the main intercostal pedicle. First, we dissected below Scarpa's fascia as the dorsal limit of the flap. Then, the adipofascial flap was elevated from the medial to the lateral side, including the perforator that pierces the serratus anterior muscle after emerging from the lateral intercostal artery. After confirming the location of the perforator, pedicle dissection was performed dorsally. Results Dominant perforators were located on the sixth to eighth intercostal space, and more than four perforators were found in fresh-cadaver angiography. In the clinical case series, the seventh or eighth intercostal artery perforators were used for the free flaps. The mean diameter of the pedicle artery was 1.36 mm, and the mean pedicle length was 61.4 mm. There was one case of partial fat necrosis. No severe complications occurred. Conclusions This is the first study of facial contour reconstruction using lateral intercostal artery perforator-based adipofascial free flaps. The use of this type of flap was effective and can be considered a good alternative for restoring facial symmetry in patients with severe facial soft tissue deficiency.

Reconstruction of Large Facial Defects via Excision of Skin Cancer Using Two or More Regional Flaps

  • Background Free-flap surgery and skin grafting can be used to reconstruct large facial defects that may result after skin cancer removal by Mohs micrographic surgery (MMS). However, these two methods may produce low postsurgical patient satisfaction with aesthetics, and free-flap surgery may produce an undue burden for the majority of skin cancer patients, who are of advanced age. Hence, the authors examined outcomes of large facial defect reconstruction using multiple flaps. Methods Among patients undergoing MMS for skin cancer at Pusan National University Hospital between January 2013 and December 2015, we evaluated 7 patients (2 males, 5 females; average age, 73.14 years) treated for large facial defects from basal cell carcinoma. Based on operative and follow-up records, we investigated the number and type of flaps used, postsurgical complications, and patients' post-surgical satisfaction. Results Two and 3 types of flaps were used for 5 and 2 patients, respectively. Most frequently used were nasolabial flaps (7 times in 6 patients) and forehead flaps (once in each of 4 patients). The average follow-up period was 14 months, with no complications-including necrosis, hematoma, or wound dehiscence-observed. Post-surgical satisfaction averaged 4.4 out of a maximum of 5 points. Conclusions Reconstruction using two or more flaps for large facial defects after skin cancer removal using MMS produced satisfactory outcomes while preventing aesthetic problems. Practitioners should consider using multiple flaps when choosing a reconstruction method for large facial defects following skin cancer removal.

이하선 종양 수술 후 신경계 합병증 발생에 관여하는 인자들에 대한 연구 (A Study of the Factors Related with Postoperative Neurologic Complications after Parotid Gland Surgery)

  • Objectives: Injuries of neurologic structures during parotid surgery are sometimes unavoidable even in benign tumors. Since the major postoperative neurologic complications such as facial nerve palsy and Frey's syndrome give a serious and heavy burden to both patients and surgeons, it is very important to know detailed information concerning risk factors, the incidence of complication, possibility of recovery, and the term before complete recovery. Materials and Methods: This report was based on 95 patients with parotid gland tumor who had been treated and followed up over 1 year at department of surgery and otolaryngology, chonnam university hospital. Results: 1) Among total 95 cases, the facial nerve palsy developed in 18 cases(l8.9%) and Frey's syndrome in 6 cases(6,3%). 2) The incidence of facial nerve palsy increased in cases with large tumor size, and in those operated with bipolar method. However, we didn't find out relationships between the incidence of facial palsy, localization, and histologic type. 3) The incidence of Frey's syndrome showed an increasing tendency in the cases with large tumor size, benign nature, and in those underwent superficial lobectomy with posterior approach, without a statistical significance. Conclusions: Risk of facial nerve palsy, especially a transient form, seems to be related to tumor size and bipolar coagulation method. Although Frey's syndrome tends to develop easily in the cases treated with superficial lobectomy under the diagnosis of benign tumor, a futhermore study is suggested to obtain a statistical significance.

말초성 안면마비 후유증의 비수술적 치료에 관한 국내외 연구 동향 (Research Trends on Non-surgical Treatment of Peripheral Facial Paralysis Sequelae)

  • Objectives : The purpose of this study was to investigate the non-surgical treatment of peripheral facial paralysis sequelae such as bell's palsy and Ramsay-hunt syndrome during last 10 years. Methods : We searched articles in the RISS, MEDLINE, CAJ from January, 2008 to June, 2018. Articles on the non-surgical treatment of bell's palsy and Ramsay-hunt syndrome sequelae were included. We extracted data about treatments, characteristics of intervention, outcomes from the included studies and classified in to 4 categories such as case studies, RCTs, nRCTs, literature reviews. Results : 132 potentially relevant studies were identified, of which 60 studies met our inclusion criteria. Of 60 included studies, 30 were case reports, 22 were RCTs, 4 were nRCTs, and 4 were reviews. China (81.8%) were the most common by country, bell's palsy(81.7%) by disease, and case reports(50%) by study type were the most common. Symptoms were lagophthalmos, asymmetry, contracture, spasm, dacryorrhea, synkinesis, paresthesia, crocodile tears mostly in the order of frequency, and these symptoms occurred at least one month after the onset of symptoms. The most common method of treatment was acupuncture, which was used in 49 studies. As the evaluation variables, the effective rate was the highest in 25, House-Brakmann grading system in 17, and Sunnybrook facial grading system in 7. In 95% of the studies, after-treatment was reported to be cured, but objectivity is low. Conclusions : This study suggests that Korean medicine such as acupuncture can play a valid role in the non-surgical treatment of peripheral facial paralysis sequelae. In the future, a systematic and well-designed clinical study is needed for treatment of peripheral facial paralysis sequelae.

Miller-Fisher 증후군에서 나타난 지연형 안면마비 및 현훈 치험 1례 (A Case of Delayed Facial Palsy and Dizziness in Miller-Fisher Syndrome)

  • Objective : Miller-Fisher syndrome(MFS) is a type of acute inflammatory polyneuropathy, a disease characterized by abnormal muscle coordination, absence of tendon reflexes, and paralysis of the eye muscle. This study is to report a case of delayed facial palsy and dizziness in Miller-Fisher syndrome, treated with traditional Korean medicine. Methods : A 57-year-old male was diagnosed with Miller-Fisher syndrome accompanied by neurological symptoms such as right facial palsy and dizziness. He received traditional Korean medicine treatment such as acupuncture(including pharmacopuncture, needle embedding therapy, electroacupuncture) and herbal medicine for 17 days. Results : Each neurological symptoms were improved after traditional Korean medicine. In case of facial palsy, House Brackmann Grading System was reduced form grade 3 to grade 2, and Sunnybrook Scale was improved from 39 to 55. Also, other symptoms such as dizziness and headache was also improved. Conclusions : This case report suggests that traditional Korean medicine may have a role in treating neurological symptoms such as facial palsy and dizziness in patient diagnosed with Miller-Fisher syndrome.

Epidemiologic Changes of Facial Bone Fracture before and after Coronavirus Disease 2019: A Level 1 Trauma Center in Korea

  • Background The coronavirus disease 2019 (COVID-19) outbreak has had a major impact worldwide. Several countries have implemented restrictions on social interaction ("social distancing"). Several studies have reported that the epidemiology of trauma patients, such as those with facial bone fractures, has changed after COVID-19 pandemic. This study aimed to further explore these specific changes. Methods This was a retrospective study of patients who presented to a single institution with facial bone fractures between January 1, 2016, and December 31, 2020. Baseline patient demographics, clinical information, type of fracture, etiology, and operative management were compared before and after COVID-19. Results Of all cases, 3,409 occurred before COVID-19, and 602 occurred after COVID-19. Since the outbreak of COVID-19, the number of patients with facial fractures has not decreased significantly. A significant increase was noted in fractures that occurred outdoors (p < 0.001). However, a decrease was observed in operative management between the groups (p < 0.001). There was no significant difference in the proportion of assault, fall-down, industrial accident, or roll-down. In contrast, the proportion of traffic accidents and slip-down categories increased significantly (p < 0.05). Moreover, a significant decrease was found in the proportion of the sports category (p = 0.001) Conclusions It was confirmed through this study that COVID-19 pandemic also affected epidemiology of facial fractures. Focusing on these changes, it is necessary to develop safety measures to reduce facial fractures.
