측두하악관절에 발생한 양성 외방성 증식병소의 증례보고 : 활액성 연골종증, 골연골종 (Case Report of Exophytic Lesion on TMJ ; Synovial Chondromatosis, Osteochondroma)

  • 측두하악관절부위에 발생하는 양성 외방성 증식은 골종, 연골종, 골연골종 및 활액성 연골종증 등이 있으며 증상으로는 통증, 개구제한 등을 보이며 때때로 교합변화 하악정중선변위등을 수반한다. 이러한 양성 외방성 증식은 측두하악장애와 비슷한 증상으로 인해 임상 증상만으로 진단 하는 것은 어려우며, 단층촬영, 자기공명영상을 포함한 방사선 사진을 평가하여야 하며, 확진을 위해서는 생검을 필요로 한다. 활액성 연골종증과 골연골증은 인체의 장골에 흔한 양성 종양이며 구강안면부위에 있어 발생이 비교적 드물다. 본 연구는 측두하악관절에 비교적 드물게 발생하는 활액성 연골종증과 골연골종에 대하여 보고하고 진단에 대해 다소의 지견을 얻었기에 문헌 고찰과 함께 보고하고자 한다.

  • The arteriovenous malformation, an uncommon lesion, is a direct communication between an artery and a vein that bypasses the intervening capillary bed. The authors experienced two cases of arteriovenous malformation in 17-year-old and 18-year-old female patients who had suffered from mandibular pain and gingival bleeding. As a result of careful analysis of clinical and angiographic radiological examinations, we diagnosed them as arteriovenous malformations, and the results were as follows : 1. Main clinical symptoms were dull pain and gingival bleeding on the mandibular body area, and reddness, pain on palpation, thrill and palpitation in the lesional sites were also observed. 2. Radiographically, well-defined radiolucent lesions with multiple osteolytic defects were observed. In radio-nuclide scan, increased uptakes of radioisotope were observed in the lesional sites. 3. Increased venous shunt supplied by the facial, lingual and inferior alveolar arteries was observed in angiography. After embolotherapy was performed, no recurrence was reported during 3 to 6 months follow up.

구강안면통증모델에서 산자나무 추출물의 진통효과 (Analgesic Effect of Hippophae rhamnoides Extract in Orofacial Pain in Rats)

  • 본 연구는 실험동물 대상으로 하는 두 가지 구강안면통증모델을 이용하여 산자나무 추출물 H. rhamnoides (sea buckthorn)의 통증조절효과를 확인하고자 하였다. 5% formalin을 실험동물 안면부 피하($50{\mu}l$) 또는 측두하악관절($30{\mu}l$) 내로 주입하여 안면부를 긁거나 문지르는 통증행위반응을 유도하였다. Formalin의 주입으로 인한 안면부통증행위반응은 15분 이후부터 증가하여 25, 30분에 가장 높게 나타났으며 40분까지 지속되다가 45분에 감소되었다. 안면부 통증 유발 30분 전 150 mg, 300 mg/kg (1 ml)의 농도로 산자나무 추출물을 희석하여 경구투여하였다. DW를 경구투여한 대조군에서는 formalin에 의한 통증행위반응의 변화에 영향을 미치지 않았으나 산자나무 추출물(150, 300 mg/kg)의 경구투여는 각 $305.3{\pm}20.7$회, $137.3{\pm}21.8$회로 포르말린으로 유도된 안면부통증행위반응을 감소시켰다. 시간에 따른 변화의 결과로, 산자나무 추출물의 300 mg/kg 경구투여는 25~40분에서 formalin에 의해 증가된 안면부통증행위반응을 현저하게 감소시켰다. 측두하악관절에 주입한 포르말린은 $306.8{\pm}31.4$회의 통증행위반응을 나타내어 턱관절 통증을 유도하였고, 산자나무 추출물(150, 300 mg/kg)의 경구투여는 턱관절통증행위반응을 각 $184.7{\pm}20.0$회, $125.7{\pm}9.8$회로 감소시켰다. 시간의 경과에 따라 결과에서 산자나무 추출물의 경구투여는 포르말린 주입 30분에서 유의한 턱관절 통증경감효과를 나타냈다. 따라서, 본 연구의 결과는 산자나무 추출물의 구강안면 통증조절에 대한 예방적 또는 치료적 제제로의 활용가능성에 대한 기초자료로 활용가능할 것으로 생각된다.

악관절내장증을 동반한 Eagle's Syndrome (A CASE REPORT OF EAGLE'S SYNDROME WITH TMJ INTERNAL DERANGEMENT)

  • A thirty two year-old male was referred with a chief complaint of mouth opening limitation, and maxillofacial pain including left TMJ area. The patient had been treated in a private clinic with medications and conservative treatments, without any improvements in symptoms. MRI findings showed a limitations in condylar head movements, and signs of disc adhesion without the Positional change. Panoramic views showed elongation of stylohyoid process. Brain C-T was taken due to the patient's complaint of headache, facial pain, and paresthesia of tongue, and the result showed no abnormalities. The first surgery included meniscoplasty and the removal of disc adhesion of left TMJ under the preliminary diagnosis of internal derangement with adhesion, and the patient displayed marked improvements in opening movements despite of persistent hemiparesthesia of left facial areas and tongue, and mild dysphasia. Therefore, the second surgery was carried out with the preliminary diagnosis of Eagle's Syndrome, and a resolution of neurologic symptoms was obtained. The final diagnosis of this case was Eagle's Syndrome with internal derangement, and the difficulties in diagnosis and treatment might be due to the existence of two separate diseases with similar symptoms.

Inflammatory granuloma caused by injectable soft tissue filler (Artecoll)

  • Artecoll (Artes Medical Inc., San Diego, CA, USA) has recently been developed as a permanent synthetic cosmetic filler. We experienced an inflammatory granuloma resulting from a previous injection of Artecoll at the upper lip, which was regarded as a rare side effect of this filler. A 50-year-old female patient complained of swelling, dull pain, and heat in the right upper nasolabial fold area, which had started one week before her visit to Kyungpook National University Hospital. The patient received topical steroid therapy at a local clinic, which was not effective. At the injection site, a hard nodule was palpated and erythema was observed with mild tenderness. Antibiotic treatment and subsequent incision and drainage did not result in complete cure of the facial swelling, and the facial swelling and pain persisted. Computed tomography showed a lesion approximately 1-cm in size without clear boundaries and relatively increased nodular thickening. Finally, a subdermal lesion was removed via an intraoral vestibular approach. The lesion was diagnosed as inflammatory granuloma by a permanent biopsy. The patient had healed at two months after the filler injection. Although the soft tissue filler is widely used for cosmetic purposes, there is potential for complication, such as the inflammatory granuloma should be considered before treatment.

파노라마 방사선사진상에서 경상돌기의 변이에 관한 연구 (A STUDY ABOUT THE VARIATION OF STYLOID PROCESSES IN PANORAMIC RADIOGRAPHS)

  • The purpose of this study was to investigate the variation in the length and shape of styloid processes and the relationships between the elongated styloid processes and the styloid process syndrome, using panoramic radiogrpahs and questionaires. The subjects were 342 patients consisted of 170 males and 172 females aged from 11 to 78 years, not showing facial asymmetry, who visited infirmary of dental college of Yonsei University. Also, the accuracy was determined for measuring the length of styloid processes from panoramic radiographs, using dried skulls. The results were as follows: 1. The length of styloid processes was magnified approximately 1.1 times, but there was no statistically significant differencies in the magnification rate of length between right and left side. 2. The mean radiographic length of styloid processes was 29.72±7.92㎜ in males, 27.93±6.69㎜ in females, and 28.82±7.37㎜ in total. And elongated styloid process (>30㎜) was seen in 31.3% of total subjects. 3. The growth in the length of styloid processes was completed in the third decade. 4. The most common shape of styloid processes was straight followed by segmented and bent form. The incidence of segmentation was reduced with increasing age. 5. The 74% of subjects with elongated styloid process (>30㎜) showed symptoms of the styloid process syndrome. The most frequent symptom was headache followed by discomfort in the neck when turning the head from left to right, tinnitus or earache, vague facial pain, discomfort or pain when swallowing, feeling that an object is caught in throat.

Quality of life of patients with nasal bone fracture after closed reduction

  • Background: Closed reduction is the standard treatment for nasal bone fractures, which are the most common type of facial bone fractures. We investigated the effect of closed reduction on quality of life. Methods: The 15-dimensional health-related quality of life survey was administered to 120 patients who underwent closed reduction under general anesthesia for nasal bone fractures from February 2018 to December 2019, on both the day after surgery and 3 months after surgery. Three months postoperatively, the presence or absence of five nasal symptoms (nose obstruction, snoring, pain, nasal secretions, and aesthetic dissatisfaction) was also evaluated. Results: The quality of life items that showed significant changes between immediately after surgery and 3 months postoperatively were breathing, sleeping, speech, excretion, and discomfort. Low scores were found at 3 months for breathing, sleeping, and distress. There were 31 patients (25.83%) with nose obstruction, 25 (20.83%) with snoring, 12 (10.00%), with pain, 11 (9.17%) with nasal secretions, and 29 (24.17%) with aesthetic dissatisfaction. Conclusion: Closed reduction affected patients' quality of life, although most aspects improved significantly after 3 months. However, it was not possible to rule out deterioration of quality of life due to complications and dissatisfaction after surgery.

소아의 하악에 발생한 Garre 골수염의 근관치료에 관한 증례보고 (GARRE'S OSTEOMYELITIS OF THE MANDIBLE RESOLVED BY ENDODONTIC TREATMENT IN CHILDREN: A CASE REPORT)

  • Garre's osteomyelitis is a unique form of osteomyelitis characterized rediographically by localized thickening of the periosteum and deposition of laminated subperiosteal bone. The most common inciting factor is a mandibular infection in permanent first molar with necrotic pulp. This disease occurs primarily in children and to date in all instances it has occured only in mandible. It usually results in hard swelling over the jaws, producing facial asymmetry with little or no pain. The overlying skin is normal but can occasionally be inflammed mostly when pain is present. Palpation reveals a usually smooth, bone-hard lesion which feel like an inherent part of the mandible. Unlike other forms of osteomyelitis, there is no marked increase in fever, white bloods cell count, sedimentation rate or alkaline phosphatase value. The treatment of Garre's osteomyelitis usually consist of elimination of the sourses of infection, i.e., either extration of an offending infected teeth or root canal therapy. This treatment almost always results in resolution of the Garre's osteomyelitis. Resistant cases have involved secondary surgery, i.e., decortication and sequestrectomy. This report presents three cases of Garre's osteomyelitis resolved by endodontic treatment. Cliniqtl examination revealed swelling on the face with no tenderness. Periapical radiograph showed deep caries lesion extending into pulp chamber and periapical radiolucency. Occlusal radiograph showed an enlargement of bone and stretching the periosteum. A clinical diagnosis of the Garre's osteomyelitis was made. Endodontic treatment was accomplished with conventional method and restored facial symmetry. Long-term check-ups are necessary to evaluate the results of endodontic treatment.

이하선 종양에 관한 임상적 고찰 (The Clinical Analysis of Parotid Gland Tumors)

  • Parotid tumors constitute about 70 to 80% of all salivary tumors. Two thirds of parotid neoplasms are benign. Women are affected more often than men. Plemorphic adenoma or benign mixed tumor is the most common parotid neoplasm, accounting for 50% of all parotid tumors. The clinical presentation is a discrete, slowly enlarging mass, rarely accompanied by pain or facial paralysis. We reviewed 69 cases of the parotid tumors admitted and treated at Department of Surgery, Kosin University Hospital from Jan, 1970 to June, 1994. The results were as follows: 1) Over all sex ratio was 1 : 1.56(M : F). The sex ratio of benign and malignant tumor was 1 : 1.43(M : F) and 1 : 2.2. 2) The mean duration of symptom was 4.6 years. 3) In the peak incidence of age, Benign tumor was in 4th decade, malignant tumor was in 2nd decade. 4) The chief complaint was painless palpable mass in 65 cases(94%) and pain in 4 cases(6%). 5) The mean size of mass was 2.5cm in diameter and the ratio of lesion site was 37 : 32(Rt : Lt). 6) Superficial parotidectomy was the most common procedure(43%). 7) According to histopathologic findings of 69 cases, Benign tumor was 56 cases(81%) and malignant tumor was 13 cases (19%). In the benign cases, pleomorphic adenoma was the most common(44 cases(65%)). In the malignant, mucoepidermoid ca. was the most common(5 cases (37%)). 8) Postoperative complication occured in 9 cases(13%), facial palsy was in 7 cases, and wound hematoma was 2 cases.

Identification of cranial nerve ganglia using sectioned images and three-dimensional models of a cadaver

  • Background: Cranial nerve ganglia, which are prone to viral infections and tumors, are located deep in the head, so their detailed anatomy is difficult to understand using conventional cadaver dissection. For locating the small ganglia in medical images, their sectional anatomy should be learned by medical students and doctors. The purpose of this study is to elucidate cranial ganglia anatomy using sectioned images and three-dimensional (3D) models of a cadaver. Methods: One thousand two hundred and forty-six sectioned images of a male cadaver were examined to identify the cranial nerve ganglia. Using the real color sectioned images, real color volume model having a voxel size of 0.4 × 0.4 × 0.4 mm was produced. Results: The sectioned images and 3D models can be downloaded for free from a webpage, anatomy.dongguk.ac.kr/ganglia. On the images and model, all the cranial nerve ganglia and their whole course were identified. In case of the facial nerve, the geniculate, pterygopalatine, and submandibular ganglia were clearly identified. In case of the glossopharyngeal nerve, the superior, inferior, and otic ganglia were found. Thanks to the high resolution and real color of the sectioned images and volume models, detailed observation of the ganglia was possible. Since the volume models can be cut both in orthogonal planes and oblique planes, advanced sectional anatomy of the ganglia can be explained concretely. Conclusions: The sectioned images and 3D models will be helpful resources for understanding cranial nerve ganglia anatomy, for performing related surgical procedures.
