A Study on the Usability of Echo Planar Image T2 FLAIR Sequence in the MRI of Acute Ischemic Cerebral Infarction (급성 허혈성 뇌경색 환자의 자기공명영상 검사 시 Echo Planar Image T2 FLAIR 기법의 유용성에 관한 연구)

  • T2 FLAIR sequence of MRI in Acute Ischemic Stroke patients is meaningful to those who have the diagnosis of cerebral hemorrhage and neurological deficiency, including the detection of cerebral infarction around Cerebrospinal fluid. However, because of the long acquisition time and mutative motion artifacts caused by movement, It is difficult to take the MRI image acquisition for non-cooperative patients who need to apply a rapid image acquisition. In this study, we applied the EPI-FLAIR sequence, which is one of the fastest pulse sequences in use, which is a combination of EPI sequence and T2 FLAIR sequence, to patients with acute ischemic cerebral infarction. Based on the qualitative and quantitative evaluation results of the EPI-FLAIR and T2 FLAIR image, we will evaluate the diagnostic usefulness of the EPI-FLAIR sequence.

Clinical Usefulness of MR FLAIR Image in Mild Head Injuries (경증 두부외상 환자에서 MR FLAIR 영상의 임상적 유용성)

  • Objectives : MR fluid-attenuated inversion recovery(FLAIR) image uses paired long inversion time and relaxation time that nulls the signal from CSF. With nulling of the CSF long echo time readout could be used to increase T2-weighting, hence improving the conspicuousness of most tissue lesions without the deleterious effects of CSF artifact seen on T2 weighted sequence. We examed the usefulness of FALIR image in the diagnosis of mild head injury. Methods : A total of 38 patients with mild head injury were examined by FLAIR image. We compared those images with CT scan and T1, T2-weighted images. Careful observation of MR images were done by two well-trained neuroradiologists. Each image was compared for conspicuousness and detectability of traumatic lesions might have shown abnormal signal intensities. The Wilcoxon signed ranks test was used for statistical evaluation. Results : The FLAIR image was significantly more sensitive than those of other images(p<0.001). T2 FFE(Fast Field Echo) image was more useful for detection of small petechial hemorrhages. Conclusion : FLAIR image is considered to be more sensitive than those of conventional MR images in the evaluation of mild head injuries.

Patients with brain metastases the usefulness of contrast-enhanced FLAIR images after delay (뇌전이 환자의 조영 증강 후 지연 FLAIR 영상의 유용성)

  • Purpose: FLAIR image is beneficial for the diagnosis of various bran diseases including ischemic CVS, brain tumors and infections. However the border between the legion of brain metastasis and surrounding edema may not be clear. Therefore, this study aims to investigate the practical benefits of delayed imaging by comparing the image from a patient with brain metastasis before a contrast enhancement and the image 10 minutes after a contrast enhancement. Materials and methods: Of the 92 people who underwent MRI brain metastases in suspected patients 13 people in three patients there is no video to target the 37 people confirmed cases, and motion artifacts brain metastases in our hospital June-December 2013, 18 people measurement position except for the three incorrect patient (male: 11 people, female: 7 people, average age: 60 years) in the target, test equipment, 3.0T MR System (ACHIEVA Release, Philips, I was 8ChannelSENSE Head Coil use Best, and the Netherlands). TR 11000 ms, TE 125 ms, TI2800 ms, Slice Thickness 5 mm, gap 5 mm, is a Slice number 21, the parameters of the 3D FFE, T2 FLAIR variable that was used to test, TR 8.1 ms, TE 3.7 ms, Slice number 240 I set to. The experiment was conducted by acquiring the FLAIR prior to contrast enhancement (heretofore referred to as Pre FLAIR), and acquiring the 3D FFE CE five minutes after the contrast enhancement, and recomposing the images in an axial plane of S/T 3mm, G 0mm (heretofore referred to as MPR TRA CE). Using the FLAIR 10 minutes after the contrast enhancement (heretofore referred to as Post FLAIR) and Pi-View, a retrospective study was conducted. Using MRIcro on the image of a patient confirmed for his diagnosis, the images before and after the contrast media, as well as the CNR and SNR of the MPR TRA CE images of the lesion and the site absent of lesion were compared and analyzed using a one-way analysis of variance. Results: CNR for Pre FLAIR and Post FLAIR were 34.35 and 60.13, respectively, with MPR TRA CE at 23.77 showing no significant difference (p<0.050). Post-experiment analysis shows a difference between Pre FLAIR and Post FLAIR in terms of CNR (p<0.050), but no difference in CNR between Post FLAIR and MPR TRA CE (p>0.050), indicating that the contrast media had an effect only on Pre FLAIR and Post FLAIR. The SNR for the normal site Pre FLAIR was 106.43, and for the lesion site 140.79. Post FLAIR for the normal site was 107.79, and for the lesion site 167.91. MPR TRA CE for the normal site was 140.23 and for the lesion site 183.19, showing significant difference (p<0.050), and post-experiment analysis shows that there was a difference in SNR only on the lesion sites for Pre FLAIR and Post FLAIR (p<0.050). There was no difference in SNR between the normal site and lesion site for Post FLAIR and MPR TRA CE, indicating no effect from the contrast media (p>0.050). Conclusions: This experiment shows that Post FLAIR has a higher contrast than Pre FLAIR, and a higher SNR for lesions, It was not not statistically significant and MPR TRA CE but CNR came out high. Inspection of post-contrast which is used in a high magnetic field is frequently used images of 3D T1 but, since the signal of the contrast medium and the blood flow is included, this method can be diagnostic accuracy is reduced, it is believed that when used in combination with Post FLAIR, and that can provide video information added to the diagnosis of brain metastases.

Benefit of Using Early Contrast-Enhanced 2D T2-Weighted Fluid-Attenuated Inversion Recovery Image to Detect Leptomeningeal Metastasis in Lung-Cancer Staging

  • Purpose: To evaluate the clinical benefit of 2D contrast-enhanced T2 fluid-attenuated inversion recovery (CE-T2 FLAIR) image for detecting leptomeningeal metastasis (LM) in the brain metastasis work-up for lung cancer. Materials and Methods: From June 2017 to July 2019, we collected all consecutive patients with lung cancer who underwent brain magnetic resonance image (MRI), including contrast-enhanced 3D fast spin echo T1 black-blood image (CE-T1WI) and CE-T2 FLAIR; we recruited clinico-radiologically suspected LM cases. Two independent readers analyzed the images for LM in three sessions: CE-T1WI, CE-T2 FLAIR, and their combination. Results: We recruited 526 patients with suspected lung cancer who underwent brain MRI; of these, we excluded 77 (insufficient image protocol, unclear pathology, different contrast media, poor image quality). Of the 449 patients, 34 were clinico-radiologically suspected to have LM; among them, 23 were diagnosed with true LM. The calculated detection performance of CE-T1WI, CE-T2 FLAIR, and combined analysis obtained from the 34 suspected LM were highest in the combined analysis (AUC: 0.80, 0.82, and 0.89, respectively). The inter-observer agreement was also the highest in the combined analysis (0.68, 0.72, and 0.86, respectively). In quantitative analyses, CNR of CE-T2 FLAIR was significantly higher than that of CE-T1WI (Wilcoxon signed rank test, P < 0.05). Conclusion: Adding CE-T2 FLAIR might provide better detection for LM in the brain-metastasis screening for lung cancer.

  • Purpose : Our aim was to evaluate the usefulness of fluid-attenuated inversion recovery (FLAIR) MR imaging for detection of acute subarachnoid hemorrhage (SAH) compared with unenhanced CT. Materials and methods ; We compared FLAIR MR images with unenhanced CT scans in 28 patients with acute SAH. Findings of SAH on CT and MR images were graded as 0 (absence), 1 (suspicious), 2 (definite) in the cerebral sulci, sylvian fissure, basal cistern, and cisterns of the posterior fossa. We also compared FLAIR MR images of 28 patients with those of 35 normal subjects, and then the sensitivity, specificity, and diagnostic accuracy of FLAIR MR image for detection of acute SAH were calculated. Results : FLAIR MR image was superior to CT in detecting SAH in the posterior fossa ($1.41{\pm}0.74{\;}vs{\;}0.78{\pm}0.80$; p<0.05) and cortical sulci ($1.11{\pm}0.80{\;}vs{\;}0.70{\pm}0.83$; p<0.05). There was no significant difference between FLAIR MR image and CT in detecting SAH in the basal cistern and sylvian fissure. The sensitivity, specificity, and diagnostic accuracy of FLAIR MR image for detection of SAH were 100% in all. Conclusion : FLAIR MR image is useful in detecting acute SAH, especially in patients with small amount of SAH or SAH in the posterior fossa.

  • T1-, and T2-weighted imagings and FLAIR (fluid attenuated inversion recovery) imaging are fundamental imaging methods in the brain. T1-weighted imaging is a spin-echo sequence with short TR and short TE and produces the tissue contrast by different T1 relaxation times. In other words, short TR maximizes the difference of the longituidinal magnetization recovery between the tissues. T2-weighted imaging is a spin-echo sequence with long TR and long TE and produces the tissue contrast by different T2 relaxation times. Long TE maximizes the difference of the transverse magnetization decay between the tissues. FLAIR is an inversion recovery sequence using 180 degree inversion pulse. 2500 msec of inversion time is applied to suppress the CSF signal.

Image Comparison of Heavily T2 FLAIR and DWI Method in Brain Magnetic Resonance Image (뇌 자기공명영상에서 Heavily T2 FLAIR와 DWI 기법의 영상비교)

  • The purpose of this study is to obtain brain MRI images through Heavenly T2 FLAIR and DWI techniques to find out strengths and weaknesses of each image. Data were analyzed on 13 normal people and 17 brain tumor patients. Philips Ingenia 3.0TCX was used as the equipment used for the inspection, and 32 Channel Head Coil was used to acquire data. Using Image J and Infinity PACS Data, 3mm2 of gray matter, white matter, cerebellum, basal ganglia, and tumor areas were set and measured. Quantitative analysis measured SNR and CNR as an analysis method, and qualitative analysis evaluated overall image quality, lesion conspicuity, image distortion, susceptibility artifact and ghost artifact on a 5-point scale. The statistical significance of data analysis was that Wilcox-on Signed Rank Test and Paired t-test were executed, and the statistical program used was SPSS ver.22.0 and the p value was less than 0.05. In quantitative analysis, the SNR of gray matter, white matter, cerebellum, basal ganglia, and tumor of Heavily T2 FLAIR is 41.45±0.13, 40.52±0.45, 41.44±0.51, 40.96±0.09, 35.28±0.46 and the CNR is 15.24±0.13, 16.75±0.23, 16.28±0.41, 15.83±0.17, 16.63±0.51. In DWI, SNR is 32.58±0.22, 36.75±0.17, 30.21±0.19, 35.83±0.11, 43.29±0.08, and CNR is 13.14±0.63, 14.21±0.31, 12.95±0.32, 11.73±0.09, 17.56±0.52. In normal tissues, Heavenly T2 FLAIR obtained high results, but in disease evaluation, high results were obtained at DWI, b=1000 (p<0.05). In addition, in the qualitative analysis, overall image quality, lesion conspicuity, image distortion, susceptibility artifact and ghost artifact aspects of the Heavily T2 FLAIR were evaluated, and 3.75±0.28, 2.29±0.24, 3.86±0.23, 4.08±0.21, 3.79±0.22 values were found, respectively, and 2.53±0.39, 4.13±0.29, 1.90±0.20, 1.81±0.21, 1.52±0.45 in DWI. As a result of qualitative analysis, overall image quality, image distortion, susceptibility artifact and ghost artifact were rated higher than DWI. However, DWI was evaluated higher in lesion conspicuity (p<0.05). In normal tissues, the level of Heavenly T2 FLAIR was higher, but the DWI technique was higher in the evaluation of the disease (tumor). The two results were necessary techniques depending on the normal site and the location of the disease. In conclusion, statistically significant results were obtained from the two techniques. In quantitative and qualitative analysis, the two techniques had advantages and disadvantages, and in normal and disease evaluation, the two techniques produced useful results. These results are believed to be educational data for clinical basic evaluation and MRI in the future.

Analysis of Signal Intensity in Choroid Plexuses by Diffusion Weighted Imaging (확산강조영상의 검사기법에 따른 맥락얼기의 신호강도 분석)

  • This report aims at picturing out the clinical usefulness by analyzing the signal intensity in choroid plexuses which produce cerebrospinal fluids by diffusion weighted imaging. At first, subjects were chosen among the patients who showed high in signal intensity by diffusion weighted imaging. The subjects were taken another test by fluid attenuated inversion recovery diffusion weighted echo planer image(FLAIR-DW-EPI) the signals of fluid attenuation. And it was found that there are differences between the signal intensities of the two methods, which showed that the signal intensity in FLAIR-DW-EPI is equal to or low than, that in the Brain. By this, it is felt that it is helpful to diagnose the disease in choroid plexus by testing another more with FLAIR-DW-EPI methods the patients who showed high in signal intensity in choroid plexus by $T2^*$ diffusion weighted echo planer image($T2^*$-DW-EPI).

The Relationship between Image Parameters and SAR for Each Sequence of MRI (MRI 검사의 시퀀스 별 영상 변수와 SAR의 관계)

  • This study analyzed the relationship between image parameters and specific absorption rate (SAR) in various sequence environments to optimize SAR. For this purpose, image parameters were adjusted for T2, T1, STIR, T1 FLAIR, and T2 FLAIR sequences in a 3.0T MRI, and the whole body (WB) SAR and head SAR calculated by the device were measured. Then, the SAR was evaluated by adjusting the number of images and the flip angle (FA) of the refocusing RF. As a result, SAR increased as the number of image increased in all sequences. T1 and T1 FLAIR had correlation coefficients (r) of 0.876, 0.876 (WB SAR, head SAR), 0.867, 0.867 (WB SAR, head SAR), respectively, and STIR had the highest correlation with 0.898 and 0.899 (WB SAR, head SAR). showed (p<0.05). When applied by increasing the refocusing FA, WB SAR and head SAR increased overall in all sequences. The T1 and T2 sequences showed high correlation with correlation coefficients (r) of 0.897, 0.898 (WB SAR, head SAR) and 0.914, 0.915 (WB SAR, head SAR), respectively, while the sequences to which the inversion recovery technique was applied had relatively low FA, showed less sensitivity to increase. Therefore, in a sequence with a relatively low TR, minimizing the number of image and applying the fast spin echo to reduce the refocusing FA in a sequence with a high duty cycle are effective in reducing SAR.

Quantitative Analysis of MR Image in Cerebral Infarction Period (뇌경색 시기별 MR영상의 정량적 분석)

  • In this study, we showed a comparison and analysis making use of DWI(diffusion weighted image) using early diagnosis of cerebral Infarction and with the classified T2 weighted image, FLAIR images signal intensity for brain infarction period. period of cerebral infarction after the condition of a disease by ischemic stroke. To compare 3 types of image, we performed polynomial warping and affined transform for image matching. Using proposed algorithm, calculated signal intensity difference between T2WI, DWI, FLAIR and DWI. The quantification values between hand made and calculated data are almost the same. We quantified the each period and performed pseudo color mapping by comparing signal intensity each other according to previously obtained hand made data, and compared the result of this paper according to obtained quantified data to that of doctors decision. The examined mean and standard deviation for each brain infarction stage are as follows ; the means and standard deviations of signal intensity difference between DWI and T2WI for each period are $197.7{\pm}6.9$ in hyperacute, $110.2{\pm}5.4$ in acute, and $67.8{\pm}7.2$ in subacute. And the means and standard deviations of signal intensity difference between DWI and FLAIR for each period are $199.8{\pm}7.5$ in hyperacute, $115.3{\pm}8.0$ in acute, and $70.9{\pm}5.8$ in subacute. We can quantificate and decide cerebral infarction period objectively. According to this study, DWI is very exact for early diagnosis. We classified the period of infarction occurrence to analyze the region of disease and normal region in DW, T2WI, FLAIR images.
