A Distributed Indexing Scheme for Wireless Data Broadcasting of Health Information FHIR Resources (의료 정보 FHIR 리소스 무선 데이터 방송을 위한 분산 인덱싱 기법)

  • FHIR, next-generation standard for health information exchange, allows to exchange health information fast and to provide various health services. In this paper, we propose an indexing scheme of FHIR resources for adopting the resources to wireless data broadcasting with a secure channel. That scheme keeps the information of users to support to download FHIR resources from the secure wireless broadcast channel and the information on the resources. Using the proposed index, massive users can download their desired FHIR resources with less energy in short time. With simulation studies, we show the proposed indexing scheme outperforms other scheme broadcasting FHIR resources.

Design of a Data Grid Model between TOS and HL7 FHIR Service for the Retrieval of Personalized Health Resources (개인화된 건강 자원 조회를 위한 TOS 와 HL7 FHIR 서비스간의 데이터그리드 모델 설계)

  • On the ICT healing platform designed to issue early disease alerts, TOS connected between the provider of personal health-related data and the service provider and relayed personalized health data. In the previous study, TOS proposed how to monitor the retrieval and management of document/measurement resources by taking mobile devices into account. Recently the healthcare field, however, defined the standard items needed for communication and data exchanges with a mobile device through HL7 FHIR. This study designed a data grid model between TOS and FHIR to provide personal health resources relayed through TOS in FHIR bundle search sets. The proposed design was organized as follows: first, it stated similarities between the method of TOS resource request and that of FHIR observation request. Then, it designed an eventbus module to process a retrieval request for FHIR service based on the imdb and cluster technologies. The proposed design can be used to expand the old service terminals of ICT healing platform to mobile health devices capable of using FHIR resources.

  • HL7 released V3 CDA(Clinical Document Architecture) and V2.x message standards for medical information exchange. Currently, these standards are successfully adopted by a number of nations across the globe. However, substantial amount of time is required to develop and implement these standards. Moreover, developers need a lot of time to understand these standards. To solve these issues from 2011, the HL7 standard framework started to discuss Fast Healthcare Interoperability Resources(FHIR) as next generation standard of healthcare information exchange. People's interests toward personal health record and smartphone penetration rate are growing and increasing rapidly. Therefore, our research team believes it is necessary to develop a PHR profiling system which could be accessed by using a smartphone and we developed the system. Through a FHIR Profile editor tool developed in Furore, we found that improvements could be made in generating and changing the profile. In order to build the PHR Profiling system, an Open-API on FHIR is used for exchanging information between electronic medical record system and PHR Profiling system. In the PHR Profiling system, the transactions of information between two systems are provided by RESTful service. In this study, we verify the efficiency of development of the PHR Profiling system through FHIR.

  • FHIR is a protocol that enables easy data exchange from the time the event occurred in health care settings as a standard for next-generation message exchange of HL7. Create a meaningful message from the ECG and medical equipment, and express the messages generated by standardized FHIR message it will be used in various medical institutions to ensure delivery to EMR, such as hospital information systems can query the results via a smartphone. In addition, it thought to be used in the future to expand SMART on FHIR, Cardiology, Cardiovascular Surgery ward in real-time remote monitoring.

  • Health Level Seven(HL7)에서 V2.x, V3 보다 향상된 FHIR(Fast Healthcare Interoperabilty Resources)를 표준을 개발하고 있고, 이를 채택한 솔루션 개발이 활발하고, 각 국가 의료환경에서의 검증이 필요하다. 현재 미국에서 성공적으로 널리 사용되는 V2.x Message와 V3 Clinical Document Architecture(CDA)가 존재 하지만, 이 기술 표준들을 개발하고 사용하는데 긴 시간 투자의 문제점이 있다. 현재 V4로 불리우는 개선된 FHIR를 사용함으로 이러한 문제점을 해결할 방법인지 확인한다. 개인건강기록 관리 또한 사회적인 관심을 끌고 있고, 스마트폰 보급률이 급격히 증가하는 함을 반영하여 개인건강기록 관리 시스템을 스마트폰으로 접속 가능한 시스템으로 개발한다. 이를 구축하기 위해서는 Electronic Medical Record(EMR) 시스템과 Personal Health Profiling(PHP) 시스템간의 정보교류를 FHIR Open API로 구성한다. PHP 시스템에서는 이들 트랜잭션을 RESTful 서비스로 제공한다. 본 연구에서는 FHIR기반의 PHP 시스템을 통해 개인건강관리 시스템의 효율성을 검증하고자 한다.

  • 최근 의료데이터 표준화에 대한 중요성이 보건의료 빅데이터 구축과 맞물려 보건의료데이터 표준화와 마이데이터 생태계 조성을 추진하고 있다. 그리고 개인들의 휴대용 기기 이용증가와 모바일 환경으로 전반적인 디지털헬스의 패러다임 변화에 따라 HL7 FHIR의 사용이 점차 확대될 것으로 예측된다. 본 논문에서는 의료정보 표준인 HL7 FHIR와 의료영상 표준인 DICOM으로 환자 정보를 전달하기 위한 다중 의료 정보 중재 플랫폼에 대해서 기술한다. 이를 구현하기 위해 HL7 FHIR의 Patient, Observation, DiagnosticReport, Bundle 리소스를 활용하여 환자 정보와 임상 리포트 정보를 전달하여 StudyList에서 보여줄 수 있도록 구현하였다. 현재 구현된 내용은 FHIR 기반의 임상데이터로 의료영상을 포함한 표준화된 정보로 제공하여 마이데이터 실증 플랫폼으로 활용될 것으로 기대된다.

  • For improving interoperability of medical information, health level 7 has initiated the development of a next-generation framework for the exchange of medical information called the Fast health interoperability resources (FHIR). However, there was no attempt to exchange the medical three-dimensional (3D) image with clinical data via FHIR. Thus, we designed a new method. The 3D image to be made from computed tomography was converted to the javascript object notation (JSON) file format, and clinical data was added. We made a test FHIR server, and the client used the postman. The JSON file was attached to the body, and was then transmitted. The transmitted 3D image could be seen through a web browser, and attached clinical data was identified in the source code. This is the first attempt to exchange the medical 3D image. Additional researches will be needed to develop applications or FHIR resources that apply this method.

  • 본 논문에서는 FHIR 인터페이스를 통해 환자에 대한 정보를 사전에 습득하여 효율적인 응급구조 활동을 지원하기 위해 FHIR를 통해 제공되는 정보의 특성을 파악하고 수집 방법을 설계한다. 환자정보를 포함한 의료정보 시스템과 연동된 FHIR 서버에서 Condition 자원을 통해 환자의 현재 유병상태를, MedicationRequest 자원을 통해 약처방 정보를 습득할 수 있는데 HAPI 및 Synthea에서 제공하는 시험 데이터를 통해 필요한 필드를 얻는 과정을 구현한다.

  • Hypertension is one of the major causes of death in the world as it is related with cardiovascular or cerebrovascular disease, so it is needed to provide continuos management for blood pressure. This study selected Health Level 7 Fast Health Interoperability Resources (HL7 FHIR) as a bio-signal data exchange service model that can provide constant blood pressure management in the rapidly growing mobile health care environment. The HL7 FHIR framework developed communicates with the IEEE 11073-10407 Personal Health Device (PHD) protocol through the bluetooth Health Device Profile (HDP) between the manager (smart phone) and the agent (hemomanometer) and acquires information about blood pressure. According to the test results, it performed its tasks successfully including hypertension patients' blood pressure monitoring, management on measured records, generation of document, or transmission of measured information. Because in the actual, clinical environment, it is possible to transmit measured information through the TCP/IP protocol, it will be needed to conduct constant research on it and vitalize it in the field of mobile health care afterwards.
