F-18 FDG PET/CT로 재구성한 담관암의 3차원 영상 (Three Dimensional Volume Rendering Fusion Images Using F-18 FDG PET/CT in Evaluation of Cholangiocellular Carcinoma)

  • A 69-year old male with cholangiocellular carcinoma (CCC) was assigned to our department for whole body PET/CT scan. $^{18}F$-FDG PET/CT images showed an intense hypermetabolic lobulating mass(SUVmax = 8.7 / size : 11.4 mm) in the right hepatic lobe with multiple metastatic lung nodules. We made three dimensional volume rendering fusion images by using advantage workstation 4.3 (GE health care) which provide quick anatomic overview and improve the planning process significantly.

  • Benign metastasizing leiomyoma(BML) is a rare condition affecting women with a history of having undergone hysterectomy or myomectomy for a benign uterine fibroid, that is found to have metastasized to extrauterine sites, usually many years after hysterectomy. 1,2) Patient with BML almost always asymptomatic, although if the lesions are large enough, they can cause compressive symptoms. Among several hypothesis of pathogenesis, most plausible theory is that these tumors represent a true metastatic lesion but are very low-grade sarcoma. 3) Because the tumor is responsive to estrogen, menopause and pregnancy have slowed the growth of these lesion 4) and it seems reasonable to perform hysterectomy in patients with a uterine mass and, at the same time, perform oophorectomy for hormonal control. BML is an unusual cause of diffuse pulmonary nodules which should be considered in females with unexplained nodules and a history of surgery for uterine leiomyoma.

  • Radiolysis is the process of decreasing in Radio-Chemical Purity (RCP) of [$^{18}F$]FDG by direct effect and indirect effect of self Radio-activity. The objective of our study was to figure out the ideal conditions which minimize damages of quality of [$^{18}F$]FDG using radical scavenger and controlling temperature of storage.

  • Infections involving the heart are becoming increasingly common, and a timely diagnosis of utmost importance, despite its challenges. F-18 fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT) is a recently introduced diagnostic tool in cardiology. This review focuses on the current evidence for the use of FDG PET/CT in the diagnosis of infective endocarditis, cardiac implantable device infection, left ventricular assist device infection, and secondary complications. The author discusses considerations when using FDG PET/CT in routine clinical practice, patient preparation for reducing physiologic myocardial uptake, acquisition of images, and interpretation of PET/CT findings. This review also functions to highlight the need for a standardized acquisition protocol.

  • Whole-body positron emission tomography (PET) imaging with 18-F deoxyglucose (FDG) is a molecular imaging modality that detects metabolic alteration in tumor cells. In various human cancers, FDG-PET shows a potential clinical benefit in screening, tumor characterization, staging, therapeutic follow-up and detecting recurrence. In gynecologic cancers, FDG-PET is also known to be effective in characterization of adnexal masses, detection of recurrence, and lymph node invasion. This review discusses the clinical feasibility and future clinical application of this imaging modality in patients with cervical cancer, ovarian cancer, and other gynecologic cancers.

폐암과 폐 염증성질환의 동적양전자방출단층검사 (Dynamic $^{18}F-FDG$ PET)의 유용성 (Usefulness of Dynamic $^{18}F-FDG$ PET Scan in Lung Cancer and Inflammation Disease)

  • 목 적: 양전자방출단층검사(Fluorine-18 2-Deoxy-D-Glucose positron emission tomography : $^{18}F-FDG$ PET)는 가시적 영상분석과 반정량적 섭취계수 분석 방법을 사용하여 폐의 염증성 질환과 폐암을 구별 및 진단하는 데 유용하다. 일반적으로 표준화섭취계수(Standardized Uptake Value : SUV)가 폐 질환의 진단에 사용되지만, 이는 폐 조직에 있어서 반정량적, 정량적 분석을 통한 동적인 정보를 포함하지 않는다. 그러므로, 본 연구는 $^{18}F-FDG$ PET 검사에서 폐 질환의 동적 섭취 분석이 보다 정확한 대상질환의 진단을 유용하게 할 수 있을 것이라는 가정하에 시간-방사능 곡선(Time-Activity Curve: TAC), 표준화섭취계수-동적곡선(Standardized Uptake Value-Dynamic Curve : SUV-DC), 패트락 분석법(Patlak analysis : Glucose Metabolic Rate(MRGlu))을 토대로 얻은 분석방법을 이용하여 진단적 유용성을 평가하였다. 대상 및 방법: 폐 질환을 가진 17명의 환자를 대상을 하여, 각각 60분간 Dynamic $^{18}F-FDG$ PET검사를 시행하였다. 획득한 정보를 통하여 폐질환의 형태를 따라 관심영역(Region of Interest: ROI)를 그린 후, 반정량적 분석인 TAC, SUV-DC와 정량적 분석인 Patlak analysis를 각각의 군에 따라 분석하여 비교 분석하였다. 결 과: Dynamic $^{18}F-FDG$ PET을 통한 분석결과, TAC 형태는 초기 혈류상에서 폐의 염증성 질환과 폐암의 구분이 어려웠으나, 시간이 지날수록 폐암의 곡선이 염증성 질환의 경우보다 확연히 증가하였다. SUV-DC의 경우는 TAC 형태와 거의 유사한 형태를 가졌다. Patlak analysis 분석결과, 대동맥 영역에서는 폐의 염증성 질환이 폐암보다 높은 혈액 방사능을 보였으나 시간이 지남에 따라 염증성 질환의 혈액 방사능이 극히 낮아졌다. 반면, 병변 조직에서는 폐암이 가장 높은 섭취를 보였으며 폐의 염증성 질환은 중간 정도에 머물렀다. 결 론: TAC와 SUV-DC 분석에서 폐암과 폐의 염증성 질환은 각각 고유한 그래프 형태를 띠었으며 Patlak analysis에서 역시 주목할 만한 차이를 보였다. 따라서 이러한 분석법을 토대로 좀더 깊이 있는 연구가 진행된다면 비침습적으로 폐의 질환을 구별하는 데 보다 적절하고 유용한 진단적 정보를 얻을 수 있을 것이다.

자궁경부암에서 $^{18}F-FDG$ PET의 임상 이용 (Clinical Application of $^{18}F-FDG$ PET in Cervix Cancer)

  • Cervix cancer is one of common gynecological cancers in the world, and staged with FIGO or TNM system. However, these clinical staging systems lack information about lymph node or distant metastases, thus imaging modalities are considered to make an appropriate therapeutic plan and enhance overall survival rate. In this context, FDG PET is recommended to pre-treatment stating and prognosis prediction, for it could noninvasively evaluate the status of lymph nodes, especially abdominal paraaortic nodes which are closely related with prognosis. Moreover, the degree of FDG uptake is correlated with prognosis. Although there is no consistent method for surveillance of cervix cancer, FDG PET seems a very important tool in detecting tumor recurrence because it is much more advantageous than conventional imaging modalities such as MRI for discerning recurrent tumor from fibrosis caused by radiation or surgery. Furthermore, FDG PET could be used to evaluate treatment response. On the other hand, recently introduced PET/CT is expected to play an ancillary role to FIGO staging by adding anatomical information, and enhance diagnostic performance of PET by decreasing false positive findings.

Correlation between glucose transporter type-1 expression and $^{18}F$-FDG uptake on PET in oral cancer

  • Objectives: Fluorine-18 fluorodeoxyglucose positron emission tomography ($^{18}F$-FDG PET) is a non-invasive diagnostic tool for many human cancers wherein glucose uptake transporter-1 (GLUT-1) acts as a main transporter in the uptake of $^{18}F$-FDG in cancer cells. Increased expression of glucose transporter-1 has been reported in many human cancers. In this study, we investigated the correlation between $^{18}F$-FDG accumulation and expression of GLUT-1 in oral cancer. Materials and Methods: We evaluated 42 patients diagnosed with oral squamous cell carcinoma (OSCC) and malignant salivary gland tumor as confirmed by histology. 42 patients underwent pre-operative $^{18}F$-FDG PET, with the maximum standardized uptake value ($SUV_{max}$) measured in each case. Immunohistochemical staining was done for each histological specimen, and results were evaluated post-operatively according to the percentage (%) of positive area, intensity, and staining score. Results: For OSCC, $SUV_{max}$ significantly increased as T stage of tumor classification increased. For malignant salivary gland tumor, $SUV_{max}$ significantly increased as T stage of tumor classification increased. For OSCC, GLUT-1 was expressed in all 36 cases. GLUT-1 staining score (GSS) increased as T stage of tumor classification increased, with the difference statistically significant. For malignant salivary gland tumor, GLUT-1 expression was observed in all 6 cases; average GSS was significantly higher in patients with cervical lymph node metastasis than that in patients without cervical lymph node metastasis. Average GSS was higher in OSCC ($11.11{\pm}1.75$) than in malignant salivary gland tumor ($5.33{\pm}3.50$). No statistically significant correlation between GSS and $SUV_{max}$ was observed in OSCC or in malignant salivary gland tumor. Conclusion: We found no statistically significant correlation between GSS and $SUV_{max}$ in OSCC or in malignant salivary gland tumor. Studies on the various uses of GLUT during $^{18}F$-FDG uptake and SUV and GLUT as tumor prognosis factor need to be conducted through further investigation with large samples.

Prognostic significance of sequential 18F-FDG PET/CT during frontline treatment of peripheral T cell lymphomas

  • Background/Aims: The prognostic significance of 18F-fluorodeoxyglucose (FDG)-positron emission tomography-computed tomography (PET/CT) in peripheral T-cell lymphomas (PTCLs) are controversial. We explored the prognostic impact of sequential 18F-FDG PET/CT during frontline chemotherapy of patients with PTCLs. Methods: In total, 143 patients with newly diagnosed PTCLs were included. Sequential 18F-FDG PET/CTs were performed at the time of diagnosis, during chemotherapy, and at the end of chemotherapy. The baseline total metabolic tumor volume (TMTV) was calculated using the the standard uptake value with a threshold method of 2.5. Results: A baseline TMTV of 457.0 cm3 was used to categorize patients into high and low TMTV groups. Patients with a high TMTV had shorter progression-free survival (PFS) and overall survival (OS) than those with a low TMTV (PFS, 9.8 vs. 26.5 mo, p = 0.043; OS, 18.9 vs. 71.2 mo, p = 0.004). The interim 18F-FDG PET/CT response score was recorded as 1, 2-3, and 4-5 according to the Deauville criteria. The PFS and OS showed significant differences according to the interim 18F-FDG PET/CT response score (PFS, 120.7 vs. 34.1 vs. 5.1 mo, p < 0.001; OS, not reached vs. 61.1 mo vs. 12.1 mo, p < 0.001). Conclusions: The interim PET/CT response based on visual assessment predicts disease progression and survival outcome in PTCLs. A high baseline TMTV is associated with a poor response to anthracycline-based chemotherapy in PTCLs. However, TMTV was not an independent predictor for PFS in the multivariate analysis.

$^{18}F$-FDG를 이용한 전신 PET 검사에서 혈당 측정 검사지의 정확한 사용에 따른 혈당 수치의 차이 분석 (The Analysis of Blood Glucose Level Difference According to the Exact Use of Blood Glucose Measurement Test Strips in $^{18}F$-FDG Wholebody PET)

  • $^{18}F$-FDG PET 검사는 포도당 대사를 이용하여 전신의 종양을 평가하는 검사로 전처치 과정에서 혈당 측정의 중요성이 매우 높다. 혈당 수치가 높은 상태에서 FDG를 투여하면 FDG와 혈중 포도당이 세포 내 흡수에 서로 경쟁하게 되고 종양으로의 FDG 섭취가 감소되어 정확한 검사가 되지 않을 가능성이 있다. 혈당 측정에 사용되는 혈당 측정 검사지는 경우에 따라 모세혈관혈 전용 측정 검사지와 모세혈관혈과 정맥혈을 모두 측정 가능한 범용 측정 검사지로 구분되어 있는데, 정확한 혈당 측정 검사지(스트립)의 사용에 따른 혈당 수치의 차이를 비교 분석 하였다. 연구 대상은 2010년 3월부터 4월까지 서울아산병원 핵의학과에서 $^{18}F$-FDG whole body PET 검사를 시행한 환자 중 46명(남자: 32명, 여자: 14명, 평균연령 $57.3{\pm}12.3$세) 을 연구의 대상으로 하였다. 실험기기는 Abbott사의 Optium Xceed혈당 측정기를 이용하였고, 모세혈관혈과 정맥혈을 모두 측정 가능한 범용 측정 검사지를 사용하여 동일 환자의 모세혈관혈과 정맥혈의 혈당 수치의 차이를 분석하였다. 또한, 모세혈관혈 전용 측정 검사지를 사용하여 동일 환자의 모세혈관혈과 정맥혈의 혈당 수치의 차이를 비교 분석하였다. 분석방법은 MINITAB 통계 프로그램을 이용하여 paired t-test 검정을 시행하였다. 모세혈관혈과 정맥혈을 모두 측정 가능한 범용 측정 검사지로 측정한 동일 환자의 모세혈관혈과 정맥혈의 혈당 수치는 각각$95.2{\pm}12.4$ mg/dL, $104.1{\pm}14.4$ mg/dL (t=8.0, p<0.001)로 통계적으로 유의한 차이가 있었다. 모세혈관혈 전용 측정 검사지로 측정한 동일 환자의 모세혈관혈과 정맥혈의 혈당 수치는 각각 $91.5{\pm}13.6$ mg/dL, $108.1{\pm}16.2$ mg/dL (t=9.2, p<0.001)로 통계적으로 유의한 차이가 있었다. 선행연구에 의하면 모세혈관혈과 정맥혈의 혈당 수치는 여러 가지 이유로 인해 차이가 있는 것으로 보고 되었고 본 연구의 결과도 유사한 차이를 확인하였다. $^{18}F$-FDG PET 검사 전에 혈당 수치를 측정할 때 모세혈관혈 전용 측정 검사지로 정맥혈의 혈당을 측정하는 것은 부정확한 혈당 수치를 측정할 수 있으므로 주의해야 할 것이다. 그러므로 적합한 측정 검사지와 규정된 측정절차를 이행하는 것이 측정 편차를 줄일 수 있어 PET 검사의 전처치를 보다 명확하게 확인할 수 있을 것이다. 또한, 혈당 측정 시에 혈액 채취 부위를 모세혈관 또는 정맥혈관 중에서 항상 동일한 부위에서 측정하는 등의 표준화 과정이 필요할 것이다.
