  • Background: The present study investigated the radiation dose distribution of balloon kyphoplasty (BKP) among surgeons and medical staff, and this is the first research to observe such exposure in Japan. Materials and Methods: The study subjects were an orthopedic surgeon (n = 1) and surgical staff (n = 9) who intervened in BKP surgery performed at the National Hospital Organization Disaster Medical Center (Tokyo, Japan) between March 2019 and October 2019. Only disposable protective gloves (0.022 mmPb equivalent thickness or less) and trunk protectors were used, and no protective glasses or thyroid drapes were used. Results and Discussion: The surgery time per vertebral body was 36.2 minutes, and the fluoroscopic time was 6.8 minutes. The average exposure dose per vertebral body was 1.46 mSv for the finger (70 ㎛ dose equivalent), 0.24 mSv for the lens of the eye (3 mm dose equivalent), 0.11 mSv for the neck (10 mm dose equivalent), and 0.03 mSv for the chest (10 mm dose equivalent) under the protective suit. The estimated cumulative radiation exposure dose of 23 cases of BKP was calculated to be 50.37 mSv for the fingers, 8.27 mSv for the lens, 3.91 mSv for the neck, and 1.15 mSv for the chest. Conclusion: It is important to know the exposure dose of orthopedic surgeons, implement measures for exposure reduction, and verify the safety of daily use of radiation during surgery and examination.

  • The immune responses of commercial layer chickens against Newcastle disease (ND) were compared among different administration methods and times of vaccination during 4 weeks of age. A total of 372 day-old chickens were devided into 4 groups of 93 birds each. Each of 3 groups was received a commercially available B$_1$ live vaccine via drinking water, eye instillation or spray method at one, 14 and 28 days of age. One group was used as an unvaccinated control. At two and 4 weeks after each time of vaccination, 15 birds from each group were challenged with virulent ND virus at the dose of 10$^{5}$ EID$_{50}$ per bird to examine the pretection rate. Ten to 15 birds from each group were bled at two weeks intervals from day old to 8 weeks of age to determine hemagglutination inhibition antibody titer. The protection rate was generally low regardless of the times of vaccination although two or more times vaccination gave higher protection than once vaccination. The low protection was considered due to low titer of the vaccine used since the vaccine titer was less than 10$^{2.5}$ EID$_{50}$ per bird. Spray method gave better protection compared to eye instillation of drinking water method which resulted in lowest response. When birds were challenged majority showed clinical signs on ND between 3 and 6 days after challenge. Death occured one or two days after onset of symptoms. Major clinical signs observed were depression (96%), drowsy(90%), anorexia (84%), diarrhoea (29%), difficult breath (15%) and torticollis (10%). Hemorrhagic lesions on post mortem were seen in duodenum (51%), trachea(36%), illeum (13%), ceacal tonsil (11%), proventriculus (10%) and some other organs. • In recent years, the safety of sunscreens have been challenged based on the reports of its adverse effects on users; dermatitis, allergic contact dermatitis, photoallergic contact dermatitis. In the toxicological aspect, the need to investigate the safety of sunscreens is steadily increasing. However, there were few studies on the correlation between sun protection factor (SPF) and the safety of sunscreens. The objective of this study was to assess whether there was a correlation between SPF and the skin irritation index of sunscreens. Octyl methoxycinnamate (OMC) was selected, which was the most frequently used (about 92% in korea and about 90% in the world) in sunscreens. In vitro SPF test was performed using the standard samples recommended by KFDA guideline. We examined primary and cumulative skin irritation and eye irritation of OMC standard sample as well as homosalate. As in vitro SPF was increased, skin irritation index was increased in primary skin irritation test ($r^2$=0.986) and cumulative skin irritation test ($r^2$=0.939). In addition, OMC was non-irritating and non-sensitizing. In this results, there was a good correlation between SPF index and skin irritation index.

  • The human eye is exposed to UV and visible light. UV light exposure becomes harmful to the eye. Protection for eyes should block all ultraviolet rays. In our study, organic-inorganic hybrid materials have been applied to UV blocking coatings with UV absorbing materials on transparent plastics. The optical properties of UV blocking coatings were investigated in PMMA, CR 39 and PC substrates. In case of all UV absorbers, the transmission of UV light decreases with an increased amount of absorber. Our findings indicate that PMMA significantly reduced the transmission of UV radiation, CR 39 showed moderate decrease, while UV-uncoated PC had some UV blocking properties. Adhesion, hot water resistance and chemical resistance of the UV-coated CR 39 lenses were good. Pencil hardness were 4H. Abrasion resistance were poor.

  • BACKGROUND/OBJECTIVE: Centella asiatica, also known as Gotu kola, is a tropical medicinal plant native to Madagascar, Southeast Asia, and South Africa. It is well known to have biological activities, including wound healing, anti-inflammatory, antidiabetic, cytotoxic, and antioxidant effects. The purpose of this study was to determine the efficacy of extracts of C. asiatica against age-related eye degeneration and to examine their physiological activities. MATERIALS/METHODS: To determine the effects of CA-HE50 (C. asiatica 50% EtOH extract) on retinal pigment cells, we assessed the cytotoxicity of CoCl2 and oxidized-A2E in ARPE-19 cells and observed the protective effects of CA-HE50 against N-methyl-N-nitrosourea (MNU)-induced retinal damage in C57BL/6 mice. In particular, we measured factors related to apoptosis and anti-oxidation and the protein levels of rhodopsin/opsin. We also measured glucose uptake to characterize glucose metabolism, a major factor in cell protection. RESULTS: Induction of cytotoxicity with CoCl2 and oxidized-A2E inhibited decreases in the viability of ARPE-19 cells when CA-HE50 was administered, and promoted glucose uptake under normal conditions (P < 0.05). In addition, CA-HE50 inhibited degeneration/apoptosis of the retina in the context of MNU-induced toxicity (P < 0.05). In particular, CA-HE50 at 200 mg/kg inhibited the cleavage of pro-caspase-3 and pro-poly (ADP-ribose)-polymerase and maintained the expressions of nuclear factor erythroid 2-related factor 2 and heme oxygenase-1 similar to normal control levels. Rhodopsin/opsin expression was maintained at a higher level than in normal controls. CONCLUSION: A series of experiments confirmed that CA-HE50 was effective for inhibiting or preventing age-related eye damage/degeneration. Based on these results, we believe it is worthwhile to develop drugs or functional foods related to age-related eye degeneration using CA-HE50.

  • The purpose of our study was to determine the eyeradiation dose when performing routine multi-detector computed tomography (MDCT). We also evaluated dose reduction and the effect on image quality of using a bismuth eye shield when performing head MDCT. Examinations were performed with a 64MDCT scanner. To compare the shielded/unshielded lens dose, the examination was performed with and without bismuth shielding in anthropomorphic phantom. To determine the average lens radiation dose, we imaged an anthropomorphic phantom into which calibrated photoluminescence glass dosimeter (PLD) were placed to measure the dose to lens. The phantom was imaged using the same protocol. Radiation doses to the lens with and without the lensshielding were measured and compared using the Student t test. In the qualitative evaluation of the MDCT scans, all were considered to be of diagnostic quality. We did not see any differences in quality between the shielded and unshielded brain. The mean radiation doses to the eyewith the shield and to those without the shield were 21.54 versus 10.46 mGy, respectively. The lens shield enabled a 51.3% decrease in radiation dose to the lens. Bismuth in-plane shielding for routine eye and head MDCT decreased radiation dose to the lenswithout qualitative changes in image quality. The other radiosensitive superficial organs specifically must be protected with shielding.

  • Interventional radiology is performed under real-time fluoroscopy, and patients are exposed to a wide range of exposures for a long period of time depending on the examination and procedure. However, studies on radiation protection for patients during an intervention are insufficient. This study aims to evaluate the doses exposed during the intervention and the applicability of 3D printing materials. The organ dose for each intervention site was evaluated using a monte carlo simulatio. Also, the dose reduction effect of the critical organs was calculated when using a shielding device using 3D printing materials. As a result, the organ dose distribution for each intervention site showed a lower dose distribution for organs located far from the x-ray tube. It was analyzed that the influence of scattered rays was higher in the superficial organs of the back of the human body where x-rays were incident. The dose reduction effect on the critical organ using the 3D printing shield showed the highest testis among the gonads, and in the case of other organs, the dose reduction effect gradually decreased in the order of the eye, thyroid, breast, and ovary. Accordingly, it is judged that the 3D printed shield will be sufficiently usable as a shielding device for the radiation protection of critical organs.

  • Exposure to HDI(hexamethylene di-isocyanate) commonly used in vehicle crash repair workshops remains a leading cause of occupational asthma. Although skin and eye contamination are considered as absorption routes, there are no occupational exposure standards for skin and ocular exposure. This is the reason why there are more empirical data should be provided. Therefore this study was to determine contamination levels of HDI on the skin, eyes, work surfaces, respirators and eye protectors. There was evidence of contamination on a variety of work surfaces, for example, door handles, bench top and spray gun, etc. A high proportion(47~80%) of skin wipe samples from neck, forehead, back hand, palm and wrist was positive for HDI contamination, even though spray time was relatively brief. The contamination levels from spraying inside spray booth were generally higher than outside booth due to poor work practices and inappropriate personal protective use like safety gloves. Apprentices had higher exposure levels than the qualified painters, likely due to lack of the recognition of safety and hygiene. The extent of contamination inside the PPE might provide an indication of the potential for respiratory & skin exposure and ocular exposure. Eye fluid samples from 4 out of 14 workers had the positive detection of HDI contamination, due to poor work practices like no or inappropriate eye protection. Considering the potential for dermal & ocular exposure to contribute to possible health symptoms including respiratory sensitization, the empirical data point to a need for improving work practices and appropriate PPE selection, use and maintenance.

  • Korea's medical law prohibited medical advertisements in principle and permitted them on an exceptional cases. However, the decision of the Constitutional Court of 20005. 10. 27. 20003 Heonga 3, it was changed to a negative system which allows advertisements in principle and restricted only exceptionally. Dramatic increase of medical advertisements was made after that and many argued more deregulation because there was actually heavy regulations. In particular, there is almost no actual regulation on the article type advertisement due to the reason of protection of the freedom of press, media and occupation. However, there may be an unjust result if a specific article or specialists' opinion is made using a newspaper, broadcasting or magazine as a form of article type advertisement to specific medical specialists or medical institution or medical treatment method that falsifies consumers or makes consumers confused by unjust medical expectations or reliability, that also deteriorates just competition and that causes the misrecognition of consumers. In fact, there were actual damages of article type advertisements on the eye whitening surgery not long after the transfer to a negative system of medical advertisements. Victims raised a medical proceeding against the doctor who carried out the surgery, but there is actually no systematic warranty except for the indemnity request. Thus, this case demonstrated a vulnerable result of a negative system. As such, it is problematic that there is no proper regulations defined in the current law and regulations because of the reason of the protection of the freedom of press, publication and occupation despite damages of such article type advertisements. Accordingly, it is urgent to apply the current prevention regulations on the article type advertisements strictly, and to set up specific regulations.
