Clinical Benefits and Complications of Cryotherapy in Advanced Lung Cancer with Central Airway Obstruction (중심성 기도 폐쇄를 동반한 폐암에서 냉동치료의 임상적 유용성 및 부작용)

  • Background: The efficacy of the use of the interventional bronchoscope for palliation of patients with central airway obstruction has been established. In the palliative setting to alleviate central airway obstruction, the use of laser resection, electrocautery, argon plasma coagulation, photodynamic therapy and cryotherapy can provide relief of an airway obstruction. Cryotherapy is the therapeutic application of extreme cold for the local destruction of living tissue. Recently, this technique has been used for endoscopic management of central airway obstructions in Korea. We report the role and complications of the use of cryotherapy for airway obstructions in patients with advanced lung cancer. Methods: We used a flexible cryoprobe for cryotherapy using nitrous oxide as a cryogen. The cryoprobe was applied through the working channel of a flexible fiberoptic bronchoscope. The temperature of the tip was approximately $-89^{\circ}C$, and the icing time was 5~20 seconds. Results: Four patients with a central airway obstruction from advanced lung cancer were treated with cryotherapy. Three of the four patients were treated successfully and the airway obstruction was improved after the cryotherapy procedure. Dyspnea, hypoxia and atelectais were improved in three cases. Two patients experienced complications- one patient experienced pneumomediastinum and the other patient experienced massive hemoptysis during the cryotherapy procedure. However, these complications resolved and did not influence mortality. Conclusion: This technique is effective and relatively safe for palliation of inoperable advanced lung cancer with a central airway obstruction.

  • Diamond-like carbon (DLC) is a convenient term to indicate the compositions of the various forms of amorphous carbon (a-C), tetrahedral amorphous carbon (ta-C), hydrogenated amorphous carbon and tetrahedral amorphous carbon (a-C:H and ta-C:H). The a-C film with disordered graphitic ordering, such as soot, chars, glassy carbon, and evaporated a-C, is shown in the lower left hand corner. If the fraction of sp3 bonding reaches a high degree, such an a-C is denoted as tetrahedral amorphous carbon (ta-C), in order to distinguish it from sp2 a-C [2]. Two hydrocarbon polymers, that is, polyethylene (CH2)n and polyacetylene (CH)n, define the limits of the triangle in the right hand corner beyond which interconnecting C-C networks do not form, and only strait-chain molecules are formed. The DLC films, i.e. a-C, ta-C, a-C:H and ta-C:H, have some extreme properties similar to diamond, such as hardness, elastic modulus and chemical inertness. These films are great advantages for many applications. One of the most important applications of the carbon-based films is the coating for magnetic hard disk recording. The second successful application is wear protective and antireflective films for IR windows. The third application is wear protection of bearings and sliding friction parts. The fourth is precision gages for the automotive industry. Recently, exciting ongoing study [1] tries to deposit a carbon-based protective film on engine parts (e.g. engine cylinders and pistons) taking into account not only low friction and wear, but also self lubricating properties. Reduction of the oil consumption is expected. Currently, for an additional application field, the carbon-based films are extensively studied as excellent candidates for biocompatible films on biomedical implants. The carbon-based films consist of carbon, hydrogen and nitrogen, which are biologically harmless as well as the main elements of human body. Some in vitro and limited in vivo studies on the biological effects of carbon-based films have been studied [$2{\sim}5$].The carbon-based films have great potentials in many fields. However, a few technological issues for carbon-based film are still needed to be studied to improve the applicability. Aisenberg and Chabot [3] firstly prepared an amorphous carbon film on substrates remained at room temperature using a beam of carbon ions produced using argon plasma. Spencer et al. [4] had subsequently developed this field. Many deposition techniques for DLC films have been developed to increase the fraction of sp3 bonding in the films. The a-C films have been prepared by a variety of deposition methods such as ion plating, DC or RF sputtering, RF or DC plasma enhanced chemical vapor deposition (PECVD), electron cyclotron resonance chemical vapor deposition (ECR-CVD), ion implantation, ablation, pulsed laser deposition and cathodic arc deposition, from a variety of carbon target or gaseous sources materials [5]. Sputtering is the most common deposition method for a-C film. Deposited films by these plasma methods, such as plasma enhanced chemical vapor deposition (PECVD) [6], are ranged into the interior of the triangle. Application fields of DLC films investigated from papers. Many papers purposed to apply for tribology due to the carbon-based films of low friction and wear resistance. Figure 1 shows the percentage of DLC research interest for application field. The biggest portion is tribology field. It is occupied 57%. Second, biomedical field hold 14%. Nowadays, biomedical field is took notice in many countries and significantly increased the research papers. DLC films actually applied to many industries in 2005 as shown figure 2. The most applied fields are mold and machinery industries. It took over 50%. The automobile industry is more and more increase application parts. Vulnerability Assessment of Cultivation Facility by Abnormal Weather of Climate Change (이상기후에 의한 재배시설의 취약성 평가)

  • Climate changes have caused not only changes in the frequency and intensity of extreme climate events, but also temperature and precipitation. The damages on agricultural production system will be increased by heavy rainfall and snow. In this study we assessed vulnerability of crop cultivation facility and animal husbandry facility by heavy rain in 232 agricultural districts. The climate data of 2000 years were used for vulnerability analysis on present status and the data derived from A1B scenario were used for the assessment in the years of 2020, 2050 and 2100, respectively. Vulnerability of local districts was evaluated by three indices such as climate exposure, sensitivity and adaptive capacity, and each index was determined from selected alternative variables. Collected data were normalized and then multiplied by weight value that was elicited in delphi investigation. Jeonla-do and Gangwon-do showed higher climate exposures than the other provinces. The higher sensitivity to abnormal weather was observed from the regions that have large-scale cultivation facility complex compared to the other regions and vulnerability to abnormal weather also was higher at these provinces. In the projected estimation based on the SRES A1B, the vulnerability of controlled agricultural facility in Korea totally increased, especially was dramatic between 2000's and 2020 year.

A Laboratory-Scale Study of the Applicability of a Halophilic Sediment Bioelectrochemical System for in situ Reclamation of Water and Sediment in Brackish Aquaculture Ponds: Effects of Operational Conditions on Performance

  • Sediment bioelectrochemical systems (SBESs) can be integrated into brackish aquaculture ponds for in-situ bioremediation of the pond water and sediment. Such an in-situ system offers advantages including reduced treatment cost, reusability and simple handling. In order to realize such an application potential of the SBES, in this laboratory-scale study we investigated the effect of several controllable and uncontrollable operational factors on the in-situ bioremediation performance of a tank model of a brackish aquaculture pond, into which a SBES was integrated, in comparison with a natural degradation control model. The performance was evaluated in terms of electricity generation by the SBES, Chemical oxygen demand (COD) removal and nitrogen removal of both the tank water and the tank sediment. Real-life conditions of the operational parameters were also experimented to understand the most close-to-practice responses of the system to their changes. Predictable effects of controllable parameters including external resistance and electrode spacing, similar to those reported previously for the BESs, were shown by the results but exceptions were observed. Accordingly, while increasing the electrode spacing reduced the current densities but generally improved COD and nitrogen removal, increasing the external resistance could result in decreased COD removal but also increased nitrogen removal and decreased current densities. However, maximum electricity generation and COD removal efficiency difference of the SBES (versus the control) could be reached with an external resistance of $100{\Omega}$, not with the lowest one of $10{\Omega}$. The effects of uncontrollable parameters such as ambient temperature, salinity and pH of the pond (tank) water were rather unpredictable. Temperatures higher than $35^{\circ}C$ seemed to have more accelaration effect on natural degradation than on bioelectrochemical processes. Changing salinity seriously changed the electricity generation but did not clearly affect the bioremediation performance of the SBES, although at 2.5% salinity the SBES displayed a significantly more efficient removal of nitrogen in the water, compared to the control. Variation of pH to practically extreme levels (5.5 and 8.8) led to increased electricity generations but poorer performances of the SBES (vs. the control) in removing COD and nitrogen. Altogether, the results suggest some distinct responses of the SBES under brackish conditions and imply that COD removal and nitrogen removal in the system are not completely linked to bioelectrochemical processes but electrochemically enriched bacteria can still perform non-bioelectrochemical COD and nitrogen removals more efficiently than natural ones. The results confirm the application potential of the SBES in brackish aquaculture bioremediation and help propose efficient practices to warrant the success of such application in real-life scenarios.

A study on the change effect of emission regulation mode on vehicle emission gas (배기가스 규제 모드 변화가 차량 배기가스에 미치는 영향 연구)

  • As the interest on the air pollution is gradually rising at home and abroad, automotive and fuel researchers have been studied on the exhaust and greenhouse gas emission reduction from vehicles through a lot of approaches, which consist of new engine design, innovative after-treatment systems, using clean (eco-friendly alternative) fuels and fuel quality improvement. This research has brought forward two main issues : exhaust emissions (regulated and non-regulated emissions, PM particle matter) and greenhouse gases of vehicle. Exhaust emissions and greenhouse gases of automotive had many problem such as the cause of ambient pollution, health effects. In order to reduce these emissions, many countries are regulating new exhaust gas test modes. Worldwide harmonized light-duty vehicle test procedure (WLTP) for emission certification has been developed in WP.29 forum in UNECE since 2007. This test procedure was applied to domestic light duty diesel vehicles at the same time as Europe. The air pollutant emissions from light-duty vehicles are regulated by the weight per distance, which the driving cycles can affect the results. Exhaust emissions of vehicle varies substantially based on climate conditions, and driving habits. Extreme outside temperatures tend to increasing the emissions, because more fuel must be used to heat or cool the cabin. Also, high driving speeds increases the emissions because of the energy required to overcome increased drag. Compared with gradual vehicle acceleration, rapid vehicle acceleration increases the emissions. Additional devices (air-conditioner and heater) and road inclines also increases the emissions. In this study, three light-duty vehicles were tested with WLTP, NEDC, and FTP-75, which are used to regulate the emissions of light-duty vehicles, and how much emissions can be affected by different driving cycles. The emissions gas have not shown statistically meaningful difference. The maximum emission gas have been found in low speed phase of WLTP which is mainly caused by cooled engine conditions. The amount of emission gas in cooled engine condition is much different as test vehicles. It means different technical solution requires in this aspect to cope with WLTP driving cycle.

A Study on the Wind Ventilation Forest Planning Techniques for Improving the Urban Environment - A Case Study of Daejeon Metropolitan City - (도시환경 개선을 위한 바람길숲 조성 계획기법 개발 연구 - 대전광역시를 사례로 -)

  • The objective of the study was to develop an Urban Windway Forest Creation Planning Technique for the Improvement of the Urban Environment using the case of Daejeon Metropolitan City. Through a spatial analysis of fine dust and heat waves, a basin zone, in which the concentration was relatively serious, was derived, and an area with the potential of cold air flow was selected as the target area for the windway forest development by analyzing the climate and winds in the relevant zone. Extreme fine dust areas included the areas of the Daejeon Industrial Complex Regeneration Business District in Daedeok-gu and Daedeok Techno Valley in Yuseong-gu. Heat wave areas included the areas of Daedeok industrial Complex in Moksang-dong, the Daejeon Industrial Complex Regeneration Business District in Daehwa-dong, and the high-density residential area in Ojeong-dong. As a result of measuring the wind speeds in Daejeon with an Automatic Weather System, the average wind speeds during the day and night were 0.1 to 1.7 m/s,, respectively. So, a plan of for a windway forest that smoothly induces the movement of cold air formed in outer forests at night is required. The fine dust/heat wave intensive management zones of Daejeon Metropolitan City were Daejeoncheon, Yudeungcheon, Gapcheon-Yudeungcheon, and Gapcheon. The windway forest formation plan case involved the old city center of Daejeon Metropolitan City among the four zones, the Gapcheon-Yudeungcheon area, in which the windway formation effect was presumed to be high. The Gapcheon-Yudeungcheon area is a downtown area that benefits from the cold and fresh air generated on Mt. Gyejok and Mt. Wuseong, which are outer forests. Accordingly, the windway forest was planned to spread the cold air to the city center by connecting the cold air generated in the Seosa-myeon forest of Mt. Gyejok and the Namsa-myeon forest of Mt. Wuseong through Gapcheon, Yudeungcheon, and street forests. After selecting the target area for the wind ventilation forest, a climate map and wind formation function evaluation map were prepared for the area, the status of variation wind profiles (night), the status of fine dust generation, and the surface temperature distribution status were grasped in detail. The wind ventilation forest planning concept and detailed target sites by type were identified through this. In addition, a detailed action plan was established according to the direction of creation and setting of the direction of creation for each type of wind ventilation forest.

Studies on the Improvement of Nursery for Better Ripening Percentage and Prevention of Red Discoloration of Rice Variety "Tongil" (통일벼의 등숙(登熟) 향상(向上)과 적고방지(赤枯防止)를 위(爲)한 묘대개선(苗垈改善)에 관(關)한 연구(硏究))

Results in Nursery This experiment was carried out on the effect of the seed treament, soil preparations, kinds of covering soil and inside covering methods in two rice varieties, 'Tongil' and 'Akibare' to find out the most reasonable model of the flat nursery bed, with which lower cost is required comparing with the tunnel nursery. The results obtained are as follows: (1) The seedling of all plots of the ordinary seed were very poor compared to the plots of sprouted seed. (2) In case that the variety 'Tongil' was cultivated on the dry nursery bed, the good seedling percentage and the plant height rat io were significantly increased but the other characteristics of the seedling were not noticeable. (3) The kinds of the covering soil had not an effect on the seedling growth significantly. (4) Inside straw mulching was seemed effective for the protection in the case of the extreme high temperature and heavy rain fall, even though there was not significant differences between inside straw mulching and no treatments at the flat type nursery. (5) Difference of seedling growth between the flat type nursery and the tunnel type nursery was not significant. And it's reason was thought that the covering period of polyethylene film was short in semi hot nursery for the common early transplanting cultivation of rice. (6) The percentage of good seedling was higher at 'Akibare' than 'Tongil', variety but the number of seedling leaf and the seedling growth ratio in height were significantly increased in the variety 'Tongil'. The other seedling characters between there two varieties were not significantly different. 2. Results after transplanting This experiment was conducted to study on the ripening percentage, rice yield and disease, appearance of the seedling from sprouted seed plots including common irrigated nursery as check plot after transplantnig. The results obtained are summarized as follows: (1) The rice yield, the yield components and the appearance of leaf discoloration of both varieties, 'Tongil' and 'Akibare' were slightly betterat the plot of the standard tunnel nursery than that of the flat nursery with inside mulching or the among these three plots. (2) For 'Tongil' variety, the ripening percentage and the rice yield were significantly decreased at the common irrigated nursery compared with semi hot nursery. (3) The ripening percentage and the rice yield of 'Akibare' contrasted with 'Tongil' were significantly decreased at thesemi 'hot-nursery compared with common irrigated nursery. The main reason was thought to be the injury of the rice stripe disease (Rice stripe disease virus). Considering above mentioned experimental result, the seedling of 'Tongil' must be cultivated on the semi bot nursery for better ripening percentage as well as rice yield and for prevention of red discoloration. And as a model of semi hot nursery, the polyethylene covering nursery of standard tunnel type is most desirable but that of flat type with inside straw mulching is thought to be desirable too.
