Use of Antimicrobial Agents for the Treatment of Inpatients in Chonbuk National University Hospital (전북대학교병원 입원환자에서의 항생제 사용 실태)

  • Purpose : The use antimicrobial agents is one of the important strategies for the treatment and prophylaxis of microbial infections. But injudicious abuse and misuse of antimicrobial agents is problem to add an extra weight on medical fee, increase of resistant bacteria and side effects according to the antibiotic use. This study was performed to establish the pertinent use of antimicrobial agent in Chonbuk National University Hospital(CNUH). Characteristics of antibiotics use was analysis by reviewing the medical records of patients admitted to CNUH during the period of May 1998. Methods : One thousand eight hundred and thirty three patients were enrolled in this study(medical division 1,014 cases, surgical division 819 cases). Medical records were retrospectively reviewed to classify the rate of antibiotics use, name of antibiotics used, appropriateness of antibiotics use. Results : The overall rate of antibiotic usage in CNUH was 67.2%(1,231/1,833), showing higher rate in surgical division(89.6%) compare to that of medical division(49.0%). Among 1,231 patients to whom antimicrobial agents were given, only 125(10.2%) were treated with single antimicrobial agents. 311(25.3%) were treated with two antimicrobial agents, and 795(64.5%) patients received 3 or more antibiotics. ${\beta}$-lactams(56.4%) were most frequently used followed by aminoglycosides(35.3%), the others(4.9%) and quinolons(3.4%). Amoxicillin-clavulanate was the mostly commonly used antibiotics followed by amoxicillin and unasyn. Prophylactic use of antibiotics was carried in seven hundred six patients(57.4%), mostly in surgical division, which can be considered somewhat inappropriate in the initiation time and duration of antibiotic use. Conclusion : Importance of monotherapy and appropriate prophylactic antibiotic use should be emphasized. Strategies of antibiotics use, such as restriction of drug use, continuous monitoring system, flow sheet system should be considered to reduce antibiotics use and establish the appropriate use of antibiotics as well as inhibiting the occurrence of resistant strains.

Effect of Acute and Chronic Treatment with Risperidone on the Serotonin and Dopamine Receptors in the Rat Brain (Risperidone의 급성 및 만성 투여가 흰쥐 뇌의 Serotonin과 Dopamine 수용체에 미치는 영향)

  • The therapeutic efficacy of antipsychotic drugs is generally attributed to their ability to block dopamine $D_2$ receptors. Classical $D_2$ antagonists are not effective to treat negative symptoms and produce extrapyramidal side effects On the other hand, atypical antipsychotic agents ameliorate negative symptoms without producing extra-pyramidal side effects, and it is reported to be associated with blockade of serotonin $5-HT_2$ receptors. The purpose of this study was to evaluate the effect of risperidone on neuroreceptors in the rat brain by Quantitative autoradiography method. In acute treatment group, risperidone was injected into Peritoneal cavity of male Wistar rats with dose of 0, 0.1, 0.25, 0.5, 1.0 and 2.0mg/kg in each group(5/group), and they were decapitated after 2 hours. In chronic treatment group, risperidone was injected with dose of 0, 0.1, and 1mg/kg(I.P.) for 21 days and decapitated after 24 hours following last treatment. The effect of risperodone on the binding of [$^3H$]spiperone to $5-HT_2$ and $D_2$ receptors were analysed in 4 discrete regions of the striatum, nucleus accumbens, and frontal cortex by quantitative autoradiography Acute treatment with risperidone reduced cortical $5-HT_2$ specific [$^3H$]spiperone binding to 32% of vehicle-treated control. Subcortical $5-HT_2$ specific [$^3H$]spiperone binding was not affected at all dose groups whereas a significant reduction (57%) in $D_2$ specific [$^3H$]spiperone binding was observed in risperidone treated group at doses of 1-2mg/kg. Chronic treatment with risperidone produced a decrease in the maximal number of cortical $5-HT_2$ receptors to 51% and 46% of control in 0.1mg/kg & 1mg/kg treated group respectively. In conclusion, risperidone is a cortical serotonin receptor antagonist with relatively weak antagonistic action on dopamine receptors. These effects oil neuroreceptors may explain the therapeutic effect of risperidone as a atypical antipsychotic agents.

  • Purpose: Whole body imaging with radioiodine can detect functioning metastases. Non-physiologic I-131 uptake detected on images usually is interpreted as suggesting functioning thyroid metastases. However, extra-thyroidal I-131 accumulation does not always imply thyroid cancer metastases and has been reported in many circumstances. In order to avoid unnecessary therapeutic interventions it is important to distinguish false-positive sites of I-131 localization. We study here to remove false-positive sites around esophagus region on I-131 whole body imaging in patients who were administrated thyroidectomy. Materials and Methods: From April to August in 2007, we had the patients who had visited our department after they received thyroidectomy due to thyroid cancer. They were given I-131, and performed radioiodine body scan after 41 to 50 hours. Patients were whole-body-scanned for the speed of 8 cm per minute. After that, we took anterior and posterior static images around the patients' neck measured by 300 thousand counts per image. We selected 44 patients who had hot spots around neck region, we divided the patients into two groups. One group was given 0.5 L of water and the other group was given 0.5 L of water with 1 g of Vitamin C dissolved. The patients were asked to drink the fluid for one minute in sitting position and after that, we measured 300 kilo counts per image again. We compared prior anterior, posterior static images with anterior, posterior images after the patients had water or water that Vitamin C resolved. Results: In using water, both observer 1 and 2 interpreted 6 patients were washed out. In the water with Vitamin C resolved, observer 1 and 2 interpreted 9 and 8 patients were washed out. Observer 1 and 2 interpreted 6 and 5 patients had 'indeterminate' when they used water. Both observer 1 and 2 interpreted 6 patients had 'indeterminate' when they used water with Vitamin C resolved. When they used water, observer 1 interpreted 10 patients had 'unchanged' and observer 2 interpreted 11 patients had 'unchanged'. Differently, when they used water with vitamin C resolved, observer1 had 8 patients having 'unchanged'and observer 2 had 9. Conclusion: As a result, by making patients drink 0.5 liter of water which has vitamin C resolved helped getting rid of false-positive sites in esophagus. Therefore, based on this study, we believe that drinking water with vitamin C dissolved is very in terms of reducing false-positive hot spot around the esophagus for the iodine-131 whole body scan.

용용과 모델 구성을 중시하는 수학과 교육 과정 개발 방안 탐색

  • This study intends to provide some desirable suggestions for the development of application oriented mathematics curriculum. More specific objects of this study is: 1. To identify the meaning of application and modelling in mathematics curriculm. 2. To illuminate the historical background of and trends in application and modelling in the mathematics curricula. 3. To consider the reasons for including application and modelling in the mathematics curriculum. 4. To find out some implication for developing application oriented mathematics curriculum. The meaning of application and modelling is clarified as follows: If an arbitrary area of extra-mathematical reality is submitted to any kind of treatment which invovles mathematical concepts, methods, results, topics, we shall speak of the process of applying mathemtaics to that area. For the result of the process we shall use the term an application of mathematics. Certain objects, relations between them, and structures belonging to the area under consideration are selected and translated into mathemtaical objects, relation and structures, which are said to represent the original ones. Now, the concept of mathematical model is defined as the collection of mathematical objcets, . relations, structures, and so on, irrespective of what area is being represented by the model and how. And the full process of constructing a mathematical model of a given area is called as modelling, or model-building. During the last few decades an enormous extension of the use of mathemtaics in other disciplines has occurred. Nowadays the concept of a mathematical model is often used and interest has turned to the dynamic interaction between the real world and mathematics, to the process translating a real situation into a mathematical model and vice versa. The continued growing importance of mathematics in everyday practice has not been reflected to the same extent in the teaching and learning of mathematics in school. In particular the world-wide 'New Maths Movement' of the 19608 actually caused a reduction of the importance of application and modelling in mathematics teaching. Eventually, in the 1970s, there was a reaction to the excessive formallism of 'New Maths', and a return in many countries to the importance of application and connections to the reality in mathematics teaching. However, the main emphasis was put on mathematical models. Applicaton and modelling should be part of the mathematics curriculum in order to: 1. Convince students, who lacks visible relevance to their present and future lives, that mathematical activities are worthwhile, and motivate their studies. 2. Assist the acqusition and understanding of mathematical ideas, concepts, methods, theories and provide illustrations and interpretations of them. 3. Prepare students for being able to practice application and modelling as private individuals or as citizens, at present or in the future. 4. Foster in students the ability to utilise mathematics in complex situations. Of these four reasons the first is rather defensive, serving to protect or strengthen the position of mathematics, whereas the last three imply a positive interest in application and modelling for their own sake or for their capacity to improve mathematics teaching. Suggestions, recomendations and implications for developing application oriented mathematics curriculum were made as follows: 1. Many applications and modelling case studies suitable for various levels should be investigated and published for the teacher. 2. Mathematics education both for general and vocational students should encompass application and modelling activities, of a constructive as well as analytical and critical nature. 3. Application and modelling activities should. be introduced in mathematics curriculum through the interdisciplinary integrated approach. 4. What are the central ideas of, and what are less-important topics of application-oriented curriculum should be studied and selected. 5. For any mathematics teacher, application and modelling should form part of pre- and in-service education.

Effects of Fresh Ginseng Size and Shape on Quality of Black Ginseng (원료삼 크기와 형태가 흑삼의 품질에 미치는 영향)

  • Current study was performed to investigate the effect of morphological properties of black ginseng such as size and shape on the quality of black ginseng. The raw ginsengs were separated based on size (medium, large, and extra-large) and shape (straight ginseng, fibrous root ginseng). Subsequently, the raw ginsengs were steamed at $95^{\circ}C$ for 3 h and dried in the presence of heated air at $50^{\circ}C$ for 30 h. This process was repeated nine times for black ginseng production. The physiochemical properties such as the content of acidic polysaccharides, ginsenosides, and antioxidative activity were evaluated. Although minor difference in physiochemical properties such as acidic polysaccharide content in raw ginseng was observed, no statistical difference in the content of acidic polysaccharides, total phenols, and ginsenosides was observed during final black ginseng production based on size classification. The minor ginsenosides in fibrous root black ginseng, such as Rk3, Rh4, Rg3, Rk1, and Rg5 were higher in content than straight black ginseng. However, no correlation between the shape of ginseng and total phenol content and antioxidative activity was observed. Therefore, present results demonstrate that the difference in ginseng size in same-age and -production area does not affect the quality of black ginseng. Furthermore, difference in ginseng shape does not influence the overall quality of black ginseng. It is hypothesized that this study would be considered as supportive data for the production of high-quality black ginseng.

Comparative Evaluation for the Effect of SUV's Due to a Residual Radio-activity Location Inside Vascular Insert Devices During PET/CT Scans (PET/CT 검사 시 혈관삽입기구 내 잔여 방사능 위치에 따른 표준섭취계수의 영향 비교 평가)

  • Purpose: Standardized uptake value (SUV) is a simple semi-quantitative method that can measure the ratio of the tissue radioactivity between the tumor and normal. SUV is commonly used in PET/CT, however, SUV is affected by various factor. The purpose of this study was to evaluate the impact of the residual activity on SUV depending on the location of catheter insertion device post injection. Materials and Methods: NEMA IEC Body Phantom was imaged using a Discovery 600 PET scanner. In 22 mm diameter sphere, the different activity of $^{18}F-FDG$ (7.4, 14.8, 22.2, 29.6, 37, 55.5 MBq) was filled and background was filled with $^{18}F-FDG$ (5.7 kBq/mL). We scaned the phantom on the assumption that the radioactivity in sphere was residual activity in insertion device. Simulation of PET was divided into three groups based on the location of sphere in Scan FOV (SFOV); inclusion, 1/2 inclusion and exclusion group. Results: Among three groups, the group of excluded sphere showed the highest SUV regardless of the amount of $^{18}F-FDG$ activity. In case of 7.4 MBq, average SUV of inclusion group, 1/2 inclusion and exclusion group was 0.780, 0.840 and 0.896 respectively. However, average SUV of 55.5 MBq showed 0.372, 0.460 and 0.508 with same order. Depend on residual radioactivity in the sphere and position of sphere, the SUV was different minimum of 10.4%, maximum of 62.8%. Conclusion: This study showed that SUV is underestimated as the residual radio-activity is increased. In addition, SUV was a changed according to the position of residual radio-activity. And among the position, exclusion group showed the difference of SUV was lowest. If we measure the residual radio-activity of inserting devices and radio-activity from extra-vasation in the patients, it seems to be more useful in clinical field.

Chemical Mass Balance of Materials in the Keum River Estuary: 1. Seasonal Distribution of Nutrients (금강하구의 물질수지: 1. 영양염의 계절적 분포)

  • As part of an on-going project investigating flux of materials in the Keum River Estuary, we have monitored seasonal variations of nutrients, suspended particulate matter (SPM), chlorophyll, and salinity since 1997. Meteorological data and freshwater discharge from the Keum River Dike were also used, Our goal was to answers for (1) what is the main factor for the seasonal fluctuation of nutrients in the Keum River Estuary? and (2) are there any differences in nutrient distributions before and after the Keum River Dike construction? Nitrate concentrations in the Keum River water were kept constant through the year. Whereas other nutrients varied with evident seasonality: high phosphate and ammonium concentrations during the dry season and enhanced silicate contents during the rainy season. SPM was found similar trend with silicate. During the rainy season, the freshwater discharged from the Keum River Dike seemed to dilute the phosphate and ammonium, but to elevate SPM concentration in the Keum Estuary. In addition, the corresponding variations of SPM contents in the estuarine water affected the seasonal fluctuations of nutrients in the Estuary. The most important source of the nutrients in the estuarine water is the fluvial water. Therefore, the distribution patterns of nutrients in the Estuary are conservative against salinity. Nitrate, nitrite and silicate are conservative through the year. The distribution of phosphate and ammonium on the other hand, display two distinct seasonal patterns: conservative behavior during the dry season and some additive processes during the rainy days. Mass destruction of freshwater phytoplankton in the riverine water is believed to be a major additive source of phosphate in the upper Estuary. Desorption processes of phosphate and ammonium from SPM and organic matter probably contribute extra source of addition. Benthic flux of phosphate and ammonium from the sediment into overlying estuarine water can not be excluded as another source. After the Keum River Dike construction, the concentrations of SPM decreased markedly and their role in controlling of nutrient concentrations in the Estuary has probably diminished. We found low salinity (5~15 psu) within 1 km away from the Dike during the dry season. Therefore we conclude that the only limited area of inner estuary function as a real estuary and the rest part rather be like a bay during the dry season. However, during the rainy season, the entire estuary as the mixing place of freshwater and seawater. Compared to the environmental conditions of the Estuary before the Dike construction, tidal current velocity and turbidity are decreased, but nutrient concentrations and chance of massive algal bloom such as red tide outbreak markedly increased.

  • In Nai-Gyung(內經), it is said that In-Yeong(人迎) means In- Yeong Maek(人迎脈), Gi-Gu m- eans Tson-Gu(寸口) or Maek- Gu(脈口), In-yeong, artery in the side of neck, is located before Yeong-Geun(瓔筋), called as Jok-Yang-Myung-Maek(足陽明脈),so the part of In-Yeong-Maek means In-Yeong-Hyul(人迎穴) of Jok-Yang-Myung- Wi-Gyung(足陽明胃經) showing the artery in the side of neck. The part of Tson-Gu-Maek is that of artery in Yo-Gol(橈骨), beating source of Soo-Tae-Eum-Maek (手太陰脈) Of In-Yeong-Gi-Gu-Maek, In-Yeong-Maek mainly consists of Yang(陽), Gi-Gu-Maek of Eum(陰), so In-Yeung means physical disease, short of extra Eum component. They said that if In-Yeong-Maek was more than Gi-Gu-Maek, it meant physical injury, while Gi-Gu-Maek was more than In- Yeo-ng-Maek, it meant internal injury. In-Yeong-Gi-Gu-Maek is the one to distinguish the external and internal, the inside and outside, Eum and Yang, but there is no definite classification method to distinguish it, also it is difficult to grasp the comparison of 1sung(1盛), 2sung(2盛), 3sung(3盛), interrelation with 12-Gyung-Rak(l2經絡), change of maek phases, so necessary to set up the clear definition for In-Yeong-Gi-Gu-Maek. For the two theories as to In-Yeong-Gi-Gu-Maek according to Nai-Gyung, the one is to diagnose the maek by comparing the In-Yeong-Hyul with Tson-Gu of Soo-Tae-Eum-Maek as Gi-Gu in the both sides of neck part, he other is to divide the left and right of Gi-Gu-Maek in to In-Yeong and Gi-Gu afterwards, but today it is difficult to compare and explain the medical theory afterwards owing to insufficient consideration of In-Yeong-Gi-Gu-Maek at Nai-Gyung. The diagnosis of In-Yeong-Gi-Gu-Maek at Nai-Gyung to distinguish the surplus and shortage of Eum and Yang up to now since Nai-Gyung has so important and diagnostic value as to grasp the exact meaning. Herewith, this researcher com-pared Nai-Gyung and medical theory afterwards, reported it to consider the fixed position and changing process of viscera and entrails arrangement of literatures introducing In-Yeong-Gi-Gu-Maek, examining the change of maek phases for normal maek and a-bnormal maek of In-Yeong-Gi-Gu-Maek, considering the 1 sung, 2 sung, 3 sung maek phases in In-Yeong-Gi- Gu-Maek. According to the above results, the conclusion was reached as follows. 1. In-Yeong as the outside indicates external disease(外感), showing the surplus and shortage of Yang symptom by having Boo-Maek(浮脈) as Py-ung-Maek(平脈), Gi-Gu as the inside indicates internal disease(內傷), showing the surplus and shortage of Eum symptom by having Chim-Maek(沈脈) as P-yung-Maek(平脈). 2. In Pyung-Maek of In- Yeong-Maek as Boo-Maek, g-radual sinking of more floated changing maek because of disease means the improvement of di-sease, in Pyung-Maek of Gi- Gu-Maek as Chim- Maek, gradual floating of more sunken changing maek because of disease means the improvement of disease. 3. They said that disease of Jok-Gyung-Rak(足經絡) is cha-nged to that of Soo-Kyung-Rak(手經絡) when Jo-Maek(躁脈) appears whether In-Yeong-Maek or Gi-Gu-Maek 4. With the exemples of 1 sung 2 sung 3 sung it was porned with the relation of Pyo-Ri-Soo-Jok(表裏手足). Therfore I can guess that this fact is a moment explained the Bu-You-Sa-Kyung(部有四經) mentioned in Nan-Gyung-18-Nan(難經 第18難). 5. I think that In-Yeong and Gi-Gu, as a diagnosis method which distinguish between the inside indicates internal disease and the outside indicates external disease, is required to study further researches.

  • Recently, there has been a rapid increase in the sale and purchase of an array of organic products. In particular, organic milk has grown in popularity. This growth could lead to expensive, premium retail prices for various organic cow's milk products. In fact, most consumers believe that (1) dairy farming to produce organic milk is beneficial for human health and (2) organic milk products are made without the use of various antibiotics, synthetic chemicals, genetic modification, extra hormones, and so on. Several factors, including breed, diet, and stage of lactation, are known to influence the composition of milk. Therefore, this review (1) presents a research outline to compare organic and conventional milk and (2) provides a summary of individual elements that affect the composition of milk.

A Study on the actual conditions of the use of them and the moves to strengthen home economics resources for school lifelong education in home economics teachers' view (가정과교사가 인식한 학교 평생교육을 위한 가정과 자원의 활용 실태 및 활성화 방안)

  • The purpose of this study was to investigate the actual conditions of the use of them and the moves to strengthen home economics resources for school lifelong education for parents and local residents. This study conducted a systematic random sampling. Questionnaires were distributed to home economics teachers from 285 middle schools, 123 high schools, and 130 of them were sampled as the subjects of this study. The results of this study were as follows. First, home economics teachers were active participating school lifelong educational program. But according to the result of research on the actual condition, the proportion of taking part in it was low. More than half of home economics teachers who took part in program's lecturer were in charge of computer courses. So they didn't show their ability as expert. Also they were in charge of several works than roll of lecturers. It was mentioned too great a burden. There is little in-service training for meeting specialization for school lifelong education. They wanted to take part of lecturers of program and preferred hours of being over class. More than half of them had intended to obtain a lifelong educator's license. The part of elective courses for obtaining a lifelong educator's license is similar to home economics educational contents. So, they have an advantage of obtaining it. Second, one-fifth of school that gave school life education carried out program of related home economics. Mostly they had mothers of students-oriented programs on artistic and leisure. But this is that home economics teachers mentioned less important teaching at society in the future. They importantly mentioned program on children's education in now and the future. Parents of students and local residents also extremely wanted it. For differentiated school life education, quality of programs is important greatly. Third, the actual condition of practical room relating home economics is only practice to cook mostly. So they are reluctant to be open it because of being responsible for the results from using there. It is necessary to ameliorate there's facilities and to increase there's area.
