Background: The purpose of this study is to compare the skeletal stability of two-jaw surgery via surgery-first approach with conventional two-jaw surgery in facial asymmetry patients by measuring the skeletal changes after surgery from a three-dimensional analysis. From January 2010 to January 2014, 40 patients with facial asymmetry who underwent two-jaw surgery in Pusan National University Hospital were included in this study. They were classified into experimental group (n = 20) who underwent two-jaw surgery via surgery-first approach and control group (n = 20) who underwent conventional two-jaw surgery. After selection of 24 landmarks and the construction of horizontal and sagittal, coronal reference planes, changes in 10 linear measurements and 2 angular measurements were compared between the surgery-first approach and conventional groups in the preoperative, immediate postoperative, and postoperative periods. The paired t test and Student t test were used for statistical analysis. The mean and standard deviation of the measurement were calculated for the experimental and control groups. Results: The statistical analysis showed that changes in skeletal measurements were similar between the surgery-first approach and conventional groups, according to each period. However, U1-SRP measurement showed statistically significant changes in surgery-first approach groups at postsurgical change (T1 to T2). Also, the mean treatment duration in the treatment group was 15.9 ± 5.48 months whereas that in the control group was 32.9 ± 14.05 months. Conclusion: In facial asymmetry patients, similar results were observed in the postoperative skeletal stability when 2-jaw surgery via surgery-first approach was compared with conventional 2-jaw surgery. However, significant lateral deviation of upper incisor midline was observed. In addition, a shorter average treatment duration was observed. To stabilize the unstable occlusion after surgery, increased wearing of the stent and proactive rubber guidance will be needed.

Purpose: The aim of this study is to compare volume and revascularization of autogenous block bone grafts in simultaneously cortical perforation of recipient beds and grafts, and only cortical perforation of recipient beds. Materials and methods: Two block bone in 8mm diameter was harvested in both skull using trephine bur on 20 New Zealand white rabbits. Harvested block bone was grafted on both inferior border of mandible. On the left side(experimental side), cortical bone of recipient beds and graft were perforated, and on the right side(control side), the only recipient bed was perforated. The rabbits had been sacrificed and infused the India ink for the observation of revascularization at 20 day and 40 day after surgery. The specimens were processed for H-E staining and quantitative analysis(independent t-test, p<0.01) was made under an optical microscope. In additional, specimens were processed for the observation of revascularization. Results: After 20 days, more bone volume was observed in experimental group, but no significant difference between two groups(p=0.106). There were significantly more bone volume in the experimental group at 40 days after surgery(p<0.01). After 20 days, more discrete vascular sprouts were observed in experimental side, but no difference at 40 days after surgery. Conclusion: We conclude that the cortical perforation of both the recipient beds and grafts improve revascularization at early stage and overall graft persistence.

The placement of different graft materials and/or the use of occlusive membranes to cover the extraction socket entrance are techniques aimed at reducing alveolar ridge resorption and enhancing bone formation. However, in spite of its clinical advantage, the use of graft materials in fresh extraction socket has been questioned because particles of the grafted material have been found in alveolar sockets with fibrous union. The purposes of this study were to evaluate whether alveolar ridge resorption following tooth extraction could be reduced and bone formation could be enhanced by the application of absorbable gelatin spongy or gelatin spongy soaked with platelet rich plasma(PRP) used as a space filler in clinical and radiographic aspects. Eighty patients who were scheduled for extraction of both third molars were participated and carried out by one experienced surgeon. Following extraction of teeth, one extracted socket were treated with gelatin spongy as an experimental group A and the other were treated with gelatin spongy and PRP as an experimental group B. The routine extracted socket were healed without any treatment as a control group. From the period of extraction to 12 weeks postoperatively, we examined the clinical course and radiographic evaluation on socket at regular interval. Both experimental groups showed faster wound healing process than control clinically. Vertical gingival height of the extraction socket were less changed statistically in both experimental groups than control. The horizontal width change of the extraction socket were not significant statistically in any group. Radiographic changes of the alveolar bone height were less changed in both experimental groups and bone density were showed higher than control. There were a little difference between experimental group A and B. In conclusion, absorbable gelatin sponge and with PRP were considered as having preservation effects of extraction socket and stimulation of bone formation process after extraction.

Introduction: This study examined the effect of cyclosporine A (CsA) on the allogenic cranial bone graft in the mice. Materials and Methods: Twenty eight 12-week-old male ICR mice weighing 40 g were used. The experimental group was injected subcutaneously with CsA (10 mg/kg/day) diluted in Caster oil for 7 days prior to the graft until sacrifice. The control group was injected with the same solution without CsA. Two full-thickness bone defects with a diameter of 3 mm were made with a trephine bur in the parietal bone lateral to the sagittal suture. A calvarial defect of a mouse was grafted with allogenic calvarial bone disc from another mouse. The experimental and control groups were injected with CsA and the solution without CsA in the same manner before surgery, respectively. The mice were sacrificed at 1 week, 2 weeks and 4 weeks after the bone graft, respectively. Results: In the experimental group, fibrous connective tissues and small amounts of inflammatory cells were observed. At 2 weeks after the allograft in the experimental group, new bone formation in fibrous collagenous tissue and around the allogenic bone was noted. At 4 weeks after the allograft, new bone formation was active along and at the periphery of the mature allogenic bone. The proliferation of blood vessels increased in bone marrow. In the control group, fibrous tissues and inflammatory cells were observed around the allogenic bone and existing bone at 1 week. At 2 weeks after the allograft, the proliferation of blood vessels accompanied by inflammatory cells were scattered in the fibrous connective tissues. New bone formation around the allogenic and existing bone could be observed. At 4 weeks after the allograft, inflammatory cells were severely infiltrated around the allogenic bone. Osteoclasts were scattered along the allogenic bone and induced bone resorption. Conclusion: These results suggest that the daily administration of CsA (10 mg/kg/day) induces efficient immunosuppression without serious complications, and this protocol might be useful for the experimental model of allogenic bone grafts.

The purpose of this study was to evaluate body temperature changes in rabbits anesthetized using water-filtered infrared-A (wIRA). Ten rabbits were used for this study. For the experimental group (wIRA group; wG, n = 5), the experimental equipment was used and irradiated using wIRA. The control group (CG, n = 5) did not have any warming device. There were no significant differences in heart rate, respiration rate, and end tidal CO2 (EtCO2) between wG and CG. After 80 min, the core body temperature of wG rabbits was significantly higher than that of CG rabbits. The surface body temperature was significantly higher while receiving wIRA support at all time points after 5 min. In conclusion, in rabbits under inhalation anesthesia, the surface body temperature was better maintained than the core body temperature when using wIRA.
Background: Most nasal bone fractures are corrected using non-invasive methods. Often, patients are dissatisfied with surgical outcomes following such closed approach. In this study, we compare surgical outcomes following blind closed reduction to that of ultrasound-guided reduction. Methods: A single-institutional prospective study was performed for all nasal fracture patients (n=28) presenting between May 2013 and November 2013. Upon research consent, patients were randomly assigned to either the control group (n=14, blind reduction) or the experimental group (n=14, ultrasound-guided reduction). Surgical outcomes were evaluated using preoperative and 3-month postoperative X-ray images by two independent surgeons. Patient satisfaction was evaluated using a questionnaire survey. Results: The experimental group consisted of 4 patients with Plane I fracture and 10 patients with Plane II fracture. The control group consisted of 3 patients with Plane I fracture and 11 patients with Plane II fracture. The mean surgical outcomes score and the mean patient dissatisfaction score were found not to differ between the experimental and the control group in Plane I fracture (p=0.755, 0.578, respectively). In a subgroup analysis consisting of Plane II fractures only, surgeons graded outcomes for ultrasound-guided reduction higher than that for the control group (p=0.007). Likewise, among the Plane II fracture patients, those who underwent ultrasound-guided reduction were less dissatisfied than those who underwent blind reduction (p=0.043). Conclusion: Our study result suggests that ultrasound-guided closed reduction is superior to blind closed reduction in those patients with Plane II nasal fractures.
Allogeneic bone grafting has recently been used in oral and maxillofacial regions to restore the cosmetic and functional problem. There are several types of allogeneic bone grafts ; bone powder, bone chips, bone blocks. Empirically, it is thought to be better to combine the allogeneic bone chips to any type of tissue adhesive not to displace during packing and condensing. But, there are no reports about using tissue adhesive in allogeneic bone grafting. This experimental study is designed to investigate the effect of the fibrin adhesive on bone healing process after demineralized allogeneic bone grafting in 60 rats. In control groups (30 rats), routine demineralized allogeneic bone grafting were done in 7 ${\times}$ 7mm calvarial bone defects which were drilled intentioally. And we used the fibrin adhesive for holding the bone particle in experimental groups (30 rats). Each experimental specimen was sacrified at 1, 2, 4, 6, 8 weeks postoperatively The results were as follows : 1. The degree of inflammatory cell infiltrations were more prominent in experimental than in control groups till 2 weeks. 2. Early fibroblast proliferation and new capillary proliferation were uncorporated around graft sites in the experimental groups later than in control groups at early stages. 3. Osteoblastic activity in control group was more prominent at 2 weeks. 4. Osteoblastic activity in experimental groups was more prominent than in control group till 4 weeks. 5. New bone formation was more in control group than experimental group till 3 weeks, but similar appearance after that time. As above results, initial bone healing within 2 weeks were more processed in without adhesive group than with adhesive group. But above 4 weeks; similar bone healing were observed.

Objective: Autologous transfusion is frequently used with orthognathic surgery. The necessity of autologous transfusion has begun to question alone in bilateral sagittal split ramus osteotomy. The aim of this study was to evaluate the availability of autologous transfusion. Methods: The chart reviews of sixty patients who had undergone bilateral sagittal ramus osteotomy were done. The subjects were divided into two groups: In experimental group, 30 patients who autodonated 320ml in the preoperative 3 weeks actually received a transfusion in the postoperative 6 hours were included. In control group, 30 patients who underwent the same operation without preoperative donation and any other transfusion were included. Hemoglobin, hematocrit, and changes of these values in both groups were evaluated. Results: 1. From postoperative 3 hours to postoperative 1 day, the increases of hemoglobin(0.8g/dL in experimental group, 0.2g/dL in control group) and hematocrit(0.3% in experimental group, 0.6% in control group) were not statistically significant between both groups. 2. From postoperative 1 day to postoperative 1 week, the increase of hemoglobin(0.6g/dL in experimental group, 0.3g/dL in control group) was not statistically significant between both groups. But the increase of hematocrit(2.5% in experimental group, 1.0% in control group) was statistically significant between both groups over the same period(hematocrit p=0.043). 3. On postoperative 1 week, the values of hemoglobin(12.3% in both groups) and hematocrit(35.6% in experimental group, 36.8% in control group) were not statistically significant between both groups. Conclusion: The autologous transfusion in surgery of just a little blood loss was not effective. The most results show that there is little availability of autologous transfusion according to changes of hemoglobin and hematocrit in bilateral sagittal split ramus osteotomy.
Purpose: This study was to develop a nursing education program for mothers with a young child undergoing cardiac surgery and determine effects of the program on her knowledge, anxiety and coping behavior. Methods: The research design was a nonequivalent control group non-synchronized design. 18 mothers were assigned to the experimental group and 18 to the control group. The effects were evaluated by measuring knowledge, anxiety, and coping behavior. The collected data was analyzed through independent t-test, and repeated measures ANOVA respectively using SPSS/WIN 12.0 program. Results: The point of knowledge in the experimental group was significantly increased tan that in the control group. The point of subjective, physiological, and behavior anxiety in the experimental group was significantly decreased than that in the control group by time change. The point of coping behavior in the experimental group was significantly higher than that in the control group. Conclusion: The nursing educational program for a young child with cardiac surgery is expected to be clinically applied as an intervention program for mothers with a young child undergoing cardiac surgery.
Purpose: The purpose of this study was to identify the effect of supportive nursing intervention on the anxiety and nursing satisfaction of mothers with children undergoing open-heart surgery. Methods: The research design involved a non-equivalent control group pretest-posttest non synchronized design. The subjects consisted of 43 mothers of children who were expected to undergo open heart surgery, and were divided into two groups; an experimental group of 22 and a control group of 21. The experiment was administered to the experimental group a total of four times, following supportive nursing intervention protocols. The research tools used were Spielberger's State Anxiety Inventory for anxiety and Park Jung-Eun's nursing satisfaction. The data were analyzed using the $x^2$-test, and t-test with SPSS/WIN 12.0 program. Results: Hypothes 1, 'The experimental group who are given supportive nursing intervention will have a lower anxiety score than the control group' was supported(t=5.658, p=.000). Hypothesis 2, 'The experimental group who are given supportive nursing intervention will have a higher nursing satisfaction score than the control gropu wasl supported(t=-9.549, p=.000). Conclusion: The supportive nursing intervention protocol was effective in reducing anxiety and increasing the nursing satisfaction of mothers with children undergoing open-heart surgery.
