Comparisons of Gluteus Maximus and Hamstring Muscle Activities according to Three Different Sling Locations during Bridge Exercise with Sling in Supine Position

  • Purpose: The study was undertaken to investigate the electromyographic activities of the gluteus maximus (GM), hamstring (HAM), and multifidus (MF) in three different sling locations during bridge exercise in the supine position. Methods: Twenty healthy male subjects participated. An electromyography device was used to measure the muscle activities of the GM, HAM, and MF muscles. Subjects were asked to perform bridge exercises with three different sling locations as follows: 1) Bridge exercise with an ankle sling; BEAS, 2) Bridge exercise with a calf sling; BECS, and 3) Bridge exercise with a knee sling; BEKS in random order. The analysis was conducted using one-way repeated ANOVA and the Bonferroni post hoc. Significance was set at α=0.01. Results: HAM muscle activity was significantly different in the three conditions (BEAS, BECS, BEKS) (adjusted p-value [padj]<0.01), and HAM muscle activity was significantly smaller during BEKS than during BEAS or BECS (padj<0.01). Muscle activity ratio (GM/HAM) during BEKS was significantly greater than muscle activity ratios during BEAS or BECS (padj<0.01). Conclusion: BEKS is recommended to inhibit HAM muscle activity and improve the GM/HAM muscle activity ratio.

삼두근 근력 운동 시 운동 자세와 전완 자세에 따른 삼두근 장두와 외측두의 근 활성도 비교 (A Comparison of EMG Activity for Long and Lateral Heads of Triceps Brachii Muscles According to Exercise and Forearm Positions During Triceps Strengthening Exercises)

  • The aim of this study was to compare electromyography activity for long and lateral heads of triceps brachii muscle according to forearm positions during different triceps strengthening exercises. The muscle activities for long and lateral head of triceps brachii were measured by surface electromyography. Fifteen healthy volunteers participated for this study and performed elbow extension in three different elbow extension exercises (elbow extension in a supine position; EES, elbow extension with shoulder abduction at 90 degrees in a prone position; EESA, and elbow extension with one arm at the side of the trunk in a prone position; EESP) and forearm positions (supination, neutral, and pronation). A two-way repeated measures ANOVA was used to compare the effects of the exercise positions and forearm positions. The EMG activities of the long head of the triceps brachii increased significantly during EESP with forearm supination, whereas the activity of the lateral head of the triceps brachii increased significantly during EESA with the forearm in a neutral position (p<.05). The results of this study suggest that exercise positions and forearm positions should be considered for selectively strengthening the long and lateral heads of triceps brachii muscles.

대퇴사두근 근력 운동 시 고관절 중립 자세와 내전 자세에 따른 내측광근과 외측광근의 근수축 개시 시간 차이 비교 (Comparisons of Vastus Medialis and Vastus Lateralis EMG Onset Time During Quadriceps Strengthening Exercise in Neutral and Adducted Hip Positions)

  • This study examined the vastus medialis oblique (VMO) and the vastus lateralis (VL) onset time differences (OTD) during quadriceps contraction in different hip positions. Twelve healthy subjects were recruited (four men, eight women). Surface EMG activities of the VMO and VL were measured during a quadriceps strengthening exercise in a long sitting condition and in a sitting at a chair with feet hanging condition. For each condition, subjects were tested in two hip positions (neutral and adduction). The OTD between the two muscles was calculated for each condition, by subtracting the onset time of the VL from the VMO. Therefore, the negative value of OTD represent earlier EMG onset of the VMO compared to the VL. The OTD was not significantly different between the hip neutral and the hip adduction position in the long sitting condition (p=.064). However, the OTD was significantly different between the hip neutral position ($15.83{\pm}109.51$ ms) and hip adduction position ($-5.58{\pm}121.08$ ms) during the sitting at a chair with feet hanging condition (p=.047). The negative OTD value in the hip adduction condition during quadriceps strengthening exercises is the result of earlier onset of the VMO than VL. Therefore, quadriceps contraction in the hip adduction position can prevent the risk of patella lateral tracking. We expect that quadriceps strengthening exercise in the hip adduction position will be a safe way to prevent patellofemoral pain syndrome resulting from abnormal patella lateral tracking.

Effects of a Trunk Stabilization Exercise with Gym Ball on Scapular Position for Patients with Arthroscopic Rotator Cuff Repair

  • Purpose: The present study was to investigate effects of scapular position and pain on a trunk stabilization exercise with gym ball for patients with arthroscopic rotator cuff repair. Methods: Subjects were patients after 2 weeks of rotator cuff repairs. Subjects were randomly assigned to conservative treatment group (CTG, n=10) or trunk stabilization with gym ball group (SBG, n=10), and participated one of those groups for 2 weeks. Measurements about pain and scapular position were assessed in before and after treatment programs. Results: Pain reduced in both rest and night in SBG compared to CTG. In the case of scapular position, decreased scapular retraction (increased protraction) was found in CTG and vice versa in SBG. Reduced scapular anterior tilting was also found on SBG. Conclusion: The present study showed that early performed trunk stabilization with gym ball could bring a scapular alignment that increases subacromial space and reduces pain. This suggests early trunk stabilization with gym ball programs to restore shoulder functions for patients with rotator cuff repair.

오십견이 있는 53세 여성환자에게 견갑골 안정화 운동 적용 후 견부 통증, 능동관절가동범위, 견갑골의 위치 변화에 미치는 영향-사례연구 (A Case Study of a 53-year-old Female Patient with Frozen Shoulder for Effect of Scapular Stabilizing Exercise on Shoulder Pain, Active Range of Motion and Position of Shoulder Blade)

  • Background: The research examined the effect of a scapular stabilizing exercise on shoulder pain, active range of motion (AROM) and changes in the position of the shoulder blade of patients with frozen shoulder. Methods: General physical therapy and a scapular stabilizing exercise were applied to a 53-year-old female patient with frozen shoulder three times a week for four weeks. Before and after the therapy, measurements were made on the visual analog scale (VAS) and of the AROM, and a lateral scapular slide test was conducted. Results: Following the therapy, VAS fell from 7 to 3. Before therapy, flexion, abduction, internal rotation, and external rotation recorded $133^{\circ}$, $102^{\circ}$, $45^{\circ}$, and $53^{\circ}$ respectively; after the therapy they increased to $150^{\circ}$, $123^{\circ}$, $55^{\circ}$, and $65^{\circ}$ respectively. The lateral scapular slide test showed sliding distances before therapy of 8.5cm at $0^{\circ}$, 9.2cm at $45^{\circ}$, and 11.3cm at $90^{\circ}$; after therapy, the distances decreased to 6.8 cm at $0^{\circ}$, 8.2 cm at $45^{\circ}$, and 9cm at $90^{\circ}$. Conclusion: The scapular stabilizing exercise was effective in alleviating shoulder pain, increasing AROM, and reducing changes in the position of the shoulder blade of frozen shoulder patients.

상지림프부종환자의 견갑골안정화운동이 안정시 견갑골 위치에 미치는 영향 (Effects of Scapular Stabilizing Exercise on Resting Scapular Position of Breast Cancer-related Lymphedema Patients)

  • PURPOSE: The purpose of this study was to confirm the effect of exercise combined with scapular stabilizing on resting scapular position (RSP) in breast cancer-related lymphedema patients. METHODS: A total of 20 patients with lymphedema after mastectomy participated in the study. All assessments of the patients edema sides (ES) and non-edema sides (NES) were evaluated. The assessment tools used wad RSP. RSP are; 1) scapular index, 2) 8th thoracic spines process (T8S) to inferior angle of scapular (IA) distance, 3) standing pectoralis minor (PM) distance, and 4) PM index (PMI). All patients carried out a scapular stabilizing exercise seven times a week for 8 weeks. The collected data were analyzed with PASW 18.0. The statistical significance (${\alpha}$) was 0.05. RESULTS: According to the results, all the variable between the ES and NES for RSP were statistically significant (p<0.05) in the pre-test. After the exercise, the differences in T8S to IA distance and the PMI between the ES and NES weren't statistically significant. The results of the RSP showed a significant improvement in T8S to IA distance, standing PM distance, and PMI. CONCLUSION: The results of this study showed that, performing the scapular stabilizing exercise had a significant effect on improving RPS in breast cancer-related lymphedema patients.

누운 자세에서의 자전거 운동 시 하지 근활성도 분석 (Analysis of Lower-Limbs Muscle Activity during Cycle Exercise in Spine Position)

  • 본 연구에서는 입원 또는 재활 초기단계에 환자가 이동하지 않고 침대에 누운 상태에서 자전거 운동이 가능한 하지 재활 훈련 시스템을 개발하였으며, 정상 성인을 대상으로 침대에 누운 자세에서 다양한 조건에서의 자전거 운동 시의 하지 근 활성도를 비교 분석하고자 하였다. 피험자는 근 골격계나 신경근육계 병력이 없는 20대~60대 사이의 건강한 성인 남녀 42명을 대상으로 하였으며, 운동 모드, 부하, 속도에 따른 자전거 운동 시 오른쪽 하지의 대퇴직근, 대퇴이두근, 전경골근, 내측 비복근, 가자미근의 근 활성도를 측정하였다. 그 결과, 부하를 1과 5단계로 제공하였을 때보다 10단계로 제공하였을 때 내측 비복근의 근활성이 높아짐을 보였다. 또한, 속도가 증가할수록 내측 비복근을 제외한 모든 근육에서 근활성도가 감소하는 결과를 보였다. 즉, 운동 조건에 따라 크게 활성화되는 근육들이 각기 달랐으며, 이러한 결과는 침상에 누워 재활훈련이 가능한 자전거 시스템 개발을 위한 기초 자료로써 활용될 것이다.

Consideration of the Exercise position for Facilitating Gluteus Medius Maximally in Normal Adults

  • Purpose: This study examined the most efficient exercise position to activate the gluteus medius (GM) and tensor fascia latae (TFL) in hip abduction in side-lying (HA-SL), clam in side-lying (CL-SL), and sling bridging in side-lying (SB-SL), which are the most representative GM exercises. Methods: Twenty-four healthy male adults aged from 20 to 40 years, whose body mass index was under 25, participated in this experiment. While all participants conducted three different positions with a counterbalanced manner, such as in AB, CL, and BR, activation of the GM and TFL was measured using 8-channel wireless EMG. Exercise was performed for 10 seconds three times in total with a five minute-break session. Results: Significant differences in GM and TFL activation were observed among the three positions (p<0.05). The highest activation of 60.69 was observed in BR followed in order by 46.03 and 12.92 in HA-SL and CL, respectively. TFL activation in HA-SL was 42.01, followed in order by 35.98 and 14.01 in BR and CL, respectively. On the other hand, there was no significant difference in TFL muscle activation between BR and HA-SL. Conclusion: These findings suggest that both BR and HA-SL in GM can be done selectively. CL has remarkably low muscle activation in GM and TFL, which makes it less valuable in GM and TFL exercise. In conclusion, selective BR and HA-SL exercise should be applied to maximally and effectively activate the GM.

엎드린 교각운동 자세에서 견갑골 전인 운동 시 익상 유무에 따른 다리 들기 차이가 체간근의 근 활성도에 미치는 영향 (The Effect of Legs Difference on The Trunk Muscle Activities With and Without Winging Scapular During Scapular Protraction in Prone-Bridge Position)

  • Background: The purpose of this study was to investigate the effect of leg lift difference on serratus and upper trapezius when exercising in a scapula in a prone position, a typical waist stabilization exercise for subjects with a winged scapula. Method: Twenty normal adults and 20 subjects with winged scapula participated in the experiment. The surface EMG recordings were obtained from external oblique muscle and internal oblique muscle during scapula protraction exercise. The presence or absence of winging of the shoulder bone was measured using an electronic digital caliper for the distance the medial border of the scapula is lifted to the rear. In prostrate pier movement posture in both groups, both legs supporting, dominant leg lifting, and non-dominant leg lifting including the scapula protraction were conducted respectively. Results: In the results of comparison between the two groups, the dominant external oblique muscle and the non-dominant internal oblique muscle tended to increase according to the difference of the leg lifting of normal people. In the winged scapula group, internal oblique muscle showed increased muscle activity more than external oblique muscle. Conclusion: It was most effective to exercise with lifting the same position leg for strengthening the same external oblique muscle, and the opposite internal oblique muscle. Also, it is effective to exercise in prone pier movement posture for trunk stability. In addition, internal oblique muscle shows increased muscle activity in subjects with winged scapula. Therefore, appropriate adjustment of external oblique muscle and internal oblique muscle may have a positive effect on scapula dysfunction for trunk stability.

Effects of Isokinetic Strength Exercise of the Hemiparetic Knee Joint on Lower Extremity Sensorimotor and Gait Functions in Patients With Chronic Stroke: Open Kinetic Chain Versus Closed Kinetic Chain

  • Background: After stroke, in order to improve gait function, it is necessary to increase the muscle strength and to enhance the propriocetive function of the lower extremity. Objects: This study aimed to compare the effects of open kinetic chain (OKC) versus closed kinetic chain (CKC) isokinetic exercise of the hemiparetic knee using the isokinetic equipment on lower extremity sensorimotor function and gait ability in patients with chronic stroke. Methods: Thirty participants with chronic hemiplegia (> 6 months post-stroke) were randomly divided into 2 equal groups: CKC group and OKC group. Patients from both groups attended conventional physiotherapy sessions 3 times a week for 6 weeks. Additionally, subjects from the CKC group performed isokinetic exercise using the CKC attachment, while those from the OKC group performed isokinetic exercise using the OKC attachment. The isokinetic knee and ankle muscles strength, position sense of the knee joint, and spatiotemporal gait parameters were measured before and after interventions. Results: The knee muscles peak torque/body weight (PT/BW) and hamstring/quadriceps (H/Q) ratio significantly increased in both groups (p<.01). In particular, ankle plantarflexors PT/BW, position sense of the knee, gait velocity, and spatial gait symmetry significantly improved in the CKC group (p<.01, p<.05, p<.01, and p<.01, respectively). Conclusion: CKC isokinetic exercise can be an effective therapeutic intervention for the improvement of sensorimotor function of the lower extremity and gait functions, such as gait velocity and symmetry. CKC position in isokinetic strength training is effective to improve functional ability in patients with chronic stroke.
