The evaluation about 10 cases of Childhood Obesity Treatment Program in Oriental Medical Center (한방병원에서 시행 한 소아 비만 관리 프로그램 10예에 대한 평가)

  • Objective : There have been many programs to treat childhood obesity, since childhood obesity has shown up as a social problem. The purpose of this study was to find out considerable matters to manage Childhood Obesity Treatment Program (COTP). Methods : This study was made with reference to clinical progress notes of children with obesity, visiting the oriental medicine center, to take COTP from July 30th, 2003 to August 25th, 2004. COTP was consist of abdominal aroma massage, abdominal low frequency acupuncture therapy, auricular acupuncture therapy and behavior modification. Results : 1. Seven children who taken COTP started fat from young age and three started from about ten-year-old ages. 2. Six children have fat fathers, a child has fat parents and three children have no fat family member. 3. The obesity levels of Obesity Index(OI), Body Mass Index(BMI) and Rohler Index(RI) were different, used for evaluating the result of treatment, though they are used for a child. The result of OI reflected more sensitive from changes of weight than those of BMI and RI. 4. We got the better results with numbers of treatment and exercise. Conclusions : More correct standard are needed to estimate degree childhood obesity. And it is necessary to carry out obesity treatment program with behavior modification, for children with obesity. to be healthy adults.

Development and Clinical Evaluation of the Upper Extremity Rehabilitation Game Program for Patients with Upper Extremity Hemiplegia After Stroke Using Smartphone: Preliminary Study (스마트폰을 이용한 뇌졸중 후 상지 편마비 환자의 상지 게임재활훈련 프로그램 개발 및 임상적 유용성 평가에 대한 예비연구)

  • In the paper, we developed the mobile based rehabilitation system for patients with upper extremity hemiplegia after stroke and evaluated clinical usefulness and effectiveness of the system. The sensors built in the smartphone were used to track patients' upper limb motion and the movements was transferred to the tablet PC through bluetooth connection so that the game contents could be interact with the movements. The rehabilitation game contents was based on Brunnstrom stage(B-stage), and was designed to lead accurate movement of upper limb. For the clinical evaluation of the effectiveness, 11 patients were recruited and make them perform an exercise of their wrist, shoulder, and forearm using the system for two weeks. The change of upper limb motor function was measured using fugl-meyer assessment(FMA), Brunnstrom stage(B-stage). And the change of quality of life was measured using EuroQoL-5 Dimension(EQ-5D), Beck Depression Inventory(BDI). The results showed significant improvement in upper limb function but not in quality of life. We verified mobile based rehabilitation program could be useful and effective for the clinical use.

An Analysis of Recognitions of Elementary School Students on Useful Classes among School Sport Clubs for Program Development using the IPA Method (IPA매트릭스를 이용한 초등학생의 학교스포츠클럽 유익한 수업 인식을 위한 프로그램 개발 분석)

  • The purpose of this study was to verify the analysis of level of importance and satisfaction in recognitions of elementary school students on useful classes among school sport clubs using Importance-Performance analysis. In order to achieve this objective, samples were taken by using convenience sampling method among non-probability sampling methods, and 384 data were used as the final valid samples for this study except 16 data with missing items or insincere responses. The results of frequency analysis, exploratory factor analysis, independent samples t-test, IPA analysis by using SPSS 20.0 were as follows. First, Iquadrant included education contents of enhance ability to ingenuity, teaching method of provides option, teaching method of encourage participation activity, education contents of the aspect of fun, evaluation of motor function and emotion, education contents of explains key contents easily, and education contents of understanding overall context. II quadrant included education environment of good sport facility, class environment of fair opportunity for activity, class atmosphere of arouses interest, and class atmosphere of autonomous and voluntary. III quadrant included diverse teaching method and instructor's demonstration, class atmosphere of systematic learning, evaluation of fairness, objectivity, and credibility, and an atmosphere that can exercise. IV quadrant included education contents of enable acquisition of knowledge and degree of improvement into consideration, and class atmosphere of trust and respect between instructor and student.

The Coordinative Locomotor Training Intervention Strategy Using the ICF Tool to Improve the Standing Posture in Scoliosis: A Case Report

  • Purpose: This study was examined to improve the standing posture of a scoliosis client using the ICF Tool. Methods: For examination, the study subject was a 16-year-old female student diagnosed with 3curve-pelvic (3CP) type scoliosis. Information about her were collected through a client interview and based on international Classification of Functioning, Disability and Health (ICF). The ICF core set was for post-acute musculoskeletal conditions, and the ICF level 2 items suggested by National Rehabilitation Information Center (NARIC) were added to the recommendations for scoliosis. For evaluation, the ICF assessment sheet was used to identify the interaction among the problems. For the diagnosis, the client's functional problems were described in ICF terms. For the prognosis, the global goals for reaching the client's functional activity and participation level were presented as the long-and short-term goals. For the intervention, a coordinative locomotor training program composed of warm-up, main exercise, and cool-down was applied 3 times a week, 50 minutes a day, for 5 weeks. For the outcome, the differences between before and after the intervention were compared with the ICF qualifier and are shown with the ICF evaluation display. Results: Clinical advantages were observed in body function and structure (7° decrease of thoracic angle, 7 score increase of trunk muscle power, 6.47s improve of one leg standing, 4 score decrease of neck pain). The activity for maintaining the standing posture, in which the client had a primary limitation, was improved. Conclusion: Applying the coordinative locomotor training program is expected to improve scoliosis client's standing posture.

The Development and Evaluation of Web-based Education Program for Lung Cancer Patient (폐암환자를 위한 웹기반 교육프로그램 개발 및 평가)

  • The purpose of this study were to develop an web-based education program for Lung cancer patients and to test its effects on patients' self-care knowledge, compliance to medical regimen, nutrition status and pain. The program was developed by the following process: first, Lung cancer patients demand on the web-based program was investigated. and second, the program was developed with the help of various reference books and then validation of experts group. last, educations effects on the patients is evaluated and compared the differences in self-care knowledge, compliance to medical regimen, nutrition status and pain between on experimental group and a control group on before discharge 1day and 3weeks after. SPSS/Win 11.0 program was used for data analysis. It was proven with $x^2$ test and t-test, and Pearson Correlation coefficient, and Chronbach's alpha coefficient were done for the reliability of measuring instruments. 1. The summary of the Program development is as follows. The program is based on patients' questionnaire and reference material and is made for users friendly. Not only Bigger font size and bright colors but also illustrations or pictures were adopted to help enhance patients' understanding. 2. The summary of the study results is as follows. 1) Compared with control group, the web-based educated experimental group showed a statistical significant difference on self-care knowledge, Especially disease, radiation treatment, medication & analgesics, chemotherapy side effect, but there was no significant difference in the field of chemotherapy, in the fields of operation, diet & general knowledge. 2) Compared with control group, the web-based educated experimental group showed a statistical significant difference on compliance to medical regimen, especially in the field of follow up care, everyday life, diet, but there was no significant difference in the field of medication, exercise. 3) Compared with control group, web-based educated experimental group showed no significant difference in nutrition status, but partially significant difference in body weight. 4) Compared with control group, the web-based educated experimental group showed no significant difference in pain level. 5) The significantly positive correalation self-care knowledge with the compliance to medical regimen. 6) Users satisfaction with the web-based education program of the contents quality, the level of recommendation to others, content layout, medical information quality, but interesting got a low mark.

Factors Associated with Perticipation in a Walking Campaign (걷기행사 참가의 결정요인)

  • Objective: The purposes of this study are to understand the characteristics of the participants in a community walking campaign and to analyze the factors related to their participation based on the Transtheoretical Model (TTM). Methods: The study composed of the description of participant characteristics and comparison of them with non-participant characteristics in a walking campaign in K province. The data were collected through a survey of 2,590 participants and 258 non-participants from the same community. The survey instrument included questions about stages of walking and exercise, knowledge and attitude toward walking, and environmental condition for walking. Results: A majority of the walking campaign participants were in the action(24.8%) and maintenance(43.6%) stages of walking and exercise behavior. The non-participant group was split between maintenance(51.6%) and precontemplation (30.6%) stages. Among the participants, effective campaign promotion channels differed by age group while motivation for participation and participation patterns were associated with both age and gender. Favorable physical environment was a significant factor of participating in walking campaign(OR=1.396, CI=1.149-1.696). Although the campaign participants scored higher than the non-participants in most attitude toward walking questions, differences in knowledge scores between two groups were less significant. Conclusion: In conclusion, further social marketing to increase the awareness and to increase the concern of population in the community is needed based on the survey result. Transtheoretical model seems appropriate to apply to the evaluation and the planning the program of the behavior change in the community. Also, more organized and sustainable support in need to maintain the good habit of walking for the participants in walking campaign.

Effects of Three Different Hip Positions in Frontal Plane on Activity of Abdominal Muscles During Active Straight-Leg Raise

  • Active straight-leg raise (ASLR) is a physical evaluation procedure to test lumbar spine stability. Several previous studies have reported various methods to control the activation of abdominal muscles during ASLR. We investigated the effects of three different hip positions in frontal plane on abdominal muscles to increase or decrease the difficulty level of lumbar spine stability exercise during ASLR in pain free subjects. Eleven young and healthy subjects voluntarily participated in this study (6 men, 5 women; mean age=$24.0{\pm}1.2$ years, height=$160.0{\pm}7.3cm$, weight=$55.0{\pm}10.6kg$, body mass index=$21.5{\pm}2.3kg/m^2$). The subjects had three trials on each ASLR with hip $10^{\circ}$ adduction, neutral hip, and hip $30^{\circ}$ abduction. Separate repeated-measures analysis of variance (ANOVA) and the post hoc Bonferroni tests (with ${\alpha}$=.05/3=.017) were performed for each muscle among the three different hip positions in frontal plane (ASLR with hip $10^{\circ}$ adduction, neutral hip, and hip $30^{\circ}$ abduction). The ipsilateral external oblique (EO), contralateral EO, ipsilateral internal oblique/transverse abdominis (IO/TrA), and contralateral IO/TrA were significantly greater in ASLR with hip $30^{\circ}$ abduction compared with ASLR with hip $10^{\circ}$ adduction. Also, the ipsilateral EO, contralateral EO, and ipsilateral IO/TrA were significantly greater in ASLR with hip $30^{\circ}$ abduction compared with ASLR with neutral hip. These results suggest that ASLR with hip $30^{\circ}$ abduction and neutral would be useful method to strengthen the EO and IO/TrA. And, ASLR with hip $10^{\circ}$ adduction would be effective in early stages of lumbar stabilization program due to low activation of EO and IO/TrA during maintaining of ASLR position with low load.

Evaluation of Saturday Nutrition Classes for Obese Elementary Students in Chungnam Province (비만 초등학생의 토요 영양 교실의 운영 효과 평가)

  • This study was designed to evaluate Saturday nutrition classes for obese elementary students in Chungnam Province. Obesity index, nutrient intake, nutrition knowledge, nutrition attitude, and self-satisfaction of 36 obese elementary students were compared before and after attending nutrition classes every other Saturday for 3 months. The average age, height, weight, body fat, % body fat, and obesity index were 11.9 years, 145.5 cm, 53.3 kg, 24.5 kg, 45.7%, and 32.4%, respectively. Most of the subjects(94.5%) viewed themselves as fat. Sixty-three percent of children were discontent with their body-image. All answered that they have experience with weight control. The major methods of weight control were exercise and reduction of snacks. The mean serum cholesterol, blood glucose, GOT/GPT, and hemoglobin were 177.4 mg/dL, 90.4 mg/dL, 25.8/25.5 IU/L, 14.5 g/dL, respectively. After attending nutrition classes, the daily intakes of energy, plant protein, plant fat, carbohydrate, crude fiber, and cholesterol increased significantly. The scores for nutrition knowledge, nutrition attitude, and self-satisfaction also increased after nutrition classes, but there were no significant differences. Percent body fat decreased significantly after nutrition classes. In conclusion, nutrition education for elementary students is effective for the prevention and control of obesity. Further research is needed to develop a systematic program of nutrition education for obese children.

Evaluation of Various Cardiae Indices and ROC Analysis in Coronary Artery Disease Employing Resting ECG Gated Blood Pool Scan (관상동맥질환에서 휴식기 심전도게이트혈액풀스캔을 이용한 각종 심기능 지표들의 평가 및 ROC 분석)

  • Gated blood pool scan is frequently used for evaluating the change in cardiac function in various cardiac diseases. But resting gated blood pool scan using only LVEF as a cardiac index has been consitently shown to have a low sensitivity, which is about 50%, in detecting coronary artery disease. So it is recommended to compare exercise gated blood pool scan to resting gated blood pool scan. Exercise tests, however, are not always possible, especially in patients with musculoskeletal diseases, recent myocardial infarction and in elderly persons. We studied the usefulness of resting gated blood pool scan using multiple indices in evaluating the patients with coronary artery disease. Studied cases were 185 patients with coronary artery disease (angina pectoris 31, myocardial infarction 154) and 25 normals with low likelihood of coronary artery disease. We used $^{99m}Tc-labeled$ RBC, 740 MBq labeled by in vivo method. The data were evaluated by Micro DELTA computer program. The results were as following: 1) The ejection rates (PER, AER) and filling rates (PFR, AFR) were different in normls and patients with angina pectoris or myocardial infarction. 2) Mean phase angle, ejection rates and filling rates could separate normals from coronary artery disease patients with normal LVEF. 3) Regional ejection fraction was decreased at the site of the infarct in patients with myocardial infarction. 4) Peak filling rate was the the most detectable index in evaluation of cardiac function in patients with coronary artery disease. 5) The threshold at 1.5 standard deviation of normal range was considered as the most reliable cut-off value from ROC analysis. These data suggest that the resting gated blood pool scan has an important role in the evaluation of cardiac functional changes using various cardiac indices in patients with coronary artery disease.

  • Local governments need to be prepared for emergency response in order to minimize damages caused by disasters, such as typhoon and chemical incidents. In this study, we have researched and analyzed current emergency preparedness status of Korea's local governments by studying laws and policies, and interviewing emergency managers. We have introduced policies to enhance emergency preparedness capabilities of United States' local governments. These policies could be useful for Korean government in developing policies to build up emergency response capabilities of Korea's local governments.
