The Effects of the Elderly's Subjective Health Perceptions and Quality of Life on Their Depression and Suicide Ideation (노인의 주관적 건강 인식과 삶의 질이 우울과 자살 생각에 미치는 영향)

  • The study thus examined and analyzed 1,478 people aged 65 or older in the Korea National Health and Nutrition Examination Survey of 2010 to investigate the effects of differences in the elderly's general characteristics and depression & suicide ideation and in their subjective health perceptions and quality of life on their depression and suicide ideation. As a result, Significant differences in their suicide ideation were also caused by such quality of life variables as ill in bed, exercise capacity, self-management, daily activity, pain and uncomfortableness, and anxiety. The logistic analysis results of depression and suicide ideation according to the degree of subjective health perception show a significant finding that the better health perception they had compared with their very poor subjective health perception, the lower the odds ratio became. In the category of quality of life, only experience of uncomfortableness caused significant results on suicide ideation and depression. The results of the study will be of great use as basic data to intervene in the suicide route of the elderly and take measures to prevent suicide.

Relationship between health factor, oral health factor and prevalence of depression in Korean elderly (한국 노인의 건강요인 및 구강건강요인과 우울증 유병여부의 관련성)

  • Objectives: The purpose of the study is to investigate the relationship between health factor, oral health factor and revalence of depression in Korea elderly by data from the 5th Korea National Health and Nutrition Examination Survey(KNHANES) 2012. Methods: The subjects were 10,938 elderly over 65 years old by rolling survey method and data were extracted from the 5th Korea National Health and Nutrition Examination Survey(KNHANES) 2012. Data of 1,421 elderly were finally selected and analyzed using kstrada. The questionnaire consisted of socio-demographical characteristics of the subjects, health factors, oral health factors and depression. The oral health factors included subjective perception of oral health, alcohol consumption, exercise, smoking, and toothache. Results: Health factors influencing on the prevalence of depression were subjective health(p<0.001), stress(p<0.001), and alcohol consumption(p<0.05). Oral health factors related to the prevalence of depression were subjective oral health(p<0.05), annual dental checkup(p<0.01), and speaking problem(p<0.01). Conclusions: It is necessary to develop the oral health promotion program for the elderly and help them maintain good quality of life and mental health.

Diagnostic Ability of Tc-99m MIBI SPECT in Coronary Artery Diseases is not Affected by the Degree of Exercise (운동 정도와 무관한 Tc-99m Methoxyisobutyl Isonitrile 심근관류 스캔의 진단능)

  • Purpose: The sensitivity of ST-depression in the electrocardiogram during exercise is influenced by the level of efforts. However, unlike the prevalence of ST-depression on exercise ECG, the degree of exercise is reported to do not influence the diagnostic ability of myocardial perfusion scan. Furthermore, the relation between the prevalence of myocardial ischemia and effort is still controversial. We evaluated the effect of the degree of exercise on the ability of SPECT imaging to detect coronary artery stenosis. Materials and Methods: The patient population was comprised of 111 patients (73 men and 38 women, mean age 56 years) who underwent an exercise test in conjunction with Tc-99m MIBI and cardiac catheterization within 3 months apart each other. The degree of exercise was classified into four groups according to the percentage of maximal predicted heart rate. The sensitivity and specificity was compared between each group. Results: The overall diagnostic sensitivity was significantly higher with Tc-99m MIBI SPECT than exercise ECG. The specificity was not significantly different between two tests. Sensitivity and specificity of Tc-99m MIBI was not different between four groups. Sensitivity for individual coronary stenosis seemed to be lower in subjects who had premature termination of exercise due to early appearance of ST depression. Conclusion: These results suggest that the overall diagnostic sensitivity of Tc-99m MIBI myocardial perfusion SPECT is not significantly affected by the degree of exercise in stable patients undergoing symptom-limited treadmill exercise testing. Myocardial perfusion SPECT imaging should be added to routine exercise stress testing for the detection of coronary artery disease.

Depression According to Health Perception and Health Practice Among the Aged in an Area (일 지역 노인의 건강인식과 건강행태에 따른 우울)

  • Purpose: This study was performed to understand health perception, health practice, and depression of the elderly and to identify their depression level according to health perception and health practice. Method: The subjects were 463 elders who have lived in the Jeju Island and data were collected from June to August in 2005. Data were analyzed by SPSS 10.0. Result: The mean score of depression was 12.94 out of 30, and had a statistically significant correlation with educational level (F=2.943 p=.033), occupation (F=4.611. p=.010), and the number of chronic diseases (F=5.303. p=.001). When the cutoff of 18 points was applied based on Jung et al. (1997), the depression ratio was 12.4%, and when the cutoff parameter by Yesavage et al (1993) was applied, themoderate depression ratio was 74.8% and the severe depression ratio was 3.4%. In health recognition, consideration of usual health state had a significant correlation with depression (F=3.553 p=.007) but consideration of health state compared to the previous year wasn't. In health practice, sleeping was in a significant correlation with depression (F=3.574 p=.011), but smoking, alcohol drinking, exercise, and rest were not. Conclusion: Based on above results, we need further study on another sample group and the development, application, and verification of health management, education, and counseling programs for the elderly. Also, additional research should be made on significant factors of elderly depression.

The Effect of Telephone-enforced Pelvic Floor Muscle Exercise on Incontinence, Urinary Function Related Satisfaction, Depression, and Quality of Life for Prostate Cancer Patients (수술 전 골반저 근육운동과 전화관리가 근치적 전립선 적출술을 받은 전립선암 환자의 요실금 지각정도, 배뇨 관련 만족도, 우울 및 삶의 질에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effects of Telephone-enforced Preoperative Pelvic Floor Muscle Exercise (TP-PFME) for prostate cancer patients on Urinary Incontinence (UI), Urinary Function (UF) related satisfaction, depression, and Quality of Life (QoL). Methods: A non-equivalent control group non-synchronized design was used with 85 participants. The two experimental groups (EG) began the PFME protocol two weeks prior to surgery. The subjects in the EG I received telephone calls to reinforce the PFME protocol. The comparison group began the PFME protocol after their surgery. Data were analyzed by the SPSS/WIN 21.0 using descriptive statistics, $x^2$ test, One-way ANOVA, Fisher's exact test, and Repeated measures ANOVA. Results: EG I showed higher performance of PFME at one (p=.001) and three months (p=.015) after surgery than the comparison group. Comparison group showed significantly more severe UI at one (p=.002) and three months (p=.006) after surgery and reported lower UF related satisfaction than EG I at one month after surgery (p=.015). Participants in both experimental groups reported higher QoL scores (p=.001) at three months following surgery than those in the comparison group. There were no significant differences in depression among the three groups. Conclusion: The findings from this study suggest that preoperative TP-PFME was effective in decreasing the perceived severity of UI as well as increasing UF related satisfaction and QoL.

The Effects of Yoga Program on Depression, Quality of Life, and Difficulty of Daily Activities in Community Elders (요가 프로그램이 재가노인의 우울, 삶의 질, 일상활동장애에 미치는 효과)

  • Objective : The purpose of this study was to investigate the effects of yoga program on depression, quality of life, and difficulty of daily activities in community elders. Methods : A Nonequivalent control group pretest-posttest design was used for this study. Elderly people who agreed to participated in the study were assigned to a experimental group(n=17) or a control group(n=17). The yoga program conducted 90minutes, 2times a week for 1 year. Study outcomes were measured by structured questionnaires from March, 2012 to Feb., 2013. For data analysis, Chi-square test, t-test, and Mann-Whitney U test were performed using SPSS version 17.0. Results : Depression was not significant difference between the two groups in pretest and posttest. But there was significant differences between the two groups in quality of life (t=3.984, p<.001), and difficulty of daily activities (t=-3.083, p=.004). Therefore, the experimental group showed significant improvement in quality of life and reduce difficulty of daily activities. Conclusion : The yoga program was effective in increasing quality of life and decreasing difficulty of daily activities in community elders. When developing exercise program to improve health in older adults, it is important to consider physical exercise plus general health management.

Chronic Fatigue and Related Factors in Adults (성인의 만성피로와 그 관련 요인에 대한 연구)

  • Purpose: To identify characteristics of chronic fatigue, difference between the duration of fatigue and characteristics of chronic fatigue, and association between chronic fatigue and related factors in adults. Method: The subjects for this study were 180 adults who had experienced fatigue for over one month The measurement tools were the Revised Fatigue Scale by Chalder et al. (1993), the Visual Analogue Scale-energy developed by Lee et al. (1991), the BEPSI (Brief Encounter Psychosocial Instrument) by Frank & Zyzanski (1988), Zung's self rating depression scale (1965) and life style items including exercise sleep, drinking smoking and diet as developed by the researchers. Results : Those who complained of fatigue over six months experienced higher disturbances in their daily life compared to those who had suffered from fatigue less than six months. There were a significant correlation between severity of fatigue and depression (r=.46, p<.001), stress (r=.41, p<.001), and sleep (r=.20, p<.01) Statistically significant relationships were found between severity of fatigue and amount of exercise per week (F=3.79. p<.05) disturbed sleep (t=-2.66, p<.01), number of times awakened during the night (F=3.48, p<.05) types of drinking (F=2.65, p<.05), and diet regularity (F=5.83, p<.01). The construction of a multiple regression model revealed an adjusted $R^2$ of .27 with the depression score serving the major predictor variables for severity of fatigue. Men and people in the younger age group were more energetic than women and those in other age groups. Also married people experienced more fatigue than people who were single. divorced or separated. Conclusion: In nursing interventions for fatigue, medical personnel should consider sociodemographic characteristics of the clients, ways to reduce of stress and counter disturbances in daily life and develop strategies for a health promoting life style.

  • This study was conducted to test whether a comprehensive health promotion program for promotion strategies and knowledge about the disease, thus leading to the improvement of health status by using repeated measure of quasi- experiment design. Eighteen RA patients who visited the RA clinic of an university hospital located in Inchon were invited to participate in the CHPPRA. According to the study results, it was shown that the CHPPRA had significant effects on the patients' health status such as pain, depression, and functional disability. Also, that the improvement of health status was achieved by a positive change in the four health promotion strategies, which consisted of goal setting, positive thinking, exercise, and knowledge about the disease. Goal setting, positive thinking, and knowledge about the disease could also affect the patients' depression. Thus it can be interpreted that the improvement of these strategies may result in a remarkable decrease of depression. In addition, alleviation of functional disability may be due to increase of exercise. However although the strategies which were directly associated with pain management were not significantly improved, pain was significantly reduced. On the other hand, the study result showed that the other health promotion strategies included in CHPPRA such as pain management, positive thinking, stress management, asking for assistance and communication were not significantly increased. although the health status such as pain, depression, and functional disability, which are final goals of the program, were significantly improved through the exposition of patients to those health promotion strategies.

Depressive Symptoms of the Population Aged 19 and Over due to Regional Gaps in Sports Facilities (생활체육시설의 지역 간 격차에 따른 19세 이상 인구의 우울증상)

  • Background: Depression is a common disease around the world. Many studies are showing that mental health can be improved through physical activity, and daily regular exercise can reduce the negative effects of depression or depressive symptoms. In order to promote individual physical activity, a physical activity-friendly environment must precede. Therefore, this study attempted to confirm whether the number of sports facilities for all affects individual depression. Methods: Among the respondents to the 2018 Community Health Survey, data from 181,086 people excluding missing value were used. Descriptive and chi-square tests were performed to understanding the general characteristics of individual level variables. A multilevel logistic regression was conducted to confirm the effect of individual and regional level variables on depressive symptoms. Results: As a result of confirming the effect of individual characteristics on depressive symptoms, it was confirmed that both socioeconomic and health behavior factors had an effect. Similar results were shown in a model that considered regional level variables, and in the case of the number of sports facilities per population, people who belongs to smaller areas were more likely to have depressive symptoms (odds ratio, 0.98; 95% confidence interval, 0.97-0.99). Conclusion: As a result of the analysis, it was confirmed that both individual level and regional level variables had a significant effect on depressive symptoms. This suggests that not only individual level approaches but also regional level approaches are needed to improve individual depressive symptoms In particular, it may be possible to consider to increase the number of sports facilities in areas where the prevalence of depressive symptoms is high and the number of sports facilities is insufficient.

Factors Influencing the Quality of Life (EQ-5D) of Patients with Diabetes Mellitus: Study Based on Data from the Second Annual Korea Health Panel survey (2019) (당뇨병 환자의 삶의 질(EQ-5D)에 미치는 영향 요인 : 2019년도 제2기 한국 의료패널 조사를 중심으로)

  • Purpose : The purpose of this study was to determine factors that affect the quality of life (QoL) of people with diabetes. We hope to provide a basis for intervention strategies for health promotion and related research among patients with diabetes in the field of physical therapy. Methods : We selected individuals diagnosed with diabetes who were aged 19 years or higher by using data from the 2019 Korean Health Panel survey and analyzed the results of 1,527 people. The influencing factors were sex, age, household income, marital status, age, education level, subjective health status, body mass index (BMI), smoking, drinking, regular exercise, experience of depression and anxiety, suicidal thoughts, stress, and frequency of walking (days/week). Frequency analysis, descriptive statistics, independent sample t-test, one-way analysis of variance, and multiple regression analysis were performed. Results : The EQ-5D score for QoL was .87 ± .12, with pain and discomfort being the most detrimental to the QoL of people with diabetes. In terms of patients characteristics, the QoL was lower among participants who were female, older, and separated/widowed/divorced; had an education level below middle school; had a poor subjective health status; were non-smokers and drinkers; and did not walk and exercise regularly, experienced a lot of stress, and experienced depression and anxiety; and had suicidal thoughts (p<.001). Being separated/bereaved/divorced; experiencing a lot of stress, depression, and anxiety; and having suicidal thoughts were negatively associated with QoL in people with diabetes (p<.01). On the other hand, age of 65 years or younger; graduation from high school, fair or better subjective health, regular exercise and alcohol consumption, and walking (days/week) were positively associated with QoL (p<.05). Conclusion : Regarding the QoL of patients with diabetes, intervention strategies should be formulated by considering general characteristics and disease-related characteristics. Among the various factors affecting QoL, the number of walking and regular exercise were found to be the factors that improved the QoL of patients with diabetes. These results can be used as a basis for the education and management of patients with diabetes in the field of physical therapy.
