Effects of virtual reality-based core stabilization exercise on upper extremity function, postural control, and depression in persons with stroke

  • Objective: The purpose of this study was to evaluate the effect of virtual reality (VR)-based core stabilization exercise on upper extremity function, postural control, and depression among persons with stroke with hemiplegia. Design: Randomized controlled trial. Methods: This study was conducted with the inclusion of 24 participants and were randomly assigned to either the VR-based trunk stability exercise group (n=12) or control group (n=12). The VR-based trunk stability exercise group performed core stabilization exercises in a VR environment for 30 minutes. Meanwhile, the control group conducted general core stabilization exercises for 30 minutes. The participants trained 3 times a week for 4 weeks. The manual functional test (MFT), Box and Block Test (BBT), Berg Balance Scale (BBS), Trunk Impairment Scale (TIS), the Geriatric Depression Scale (GDS) were used to assess all participants before and after the intervention. Results: The VR-based core stabilization exercise group had a significant improvement in upper extremity function (MFT, BBT) and postural control (BBS) compared with the control group (p<0.05). The VR-based core stabilization exercise showed a significant difference after intervention in the TIS and GDS scores (p<0.05), but they did not significantly differ between the two groups. Conclusions: The result showed that VR-based core stabilization exercise can be effective in improving upper extremity function and postural control among patients with stroke more than the sole application of general physical therapy.

The Effect of Fumanet Exercise Program for Life care on Cognition Function, Depression in Dementia (라이프케어 증진을 위한 후마네트 운동프로그램이 치매노인의 인지기능, 우울기능에 미치는 영향)

  • Purpose: As dementia progresses, cognitive function decreasing leads to memory loss, speech degradation, time and space degradation and judgment degradation, which causes difficulties in carrying out tasks related to daily life. It was said that community-based non-drug intervention therapy for early dementia patients was important to participate in entertainment treatment, including activities such as awareness and exercise therapy, exercise rehabilitation, aerobic exercise, and art. Methods: This study conducted 15 experimental and 15 control groups(experimental group : Fumanet exercise, control group : general occupational therapy) for eight weeks at the Daycare Center in Gyeonggi-do to find out the impact of the Fumanet exercise program on cognitive and depression functions of the elderly. The pre-post evaluation used KGDS, MMSE. Results: There were significant differences between the two groups in the function of menopause, memory recall, attention concentration and calculation, and depression, and no significant results were obtatined in memory registration, language function, understanding and fracture. The Fumanet movement was judged to be effective in improving cognitive function and reducing depression for the elderly with dementia. Conclisions: The Fumanet movement was judged to be effective in improving cognitive function and reducing depression for the elderly with dementia.

Changes of the Level of Physical Self-Efficacy, Depression and Stress of Middle-Aged Men According to the Stage of Exercise Change (중년기 남성의 운동변화단계에 따른 신체적 자기효능감과 우울 및 스트레스 수준 변화)

  • The purpose of this study was to verify whether there are any differences in the level of physical self-efficacy, depression and stress of meddle-aged men according to the stage of exercise change. As for the study objects, men at the ages between 40 and 50, residing in Seoul and mostly having temporary jobs, office jobs and professional jobs, were surveyed with the questionnaire prepared, and 299 copies of the questionnaire in total were collected and used for the final analysis. SPSS 12.0 was used for the processing of the data, while the results were deduced by using exploratory analysis, reliability analysis, 1-way ANOVA and correlation analysis. The study results were as follows: Firstly, their Physical self-efficacy showed significant differences according to the stage of exercise change of the study objects, and as a result of the post-hoc analysis, it was found that as the stage became higher, their physical self-efficacy became higher as well. Secondly, the level of depression showed significant differences according to the stage of exercise change of the study objects, and as a result of the post-hoc analysis, it was found that as the stage generally became higher, their depression showed lower levels. Thirdly, their stress showed significant differences according to the stage of exercise change of the study objects, and as a result of the post-hoc analysis, it was found that the preparation stage had a lower level of stress than the maintenance stage while the before-plan stage had a lower level of stress than the before-plan stage and the maintenance stage. Fourthly, it was found that as the stage of exercise change became higher, their stress tended to decrease while their self-efficacy tended to increase.

Frailty and its Related Factors in the Vulnerable Elderly Woman by Urinary Incontinence (지역사회 거주 여성노인의 요실금 유무에 따른 허약정도와 허약 영향요인)

  • This study aimed to investigate factors affecting frailty by urinary incontinence groups among the vulnerable elderly woman in Korea. In this secondary analysis, data were collected from records for 3,251 elders registered in the Visiting Health Management program of Public Health Centers in 2012. body mass index, waist circumference, timed up & go, depression, self rated helath, walking exercise, flexibility exercise, strengh exercise and frailty were assessed. Data were analyzed using $x^2$-test, t-test, ANOVA, Pearson's correlation and stepwise regression to determine the associated factors of frailty by urinary incontinence. Depression, walking exercise, timed up & go and age were found to be factors significantly associated with frailty among the elders with incontinence(F=38.321, p<.001). Age, depression, walking exercise, self rated health and tined up & go were found to be factors associated with frailty in the elders without incontinence(F=265.666 p<.001). The findings show that frailty of elders and associated factors were different by urinary incontinence, and common factors affecting frailty were depression, walking exercise, timed up & go, age. Thus, these factors should be considered in the development of intervention program for care and prevention of frailty and program should be modified according to urinary incontinence.

Prediction Model of Exercise Behaviors in Patients with Arthritis (by Pender's revised Health Promotion Model) (관절염 환자의 운동행위 예측모형 (Pender의 재개정된 건강증진 모형에 의한))

  • The aims of this study were to understand and to predict the determinent factors affecting the exercise behaviors and physical fitness by testing the Pender's revised health promotion model, and to help the patients with rheumatoid arthritis and osteoarthritis perform the continous exercise program, and to help them maximize the physical effect such as muscle strength, endurance, and functional status and mental effects including self efficacy and quality of life, and improve the physical and mental well being, and to provide a basis for the nursing intervention strategies. Of the selected variables in this study, the endogenous variables included the physical fitness, exercise score, exercise participation, perceived benefits of action, perceived barriers of action to exercise, activity-related affect(depression) and perceived self-efficacy, interpersonal influences(family support), situational factors(duration of arthritis, fatigue) and the exogenous variables included personal sociocultural factor(education level), personal biologic factor(body mass index), personal psychologic factor(perceived health status) and prior related behavior factors(previous participation in exercise, life-style). We analyzed the clinical records of 208 patients with rheumatoid arthritis and degenerative arthritis who visited the outpatient clinics at H university hospital in Seoul. Data were composed of self reported qustionnaire and good of fitness score which were obtained by padalling the ergometer of bicycle for 9 minutes. SPSS Win 8.0 and Window LISREL 8.12a were used for statistical analysis. Of 75 hypothetical paths that influence on physical fitness, exercise participation, exercise score, perceived benefits of action, perceived barriers of action to exercise, activity-related affect(depression) and perceived self-efficacy, interpersonal influences(family support), situational factors(duration of arthritis, fatigue), 40 were supported. The physical fitness was directly influenced by life-style, perceived health status, education level, family support, fatigue, which explained 12% of physical fitness. The exercise participation were directly influenced by life-style, education level, past exercise behavior, perceived benefits of action, perceived barriers of action, depression and duration of arthritis, which explained 47% of exercise participation. Exercise score were directly affected by perceived self efficacy. BMI, life-style, past exercise behavior, perceived benefits of action, family support, perceived health status. perceived barriers of action, and fatigue, which explained 70%. Perceived benefits of action was directly influenced by BMI, life-style, which explained 39%. Perceived barriers of action were directly influeced by past exercise behavior, perceived health status, which explained 7%. Perceived self efficacy were directly influeced by level of education, perceived health status, life-style, which explained 57%. Depression were directly influeced by past exercise behavior, BMI, life-style, which explained 27%. Family support were directly influeced by life-style, perceived health status, which explained 29%. Fatigue were directly influeced by BMI, life-style, perceived health status. which explained 41%. Duration of arthritis were directly influeced by life-style, past exercise behavior, BMI, which explained 6%. In conclusion, important variables for physical fitness were life-style, and variable affecting exercise participation were life-style. Perceived self-efficacy of exercise was a significant predictor of exercise score. BMI, Life-style, perceived benefits of action, family support, past exercise behavior showed direct effects on perceived self-efficacy. Therefore, disease related factor should be minimized for physical performance and well being in nursing intervention for patients with rheumatoid arthritis, and plans to promote and continue exercise should be seeked to reduce disability. In addition, Exercise program should be planned and performed by the exact evaluation of exercise according to the ability of the patients and the contents to improve the importance of exercise and self efficacy in self control program, dedicated educational program should be involved. This study suggest that the methods to reduce the disease related factors, the importance of daily life-style, recognition of benefit of exercise, and educational program to promote self efficacy should be considered in the exercise behavior promotion and nursing intervention for continous performance. The significance of this study is also thought to provide patients with chronic arthritis the specific data for maximal physical and mental well being through exercise, chronic therapeutic procedure, daily adaptation and confrontation in nursing intervention.

Effects of Activity of Daily Living and Depression on Quality of Life among Elderly Women in Rural Areas (농촌 여성 노인의 일상생활수행능력과 우울이 삶의 질에 미치는 영향)

  • Purpose: This study was done to examine factors affecting quality of life among community-dwelling elderly women who live in rural areas. Methods: The design of this research was cross-sectional descriptive study. The participants were 92 community-dwelling women aged 65 or older. Data were collected from November 1 to 15, 2013. ADL (Kart's Index), GDS-SF (Geriatric Depression Scale Short Form) and GQOL (Geriatric Quality of Life Scale) were used to measure variables. Data were collected using self-administered questionnaires and analyzed using descriptive statistics, Spearman's coefficient and stepwise multiple regression. Results: The participants' mean age was 77.5, and 85.9% had elementary school graduation or less education, and for 64.1%, their economic status was low. Spearman's rho coefficient analysis found that QOL was significantly associated with depression (r=-.72, p<.001), perceived health (r=.58, p<.001), regular exercise (r=.47, p<.001), education level (r=.29, p=.005), and ADL (r=-.21, p=.043). Multiple regression analysis showed that 65.9% of their QOL was explained by depression (${\beta}=-.72$), perceived health (${\beta}=.24$), ADL (${\beta}=-.16$), exercise (${\beta}=.22$) and number of diseases (${\beta}=.19$). Conclusion: These results indicate that older women who live rural areas need support for ADL, and prevention of depression to improve their quality of life.

Effects of the Neurofeedback Program on Psychogenic Factors in Breast Cancer Patients (뉴로피드백 훈련이 유방암 환자의 심인성 변화에 미치는 영향)

  • Background: This report determines the effects of a neurofeedback program on patients with breast cancer through a study of psychogenic factors(distress, depression, anxiety). Design: Randomized controlled trial. Methods: The study selected 28 patients with breast cancer and divided them into two groups: 14 subjects in the neurofeedback program group and 14 subjects in the exercise intervention program group. Both groups conducted one-and-hour training per session five times a week for a total of five weeks. The experimental group performed neurofeedback, and the control group performed a exercise intervention. The distress was measured using the distress management Test. Beck's Depression Inventory was used to measure depression. Beck's Anxiety Inventory was used to measure Anxiety Results: Both the experimental and control groups showed significant differences in distress, depression, and anxiety after the intervention (p<0.05) in the within-group comparisons. It was found that the experimental group showed more significant differences in distress, depression, and anxiety than the control group (p<0.05) when the two groups were compared. Conclusion: The results obtained in this study show that the neurofeedback program had a positive effect on distress, depression, and anxiety of life in patients with breast cancer. The study thereby proposes that the neurofeedback program should be applied as an intervention method for clinical use on patients with breast cancer.

The Effect of Qigong Exercise on SF-36 and Psychological Factors of Middle-aged Obese (기공 운동이 비만인의 건강관련 삶의 질(SF-36)과 심리적 요인에 미치는 영향)

  • Obese people experience dissatisfaction of body, depression, anxiety, and emotional disturbance by social prejudice and discrimination, further they represent decline of health-related quality of life. Exercise therapy is recommended as one of the positive treatments to improve the emotional pain of obese people. This study is to provide guideline of exercise prescription for obese people by investigating the effects of qigong exercise on psychological factors such as health-related quality of life (SF-36), self-efficiency, depression, anxiety, and fatigue in obese people. The subjects of this study were 50~60s adults participated in Golden-Wellbeing program at S university in G gu of S city and were devided into exercise (N=17) and control (N=16) group. Qigong exercise was performed twice per week for 12 weeks and SF-36, self-efficiency, depression, anxiety, and fatigue were measured before and after 12 weeks of exercise. In the 8 items of SF-36 before and after 12 weeks of qi-gong exercise, there was no significance in role limitation-emotional (F=0.187, p=0.668), mental health (F=2.043, p=0.163) between groups, but there was significance in physical functioning (F=15.151, p<0.001), role limitation-physical (F=18.278, p<0.001), social functioning (F=4.957, p=0.033), vitality (F=11.485, p=0.002), bodily pain (F=6.623, p=0.015), and general health (F=4.498, p=0.042) between groups. Also, anxiety (F=0.631, p=0.433) was not significant, whereas self-efficiency (F=6.124, p=0.019), depression (F=5.109, p=0.031), fatigue (F=7.998, p=0.008) was significant between groups. Even though qigong is slow motion and low intensity of exercise, it was found that qi-gong has exercise effect which induces mental and psychological improvement through this study.

The Effect of the DanJeon Breathing Exercise Program Applied to Health Promotion in Women in Midlife (중년여성의 건강증진을 위한 단전호흡 운동프로그램의 적용 효과)

  • The purpose of this study was to develop a DanJeon Breathing Exercise Program for health promotion and to examine the effects of a DanJeon Breathing Exercise Program on the physical and emotional health promotion of women in midlife. The design utilized for this study was quasi-experimental with a nonequivalent control group pretest-posttest design. This study was done between February 1 and July 4, 2000 and the subjects of the study were 40 women in midlife living in Seoul. There were 20 members in experimental group who participated in the DanJeon Breathing Exercise Program and 20 members in the control group who were chosen as matched to members of the experimental group according to age, education and religion. The DanJeon Breathing Exercise Program was carried out for 80 minutes a day, 3 times a week for 12 weeks. The percent of body fat, back strength, flexibility (trunk flexion), balance and vital capacity were measured using the Health Management System developed by the Korea Physical Science Institution. Blood tests were done at D infirmary for total cholesterol, and the atherogenic index was calculated using an established formula. The scores of anxiety and depression were measured by a questionnaire with 10 questions on anxiety and 13 questions on depression. It was developed from the Korean Manual of Symptoms-Checklist-90 revision. The collected data were processed using the SPSS PC program and analyzed using $\chi^2$-test and t-test. The result of this study are as follows : 1. The percent of body fat for the experimental group who participated in the DanJeon Breathing Exercise Program was lower than that of the control group. 2. The degree of back strength, flexibility (trunk flexion) and balance of the experimental group that participated in the DanJeon Breathing Exercise Program was higher than that of the control group. 3. The vital capacity of the experimental group that participated in the DanJeon Breathing Exercise Program was higher than that of the control group. 4. The total cholesterol level and atherogenic index of the experimental group that participated in the DanJeon Breathing Exercise Program were lower than those of the control group. 5. The scores for anxiety and depression in the experimental group that participated in the DanJeon Breathing Exercise Program were lower than those of the control group. In conclusion, DanJeon Breathing Exercise Program promotes the physical and emotional health of women in midlife. The DanJeon Breathing Exercise Program can be utilized as a nursing intervention for the promotion of health in women in midlife.

Development and Examination of the Online and Offline Educational Programs for Ankylosing Spondylitis Patients (강직성 척추염 환자의 오프라인 교육과 온라인 교육 프로그램 개발 및 효과 검증)

  • Ankylosing spondylitis(AS) is one of the most common forms of chronic arthritis causing pain and progressive disability. The purposes of this study were 1) to develop an offline educational program and online educational program for the patients with ankylosing spondylitis; and 2) to test the effectiveness of each program in terms of changes in ankylosing spondylitis self-efficacy, exercise continuity disease activity, physical functioning, and depression. The convenient samples for three groups were selected acceding to the inclusion criteria. Online education group consists of 7 patients, and 14 patients were included for the offline group, while 12 patients were assigned to the control group. Outcomes variables included self efficacy, exercise continuity, disease activity, physical function, and depression and the reliability of each measure was ranged from .88 to .95 in the present study. The offline educational program demonstrated positive effects on self-efficacy, exercise continuity, disease activity, and depression, whereas the online educational program on exercise continuity, disease activity, and depression. The self-efficacy was showed positive effect only in the offline educational program, whereas physical functioning did not improve in both intervention groups. These partial improvements may be due to the small sample size and the methods of online education program. In conclusion, the effectiveness of the offline and online educational programs was partially supported in the present study. For the improvement of these programs, the further study would be necessary to apply this kind of intervention program with larger sample and to test the psychometric of the newly development tools.
