Effects of virtual reality-based core stabilization exercise on upper extremity function, postural control, and depression in persons with stroke

  • Objective: The purpose of this study was to evaluate the effect of virtual reality (VR)-based core stabilization exercise on upper extremity function, postural control, and depression among persons with stroke with hemiplegia. Design: Randomized controlled trial. Methods: This study was conducted with the inclusion of 24 participants and were randomly assigned to either the VR-based trunk stability exercise group (n=12) or control group (n=12). The VR-based trunk stability exercise group performed core stabilization exercises in a VR environment for 30 minutes. Meanwhile, the control group conducted general core stabilization exercises for 30 minutes. The participants trained 3 times a week for 4 weeks. The manual functional test (MFT), Box and Block Test (BBT), Berg Balance Scale (BBS), Trunk Impairment Scale (TIS), the Geriatric Depression Scale (GDS) were used to assess all participants before and after the intervention. Results: The VR-based core stabilization exercise group had a significant improvement in upper extremity function (MFT, BBT) and postural control (BBS) compared with the control group (p<0.05). The VR-based core stabilization exercise showed a significant difference after intervention in the TIS and GDS scores (p<0.05), but they did not significantly differ between the two groups. Conclusions: The result showed that VR-based core stabilization exercise can be effective in improving upper extremity function and postural control among patients with stroke more than the sole application of general physical therapy.

라이프케어 증진을 위한 후마네트 운동프로그램이 치매노인의 인지기능, 우울기능에 미치는 영향 (The Effect of Fumanet Exercise Program for Life care on Cognition Function, Depression in Dementia)

  • 치매가 진행되면서 인지기능 저하로 인해 기억력 감퇴, 언어능력 저하, 시공간 파악능력 저하, 판단력 저하가 오게 되어 일상생활과 관련된 과제들을 수행하는데 어려움이 발생하게 된다. 경도인지장애를 동반한 치매 환자들을 위한 지역사회 기반 비 약물적 중재치료는 인지, 운동치료, 예술과 같은 활동을 포함 작업, 운동, 오락치료가 있고, 환자들의 삶의 질, 라이프케어의 증진에 영향을 준다. 본 연구는 라이프케어 증진을 위한 후마네트 운동 프로그램이 노인의 인지기능, 우울기능에 미치는 영향을 알아보고자, 경기도에 소재한 데이케어센터에서 실험군 15명, 대조군 15명을 8주간 실시하였다. 두 집단간에 지남력, 기억회상, 주의집중 및 계산, 우울기능에 유의한 차이가 있었고, 기억등록, 언어기능, 이해 및 판단에는 유의한 결과를 얻지 못하였다. 후마네트 운동은 치매 노인에게 인지기능 향상과, 우울기능에 효과가 있다고 판단되었다. 집안 내 생활이 많아지고, 운동기능, 우울기능, 인지기능이 감소될 수 있는 노인, 치매, 경도인지장애 환자들을 대상으로 라이프케어 증진을 위한 후마네트 운동 프로그램을 적용할 수 있는 방안을 마련하고 그 효과를 반복 측정하는 연구를 제언한다.

중년기 남성의 운동변화단계에 따른 신체적 자기효능감과 우울 및 스트레스 수준 변화 (Changes of the Level of Physical Self-Efficacy, Depression and Stress of Middle-Aged Men According to the Stage of Exercise Change)

  • 본 연구는 중년기남성의 운동변화단계에 따른 신체적 자기효능감과 우울 및 스트레스의 수준에 차이가 있는지를 검증하는데 목적이 있다. 연구대상은 서울에 거주하는 만 40세부터 50세의 남성으로 일용직, 사무직, 전문직 종사자를 대상으로 자료를 수집하여 최종 289부의 설문을 최종분석에 이용하였다. 자료처리는 SPSS 12.0을 사용하여 탐색적 요인분석, 신뢰도분석, 1-way ANOVA, 상관관계분석을 이용하여 결과를 도출하였다. 연구결과 첫째, 연구대상의 운동변화단계에 따라 신체적 자기효능감은 유의한 차이를 보였고 사후검증 결과 단계가 높아질수록 신체적 자기효능감이 높아지는 것으로 나타났다. 둘째, 연구대상의 운동변화단계에 따라 우울의 수준은 유의한 차이를 보였고 사후검증 결과 전반적으로 단계가 높아질수록 우울이 낮은 수준을 나타내는 것으로 나타났다. 셋째, 연구대상으로 운동변화에 따른 스트레스는 유의한 차이를 보였고 사후검증결과 준비단계가 유지단계보다, 계획전단계가 행동단계와 유지단계보다 낮은 스트레스 수준을 나타냈다. 넷째, 운동변화단계가 높아질수록 우울, 스트레스가 감소하고 신체적 자기효능감은 증가하는 경향을 나타냈다.

지역사회 거주 여성노인의 요실금 유무에 따른 허약정도와 허약 영향요인 (Frailty and its Related Factors in the Vulnerable Elderly Woman by Urinary Incontinence)

  • 본 연구는 지역사회에 거주하는 여성 노인의 요실금 유무에 따른 허약정도와 허약에 영향을 미치는 요인을 파악하기 위하여, 2012년 S시 8개구의 보건소 방문건강관리사업 대상자 중 65세 이상 여성노인 3,251명의 자료를 이차 분석하였다. 요실금 유무별 여성노인의 체질량 지수, 허리둘레, 복합적 이동능력, 우울, 주관적 건강과 걷기운동, 유연성 운동 및 근력운동에 따른 허약의 차이를 파악하기 위하여 $x^2$-test, t-test, ANOVA를 실시하였으며, 사후검증은 Scheffe test를 실시하였다. 또한, 요실금 유무에 따른 제 변수별 상관관계는 Pearson's correlation을 실시하였고, 요실금 유무에 따른 허약에 가장 영향을 미치는 요인을 파악하고자 위계적 회귀분석을 수행하여 비교하였다. 연구결과, 요실금이 있는 여성노인의 허약에 가장 영향을 미치는 요인은 우울, 걷기 운동, 복합적 이동능력, 연령순으로 높게 나타났으며(F=38.321, p<.001), 이들 변수에 의해 허약이 36.6% 설명되었다. 요실금이 없는 집단에서 유의한 변인은 연령, 우울, 걷기 운동, 주관적 건강, 복합적 이동능력 순으로 높게 나타났으며(F=265.666, p<.001), 이들 변수에 의해 허약 정도가 설명되는 변량은 30.7%였다. 향후 지역사회에 거주하는 여성 노인, 특히 요실금을 갖고 있는 여성노인의 허약을 예방하기 위하여, 하루 10분 이상, 주 5일 이상의 규칙적인 걷기 운동 프로그램과 우울을 예방하고 관리하는 프로그램을 제언한다.

관절염 환자의 운동행위 예측모형 (Pender의 재개정된 건강증진 모형에 의한) (Prediction Model of Exercise Behaviors in Patients with Arthritis (by Pender's revised Health Promotion Model))

  • The aims of this study were to understand and to predict the determinent factors affecting the exercise behaviors and physical fitness by testing the Pender's revised health promotion model, and to help the patients with rheumatoid arthritis and osteoarthritis perform the continous exercise program, and to help them maximize the physical effect such as muscle strength, endurance, and functional status and mental effects including self efficacy and quality of life, and improve the physical and mental well being, and to provide a basis for the nursing intervention strategies. Of the selected variables in this study, the endogenous variables included the physical fitness, exercise score, exercise participation, perceived benefits of action, perceived barriers of action to exercise, activity-related affect(depression) and perceived self-efficacy, interpersonal influences(family support), situational factors(duration of arthritis, fatigue) and the exogenous variables included personal sociocultural factor(education level), personal biologic factor(body mass index), personal psychologic factor(perceived health status) and prior related behavior factors(previous participation in exercise, life-style). We analyzed the clinical records of 208 patients with rheumatoid arthritis and degenerative arthritis who visited the outpatient clinics at H university hospital in Seoul. Data were composed of self reported qustionnaire and good of fitness score which were obtained by padalling the ergometer of bicycle for 9 minutes. SPSS Win 8.0 and Window LISREL 8.12a were used for statistical analysis. Of 75 hypothetical paths that influence on physical fitness, exercise participation, exercise score, perceived benefits of action, perceived barriers of action to exercise, activity-related affect(depression) and perceived self-efficacy, interpersonal influences(family support), situational factors(duration of arthritis, fatigue), 40 were supported. The physical fitness was directly influenced by life-style, perceived health status, education level, family support, fatigue, which explained 12% of physical fitness. The exercise participation were directly influenced by life-style, education level, past exercise behavior, perceived benefits of action, perceived barriers of action, depression and duration of arthritis, which explained 47% of exercise participation. Exercise score were directly affected by perceived self efficacy. BMI, life-style, past exercise behavior, perceived benefits of action, family support, perceived health status. perceived barriers of action, and fatigue, which explained 70%. Perceived benefits of action was directly influenced by BMI, life-style, which explained 39%. Perceived barriers of action were directly influeced by past exercise behavior, perceived health status, which explained 7%. Perceived self efficacy were directly influeced by level of education, perceived health status, life-style, which explained 57%. Depression were directly influeced by past exercise behavior, BMI, life-style, which explained 27%. Family support were directly influeced by life-style, perceived health status, which explained 29%. Fatigue were directly influeced by BMI, life-style, perceived health status. which explained 41%. Duration of arthritis were directly influeced by life-style, past exercise behavior, BMI, which explained 6%. In conclusion, important variables for physical fitness were life-style, and variable affecting exercise participation were life-style. Perceived self-efficacy of exercise was a significant predictor of exercise score. BMI, Life-style, perceived benefits of action, family support, past exercise behavior showed direct effects on perceived self-efficacy. Therefore, disease related factor should be minimized for physical performance and well being in nursing intervention for patients with rheumatoid arthritis, and plans to promote and continue exercise should be seeked to reduce disability. In addition, Exercise program should be planned and performed by the exact evaluation of exercise according to the ability of the patients and the contents to improve the importance of exercise and self efficacy in self control program, dedicated educational program should be involved. This study suggest that the methods to reduce the disease related factors, the importance of daily life-style, recognition of benefit of exercise, and educational program to promote self efficacy should be considered in the exercise behavior promotion and nursing intervention for continous performance. The significance of this study is also thought to provide patients with chronic arthritis the specific data for maximal physical and mental well being through exercise, chronic therapeutic procedure, daily adaptation and confrontation in nursing intervention.

농촌 여성 노인의 일상생활수행능력과 우울이 삶의 질에 미치는 영향 (Effects of Activity of Daily Living and Depression on Quality of Life among Elderly Women in Rural Areas)

  • Purpose: This study was done to examine factors affecting quality of life among community-dwelling elderly women who live in rural areas. Methods: The design of this research was cross-sectional descriptive study. The participants were 92 community-dwelling women aged 65 or older. Data were collected from November 1 to 15, 2013. ADL (Kart's Index), GDS-SF (Geriatric Depression Scale Short Form) and GQOL (Geriatric Quality of Life Scale) were used to measure variables. Data were collected using self-administered questionnaires and analyzed using descriptive statistics, Spearman's coefficient and stepwise multiple regression. Results: The participants' mean age was 77.5, and 85.9% had elementary school graduation or less education, and for 64.1%, their economic status was low. Spearman's rho coefficient analysis found that QOL was significantly associated with depression (r=-.72, p<.001), perceived health (r=.58, p<.001), regular exercise (r=.47, p<.001), education level (r=.29, p=.005), and ADL (r=-.21, p=.043). Multiple regression analysis showed that 65.9% of their QOL was explained by depression (${\beta}=-.72$), perceived health (${\beta}=.24$), ADL (${\beta}=-.16$), exercise (${\beta}=.22$) and number of diseases (${\beta}=.19$). Conclusion: These results indicate that older women who live rural areas need support for ADL, and prevention of depression to improve their quality of life.

뉴로피드백 훈련이 유방암 환자의 심인성 변화에 미치는 영향 (Effects of the Neurofeedback Program on Psychogenic Factors in Breast Cancer Patients)

  • Background: This report determines the effects of a neurofeedback program on patients with breast cancer through a study of psychogenic factors(distress, depression, anxiety). Design: Randomized controlled trial. Methods: The study selected 28 patients with breast cancer and divided them into two groups: 14 subjects in the neurofeedback program group and 14 subjects in the exercise intervention program group. Both groups conducted one-and-hour training per session five times a week for a total of five weeks. The experimental group performed neurofeedback, and the control group performed a exercise intervention. The distress was measured using the distress management Test. Beck's Depression Inventory was used to measure depression. Beck's Anxiety Inventory was used to measure Anxiety Results: Both the experimental and control groups showed significant differences in distress, depression, and anxiety after the intervention (p<0.05) in the within-group comparisons. It was found that the experimental group showed more significant differences in distress, depression, and anxiety than the control group (p<0.05) when the two groups were compared. Conclusion: The results obtained in this study show that the neurofeedback program had a positive effect on distress, depression, and anxiety of life in patients with breast cancer. The study thereby proposes that the neurofeedback program should be applied as an intervention method for clinical use on patients with breast cancer.

기공 운동이 비만인의 건강관련 삶의 질(SF-36)과 심리적 요인에 미치는 영향 (The Effect of Qigong Exercise on SF-36 and Psychological Factors of Middle-aged Obese)

  • 많은 비만인들은 사회적인 편견과 차별로 인하여 신체상에 대한 불만족, 우울, 불안 및 정서장애를 경험하고 있으며, 더 나아가 건강과 관련된 삶의 질의 저하를 나타내고 있다. 이러한 비만인들의 정신적 고통을 개선하기 위하여 운동요법은 하나의 긍정적인 치료 방법으로 권장되고 있다. 본 연구는 동양의 전통 운동형태 중 기공 운동이 비만인에게 건강관련 삶의 질(SF-36), 자기효능감, 우울, 불안, 피로의 심리적 요인에 어떠한 효과를 나타내는지를 살펴봄으로써 비만인을 위한 운동처방 지침을 제공하고자 한다. 본 연구의 피검자는 S시 G구에 소재한 S대학교 골든웰빙 운동프로그램에 참가하는 50-60대의 성인으로 통제군 16명, 운동군 17명으로 분류하였다. 기공 운동은 12주 동안 주2회의 빈도로 실시하여, 운동 전 후로 건강관련 삶의 질(SF-36), 신체적 자기효능감, 우울, 불안, 피로도 변인을 측정하였다. 12주간의 기공 운동 전 후 건강관련 삶의 질(SF-36)의 8항목 중 감정역할 제한(F=0.187, p=0.668), 정신건강(F=2.043, p=0.163)은 집단 간의 유의한 차이를 나타내지 못하였으나, 신체기능(F=15.151, p<0.001), 신체역할 제한(F=18.278, p<0.001), 사회적 기능(F=4.957, p=0.033), 활력/피로(F=11.485, p=0.002), 신체통증(F=6.623, p=0.015), 일반적인 건강(F=4.498, p=0.042)의 6가지 변인은 집단 간의 유의한 차이를 나타내었다. 불안(F=0.631, p=0.433)은 집단 간의 유의한 차이를 나타내지 못하였으나, 신체적 자기효능감(F=6.124, p=0.019), 우울(F=5.109, p=0.031), 피로(F=7.998, p=0.008)의 3가지 변인은 집단 간의 유의한 차이를 나타내었다. 비만인에게 있어서 기공 운동은 비록 느린 동작으로 수행되는 낮은 강도의 운동형태일지라도 운동자체가 가져다주는 정신 심리적인 개선효과가 분명하게 나타나는 운동임을 본 연구를 통하여 확인할 수 있었다.

중년여성의 건강증진을 위한 단전호흡 운동프로그램의 적용 효과 (The Effect of the DanJeon Breathing Exercise Program Applied to Health Promotion in Women in Midlife)

  • The purpose of this study was to develop a DanJeon Breathing Exercise Program for health promotion and to examine the effects of a DanJeon Breathing Exercise Program on the physical and emotional health promotion of women in midlife. The design utilized for this study was quasi-experimental with a nonequivalent control group pretest-posttest design. This study was done between February 1 and July 4, 2000 and the subjects of the study were 40 women in midlife living in Seoul. There were 20 members in experimental group who participated in the DanJeon Breathing Exercise Program and 20 members in the control group who were chosen as matched to members of the experimental group according to age, education and religion. The DanJeon Breathing Exercise Program was carried out for 80 minutes a day, 3 times a week for 12 weeks. The percent of body fat, back strength, flexibility (trunk flexion), balance and vital capacity were measured using the Health Management System developed by the Korea Physical Science Institution. Blood tests were done at D infirmary for total cholesterol, and the atherogenic index was calculated using an established formula. The scores of anxiety and depression were measured by a questionnaire with 10 questions on anxiety and 13 questions on depression. It was developed from the Korean Manual of Symptoms-Checklist-90 revision. The collected data were processed using the SPSS PC program and analyzed using $\chi^2$-test and t-test. The result of this study are as follows : 1. The percent of body fat for the experimental group who participated in the DanJeon Breathing Exercise Program was lower than that of the control group. 2. The degree of back strength, flexibility (trunk flexion) and balance of the experimental group that participated in the DanJeon Breathing Exercise Program was higher than that of the control group. 3. The vital capacity of the experimental group that participated in the DanJeon Breathing Exercise Program was higher than that of the control group. 4. The total cholesterol level and atherogenic index of the experimental group that participated in the DanJeon Breathing Exercise Program were lower than those of the control group. 5. The scores for anxiety and depression in the experimental group that participated in the DanJeon Breathing Exercise Program were lower than those of the control group. In conclusion, DanJeon Breathing Exercise Program promotes the physical and emotional health of women in midlife. The DanJeon Breathing Exercise Program can be utilized as a nursing intervention for the promotion of health in women in midlife.

강직성 척추염 환자의 오프라인 교육과 온라인 교육 프로그램 개발 및 효과 검증 (Development and Examination of the Online and Offline Educational Programs for Ankylosing Spondylitis Patients)

  • Ankylosing spondylitis(AS) is one of the most common forms of chronic arthritis causing pain and progressive disability. The purposes of this study were 1) to develop an offline educational program and online educational program for the patients with ankylosing spondylitis; and 2) to test the effectiveness of each program in terms of changes in ankylosing spondylitis self-efficacy, exercise continuity disease activity, physical functioning, and depression. The convenient samples for three groups were selected acceding to the inclusion criteria. Online education group consists of 7 patients, and 14 patients were included for the offline group, while 12 patients were assigned to the control group. Outcomes variables included self efficacy, exercise continuity, disease activity, physical function, and depression and the reliability of each measure was ranged from .88 to .95 in the present study. The offline educational program demonstrated positive effects on self-efficacy, exercise continuity, disease activity, and depression, whereas the online educational program on exercise continuity, disease activity, and depression. The self-efficacy was showed positive effect only in the offline educational program, whereas physical functioning did not improve in both intervention groups. These partial improvements may be due to the small sample size and the methods of online education program. In conclusion, the effectiveness of the offline and online educational programs was partially supported in the present study. For the improvement of these programs, the further study would be necessary to apply this kind of intervention program with larger sample and to test the psychometric of the newly development tools.
