건강한 만삭아에서 모유황달의 발생 빈도 (Incidence of breast milk jaundice in healthy full-term infants)

  • 목 적 : 엄마 젖을 먹는 신생아에서 조기 모유황달과 모유 황달의 발생 빈도가 우리나라에서는 더 높은 것으로 추정되나 이에 대한 연구가 부족하여, 모유를 수유하는 우리나라 건강한 만삭아의 모유 황달의 발생 빈도를 알아보고자 하였다. 방 법 : 2005년 1월부터 2005년 12월까지 1년 동안 제일 병원에서 출생하고 제일 병원 산후 관리 센터에 입실하여 7일 이상 추적 관찰이 가능한 건강한 만삭아 839명을 대상으로 하였고, 육안적으로 황달이 관찰된 경우 경피적 빌리루빈 측정기 JM-103 jaundice meter(Konica Minolta sensing, Inc., Osaka, Japan)를 이용하여 매일 경피적 빌리루빈치의 변화를 조사하여, 이를 통하여 모유 수유를 하는 건강한 만삭아의 황달 발생 빈도를 알아 보았다. 결 과 : 대상아 중 완전 모유 수유 군은 251명(29.9%), 혼합 수유 군은 568명(67.7%), 조제 분유 수유 군은 20명(2.4%) 이었으며, 세 군간에서 성별, 출생 체중, 아프가 점수, 산과적 위험 인자 등의 유의한 차이는 없었으나, 분만 형태에서는 완전 모유 수유 군에서 질식 분만이 유의하게 많았다. 모유 황달은 완전 모유 수유 군에서 31예(10.8%), 혼합 수유 군에서 21예(4.4%)로 전체 모유 수유아의 6.3%에서 나타났고, 완전 모유 수유 군에서 혼합 수유 군보다 통계적으로 유의하게 높았으며, 조기 모유 황달은 완전 모유 수유 군에서 63예(24.3%), 혼합 수유 군에서 149예(26.2%)로 모유 수유아의 25.6%에서 나타났으나, 완전 모유 수유 군과 혼합 수유 군에서 통계적으로 유의한 차이는 없었다. 결 론 : 모유를 수유하는 우리나라의 건강한 만삭아에서 모유 황달의 발생 빈도는 6.3%, 조기 모유 황달의 발생 빈도는 25.6%였으며, 서양인에 비하여 모유 황달의 발생 빈도가 높은 것을 알 수 있었다.

Feeding characteristics in infancy affect fruit and vegetable consumption and dietary variety in early childhood

  • BACKGROUND/OBJECTIVES: Previous studies have shown an association between breastfeeding and higher fruit and vegetable consumption and the level of dietary variety in children. However, few studies have reported this association on the feeding characteristics. Therefore, this study examined the association of the feeding characteristics with the consumption of fruit and vegetable and dietary variety in children. SUBJECTS/METHODS: This study recruited 802 participants from their parents with information on their feeding, and 24-h dietary recall. The associations of the feeding characteristics with fruit and vegetable consumption and dietary variety score (DVS) were analyzed using a multiple logistic regression model. RESULTS: Compared to the feeding type of exclusive breastfed children, exclusive formula-fed children had a significant association with a lower DVS (odds ratio [OR], 0.42, 95% confidence interval [CI], 0.23-0.77). Fruit and vegetable consumption was classified into 6 groups: non-salted vegetables (NSV), salted vegetables (SV), fruit (F), total vegetables (TV), non-salted vegetables + fruit (NSVF), and total vegetables + fruit (TVF). According to the mean level of fruit and vegetable consumption, compared to the duration of total breastfeeding for 6 month or less, a greater duration of breastfeeding for 12 mon had a significant association with a higher intake of NSVF and TVF (OR, 1.85, 95% CI, 1.20-2.85 and OR, 1.89, 95% CI, 1.22-2.92). On the other hand, the early introduction of formula feeding for 4 mon had a significant association with a lower intake of F and NSVF (OR, 0.59, 95% CI, 0.38-0.91 and OR, 0.63, 95% CI, 0.40-0.99). CONCLUSIONS: These results confirm that breastfeeding is associated with higher fruit and vegetable consumption and dietary variety, whereas formula feeding is associated with lower fruit and vegetable consumption and dietary variety. Therefore, the feeding characteristics in infants may affect fruit and vegetable consumption and dietary variety in children.

산후 1, 6개월 완전모유수유율과 영향 요인 (Exclusive Breastfeeding Rates and it's Influencing Factors by the 1st and 6th Month of Postpartum)

  • 본 연구는 산후 1개월 및 6개월 완전모유수유율을 파악하고 이에 영향을 미치는 요인을 파악하기 위해 수행하였다. 본 연구는 서술적 조사연구로, 연구자료는 2015년 전국 출산력 및 가족보건복지실태 조사 원시 자료를 이용하였다. 연구대상자는 조사 완료된 기혼여성 중 2013년 1월부터 조사시점에 추산 후 6개월이 경과한 2015년 2월 사이에 자녀를 출산한 1,839명이었다. 자료분석은 SPSS 25.0 프로그램을 이용하여 기술 통계분석, 카이제곱분석 및 다중로지스틱분석을 실시하였다. 연구결과 분만 후 1개월 및 6개월 완전모유수유율은 각각 54.0%, 9.4%로 월령이 증가함에 따라 완전모유수유율이 감소하였다. 산후 1개월 완전모유수유율에 영향을 미치는 요인은 산모의 나이, 아기의 출생순위, 분만형태, 분만 후 1시간 이내 모유수유 여부였으며, 산후 6개월 완전모유수유율에 영향을 미치는 요인은 산모의 나이, 거주지역, 아기의 출생순위, 산후 1개월 완전모유수유 여부였다. 본 연구결과 완전모유수유에 영향을 미치는 요인은 출산 후 시기에 따라 다른 것으로 확인되었다. 따라서 완전모유수유율 향상을 위해 출산 후 각 시기별 완전모유수유에 영향을 주는 요인을 고려한 맞춤형 중재가 필요하다.

Human Milk Oligosaccharides as a Missing Piece in Combating Nutritional Issues during Exclusive Breastfeeding

  • Extensive studies have shown that breast milk is the best source of nutrition for infants, especially during the first six months, because it fulfills almost all of their nutritional needs. Among the many functional building blocks in breast milk, human milk oligosaccharides (HMOs) have been receiving more attention recently. Furthermore, it is the third most common group of compounds in human milk, and studies have demonstrated the health benefits it provides for infants, including improved nutritional status. HMOs were previously known as the 'bifidus factor' due to their 'bifidogenic' or prebiotic effects, which enabled the nourishment of the gastrointestinal microbiota. Healthy gastrointestinal microbiota are intestinal health substrates that increase nutrient absorption and reduce the incidence of diarrhea. In addition, HMOs, directly and indirectly, protect infants against infections and strengthen their immune system, leading to a positive energy balance and promoting normal growth. Non-modifiable factors, such as genetics, and modifiable factors (e.g., maternal health, diet, nutritional status, environment) can influence the HMO profile. This review provides an overview of the current understanding of how HMOs can contribute to the prevention and treatment of nutritional issues during exclusive breastfeeding.

Association between Maternal Feeding Practices and Excessive Weight Gain in Infants

  • Purpose: The purpose of this study is to identify the association between maternal feeding practices and excessive weight gain in infants. Methods: This study adopted a cross-sectional design and included 240 pairs of mothers and their infants (129 boys and 111 girls) in public healthcare centers in the Daejeon area in South Korea. Via multivariate analyses, the association between maternal feeding practices and excessive weight gain in infants was identified. Results: Among 240 infants in this study, 39 (16.3%) infants gained excessive weight during 12 months after birth. Using multivariate logistic regression with adjustment for covariates, more than 7 months of exclusive breastfeeding was associated with a reduced likelihood of excessive weight gain in infants during the 12 months after birth (adjusted odds ratio: 0.39, 95% confidence interval: 0.02~0.81, p=.029). Conclusion: Based on these results, nurses in communities and clinics should educate mothers on the importance of longer durations of exclusive breast feeding and develop strategies for encouraging such behavior. Furthermore, support for exclusive breast feeding should be provided in various settings.

모유수유 임파워먼트 프로그램이 완전모유수유에 미치는 효과 (Effects of a Breast-Feeding Empowerment Program on Exclusive Breast-Feeding)

  • Purpose: The purpose of this study was to develop an antepartum-postpartum breast-feeding program titled "Breastfeeding Empowerment Program" and to measure the effects of this program on breast-feeding empowerment and the rate of exclusive breast-feeding. Methods: The study design was a nonequivalent control group design with repeated measures. Results: The scores for breast-feeding empowerment of the experimental group (EG) were higher than those of the control group (CG) at week 1, 4, 8, 12. The rates for exclusive breast-feeding of the experimental group were also higher than those of the control group: 86.4% in the EG and 40.9% in the CG at week 1; 100% vs 42.9% at week 4; 86.4% vs 38.1% at week 8; 95.5% vs 38.1% at week 12; 94.7% vs 31.6% at 6 months; 89.5% vs 26.3% at 12 months. Conclusion: The study provided evidence that raising consciousness and strengthening mother's will and skills for exclusive breast-feeding helps them to achieve successful breast-feeding by themselves. In addition mothers need to be empowered to breastfeed starting before delivery and continuing after delivery. This approach could make long-term exclusive breast-feeding successful.

Positive Effect of Baby-Friendly Hospital Initiatives on Improving Mothers' Intention for Successful Breastfeeding in Korea

  • Background: In Korea, the breastfeeding (BF) rate of infants aged 6 months or more is drastically decreasing, and this phenomenon is particularly worrisome for the future health of the population. The present study aimed to identify an antenatal strategy for initiation and continuation of human BF, and to identify how Baby-Friendly Hospitals (BFHs) may positively influence the intention to breastfeed. Methods: A total of 414 pregnant Korean antenatal women were surveyed using questionnaires to determine current knowledge of the benefits of human breast milk, whether they planned to breastfeed after delivery, to continue BF after reinstatement in the workforce, are willing to abide by rooming-in care for infants, and plan to give birth at BFHs. Results: We found that planning room-in care, greater awareness of BF benefits for infant and mother, participation in antenatal education programs, and provision of BF facilities in the workplace were positively associated with plans for exclusive breastfeeding (EBF) and longer BF duration. The mothers who planned to give birth at BFHs also desired to breastfeed immediately after birth, implement in-room care, continue BF at their workplace, participate in antenatal BF educational programs, and were more aware of the benefits of BF. Conclusion: If the beneficial effects of BFHs were well known to individuals, these would enhance the success rate of BF in Korea. Antenatal education and consequent acquisition of better knowledge of the benefits of BF are important for increasing the rate of BF practices.

Safety of a New Synbiotic Starter Formula

  • Purpose: Breastfeeding is the best way to feed all infants, but not all infants can be (exclusively) breastfed. Cow's milk based infant formula is the second choice infant feeding. Methods: The safety of a new synbiotic infant formula, supplemented with Bifidobacterium lactis and fructo-oligosaccharides, with lactose and a whey/casein 60/40 protein ratio was tested in 280 infants during 3 months. Results: The median age of the infants at inclusion was 0.89 months. Weight evolution was in accordance with the World Health Organization growth charts for exclusive breastfed infants. The evolution of all anthropometric parameters (weight-for-length z score and body mass index-for-age z score) was within the normal range. The incidence of functional constipation (3.2%), daily regurgitation (10.9%), infantile crying and colic (10.5%) were all significantly lower than the reported median prevalence for a similar age according to literature (median value of 7.8% for functional constipation, 26.7% for regurgitation, 17.7% for infantile colic). Conclusion: The new synbiotic infant starter formula was safe, resulted in normal growth and was well tolerated. Functional gastro-intestinal manifestations (functional constipation, regurgitation and colic) were significantly lower than reported in literature. Synbiotics (Bifidobacterium lactis and fructo-oligosaccharides) in cow's milk based infant formula bring the second choice infant feeding, formula, closer to the golden standard, exclusive breastfeeding.

  • Previous infant feeding guidelines recommended a delayed introduction of solids to beyond 6 months of age to prevent atopic diseases. However, scientific evidence supporting a delayed introduction of solids for prevention of atopic diseases is scarce and inconsistent. Current evidence does not support a major role for maternal dietary restrictions during pregnancy or lactation in the prevention of atopic disease. In studies of infants at high risk of developing atopic disease, there is evidence that exclusive breastfeeding for at least 4 months compared with feeding intact cow milk protein decreases the incidence of atopic dermatitis, cow milk allergy, and wheezing in early childhood. For infants at high risk of developing atopic disease who are not breastfed exclusively for 4 to 6 months, there is modest evidence that atopic dermatitis may be delayed or prevented by the use of extensively or partially hydrolyzed formulas, compared with cow milk formula, in early childhood. There is no convincing evidence that a delayed introduction of solid foods beyond 4 to 6 months of age prevents the development of atopic disease. For infants after 4 to 6 months of age, there are insufficient data to support a protective effect of any dietary intervention for the development of atopic disease.

Association of Infant Feeding Characteristics With Dietary Patterns and Obesity in Korean Childhood

  • Objectives: Young children's feeding characteristics can play an important role in eating habits and health during later childhood. This study was conducted to examine the associations of feeding characteristics with dietary patterns and obesity in children. Methods: This study utilized data from the Korea National Health and Nutrition Examination Survey conducted between 2013 and 2017. In total, 802 toddlers were included, with information on their demographic characteristics, feeding practices and duration, and 24-hour recall obtained from their parents. Feeding characteristics were categorized into feeding type, duration of total breastfeeding, duration of total formula feeding, duration of exclusive breastfeeding, and age when starting formula feeding. Dietary patterns were identified based on factor loadings for the food groups for 3 major factors, with "vegetables & traditional," "fish & carbohydrates," and "sweet & fat" patterns. Overweight/obesity was defined as ≥85th percentile in body mass index based on the 2017 Korean National Growth charts for children and adolescents. Multiple regression analysis was conducted to examine associations between feeding characteristics and dietary patterns. The association between dietary patterns and obesity was analyzed using multivariable logistic regression analysis. Results: The early introduction of formula feeding was inversely associated with the "vegetables & traditional" pattern (β=-0.18; 95% confidence interval [CI], -0.34 to -0.02). A higher "vegetables & traditional" intake was associated with a lower risk of obesity (odds ratio, 0.48; 95% CI, 0.24 to 0.95). Conclusions: Feeding characteristics are associated with dietary patterns in later childhood, and dietary patterns were shown to have a potential protective association against obesity.
