Dosimetric Comparison of Radiation Treatment Techniques for Breast Cancer : 3D-CRT, IMRT and VMAT (유방암 방사선치료 기법에 따른 선량 비교 : 3차원 입체조형치료, 세기 변조 방사선치료, 입체세기조절회전 방사선치료)

  • The purpose of this study is to compare method in the treatment of breast cancer using dose index. And, it is to find the optimized treatment technique to the patient. The phantom filled with tissue-equivalent material were used simulation and treatment as techniques of 3D-CRT, IMRT, VMAT was planned using Eclipse v10. By using HI(homogeneity index), CI(Conformity index), OED(Organ equivalent dose), EAR(Excess Absolute Risk), were assessed for each treatment plans. HI and CI of 3D-CRT, IMRT, VMAT were calculated 16.89, 11.21, 9.55 and 0.59, 0.61, 0.83. The organ average doses of Lt lung, Rt lung, liver, heart, esophagus, cord, Lt breast, trachea and stomach were 0.01 ~ 2.02 Gy, 0.36 ~ 5.01 Gy, 0.25 ~ 2.49 Gy, 0.14 ~ 6.92 Gy, 0.03 ~ 2.02 Gy, 0.01 ~ 1.06 Gy, 0.25 ~ 6.08 Gy, 0.08 ~ 0.59 Gy, 0.01 ~ 1.34 Gy, respectively. The OED, EAR of the IMRT and VMAT show higher than 3D-CRT. As the result of this study, we could confirm being higher dose index(HI, CI) in IMRT and VMAT than 3D-CRT, but doses of around normal organs was higher IMRT, VMAT than 3D-CRT.

Health Risk Assessment of Heavy Metals in Fine Particles Collected in Seoul Metropolitan Area (서울북부 지역 미세먼지에 함유된 유해 중금속의 분석 및 건강위해성평가)

  • Particulate materials (PM) less than 10 ${\mu}m$ in diameter are of special interest in air pollution because they are respirable and responsible for the increasing mortality rate of lung cancer and cardiovascular diseases. These particles are often referred to as $PM_{10}$ and they are divided into a coarse fraction and a fine fraction which is also often referred to as $PM_{25}$. In this study, we monitored the TSP, $PM_{10},\;PM_{2.5}$ concentration of ambient air collected in northern part of Seoul in early spring and measured the concentration of heavy metals; Cr, Mn, Zn, As, Cd, and Pb. All the heavy metals were found in the collected particles and the concentrations were variable in the $PM_{10},\;and\;PM_{2.5}$ respectively. The detected concentration ranges were Cr: $ND\~2,889ng/m^3,\;Mn:2.4\~257.9ng/m^3,\;Zn:ND\~353.7ng/m^3,\; As:ND\~22.3ng/m^3,\;Cd:0.1\~2.9ng/m^3,\;and\;Pb:ND\~392.2ng/m^3$ in fine particles. Heavy metal toxicity of the particles were also tested in $H_9C_2$ cell line derived from rat cardiomyocytes. As for the results of health risk assessment calculated by unit risk of IRIS, heavy metals in ambient air of Seoul metropolitan area were found to be responsible for the increase of total excess cancer risk. Among them, chromium (hexavalent) was found to be the most risky in fine particles of ambient air collected in the northern part of Seoul in early spring.

  • Background : Although there are growing concerns about the adverse health effect of air pollution, not much evidence on health effect of current air pollution level had been accumulated yet in Korea. This study was designed to evaluate the chronic health effect of ai. pollution using Korean Medical Insurance Corporation (KMIC) data and air quality data. Medical insurance data in Korea have some drawback in accuracy, but they do have some strength especially in their national coverage, in having unified ID system and individual information which enables various data linkage and chronic health effect study. Method : This study utilized the data of Korean Environmental Surveillance System Study (Surveillance Study), which consist of asthma, acute bronchitis, chronic obstructive pulmonary diseases (COPD), cardiovascular diseases (congestive heart failure and ischemic heart disease), all cancers, accidents and congenital anomaly, i. e., mainly potential environmental diseases. We reconstructed a nested case-control study wit5h Surveillance Study data and air pollution data in Korea. Among 1,037,210 insured who completed? questionnaire and physical examination in 1992, disease free (for chronic respiratory disease and cancer) persons, between the age of 35-64 with smoking status information were selected to reconstruct cohort of 564,991 persons. The cohort was followed-up to 1995 (1992-5) and the subjects who had the diseases in Surveillance Study were selected. Finally, the patients, with address information and available air pollution data, left to be 'final subjects' Cases were defined to all lung cancer cases (424) and COPD admission cases (89), while control groups are determined to all other patients than two case groups among 'final subjects'. That is, cases are putative chronic environmental diseases, while controls are mainly acute environmental diseases. for exposure, Air quality data in 73 monitoring sites between 1991 - 1993 were analyzed to surrogate air pollution exposure level of located areas (58 areas). Five major air pollutants data, TSP, $O_3,\;SO_2$, CO, NOx was available and the area means were applied to the residents of the local area. 3-year arithmetic mean value, the counts of days violating both long-term and shot-term standards during the period were used as indices of exposure. Multiple logistic regression model was applied. All analyses were performed adjusting for current and past smoking history, age, gender. Results : Plain arithmetic means of pollutants level did not succeed in revealing any relation to the risk of lung cancer or COPD, while the cumulative counts of non-at-tainment days did. All pollutants indices failed to show significant positive findings with COPD excess. Lung cancer risks were significantly and consistently associated with the increase of $O_3$ and CO exceedance counts (to corrected error level -0.017) and less strongly and consistently with $SO_2$ and TSP. $SO_2$ and TSP showed weaker and less consistent relationship. $O_3$ and CO were estimated to increase the risks of lung cancer by 2.04 and 1.46 respectively, the maximal probable risks, derived from comparing more polluted area (95%) with cleaner area (5%). Conclusions : Although not decisive due to potential misclassication of exposure, these results wert drawn by relatively conservative interpretation, and could be used as an evidence of chronic health effect especially for lung cancer. $O_3$ might be a candidate for promoter of lung cancer, while CO should be considered as surrogated measure of motor vehicle emissions. The control selection in this study could have been less appropriate for COPD, and further evaluation with another setting might be necessary.

Characterization of Worker Exposure to Airborne Asbestos in Asbestos Industry (석면취급 사업장 근로자의 석면폭로 특성에 관한 연구)

  • This study was conducted to evaluate worker exposure to airborne asbestos fibers by industry, and to evaluate polarized-light microscopy for determining airborne asbestos fibers. A total of 11 plants including asbestos textile, brake-lining manufacturing, slate manufacturing, and automobile maintenance shops were investigated. Rsults of the study are summarized as follows. 1. Worker exposure levels to airborne asbestos fibers were the highest in asbestos textile industry, followed by brake-lining manufacturing, slate manufacturing, and automobile maintenance shops, in order. In asbestos textile industry, large variation of asbestos levels was found by plants. The worst plant indicated airborne fiber concentrations in excess of 10 fibers/cc, however, the best plant showed concentrations within 0.50 fibers/cc. 2. Characterization of airborne fibers by industry indicated that fibers from asbestos textile industry were the longest with the largest aspect ratio. Fibers from automobile maintenance shops were the shortest with the smallest aspect ratio. Based on characteristics of fibers and the highest levels of concentrations, it is concluded that workers in the asbestos textile industry are exposed to the highest risk of producing asbestosis, lung cancer, and mesothelioma. 3. Result s obtained using polarized-light microscopy were $43.7{\pm}12.3%$ of the results obtained using phase contrast microscopy. This may be resulted from the worse resolution of polarized-light microscopy than that of phase contrast microscopy. Based on the results, it is recommended that polarized-light microscopy be used for mainly bulk sample analyses and further study be performed to improve the method for determining airborne samples. However, polarized-light microscopy can be used for determining thick fibers.
