A Study of Stress Factors Experienced by the Hospitalized Patients (입원이 불안감(Stress)으로서 환자에게 미치는 영향에 관한 일 연구)

  • As the hospitalized patients will be facing new stress situation due to change of his environment from home to hospital it will be very important to understand the psychological stress experienced by hospital patients not only for helping patients in the process of recovery from illness but also fulfil1ing the objective of comprehensive nursing care by understanding the needs of the patients. There is no doubt that it would be very helpful for treatment of patients as well as for improvement of nursing care if we know more about psychological needs of patients and give them adequate support to meet these needs. The study to find out the causes and degree of stress events experienced by hospitalized patients, with the objective of instituting improvement of nursing care program based on the needs of patients, was conducted during the month of September 1974 with 60 patients randomly selected from those admitted to medical and surgical wards at Yonsei Medical Center in that period The questionnaire form included 36 questions which are considered to be stress events for hospital patients, and was devide into five areas namely, such events related to 1) disease itself, 2) hospital environment, 3) nursing care and treatment, 4) communication and human relations, and 5) family and economic problems. The results of the study were as follows: 1. It was confirmed that hospitalization considered to be a stress producing factor and most patients perceived the admission to hospital as a stress factor. 2. According to the rating scale, it was found that degree of perceived stress shows a variation according to the source of stress producing event. 3. No significant differences in the mean values were observed statistically with the perceived stress levels according to demographic and other variables of patients related to hospitalization. 4. Among the questions related to disease itself, "Admission for surgery" was perceived most frequently as a stress event (97.14%) by patients. 5. With regard to the questions related to hospital environment, "death of the patient room-mate" was the most serious stress event perceived by patients (90%) and "living with hospital regulations"was considered to be less serious stress event (23.33%). 6. As for the questions related to nursing care and treatment, "limitation of freedom" was perceived as a stress factor most frequently (70.91%) by the patients and "worry for wrong treatment" turned out to be less frequent stress event (50.0%). 7. As for the questions related to communication and human relations, "difficulty to meet doctors when wanted"appeared to be the most frequent stress event by the respondents (75.86%) , followed by "no explanation about treatment or examination"(75.0%) and "no explanation about nursing care procedures"(71.66%). 8. With regard 111 tile questions related to family and economic problems, "inadequate finances for family living due to hospitalization"and "high cost of hospitalization" were the most frequent cause of stress mentioned by the patients. (80.0%). 9. As a result of application of the stepwise regression analysis, it was found that about 89% was explained by those events associated with disease itself, hospital environment and family and economic problems. By adding those events related to "nursing care and treatment" and "communication and human relation", 100% of stress associated with hospitalization was explained.

The Effect of Stress on Borehole Deformability (응력이 공내 변형률에 미치는 영향)

  • Modulus measurements in vertical boreholes under simulated horizontal in-situ stress conditions were performed on laboratory rock specimens. The experimental program was focused on the examination of modulus change with the variation of the orientation, magnitude and ratios of horizontal biaxial stresses. The experiment results show that the modulus increases when the magnitude of the horizontal stresses increases. The modulus measured in the minimum principal direction increased when the ratio between the horizontal principal stresses increased, while the modulus measured in the maximum principal direction decreased when the ratio of the horizontal principal stresses increased. These were caused by the tangential stresses that vary depending upon the magnitude of horizontal stresses, the applied pressure and the orientation of measurement. Also, the measured moduli were determined under tensile stress, compressive stress, or both stresses. Thus, the stress effect on deformation modulus should be considered, not only for the interpretation of the results of borehole deformability measurement, but also for the design of underground gas storage and pressure tunnel, and for the interpretation of tunnel monitoring.

A study on the stressors and coping patterns of some dental hygiene students (일부 치위생과 학생의 스트레스원과 대처방식에 관한 연구)

  • Purpose : The purpose of this study was to examine the stressors and coping patterns of dental hygiene students in a bid to boost the efficiency of guidance and counseling. Subjects and Method : The subjects in this study were 359 dental hygiene students from some colleges. After a survey was conducted, the collected data were analyzed with SPSS 13.0 program. Statistical data on frequency and percentage were obtained, and ANOVA and t-test were utilized. Findings : 1. The dental hygiene students investigated suffered the most stress with excessive workload, and anxiety about the national examination was the greatest stressor. Among the areas of stressors, school life put the most pressure on them(3.25). 2. The most dominant way for the dental hygiene students to handle stress was talking with friends(4.32). Out of the areas of stress-coping patterns, mental coping was most prevalent(2.94), and the least common stress-coping area was physical coping(2.23). 3. As for the relationship between the place where they were raised and stress-coping styles, physical coping and direct coping were more widespread among the students who were brought up in large urban communities than the others. Conclusion : As the dental hygiene students were different in stress-coping styles according to the place where they were grown, educators who nurture dental hygienists should offer customized education and counseling in consideration of where students were brought up.

Stress and Burn-Out Experience in Caregivers of Patients with Senile Dementia (치매환자 부양자의 스트레스와 소진경험)

  • Purpose: This study was done to identify the experience of stress and burn-out in caregivers of patients with senile dementia. Method: To evaluate the degree of stress and burn-out in caregivers of patients with senile dementia, 64 caregivers and matched to 64 patients with senile dementia at a Primary Health Care Post in South Kyung Sung Province were selected. The study was carried out from March 6 to March 30, 2001. Data on the degree of dementia in the patients was measured by the MMSE-K (Mini-Mental State Examination-Korea) and caregiver characteristics such as, sex, age, marital status, educational level, job, socioeconomic status, religion, number in family, relationship with patient, duration of care, and chronic disease in caregiver were collected by direct interview with a questionnaire. Results: Of 64 patients with senile dementia, 15.6% were classified as mild dementia (MMSE score 20-24) and 84.4%, as severe dementia. There were no significant characteristics of caregivers associated with the degree of stress and burn-out experience. The degree of burn-out in these caregivers of patients with severe dementia (mean value 94.3) was significantly higher than the 81.4 for those caring for patients with mild dementia (p<0.05). However, the degree of stress was not significantly related with the degree of dementia. The proportion experiencing severe burn-out (above score 4) was 54.7% in the physical domain, 90.6% in the emotional domain, and 73.4% in psychiatric domain, respectively. Conclusion: The above findings suggest that the degree of stress and burn-out experienced by caregivers of patients with senile dementia are high. Also the degree of burn-out experienced by in caregivers of patients with severe dementia was higher than for those caring for patients with mild dementia.

Psychiatric Factors Associated with Farmers' Syndrome (농부중의 정신적 원인에 대한 연구)

  • Backgrounds : There has been many studies investigating the causes of farmers' syndrome. In some studies, psychologic stress is related to farmer's syndrome. And the diagnostic criteria of farmer's syndrome is similar to those of generalized anxiety disorder. So we carried out this study to investigate the psychiatric causes of farmers' syndrome. Methods : This study was done in some rural and urban areas of Kyoungsangnam Province, July, August, October and November of 1996. Those who came to free medical service and completed interview, medical examination and laboratory examination and 20-59 years old were 150 persons. And those who came to health center for health examination and completed only interview and 20-59 years old were 94 persons. The questionnaire was composed of sociodemographic factors, health risk factors, farmer's syndrome, work load, BEPSI(inventory to measure stress), Spielberger's state-trait anxiety inventory, self-rating depression scale. To examine statistical significance, we used X2-test, Mantel-Haenszel test for linear association, t-test, ANCOVA, correlation, multiple regression, logistic regression. Results : The prevalence of farmer's syndrome, adjusted for age and sex with population of Kyungsangnam Province of 1993 was 208 per 1,000(90 per 1,000 in men and 329 per 1,000 in women). In bivariate analysis, farmer's syndrome was significantly related to age, sex, job, income, smoking, alcohol drinking, work load, BEPSI, state anxiety, trait anxiety, depression, body mass index, Hwa-Byung, hypertension, anemia. However, when age and sex were adjusted, job was not significantly related to farmer's syndrome. The score of farmer's syndrome was significantly related to age, sex, work load, BEPSI, trait anxiety by multiple regression. Farmer's syndrome was significantly related to increasing age(odd ratio 1.079, 95% C.I. 1.060 - 1.099), sex(odds ratio of male 0.434, 95% C.I. 0.349 - 0.540), and BEPSI(odds ratio 1.231, 95% C.I. 1.148 - 1.320) by logistic regression. Results of logistic regression analysis of the component symptoms of farmer's syndrome were as follows. Shoulder stiffness was significantly related to increasing age, female sex and BEPSI. Lumbago was significantly related to increasing age, female sex and trait anxiety. Numb limbs and nocturia was significantly related to increasing age and female sex. Breathlessness was significantly related to work load, sleeplessness was significantly related to depression, dizziness was significantly related to job and state anxiety, and abdominal fullness was significantly related to female sex. Conclusion : Farmers' syndrome was related to work load, but was more related to psychiatric factors such as BEPSI and trait anxiety. And the occupation was not risk factor of farmers' syndrome in this study, so further study is needed to investigate the cause of farmers' syndrome.

The Effect of Korean Adult's Mental Health On QOL(Quality Of Life) -The Fifth Korea National Health and Nutrition Examination Survey, 2010 (연령대별 성인의 정신건강이 삶의 질에 미치는 영향 -제5기(2010)국민건강영양조사를 중심으로)

  • The purpose of this study is to find out how the adult's mental health in Korea has an impact on the quality of the life. By using '2010 National Health and Nutrition Examination Survey' data, 4,211 people among 20-year-old to 59-year-old adults were enrolled in this study. Frequency and percentage t-test using the SPSS WIN18.0 Multiple regression analysis was conducted and the results of this study are as follows. First, it is showed that in the 20s and 30s the females are a little more sleep than the males regarding the relationship between sleeping hours and the quality of the life of adult men and women. However, there are no differences between the 40s and 50s groups. Second, it is indicated that adult men are higher than women for the subjective health status when it comes to the subjective health status and the usual perceived stress. Although there are no statistical differences according to the gender in the 40s and the 50s, they have a little stress in general. Third, it is confirmed that only the subjective health status and usual stress perception give an beneficial influence on the quality of adult's life in the adult's mental health variations and the quality of life. In summary, this study was found that the adults in the 20s and 50s could increase their satisfaction of their life if they have more positive thinking and less stress from their life. Therefore, the systematical education and programs to manage the health and stress are required.

Nicotine Dependence and Stress Susceptibility in E-Cigarette Smokers: The Korea National Health and Nutrition Examination Survey 2013-2017

  • Background: E-cigarettes are steadily gaining popularity in Korea. However, the characteristics of e-cigarette smokers, especially nicotine dependence and stress susceptibility, have not been evaluated in comparison to those of nonsmokers or combustible cigarette smokers in Korea. Methods: In this study, 28,059 participants from the Korea National Health and Nutrition Examination Survey (2013-2017) were classified into the following three groups: non-smokers, smokers (current smokers and ex-smokers of combustible cigarettes only), and e-smokers (current smokers and ex-smokers of e-cigarettes regardless of combustible cigarette use). Results: Among the participants, 16,980 (60.5%), 9,247 (33.0%), and 1,832 (6.4%) subjects were non-smokers, smokers, and e-smokers, respectively. E-smokers were younger, more educated, and had a higher household income than non-smokers or smokers. The number of e-smokers who smoked within 5 minutes of waking up (31.5% vs. 19.8%, p<0.001) and who planned to quit smoking within 6 months (39.1% vs. 35.7%, p<0.05) was greater than that of smokers. E-smokers perceived stress as "very much" (7.0% vs. 4.4%, p<0.001) and "a lot" (29.1% vs. 20.5%, p<0.001) compared to non-smokers. Suicidal ideation (6.5% vs. 4.7%, p<0.001), plans (2.4% vs. 1.3%, p<0.001), and attempts (1.1% vs. 0.5%, p<0.001) were higher in e-smokers than in non-smokers. Depressive episodes in 1 year (14.2% vs. 11.4%, p<0.05) and suicidal plans (2.4% vs. 1.8%, p<0.05) were more frequent among e-smokers than among smokers. Conclusion: E-smokers were younger, more educated, and had a higher income, but they were more dependent on nicotine and susceptible to stress than non-smokers and smokers. Smoking cessation counseling should be tailored according to the characteristics of e-smokers.

Analysis of Influencing Factors on Subjective Health Status by Life Cycle (KNHANES VII-3) (생애주기별 대상의 주관적 건강상태에 미치는 영향 요인 분석 (KNHANES VII-3))

  • The purpose of this study was to identify the influencing factors of subjective health status among the subjects of the 7th year 3 (2018) National Health and Nutrition Examination Survey, and to help provide programs for improving subjective health status in the future. Data from the 7th Korea National Health and Nutrition Examination Survey(2018) were used. Variables were age, gender, age, subjective health status, subjective body shape recognition, weight change over the past year, usual stress level, walking days per week, and strength training days per week. The higher the number of walking days per week, the lower the usual stress perception level. As the number of strength training days per week increased, the level of stress perception was lower (p<.001). The subjects' subjective health status was found to have a negative effect on their usual stress perception (β=-.759), subjective body shape (β=.111), age (β=.421), and number of days walking per week. (β=.968). Based on this study, it is intended to provide basic data for preparing programs to help improve stress perception and subjective health status in the future.

Effects of Blood Indicators and Stress on Obesity by Gender in Korean Adults (성별에 따른 혈액 지표 및 스트레스가 비만에 미치는 영향)

  • This study aimed to investigate the effect of subjective factors (figure recognition and stress) and objective factors (blood biochemical parameters) on body mass index by gender difference in Korean adults. This study analyzed 3,343 subjects older than 20 years according to gender using the Sixth Korean National Health and Nutrition Examination Survey database. Specifically, male subjects had higher body mass index than female subjects (p < .001) and were more frequent in the obese group. Female subjects had higher stress and platelet score than male subjects (p < .001). Male subjects had significantly higher white blood cell counts (p < .001). The factors affecting the occurrence of obesity by gender difference was highly associated with subjective figure recognition (95% CI = 0.026~0.045, p < .001), stress (95% CI = 0.487~0.925, p = .015) and white blood count (95% CI = 1.232~1.392, p < .001) in females. These findings demonstrate a gender-specific difference in recognizing subjective factors and especially the objective factors of white blood cells and platelets affecting obesity. These results can be used for obesity education or fundamental data for establishing a nursing arrangement program for Korean adults.

Development of Elbow Joint X-ray Examination Aid for Medical Imaging Diagnosis (의료영상 진단을 위한 팔꿉관절 X-선 검사 보조기구 개발)

  • The elbow joint is made up of three different bones. X-rays or other radiological exams are commonly used to diagnose elbow injuries or disorders caused by physical activity and external forces. Previous research on the elbow joint reported a new examination method that meets the imaging evaluation criteria in the tilt position by Z-axis elevation of the forearm. Therefore, this study aims to design an optimized instrument and develop an aid applicable to other upper extremity exams. After completing the 2D drawing and 3D modeling design, the final design divided into four parts was fabricated with a 3D printer using ABS plastic and assembled. The developed examination aid consists of a four-stage Z-axis elevation tilt angle function (0°, 5°, 10°, and 15°) and can rotate and fixate 360° in 1-degree increments. It was designed to withstand a maximum equivalent stress of 56.107 Pa and a displacement of 1.6548e-5 mm through structural analysis to address loading issues caused by cumulative frequency of use and physical utilization. In addition to X-ray exams of the elbow joint, the developed aid can be used for shoulder function tests by rotating the humerus and also be applied to MRI and CT exams as it is made of non-metallic materials. It will contribute to the accuracy and efficiency of medical imaging diagnosis through clinical applications of various devices and medical imaging exams in the future.
