인공수정자의 법적 지위에 관한 연구 (A Study of Legal Status of Artificial Insemination Child)

  • 난임과 인공수정은 증가하고 있음에 비해 이에 대한 법적 규율은 미비한 상태이며, 인공수정으로 출생한 자녀의 법적 지위는 불안하다. 현재는 해석에 의해 해결하고 있지만 향후 입법이 필요한 분야로, 우리 문화와 현재 사회의 인식 정도, 의료 수준의 발달 정도, 국외 여러 나라의 선행 입법례 등을 종합적으로 검토하여야 한다. 특히 입법론에서 고려하여야 할 것은 법을 위반하여 자녀가 출생한 경우, 그의 복리이다. 법이 미래의 모든 사항을 규율할 수는 없기 때문이다. 인공수정자의 복리는 이에 관한 상세한 입법이 없는 현재에서도 중점적 고려사항이다. 자선의 자유 의지와 적극적 행위로 인공수정을 선택한 성인과 달리 인공수정자는 출생에의 선택권이 없기 때문이다. 이하 본고에서는 인공생식 기술의 유형과 현황을 설명하고, 해석과 판례에서 나타나는 인공수정자의 법적인 지위와 기타 문제 등을 살펴보겠다. 그리고 해외 입법례를 통해 향후 입법안의 방향을 제시하겠다. 인공수정의 기술적 부분에 관한 입법안은 생명윤리 및 안전에 관한 법률에 포함시키고, 인공수정자의 친자관계에 관한 법적 지위 부분은 민법전에 편입하는 방식을 취하겠다. 인류의 미래에 도움을 주는 생명과학의 발전은 장려되어야 하지만 인간의 창조나 개량을 향한 연구는 지양해야 한다. 그러므로 본 연구가 현실과 과학을 접목하여 법적인 기준을 제시할 수 있는 단조가 되기를 기대해본다.

Prostate Cancer Screening in a Healthy Population Cohort in Eastern Nepal: an Explanatory Trial Study

  • Background: Prostate cancer features a substantial incidence and mortality burden, similarly to breast cancer, and it ranks among the top ten specific causes of death in males. Objective: To explore the situation of prostate cancer in a healthy population cohort in Eastern Nepal. Materials and Methods: This study was conducted in the Department of General Surgery at B. P. Koirala Institute of Health Sciences, Dharan, Nepal from July 2010 to June 2011. Males above 50 years visiting the Surgical Outpatient Department in BPKIHS were enrolled in the study and screening camps were organized in four Teaching District Hospitals of BPKIHS, all in Eastern Nepal. Digital rectal examination (DRE) was conducted by trained professionals after collecting blood for assessment of serum prostatic specific antigen (PSA). Trucut biopsies were performed for all individuals with abnormal PSA/DRE findings. Results: A total of 1,521 males more than 50 years of age were assessed and screened after meeting the inclusion criteria. The vast majority of individuals, 1,452 (96.2%), had PSA ${\leq}4.0$ ng/ml. Abnormal PSA (>4 ng/ml) was found in 58 (3.8%). Abnormal DRE was found in 26 (1.72%). DRE and PSA were both abnormal in 26 (1.72%) individuals. On the basis of raised PSA or abnormal DRE 58 (3.84%) individuals were subjected to digitally guided trucut biopsy. Biopsy report revealed benign prostatic hyperplasia in 47 (3.11%) and adenocarcinoma prostate in 11 (0.73%). The specificity of DRE was 66.0%with a sensitivity of 90.9% and a positive predictive value of 38.5%. The sensitivity of PSA more than 4ng/ml in detecting carcinoma prostate was 100% and the positive predictive value for serum PSA was 19.0% Conclusions: The overall cancer detection rate in this study was 0.73% and those detected were locally advanced. Larger community-based studies are highly warranted specially among high-risk groups.

국민건강영양조사 자료를 이용한 지방산 데이터베이스 구축 (Development of a fatty acids database using the Korea National Health and Nutrition Examination Survey data)

  • 본 연구에서는 국가적 수준에서 지방산 섭취량을 추정할 수 있도록 국민건강영양조사에서 사용하는 식품목록에 수록된 5,144개의 식품에 대해 지방산 DB를 구축하고, 그 과정을 단계별로 정리하여 향후 개정에 활용할 수 있도록 하였다. 지방산 DB는 국내 국립기관에서 발행한 식품의 지방산 함량 분석 자료를 기본으로 하여, 국외의 권위 있는 기관에서 발간한 자료와 학술지에 보고된 함량 분석 자료를 수집한 후 자료의 질을 검토하고 대체하거나 계산하는 방법으로 구축되었다. 구축된 DB를 2011년 국민건강영양조사 자료에 적용하여 지방산 섭취량을 추정한 결과, 총 지방산 섭취량은 지방섭취량의 84% 수준으로 학술지에 보고되고 있는 섭취량 추정수준보다는 높은 수준이나 DB 구축과정에서 확인된 문제점들을 보완하여 자료의 질을 향상시키는 것이 필요할 것으로 사료된다. 본 연구는 관련 연구 분야와 국가 영양정책의 중요한 기본 자료를 확보하였다는데 의의가 있다.

세계유산 등재대상 함양 남계서원의 경관보존관리방안 (Measures to Implements the Landscape Conservation and Management in the Seowon Hamyang Namgye inscribed on the World Heritage List)

  • 본 연구는 세계유산 등재대상 함양 남계서원의 유산 가치를 구명하기 위해 문화적 맥락을 고려한 보호구역을 설정하고, 이에 따른 경관보존 및 관리방안마련을 목적으로 한다. 서원 일원의 보호구역 설정은 남계서원의 입지특성과 배향인물과 관련된 장소, 풍영루를 중심으로 가시권 분석, 현 문화재 보호구역 및 현상변경영향검토구역 및 토지이용 현황 등을 고려하였다. 특히 서원의 조망경관은 원경인 백암산과 더불어 대봉산의 천황봉이 가시되며, 근경의 대규모 방음벽과 송전탑 등이 조망경관의 저해 요소로 작용함을 파악할 수 있었으며, 향후 서원의 원형경관회복을 위해서는 세계유산의 진정성, 완전성 등의 가치를 충족시키기 위한 보존관리계획 수립이 필요하다고 판단된다.

정상 한국 성인에서 발목관절 외측 안정성의 방사선학적 계측 (Radiographic Measurement of Ankle Lateral Stability in Normal Korean Adults)

  • Purpose: Anterior drawer and varus stress test are commonly used for radiologic evaluation of chronic lateral ankle instability. However, there are controversies regarding the method of measurement and the normal value. This study was performed to investigate radiologic normal values in normal Korean adults and to analyze differences by age and gender. Materials and Methods: Sixty Korean adults were recruited and divided in three groups (20 in their twenties, 20 in their thirties, 20 in their forties). There were 10 males and 10 females in each group. The selection criteria were no history of ankle injury and no evidence of instability on physical examination. Radiologic measurement of varus talar tilt and anterior talar translation were performed through anterior and varus stress radiographs using Telos device (150N force). The measurement was repeated twice by three researchers, and intraobserver reproducibility and interobserver reliability were analyzed. The average talar tilt and anterior talar translation were obtained. Results: Talar tilt and anterior talar translation on ankle stress radiographs had good intraobserver reproducibility and interobserver reliability. Talar tilt was average $3.7^{\circ}$ and $5.1^{\circ}$ in male and female of twenties of age, $3.9^{\circ}$ and $4.8^{\circ}$ in their thirties, $3.4^{\circ}$ and $4.5^{\circ}$ in their forties. Anterior talar translation was average 3.5 mm and 4.2 mm in their twenties, 4.1 mm and 3.8 mm in their thirties, 3.6 mm and 4.1 mm in their forties. There was no significant difference in talar tilt and anterior talar translation by age. However, there was significant difference in talar tilt by gender. Conclusion: Normal range of talar tilt angle in Korean adults was below $8.3^{\circ}$, and normal range of anterior talar translation was below 7.6 mm. It seems to be able to serve as a good reference for radiologic evaluation and for treatment of chronic lateral ankle instability.

외래식물의 공간분포와 토지피복간의 상관성 연구 - 전국자연환경조사 자료를 중심으로 - (Correlations between Spatial Distribution of Alien Plants and Land Cover - Focused on National Ecosystem Survey -)

  • The aim of this study is to seek application plans of National Ecosystem Survey database based on comparison and examination of feasible analysis methods for distribution characteristics of alien plants. In order to set up a correlation analysis method between alien plants and environmental factors, we had reviewed the 3rd National Ecosystem Survey guide book and consequently, two kinds of analysis method were tested. One was 1/25,000 scale map boundary based analysis and the other was representative mountain area based analysis. In this study we restricted the research area to select reliable surveyed database from whole "2011 National Ecosystem Survey flora database" according to two major criteria. First, an area defined by 1/25,000 scale map boundary and representative mountain area where the number of surveyed flora records should be within top 20%. Second, land cover map should also be built up inside that area. As a result, 25 map boundaries and 25 representative mountain areas were extracted to be analyzed. To limit a boundary for every representative mountain area we had analyzed distribution of environmental factors around that area by manual inspection with SPOT-5 remote sensed satellite image then designated 3km buffer zone from each alien plant location in that area. After then, naturalized index (NI) and urbanized index (UI) was calculated and correlations analysis was carried out. With the result of correlation analysis by map boundary only agricultural land area showed significant value of r (0.4~0.6, correlated) and the rest of factors did not. In the case of representative mountain area, the result showed that agricultural land, road and forest area showed significant value of r (0.6~0.8, highly correlated) which was corresponding to existing researches. Therefore, representative mountain area based method is preferable when using the alien plants database of National Ecosystem Survey for species distribution analysis. And also, considering the way of database utilization is strongly suggested at the first stage of survey planning for promoting active use of national ecosystem survey database.

Helicobacter pylori의 in vivo 연구를 위한 ethanol-pretreating animal model의 개발 (Establishment of ethanol-pretreating animal model to study Helicobacter pylori infection)

  • A stable and reliable Helicobacter pylori (H. pylori) infection animal model would be necessary for evaluating vaccine efficacy and helpful for understanding the pathological mechanism of the organism. The aim of the present study is to investigate the effect of ethanol treatment prior to H. pylori inoculation on associated gastric mucosal injury and to establish ethanol-pretreating animal model to study H. pylori infection. Male Mongolian gerbils were used for the study. H. pylori was orally inoculated after 12 h fasting. 3 h prior to H. pylori inoculation, a group of gerbils was orally treated with absolute ethanol, 60% and 40% ethanol respectively. Another group of animals was treated either with H. pylori culture media alone or with different concentrations of ethanol plus culture media. Gerbils were killed 4 or 8 weeks after H. pylori inoculation. The colonization of H. pylori was confirmed by both histological examination and rapid urease test. Mucosal damage was evaluated grossly and histologically according to the criteria. The colonization of H. pylori and pathological changes in gastric mucosa of the animals were also observed. Although no significant change to the gastric mucose was observed in the animals treated either with H. pylori culture media alone or with different concentrations of ethanol plus culture media, persistent H. pylori infection was seen in the mucosa and mucosal leucocyte infiltration and severe epithelial damage was observed in the Helicobacter and ethanol + Helicobacter groups after 4 weeks. The gross and histological scores were higher in the ethanol + Helicobacter than in the Helicobacter alone group. As the results, ethanol-pretreatment with 60% concentration induced severe pathogenic changes by H. pylori infection in 5 weeks-old Mongolian gerbils. These results suggested that ethanol-pretreatment before H. pylori inoculation could increase the severity of gastric mucosal inflammation and enhance the colonization of H. pylori. The established ethanol-pretreating animal model would contribute to screen new drugs against H. pylori and be used as an useful tool for various animal experiments with H. pylori strains.

시판 식물성 식품의 오염지표세균 분포 및 저장온도, 기간별 오염지표세균의 변화 (Distribution of Indicator Organisms and Influence of Storage Temperature and Period in Commercial Plant Food)

  • There were few data for the distribution of the indicator organisms in the commercial plant foods, and for the normal flora and for the foodborne agents within the country. First of all it must be investigated the distribution of the indicator organisms. And also it is very important to prepare the sanitation criteria for the plant foods through the microbiological examination and the investigation of tendency to change of the indicator organisms according to the storage temperature and period. The average number of total viable counts for grains was 2.9$\times$105/g, psychrophilic bacteria 2.9$\times$105/g, heterotrophic bacteria 3.1$\times$105/g, heat-resistant bacteria 2.1$\times$103/g, Pseudomonas aeruginosa 23/g. That for beans was 6.3$\times$102/g, psychrophile 34/g, heterotroph 1.7$\times$102/g. That for sesames was 1.4$\times$105/g, coliform 350/g, psychrophile 7.4$\times$104/g, heterotroph 5.8$\times$104/g, Pseud. aeruginosa 2.3$\times$103/g. heat-resistant bacteria 150/g. That for potatoes was 2.0$\times$107/g, coliform 5.0$\times$104/g, psychrophile 1.8$\times$107, heterotroph 1.4$\times$107/g, heat-resistant bacteria 3.3$\times$104/, Staphylococcus 2.7$\times$105/g, fecal streptococcus 4.5$\times$103/g, Pseud. aeruginosa 7.0$\times$103/g. That for mushrooms was 1.2$\times$108/g, psychrophile 9.4$\times$107/g, heterotroph 1.0$\times$109/g, heat-resistant bacteria 1.6$\times$105/g, Pseud. aeruginosa 1.3$\times$103/g. That for vegetables was 5.9$\times$1011/g, coliform 1.8$\times$106g/, Staphylococcus 1.1$\times$1012/g, heterotroph 8.4$\times$1011/g, heat-resistant bacteria 7.6$\times$106/g, Staphylococcus 1.1$\times$107/g, fecal streptococcus 1.1$\times$104/g, Pseud. aerugniosa 5.2$\times$104/g. That for nuts 3.9$\times$104/g, coliform 3.9$\times$103/g, psychrophile 4.0$\times$104/g, heterotroph 3.2$\times$104/g, heat-resistant bacteria 400/g. In commercial grains and beans, SPC, psychrophile, heterotroph and heat-resistant bacteria stored at 1$0^{\circ}C$, 2$0^{\circ}C$, 3$0^{\circ}C$ were constant. Staphylococcus, coliform, Pseud. aeruginosa were decreased a little n grains, but were not detected in beans. In mushrooms, all indicator organisms were increased as time goes on and were increased rapidly at 2$0^{\circ}C$. In sesames, coliform was not detected at all temperature. psychrophile was increased for 7 days, the others were constant. In potatoes, SPC, psychrophile, heat-resistant bacteria, heterotroph had a tendency to increase and the others were constant. In vegetables, indicator organisms were had a tendency to increase, psychrophile, heterotroph were rapidly increased after 7 days. In nuts, SPC, coliform, psychrophile heterotroph, heat-resistant bacteria, Pseud. aeruginosa were constant, staphylococcus and fecal streptococcus were not detected.

나일틸라피아의 임파조혈조직내 대식세포(大食細胞) 분포변화(分布變化)에 관한 병리조직학적(病理組織學的) 연구(硏究) (Histopathological studies on the macrophage behavior in lymphomyeloid tissues of tilapia, Oreochromis niloticus)

  • 경골어류의 질병진행과정에 따른 임파조혈조직내 대식세포의 분포변화 특성을 밝히고자 Edwardsiella tarda균과 그 분비독소인 Extracellula products(ECP)를 각각 진골어류의 일종인 틸라피아에 복강 주사한 후, 임상증상을 발현하지 않은 개체에 대하여 비장 및 두신의 병리조직학적 변화를 경시적으로 조사하였다. E. tarda 생균을 주사 1주일후, 비장에서는 주로 유초동맥을 중심으로, 두신에서는 조혈 조직전반에 걸쳐 대식세포 집단이 출현하였다. 또한 이 집단들은 시간이 경과함에 따라 MMC와 유사한 밀집구조를 이루었고, 1주째에는 이들 응집체가 더욱 치밀해지면서 주위의 실질은 어느 정도 회복경향을 보였다. 대식세포 집단은 2주째가 되면서 대부분 사라졌고, 거의 정상적 조직배열상을 보였다. ECP주사에 의한 비장과 두신의 경시적 조직학적 변화는 생균주사의 경우와 매우 유사하였으나 생균주사의 경우에 비해 대식세포의 응집이 더욱 빠르게 일어났을 뿐 아니라, 주사 후 1시간째의 비장 및 두신조직에서도 유초동맥의 증식비후(비장) 또는 대식세포 집단의 출현(두신)이 인정되었다. 따라서 E. tarda 생균 또는 그 ECP를 틸라피아에 주사했을 경우, 임파조혈조직내의 이와 같은 대식세포 행동특성은 에드워드증(Edwardsiellosis) 또는 기타 감염증의 조기진단을 위한 중요한 병리조직학적 정보로 활용될 수 있을 것으로 사료되었다.

목조주택 내 마감자재에 따른 카르보닐화합물의 방산특성 (Emission Characteristics of Carbonyl Compounds Depending on Different Finishing Materials in a Wooden House)

  • 본 연구는 실내마감자재가 실내공기질에 미치는 영향을 조사하기 위하여 목재주택 내 친환경 마감자재로 처리한 시험실 내부의 카르보닐화합물의 방산특성을 조사하였다. 실험 결과 포름알데히드, 아세톤 및 헥사알데히드가 카르보닐화합물의 대부분을 차지하였으며, 이 중 포름알데히드가 25.6~30.5%로 가장 높은 비중을 차지하였다. 시험실 R2-2를 제외한 모든 시험실에서 포름알데히드 방산량이 신축 공동주택의 실내공기질 권고기준치 이하를 나타내었고, 아세트알데히드의 경우 시험실 R1-3을 제외한 나머지 시험실에서 일본 후생노동성에서 제시한 기준($48{\mu}g/m^3$)을 초과하는 것으로 나타났다. 황토벽지를 사용한 R1-1에서 상대적으로 높은 방산량을 나타내었고, 대나무숯 패널을 설치한 R2-2에서 모든 카르보닐화합물에 대해 높은 방산량을 나타내었다. 편백으로 치장한 거실인 시험실 R1-3은 나머지 시험실에 비해 아세톤을 제외하고 전반적으로 낮은 포름알데히드와 카르보닐화합물 방산량을 나타내었다.
