Research methodology on Traditional Medicine in East Asia refers to logical thinking system, empirical positivism system and methodology of developing these knowledge systems. Logical thinking system of abstract concepts such as analogy or abduction and positivism system of reasonable explanation such as the five elements and their characteristic theory have been used in various ways empirically or in the form of humanities and knowledge system was developed through parallel structure of empirical positivism and exegetical studies. After the 16th century, evidence was required along with the tradition of putting emphasis on rationality, logicality and empirical positivism and characteristics of medical humanities can be found in emphasizing on medical ethics. Data that can be considered as structural review paper or meta analysis from original data of research on Traditional East Asian Medicine should be evaluated as historical evidence which is equivalent to specialist opinion, descriptive disease research, single case report or case series. Historical evidence based medicine is a research method using Historical evidence to selectively support data that are faithful to traditional theory with higher possibility to be used in future traditional east Asian medicine that links between traditional knowledge and scientific research methodology. Moreover, historical evidence based medicine tries to re-evaluate the value of traditional knowledge and ultimately, guides the direction of development of traditional medicine through scientific rationality based on history and culture.
Objectives: The purpose of this study is to analyze the current state of quantitative literature evidence for the prescription of 56 herbal medicines covered by health insurance that have been studied in Korea for the past 30 years, to evaluate the reliability of the evidence, and to find out the research direction of herbal medicine prescription in the future. Methods: 56 kinds of herbal medicine prescriptions were searched in domestic literature search databases OASIS, DBpia, and overseas PubMed, classified into chemistry, toxicity, cells, animals, clinical cases, and clinical trial studies, and built into an EBM pyramid structure. Results: When classified according to research contents, there were 61 cases (7.5%) of physicochemical analysis to identify constituent substances, 80 cases (9.8%) of toxicity evaluation, and 672 cases (82.7%) of efficacy evaluation. The efficacy evidence was classified according to the evidence-based medical pyramid structure: 196 cell trials (29.1%), 372 animal trials (55.4%), 89 case and case reporting series (13.3%), 7 comparative case studies (1.1%), and 8 randomized control clinical trials (1.2%). In the pyramid composition, the basis for the validity of 56 kinds of herbal medicines prescribed was 568 cases (84.5%) in cell and animal units, which could not be said to be highly reliable. There was no relationship between the ranking of quantitative literature evidence for herbal medicine prescriptions and the ranking of salary administration. Conclusions: In an era that continues to require scientific evidence for herbal medicine, traditional herbal medicine should secure the basis for safety validity even for the 10th most frequent prescription among 56 herbal medicine prescriptions for consumers. In particular, traditional herbal medicine should increase the quantitative and qualitative level of case reports on related herbal medicine prescriptions, focusing on each clinical society, and move toward comparative case studies and randomized clinical trial so that traditional herbal medicine is positioned as Evidence-based medicine.

Evidence based practice (EBP) is a system of applying the most current and valid high quality evidence to support clinical decision making in a healthcare setting. In the twenty five years since its inception, EBP has become the accepted benchmark for excellence in healthcare. Although the system emerged within the biomedical sciences, in the years since EBP has become normative across all healthcare modalities from dentistry, allied health to complementary and alternative medicine (CAM). Practicing evidence based medicine within any modality potentially offers the patient the best available care based on high quality evidence. Yet it is the nature of the evidence that provokes some questions about the suitability of EBP across all modalities of healthcare. The meta analysis of randomized controlled trial (RCT) stands at the pinnacle of the hierarchy of evidence in EBP. This forms a challenge to CAM due to the difficulty in reducing the elementals of a holistic naturopathic assessment of a patient into an answerable question to be tested within a RCT. On one level this makes EBP paradigmatically incompatible with CAM, yet on another level it presents the opportunity to redefine the parameters of what is considered high level evidence. EBP has become a tool, and at times a weapon wielded by governments and health insurance companies to direct healthcare funding and policy. The implications of the nature of accepted evidence are becoming far reaching. The pursuit of the best available healthcare for each individual is the focus of EBP. However, the injudicious use of this system to direct health policy is fraught with biomedical bias and dominance. This issue raises the challenge to CAM to present high level evidence according to the rules of evidence, or face the annihilation of centuries of empirical knowledge.
Background: We performed this meta-analysis to assess the effectiveness and safety of axillary dissection in old women. Methods: The Cochrane Library, PubMed, EMBASE and Chinese Biomedical Literature Database were searched and all randomized controlled trials of axillary dissection in old women (at least 60 years old) were considered. Meta-analyses were completed using RevMan5.1. Results: Three eligible randomized controlled trials (RCTs) including 5,337 patients were considered. There was weak evidence in favour of axillary dissection (AD) in old women. The meta-analysis showed that the overall survival (OS) after 1, 3, 5 and 7 years and the disease free survival (DFS) after 1, 3 and 5 year were not statistically significantly different between AD and no AD groups. However, there was a difference in the 7 year DFS. Conclusions: Axillary dissection did not provide survival benefit to the old women with breast cancer analysed. Therefore, axillary dissection is not well-indicated in old women with breast cancer.
Objectives : The inclusion of Korean Medicine in the national health care system led to the need of a serious discussion on its modernization. This paper looked at how these discussions were progressed. Especially, a difference in the meaning of 'science' in the 1950s and the 1990s was observed. Methods : Articles related to the medical dispute between Korean and Western Medicine in "EE LIM", especially the ones from the 1950s, were selected, classified and analyzed. Research papers on Evidence-Based Medicine were also analyzed. Evidence-Based Medicine was a new medical research methodology in the 1990s. Articles on Evidence-Based Medicine were searched and selected from journal databases: DBpia, KISS, RISS, and OASIS. Results : The problem of establishment of modernity of Korean Medicine in the recent times was addressed in the context of scientification and systematization. Accordingly, the academic societies of Korean Medicine aimed for scientification. But efforts to secure the identity of Korean Medicine opposed to western science were also made. Meanwhile, Evidence-Based Medicine established in 1992 presented a different concept of science. This led to a new means of acknowledgement of Korean Medicine in the modern society. Conclusions : The modernization process of Korean Medicine up to now was a course of scientification. But as this process did not fully reflect its characteristics, the modernization of Korean Medicine still remains a pending question. A modern study methodology that fully reflects the identity of Korean Medicine needs to be established.

Objects : To establish directions to evidence-based medical interventions and diagnosis technology in oriental medicine, we did survey research among oriental medical doctors. Methods : Systematically-organized questionnaires were provided for survey. 105 Oriental medical doctors nationwide participated in the survey. We investigated diseases of their patients, differential diagnosis methods, frequently used medical interventions, needs for diagnosis devices, evidence-based clinical manuals, etc. Results : In oriental medical clinics, the most frequent patient class was patients with musculo-skeletal diseases. Oriental medical doctors assumed that the preparation of care solution for chronic life-style diseases was the most urgent. Dong-Eui-Bo-gam(東醫寶鑑) was the most frequently used reference for their herbal medication prescription. Fixed document for diagnosis and treatment were thought to be the most important in the developing evidence-based clinical manuals. Conclusions : Validity and reliability should be considered as very important in developing oriental diagnosis devices. Evidence-based clinical manuals are needed to build standardized document for diagnosis and treatment and to verify efficacy and safety of oriental medical diagnosis and treatment.

Background: The incidence rate of thyroid cancer has been increasing worldwide in recent years, and it is also the most prevalent cancer when it comes to the number of patients among Korean women. With it, ultrasonographic screening test has also become very common. However, there is still controversy over the performance of this screening test. Therefore, the National Evidence-based Healthcare Collaborating Agency (NECA) organized a Round-table Conference on the issues regarding ultrasonographic screening for thyroid cancer in Korea. The objective of the conference was mainly about delivering worthwhile information reflecting social value for the current situation, which was based on evidence surrounding thyroid cancer screening that relevant experts investigated and agreed on. The significance of this Round-table Conference lies in the fact that we reviewed the current evidence, and we were able to discuss the social value and future direction for ultrasonographic screening in Korea.
Evidence-based dentistry is a new, worldwide trend in the field of clinical dentistry. Despite this fact, it has been neglected in Korean dental community. Thus, its basic concepts and the 5-step procedures are introduced and described in this 2-part series report. Evidence-based dentistry is a method of clinical practice where the clinicians use the best available research evidence for their own clinical decision making. By using this method, the clinical outcomes can be improved and the clinicians themselves can have the ability to keep watch over their daily practices. To clinically apply the principles of evidence-based dentistry, dental practitioners have to formulate a clear question, find the best available research evidence efficiently, critically appraise the evidence systematically, apply the results of the evidence to their practice, and evaluate the clinical outcomes comprehensively. In building a clinical question, it is essential to categorize questions according to their types followed by their organization using the key elements. Then, literature search must be done. There are many web sites for searching evidence, but PubMed is considered to be the leading site. To efficiently search the literature, search term must be selected appropriately and the search results should be limited.
