When modeling a complex system we use an event tree to analyze propagation of failure. An event tree cannot represent the statistical interrelationships among parameters, but it can be represented as a statistically identical influence diagram so that parameter updating can be easily performed. After updating parameters we can calculate posterior distribution of the failure rate for each path. But exact distribution requires considerably complex numerical integration. We propose an approximation method to calculate the posterior and derive the predictive distribution of the time to next failure. Finally we introduce the system which implements our methodology.
In a seismic PSA, dependency among seismic failures of components has not been explicitly modeled in the fault tree or event tree. This dependency is separately identified and assigned with numbers that range from zero to unity that reflect the level of the mutual correlation among seismic failures. Because of complexity and difficulty in calculating combination probabilities of correlated seismic failures in complex seismic event tree and fault tree, there has been a great need of development to explicitly model seismic correlation in terms of seismic common cause failures (CCFs). If seismic correlations are converted into seismic CCFs, it is possible to calculate an accurate value of a top event probability or frequency of a complex seismic fault tree by using the same procedure as for internal, fire, and flooding PSA. This study first proposes a methodology to explicitly model seismic dependency by converting correlated seismic failures into seismic CCFs. As a result, this methodology will allow systems analysts to quantify seismic risk as what they have done with the CCF method in internal, fire, and flooding PSA.

This Paper proposes a new methodology for assessing the reliability of an accident management, which Is based on the reliability physics and the scheme to generate dynamic event tree. The methodology consists of 3 main steps: screening; uncertainty propagation; and probability estimation. Sensitivity analysis is used for screening the variables of significance. Latin Hypercube sampling technique and MAAP code are used for uncertainty propagation, and the dynamic event tree generation method is used for the estimation of non-success probability of implementing an accident management strategy. This approach is applied in assessing the non-success probability of implementing a cavity flooding strategy, which is to supply water into the reactor cavity using emergency fire systems during the sequence of station blackout at the reference plant.
At the design stage of a plant, the plausible causes and pathways of release of hazardous materials are not clearly known. Thus there exist large amount of uncertainties on the consequences resulting from the operation of a fusion plant. In order to better handle such uncertain circumstances, we utilize the Probabilistic Risk Assessment(PRA) for the safety analyses on fusion power plant. In this paper, we concentrate on the tritium release accident. We develop a simple model that describes the process and flow of tritium, by which we figure out the locations of tritium inventory and their vulnerability. We construct event tree models that lead to various levels of tritium release from abnormal initiating events. Branch parameters on the event tree are assessed from the fault tree analysis. Based on the event tree models we construct influence diagram models which are more useful for the parameter updating and analysis. We briefly discuss the parameter updating scheme, and finally develop the methodology to obtain the predictive distribution of consequences resulting from the operating a fusion power plant. We also discuss the way to utilize the results of testing on sub-systems to reduce the uncertain ties on over all system.

A HAZID is a brainstorming workshop to identify hazards in an early phase of a project. It should be flexible to capture all probable accidents allowing experienced participants to exploit their expertise and experiences. A bowtie analysis is a graphical representation of major accident hazards elaborating safety measures i.e. barriers. The result of these workshops should be documented in an organized manner to share as good as possible details of the discussion through the lifetime of the project. Currently results are documented using a three-step representation of an accident; causes, top event and consequences, which cannot capture correctly sequence of events leading to various accidents and roles of barrier between two events. Another problem is that barriers would be shown repeatedly leading to a misunderstanding that there are an enough number of safety measures. A new bowtie analysis method is proposed to describe an accident in multiple steps showing relations among causes or consequences. With causes and consequences shown in a format of a tree, the frequencies of having the top event (Fault tree analysis) and various consequences (Event tree analysis) are evaluated automatically based on the frequency of initiating causes and the probabilities of failure of barriers. It will provide a good description of the accident scenario and help the risk to be assessed transparently.

Fault tree analysis(FTA) is available to the engineer for determining reliability of complex industrial safety system. Therefore quantitative aspects of FTA greatly multiply its power this paper proceeds of presenting the methodology of FTA, including an approach to constructing in fault tree. A working guide to the use of FTA for the purpose of cost/benefit determination in industrial safety system is given. Finally, an analytic method for uncertainty analysis of the top event of a complex system is described.
In order to overcome the difficulties of quantitative risk analysis such as complexity of model, we propose a systematic methodology for risk quantification of railway system which consists of 6 steps: The identification of risk factors, the determination of major scenarios for each risk factor by using event tree, the development of supplementary fault trees for evaluating branch probabilities, the evaluation of event probabilities, the quantification of risk, and the analysis in consideration of accident situation. In this study, in order to address the feasibility of the propose methodology, this framework is applied to the prototype risk model of nation-wide railway level crossings. And the quantification result based on the data of 2005 in Korea will also be presented.

This article describes an integrated Level 1-Level 2 probabilistic risk assessment (PRA) methodology to evaluate the radiological risk during postulated accident scenarios initiated during the decommissioning phase of a typical Mark I containment boiling water reactor. The fuel damage scenarios include those initiated while the reactor is permanently shut down, defueled, and the spent fuel is located into the spent fuel storage pool. This article focuses on the integrated Level 1-Level 2 PRA aspects of the analysis, from the beginning of the accident to the radiological release into the environment. The integrated Level 1-Level 2 decommissioning PRA uses event trees and fault trees that assess the accident progression until and after fuel damage. Detailed deterministic severe accident analyses are performed to support the fault tree/event tree development and to provide source term information for the various pieces of the Level 1-Level 2 model. Source terms information is collected from accidents occurring in both the reactor pressure vessel and the spent fuel pool, including simultaneous accidents. The Level 1-Level 2 PRA model evaluates the temporal and physical changes in plant conditions including consideration of major uncertainties. The goal of this article is to provide a methodology framework to perform a decommissioning Probabilistic Risk Assessment (PRA), and an application to a real case study is provided to show the use of the methodology. Results will be derived from the integrated Level 1-Level 2 decommissioning PSA event tree in terms of fuel damage frequency, large release frequency, and large early release frequency, including uncertainties.
Accidental oil and gas leak is a critical concern for the offshore industry because it can lead to severe consequences and as a result, it is imperative to evaluate the probabilities of occurrence of the consequences of the leakage in order to assess the risk. Event Tree Analysis (ETA) is a technique to identify the consequences that can result from the occurrence of a hazardous event. The probability of occurrence of the consequences is evaluated by the ETA, based on the failure probabilities of the sequential events. Conventional ETA deals with events with crisp failure probabilities. In offshore applications, it is often difficult to arrive at a single probability measure due to lack of data or imprecision in data. In such a scenario, fuzzy set theory can be applied to handle imprecision and data uncertainty. This paper presents fuzzy ETA (FETA) methodology to compute the probability of the outcomes initiated due to oil/gas leak in an actual offshore-onshore installation. Post FETA, sensitivity analysis by Fuzzy Weighted Index (FWI) method is performed to find the event that has the maximum contribution to the severe sequences. It is found that events of 'ignition', spreading of fire to 'equipment' and 'other areas' are the highest contributors to the severe consequences, followed by failure of 'leak detection' and 'fire detection' and 'fire water not being effective'. It is also found that the frequency of severe consequences that are catastrophic in nature obtained by ETA is one order less than that obtained by FETA, thereby implying that in ETA, the uncertainty does not propagate through the event tree. The ranking of severe sequences based on their probability, however, are identical in both ETA and FETA.
This paper presents a new dynamic approach for assessing feasibility associated with the implementation of accident management strategies by the operators. This approach includes the combined use of both the concept of reliability physics and a dynamic event tree generation scheme. The reliability physics is based on the concept of a comparison between two competing variables, i.e., the requirement and the achievement parameter, while the dynamic event tree generation scheme on the continuous generation of the possible event sequences at every branch point up to the desired solution. This approach is applied to a cavity flooding strategy in a reference plant, which is to supply water into the reactor cavity using emergency fire systems in the station blackout sequence. The MAAP code and Latin Hypercube sampling technique are used to determine the uncertainty of the requirement parameter. It has been demonstrated that this combined methodology may contribute to assessing the success likelihood of the operator actions required during accidents and therefore to developing the accident management procedures.
