Human Bites Over Nose: Management and Reconstruction

  • Background Human bite wounds in emergency department need evaluation in regard of reconstruction. These are due to occlusive bite injuries over face. Most commonly, human bites over face involve ear and nose, and may lead to avulsion injury. Defects over nose can be reconstructed immediately after debridement or delayed till the wound heals and scar becomes supple. Thorough wash and lavage with broadspectrum antibiotic cover has utmost importance in preventing cartilage infection. Methods We report 20 cases of human bite injuries over nose who presented to us in emergency department between 2018 and 2020. At the time of presentation the wound was assessed for closure. If not possible, patient was planned for delayed reconstruction after 3 months. In case delayed reconstruction was planned, the skin and nasal mucosa were approximated at first presentation. The patients underwent paramedian forehead flap after recreation of defect with conchal cartilage graft. Second stage of flap detachment and insetting was done after 3 weeks. After three weeks of second stage, third stage of flap thinning was done. Patients were followed for 3-6 months and subjective satisfaction was noted. Results Nineteen patients underwent delayed staged reconstruction with paramedian forehead flap and one underwent primary wound closure. The flap survival was 100%. The patient satisfaction was excellent in most cases. Conclusion We recommend delayed reconstruction for human bite nasal injuries. For reconstruction, paramedian forehead flap with conchal cartilage graft, if required, provides excellent reconstructive option with good contour and color match and minimal donor site scar.

Recreation of Korean Traditional Nuruk and the Analysis of Metabolomic Characteristics

  • Korean traditional Nuruk has been developed with various materials and shapes according to geographical environments and climates of their origins. Nuruk is also known as kokja in Korea, reflecting the understanding that microorganisms such as wild fungi, yeasts, and lactobacillus bacteria are naturally inoculated and reproduced. The objective of this study is to identify the characteristics of traditional Nuruk through recreating traditional production methods detailed in ancient Korean documents. In the present study, a total of 58 different kinds of Korean traditional Nuruk were prepared, including 46 kinds of recreated products. Each Nuruk sample was evaluated for its enzymatic activities, including glucoamylase, protease, and glucanase. Their suitability for alcoholic beverage production were compared to each other. To isolate valuable microorganisms from Nuruk samples, alcoholic beverages produced using each sample were subjected to sensory evaluation to determine their taste. In addition, metabolite changes in traditional alcoholic beverages fermented with different kinds of Nuruk were analyzed through mass-based metabolomics approach. This study presents, for the first time, the traditional production methods written in ancient Korean documents using workable production methods supported by modern technologies. In addition, this study analyzed the characteristics of reproduced Nuruk. It could be utilized as a basis for studying traditional Korean traditional alcoholic beverages and their valuable microorganisms.

The Impact of Recreational Sports Enthusiasts' Willingness to Engage in the Leisure Consumption

  • Purpose: The present research guides to a middle ground where recreational sports and enjoyment justify the basic concept of leisure consumption, exploring an exciting field of study, where participation is the motivational source of recreational sports fans for transferring into the motivation of leisure activities and leisure consumption. Research design, data and methodology: This research took a close look at the current literature to investigate impacts of recreational sports enthusiasts' willingness to engage in the leisure consumption. The screening and excluding procedure took a place to select a qualified prior resources and the present author finally obtained a total of 19 highly relevant papers. Results: The evaluation of the presented research shows that the enthusiasts' participation in avocational sports has four significant implications on their lifestyle and leisure consumption. These impacts are categorized as follows: Economic influence, Psychological influence, Social influence, and Environmental influence. Conclusions: In sum, this study has shown the various ways and magnitude in which willingness to participate in leisure affects its consumption by the recreational sports enthusiast. These impacts have brought critical implications for the viability of the sport leisure business in the industry, particularly in urging it to be the right strategic direction to cater to this market segment.

신조 운항실습선의 여름철 실내 온열환경 실측평가 (A Measurement and Evaluation on the Indoor Thermal Conditions in Summer of a New Training ship)

  • The purpose of this study is to measure and analyze the ship's indoor thermal conditions and also to integrate experimental database of those which are supplied and controlled by marine HVAC. On this study, temperature, humidity and air volume of 6 different needs' cabin are measured like previous report on a newly-launched training ship during 25th through 27th of July, 2007. Followings are the results of this study. (1)The air supply volumes to each cabins are measured 250CMH(Recreation room), 800CMH(Conference room), 1.000CMH(Bridge), 5,100CMH(Lecture room) respectively. (2)The temperatures are maintained at $21{\sim}27^{\circ}C$ in almost cabins through measuring period, but the temperatures are fluctuated over ${\pm}4^{\circ}C$ at the bridge and conference room. (3)The relative humidities are shown between $40{\sim}60%$ known as comfort conditions, but the conference room is needed to dehumidified because of over 70% humidity. (4)From the student cabins' measurements which have different supply diffuser(s), it is clear that the design is suitable for this case. (5)Because of temperature diversities, only 32% among the measured data are satisfied with the comfort standard range proposed by ASHREA.

확률효용모형 분석을 통한 국립공원의 경제적 가치 평가 (Random Utility Models and the Value of National Parks in Korea)

  • 소비자들이 실제로 선택한 행위를 분석하여 국립공원과 같은 자연생태계에 대한 수요를 분석하고 그 경제적 가치를 도출하는 대표적인 방법은 여행비용법과 이산선택모형을 이용하는 방법이다. 한국에서는 여행비용법의 적용사례는 상당히 많이 있으나, 상대적으로 많은 장점을 가짐에도 불구하고 이산선택모형을 적절히 적용한 연구사례는 아직 없는 실정이다. 본 연구는 전국적인 표본조사를 통해 18개 국립공원 가운데 방문지를 선택하는 행위를 조건부로짓모형과 계층로짓모형을 추정해 실증분석하고, 그 결과를 이용해 각 국립공원별 경제적 가치와 국립공원 특성별 경제적 가치를 도출한다.

소규모 생태연못(원흥이 방죽)의 부영양화 평가 (Evaluation of Trophic State of a Small-scale Pond (Wonheung) in Ecological Park)

  • Many small-scale ponds that serve as ecological habitat, recreation and irrigation are faced to eutrophication problem, which causes aesthetic nuisance and ultimately loss of their functions. Thus accurate evaluation of the trophic state of these ponds is essential to provide rational information to the stakeholders so that they can develop effective management actions. In this study, the trophic state of a small pond (Wonheung) that experiencing water quality degradation due to vicinity land development was assessed using various Trophic State Indexes (TSIs) and statistical analysis including Principal Components Analysis (PCA) based on the field monitoring data obtained from May to December, 2007. The results showed that the pond is under eutrophic state with average total nitrogen (T-N) and total phosphorus (T-P) concentrations of $708.1{\mu}g/L$ and $59.3{\mu}g/L$, respectively. The factor loading plot obtained from PCA showed distinct two influencing factors, PC 1 and PC 2. PC 1 was grouped by T-P, Chlorophyll a (Chl-a), suspended solids (SS), TN/TP ratio, and transparency that all strongly related to the eutrophication state, while PC 2 by temperature, conductivity, dissolved oxygen (DO) and turbidity that explains the seasonal water quality variations. The limiting factor was identified as light rather than phosphorus by both T-N/T-P ratio and TSI indexes analysis. The results and methodology adopted in this study can be used for water quality assessment for other small ponds and lakes.

통나무집의 동절기 실내 온열환경 요소의 측정과 온열감 평가에 관한 연구 (A Study on Examination of Indoor Thermal Environment Elements and Thermal Sensation Vote of Log Cabins in Winter Season)

  • Various buildings constructed by environmentally friendly resources are being built in KOREA. Especially as the wood has distinctive ecological merits in comparison with reinforced concrete and brick, the buildings made by the wood are acknowledged with its superiority of ecological value. Enough field studies for their thermal environment, however, haven't been done. In this study, to investigate indoor environmental condition and occupants' response to it of Log Cabin in Gyeongsangnam-do Hamyang Country Jirisan Natural Recreation Forest, examination of indoor thermal environment and field subjective evaluation have been done in that fundamental information of thermal environment characteristics can be suggested. The results are following; 1) Thermal environment of the Log Cabins; Indoor and outdoor mean dry bulb temperature were $21.9^{\circ}C$ and $-3.1^{\circ}C$, and Indoor and outdoor average relative humidity were 25.8% and 52.1%. These results are below ASHRAE; dry bulb temp. $22.0^{\circ}C$, humidity 30%, and above domestic standards; dry bulb temp. $18{\sim}20.0^{\circ}C$, humidity $40{\sim}60%$. 2) Result of subjective evaluation; Thermal sensation and its comfort were evaluated as 'slightly uncomfortable' because of 'slightly warm'. And humid sensation and its comfort were evaluated as 'slightly uncomfortable' because of 'slightly warm'. 3) Result of vertical temperature and humidity; Vertical temperature difference from head to ankle was $0.54^{\circ}C$ which means most occupants may feel comfortable.

정신건강 어려움이 있는 국내 초등학생의 학교-기반 중재 : 체계적 고찰 (School Based Intervention with Mental Health Problem in the Elementary School : Systematic Review)

  • 본 연구는 정신 건강에 어려움을 가진 국내 초등학생의 학교기반 중재를 체계적으로 고찰하여 추후 더욱 효과적인 정신건강 관련 학교 기반 중재를 모색하고자 하였다. PRISMA 체크리스트와 흐름도를 통해 체계적 고찰을 시행하였고, 데이터베이스는 한국교육학술정보원, 국가과학기술센터, 국회도서관을 이용하였다. 대상 연구의 중재 특성, 대상자 특징, 평가도구로 분석하였다. 최종적으로 25개의 연구를 선정하였으며, 학교 기반 중재 특징에 따라 심리교육 중재(자기관리기술, 사회기술, 사회정서학습), 행동 중재(긍정적 행동지원, 모델링), 심리치료 중재(미술치료, 상담치료 및 부모교육), 레크리에이션 중재(로봇 중재, 원예치료)로 분류하였다. 연구의 대상자로는 지적장애가 가장 많았고, 주의력 결핍 과잉행동장애가 그 다음으로 많았다. 평가 영역은 주로 정서의 변화, 주의력의 변화를 알아보는 도구가 사용 되었다. 본 연구를 통해 학교기반 중재의 연구 분야, 대상자, 평가도구의 근거를 마련하였다. 이는 추후 더욱 효과적인 정신건강 관련 학교기반 중재를 마련하는데 있어 중요한 기본 자료로 활용 할 수 있을 것이다.

걷고 싶은 거리 조성사업의 경제적 가치 추정 - 과천 특화거리 사업 중 '환영의 거리'를 사례로 - (An Estimation of Economic Value on Street Plan for a Walking Tour - In the Case of 'Welcome Avenue Project' in Gwacheon -)

  • 최근 쾌적한 도시환경과 삶의 질에 대한 관심과 욕구가 급증하면서 많은 걷고 싶은 거리 조성사업이 추진되고 있지만 이에 대한 지역주민의 지지 등의 태도의 평가나 경제적 가치의 추정 등은 미흡한 실정이다. 걷고 싶은 거리는 말 그대로 누구나 보행할 수 있는 공간으로서 공공서비스 사업이자 지역 명소화 차원에서 추진되고 있으며, 대표적인 공공재이자 비시장재화이다. 본 연구는 비 시장가치 추정법 중 조건부시장가치법(CVM)에 의해 공공서비스 사업이자 지역 명소화 차원에서 추진되고 있는 걷고 싶은 거리 조성사업에 대한 경제적 가치 평가를 목적으로 하였다. 과천시에서 추진 중인 걷고 싶은 거리 조성사업에 대하여 과천시민 152명과 경마공원 이용객 175명을 대상으로 과천시와 경마공원의 이미지 개선효과 등의 분석과 경제적 가치를 추정하였다. 분석 결과, 과천시가 제안한 걷고 싶은 거리 사업에 대하여 지역주민의 69.8%와 경마장 이용자의 60%가 관심을 나타냈고, 지역주민의 81.6%와 이용자의 89.7%가 찬성하였으며, 지역주민의 67.8%와 이용자의 69.7%가 세금 또는 입장료의 지불의사를 나타냈다. 또한, 5년간 지역주민은 가구당(연간) 11,721원의 세금을, 이용자 집단은 750원의 입장료를 추가적으로 지불할 의사가 있어서 전체적으로 걷고 싶은 거리 조성사업은 총사업비의 약 1.4배에 해당하는 9,997백만원의 경제적 가치를 가지는 것으로 분석되었다. 이는 공공재로 제공되는 걷고 싶은 거리 조성사업에 대하여 매우 긍정적 평가와 기대를 나타내는 것으로 이론적 그리고 실무적 측면에서 매우 고무적인 연구결과라 할 수 있다.

자연휴양공간(自然休養空間)의 이용행동(利用行動) 및 인지적(認知的) 평가(評價) (Active and Cognitive Evaluating of the Recreational Spaces in Natural Settings)

  • 휴양공간(休養空間)의 환경특성(環境特性)과 이용자의 행동(行動) 및 인지반응(認知反應)과의 상호관계를 밝혀, 자연지역에 존재하는 다양(多樣)한 이용 공간에 대한 휴양(休養) 평가(評價)를 실시하였다. 본 연구는 일본(日本) 대판부(大阪府)의 자연공원내(自然公園內)에 있는 수림(樹林), 수변공간(水邊空間), 자유광장(自由廣場) 및 기타 휴양시설(休養施設)들을 연구대상 공간으로 설정하고, 현지 이용자에 대한 직접(直接) 설문 조사를 실시하여 얻어진 정보를 분석한 연구 결과는 다음과 같다. 1) 수림(樹林)과 수변공간(水邊空間)은 야외 휴양 공간의 기본적 구성요소인 것으로 판단된다. 수림은 임층구성(林層構成)에 따라 휴양 평가의 차이가 있다. 특히 활엽수림(闊葉樹林)은 그 효용성이 높은 것으로 나타났다. 2) 자유광장(自由廣場)은 활용성이 광범위한 휴양 공간이지만 이용 형태에 대응한 공간규모나 지형 등을 설정하여야 하며, 지피식물(地被植物)의 도입과 철저한 관리가 요청된다. 3) 야외 휴양 공간내의 인공시설(人工施設)(전망대(展望臺), 곤충관(昆蟲館), 캠프장(場))은 휴양적(休養的) 평가치(評價値)가 낮은 것으로 나타났으나 다양한 이용성이 있으므로 야외 휴양 활동에 반드시 필요한 것으로 생각된다. 다만 그 설치에 있어서 주변의 자연경관(自然景觀)과 환경용량(環境容量)을 신중히 고려하여야 한다. 4) 전통적인 역사(歷史), 문화시설(文化施設)도 야외 휴양자원의 귀중한 구성요소가 되고 있음이 인정됨으로 이에 대한 지속적인 보전 대책이 강구되어야 할 것이다.
