A Study on Quality of Life and Related Factors of Ostomates (장루보유자의 삶의 질 및 관련 요인에 대한 연구)

  • This is a descriptive study on quality of life(QOL) and related factors of ostomates to provide a basic data for development of nursing interventions. The subjects were 110 ostomates who were members of the Daegu or Daejon branches of the Korean Ostomy Association. Data collection was performed between March 1st and April 6th, 1998. Measurements of QOL, self-care, family support, self-esteem, and hope were used as the study tools. Data were analyzed with the SAS program by using t-test, ANOVA Pearson correlation and stepwise multiple regression. The results are as follows : 1) The score on the QOL scale ranged from 97 to 226 with a mean of 164.53($\pm$28.29). 2) The score of QOL on the general and ostomate-related characteristics showed significant differences according to monthly income, monthly participation in ostomate meetings, combined treatments after operation, types of evacuation management, problems of ostomy, help in caring for the ostomy, length of time since ostomate surgery, and presence or absence of readmission after discharge. 3) There were significant positive correlations between quality of life and other factors : self-esteem(r=0.7107, P<0.001), hope(r=0.6584, P<0.001) family support(r=0.6191, P<0.001), perception of health condition(r=0.6017, P<0.001), and self-care (r=0.2286, P<0.05). 4) The variables that affected the quality of life of the subjects were self-esteem, level of family support, perception of health condition, monthly participation in ostomate meetings, combined treatments after operation, monthly income, level of hope, and age in that order. The variance of quality of life was $R^2$=77.20 percent by calculating the sum of those variables. In conclusion, it is necessary for nursing intervention to promote self-esteem, family support, and hope in the care of ostomates in order to improve QOL.

  • Natural disasters caused by abnormal climates are continuously increasing, and the types of natural disasters that cause the most damage are flood damage caused by heavy rains and typhoons. Therefore, in order to reduce flood damage, this paper proposes a system that can predict the water level, a major parameter of flood, in real time using LSTM and GRU. The input data used for flood prediction are upstream and downstream water levels, temperature, humidity, and precipitation, and real-time prediction is performed through the pre-trained LSTM-GRU model. The input data uses data from the past 20 hours to predict the water level for the next 3 hours. Through the system proposed in this paper, if the risk determination function can be added and an evacuation order can be issued to the people exposed to the flood, it is thought that a lot of damage caused by the flood can be reduced.

Field Experiment on Influence of Stack Effect to Pressure Differential System for Smoke Control (연돌효과가 급기가압 제연시스템에 미치는 영향에 대한 현장실험)

  • In order to design and operate successfully the pressure differential system for smoke control which uses difference of pressure between compartments of building, architectural factors affecting the pressure field of building should be examined and the stack effect is one of the important factors. The field experiments on pressure field in two buildings of 21 stories and 31 stories in summer and winter season with regard to on/off condition of the pressure differential system are carried out to evaluate the influence of stack effect to evacuation and smoke management of high-rise building. In winter season when the stack effect increases, as the pressure differential system starts to operate, the pressure in upper stair rises largely due to the combination effect of the air infiltration from lobby to stair and the stack effect.

  • Today the issue of deterioration of industrial complexes that are located close to life space of residents has been raised as a cause of threats to the safety of local communities. In this study, in order to improve the current risk analysis and scope of community notification, simulated threat zones were comparatively analyzed by utilizing the threat zones of alternative accident scenarios and modes of seasonal weather, and the area with a high probability of damage upon the leakage of toxic substances was predicted by examining wind directions observed at each time slot for each season. In addition, limit evacuation time and minimum separation distance to minimize casualties were suggested, and a proposal to enable more reasonable safety measures for on-site workers and nearby residents made by reviewing the risk management plan currently utilized for emergency response.

  • In this study, a complex simulation prototype was developed for rapid and accurate prediction of chemical dispersion range in order to reduce human casualties caused by chemical accidents. Complex simulation considered the leakage momentum during the near-field dispersion to take into account the leakage characteristics of the chemical. In the far-distance dispersion process, the wind distribution of the existing model, which was presented uniformly, was improved using weather and topographical information around the accident site, to realize a wind field similar to the actual one. Finally, the damage range was more precise than the existing model in line with the improved near- and far-distance dispersion process. Based on the results of damage range prediction of the complex simulation, it is expected that it will be highly utilized as a system to support policy decision-making such as evacuation and return of residents after a chemical accident.

Characteristics of Tsunamis and Mitigation Planning (지진해일의 특성 및 방재대책)

  • Recently, many tsunamis triggered by impulsive undersea ground motions occurred in subduction zones around the Pacific Ocean area including the East Sea surrounded by Korea, Japan and Russia. The wave height of a tsunami may be in the order of several meters, while the wavelength can be up to 1,000 km in the ocean, where the average water depth is about 4 km. A tsunami could cause a severe coastal flooding and property damage not only at neighboring countries but also at distant countries. A fundamental and economic way to mitigate unusual tsunami attacks is to construct tsunami hazard maps along coastal areas vulnerable to tsunami flooding. These maps should be developed based on the historical tsunami events and projected scenarios. The map could be used to make evacuation plans in the event of a real tsunami assault.

  • The number of motor vehicle registrations in Korea is increasing steadily each year, driven by industry development and economic growth. The number of traffic accidents is also rapidly increasing. Korea has a relatively high number of traffic accidents among OECD member countries, and it ranks among the highest in traffic accident death rates. This death rate is higher compared to death rates as a proportion of the number of traffic accidents in each country. It is very common for drivers to lose consciousness in traffic collisions, which leads to a failure to carry out early emergency measures. In order to prevent such situations as well as hit-and-runs and people left uncared for after traffic accidents, there is a need for motor vehicle black boxes and accident report systems. This study addressed the need for an emergency evacuation system for people injured in traffic accidents and a secondary traffic accident prevention system by developing a motor vehicle emergency situation detection and report system combined with a black box, and materializing it as an actual system.

A Study of Smoke Movement in a Short Tunnel (짧은 터널 내의 연기거동에 관한 연구)

  • This paper concerns smoke propagation in tunnel fires with various size of fire source. Experiments carried out in model tunnel and those results were compared with numerical results. The Froude scaling law was used to scale model tests for comparison with larger scale tests. In order to validate for numerical analysis, temperature distribution of predicted data was compared with measured data. Examining the temperature distribution, we found that smoke layer does not come down under 50% of tunnel heights for a short tunnel heights for a short tunnel firs without ventilation. Front velocity of smoke layer is proportional to the cube root of heat release rate. And it is in good agreement with existing empirical expression and numerical prediction. In a short tunnel fire, horizontal propagation of smoke layer is more important than vertical smoke movement for evacuation plan.

Field survey of 1983 central East Sea Tsunami : Imwon Port (1983년 동해 중부 지진해일 현장조사 : 임원항)

  • This study has been focus to certify the run-up heights, losses of human lives and property damages due to the 1983 Central East Sea tsunami. We have conducted the interview with indigenous inhabitant and field surveying at the Imwon port, East sea in Korea in order to inquire into the state of things occurred during that period. It is also investigated how much well they are aware of the emergency action plan including the evacuation system. Base on the reliable interviews, we selected and surveyed 10 places at the Imwon port, where the historical maximum overflowing occurred due to the 1983 Central East Sea tsunami. The measured run-up heights are approximately $3.3m{\sim}4.0m$ at the selected 10 places and it is found that the sea water ran over the banks in Imwon stream about 700m upstream from the Imwon port. From this study we can suggest supplementing the present emergency action plan and supply the state-of-the-art inundation map.

A Nuclear Medicine Study on the Effect of High Energy Gamma Irradiation on Rabbit Liver (고(高)에너지 방사선(放射線) 조사(照射)가 가토(家兎) 간장(肝臟)에 미치는 영향(影響)에 관(關)한 핵의학적(核醫學的) 연구(硏究))

  • In order to investigate the in vivo effect of $^{60}Co$ radiation on rabbit liver, the uptake ratio and regional excretory value in hepatocytes and Kupffer cells were estimated during acute and chronic hepatic injuries. The left lobe of liver was irradiated at 15 Gy or 30 Gy with a single dose and subsequent changes were analysed with a seial nuclear medicine imaging by using $^{99m}Tc-phytate,\;^{99m}Tc-DISIDA\;and\;^{99m}Tc-HSA$ and resulting data were computerized. The degree of hepatic damage, duration of the injury, and recovery pattern after the irradation were in agreement with the findings of other investigations. However, out values were more quantitative evacuation than those of other publications. Recovery of decreased uptake of $^{99m}Tc-phytate$ was delayed approximately $2{\sim}3$ days later than that of $^{99m}Tc-DISIDA$. In acute radiation induced injury, the results demonstrated that the recovery of Kupffer cells was delayed more than that of hepatocytes. This discrepancy was considered due to the differences in repair activities between these cell types. The decreased of regional excretory value in irradiated area was found to be dose-dependent but had no corelation with regional uptakes of DISIDA and phytate. The decreased of regional excretory value observed in non-irradiated region suggested that irradiated liver might induce an indirect effect.
