Literatural Consideration on the Classification of cause and Treatment of Tinnitus (耳鳴의 原因別 分類 및 治法에 關한 文獻的 考察)

  • I have been studied the tinnitus. The results are summarized as follows. 1. The etiologies of tinnitus is classified the Zang fa endogenous factors of the Jong-Maek-Hae and exogenous factors of the Oun-Gi, in the Nei Ching. 2. In the endogenous factors the etiologies of the Zang fa is mostly hased deficiency of the kidney, which is concerned with Sim-Hae Gan-Darn-Hae and Bi-Wae-Hae, the etiologies of the phlegm fire is fire is divided into Sin-Hae, Om-Ju-Hu-Mi and No-Gi-Oaek-Sang. 3. The etiologies of the Jong-Maek-Hae is divided into deficiency of the stomach xu of both gi am blood and xu of the kidney. 4. In Nei ching,the etiologies of Oun-Gi divided into Gul-Eum-Pung-Mok and So-Yang-Sang-Hwa of the exgeous factors is regarded to wind and fire as following generations is regrded to wind the endogenous factors caused Sin-Hae Gi-Hae. 5. In the Nei ching, Since the O-Mi-Bo-Sa-Bub is uttered main treated of tinnitus is friquently used Bo-Sin Young-Sim-Sun-Gi and Choung-Gan-Sul- You1 as Zang-Fu Choung-Dam-Gang-Hwa as the Phlegm fire Bo-bi-Sin as the Jong Maek Hae and Gye-Pung-San-Hwa as the Oun-Gi.

Etiologic Classification of Recurrent Spontaneous Abortion (습관성 유산의 원인적 분류)

  • Etiologic classification was performed in 155 patients with recurrent spontaneous abortion history. The incidence was 9.3% among 1658 pregnant women at Hanyang university hospital during the same period. In etiologic classification, 37 cases of on-going patients for diagnostic evaluation were excluded. Of the remained 118 patients, the patients with unknown etiology were 32 cases (27.1%), and 86 cases were classified into each etiologies using appropriate diagnostic modalities. Anatomic causes were the largest etiology, which revealed 46.6% (55 among 118 patients). The next etiology was immunologic cause, which revealed 24.6% (29 patients). Of the 86 patients who have at least one cause, 40.7% (35 among 86) have two or more etiologies. Furthermore, six cases (7%) have 3 combined etiologies. The uterine synechia and/or incompetent internal os of the cervix, namaly anatomic causes, revealed highest combination ratio than other etilogies. The popular abortion technique in Korea, D&C, seems to be main factors for these anatmic causes. The combined causes rather than single cause of recurrent spontaneous abortion were unique situation in Korea especially with anatomic etiologies. From this point of view, we think anatomic etiologies should be rule out first in the evaluation of the patients. For this purpose, we believe hysteroscopy would be helpful as a diagnostic work-up and treatment modality in the management of recurrent spontaneous abortion patients in Korea.

  • Rectal bleeding is a common and worrisome problem in pediatric practice. The condition is usually alarming to patient and children. Although most cases are benign and self-limited, sometimes serious pathology also present in this manner. And severe rectal bleeding necessitates prompt diagnostic notice and appropriate treatment. The etiologies of rectal bleeding are somewhat specific to certain age group in children. Accurate history and through physical examination are paramount importance to help differentiate of the diverse etiologies causing rectal bleeding in children. This review describes patterns of rectal bleeding, historical and physical aspect, differentiation of clinical characteristics of rectal bleeding according to etiology, and diagnostic investigations.

  • Recent genetic advances allow for identification of the genetic etiologies of epilepsy within individual patients earlier and more frequently than ever. Specific targeted treatments have emerged from improvements in understanding of the underlying epileptogenic pathophysiology. These targeted treatment strategies include modifications of ion channels or other cellular receptors and their function, mechanistic target of rapamycin signaling pathways, and substitutive therapies in hereditary metabolic epilepsies. In this review, we explore targeted treatments based on underlying pathophysiologic mechanisms in specific genetic epilepsies.

Clinical Experience of Acute Pericarditis with Effusion (삼출액을 동반한 급성심낭염의 임상적 고찰)

  • Clinical experience of 48 acute pericarditis with effusion was reviewed and presented. There were 28 male and 20 female patients ranging from 3 years to 77 years old. Malignant effusion; Twenty patients had underlying malignancy. These etiologies were lung ca[8 patients, 40%], breast ca[7 patients 35%], lymphoma[2 patients, 10%], esophageal ca[1 patients, 5%], stomach ca[1 patient, 5%], ovarian ca[1 patient, 5%]. Uremic effusion; 15 patients with renal failure required surgical intervention. Traumatic effusion; 7 patients had traumatic pericarditis. These etiologies were stab wound [5 patients, 71.4%] and aspiration[2 patients, 28.6%]. Pyogenic effusion: 6 patients had pyogenic pericarditis. These etiologies were empyema thoracis[3 patients, 50%], liver abscess[2 patients, 33.3%], pneumonia[1 patient, 16.7%]. The patients were treated by pericardiocentesis, subxiphoid tube drainage, pericardiectomy: 4 of them underwent pericardiocentesis; 37, subxiphoid tube drainage; 5, pericardiectomy. We conclude that subxiphoid tube pericardial drainage was effective for treatment of pericardial effusion.

  • Based on the research of cataract, glaucoma, bluish glaucoma, we have abtained the following results. 1. Main etiologies of the cataract are deficiency of the liver and kidneys, main symptoms are no nebula, no pain, no tear and sometimes light blue and white vision appear. 2. Main etiologies of glaucoma are receives heat from the liver, stagnation of the liver and gall bladder etc, main symptoms are both forehead corner pull each ether, pain from eye ball to nasal septum, headache, extending pain of the eyes, sometimes red and white flower vision appear, etc. 3. main etiologies of the bluish glaucoma are melancholy, wrath, stagnation of the liver, internal damage, flaming up fire of the phlegm, deficiency of the liver, deficiency of blood, etc, main symptoms are numbness, visual field constriction, blindness, reduce of visual capacity, blue flower vision sometimes appear, etc. 4. Treatments are mainly uses nourish Um(陰) to reduce pathogenic fire, relieve stagnancy of Ki(氣) of the liver, nourish Um(陰) of the liver and kidneys, etc, acupuncture points are mostly contended arounded eyes such as BL10)(天柱), GV16(風府), LR3)(太衝), HT5)(通里) and in the case of nauses and vomitting PC6)(內關, ST36)(足三里) are included in addition to the above.

Pediatric Hand Trauma: An Analysis of 3,432 Pediatric Hand Trauma Cases Over 15 Years

  • Purpose: Pediatric hand trauma is common and sometimes causes deformity or disability. The incidence and etiologies of hand trauma in children are different from those in adults. This study analyzed the characteristics of pediatric hand trauma cases and patients over a 15-year period. Methods: We conducted a retrospective medical record review of 3,432 children (2,265 boys, 1,167 girls, under 18 years of age) with hand injuries from January 2005 to December 2019. We evaluated the sex distribution and injury etiologies. Injuries were classified by type as burns, amputations, crushing injuries, lacerations, extensor and flexor tendon injuries, open and closed fractures, and nerve injuries. Results: Among the pediatric hand injury patients, males were predominant (1.94:1). Simple lacerations (58.4%) were the most common injury type, followed by fractures (22.8%). Lacerations and burns tended to be common in younger age groups, while tendon injuries, nerve injuries, and crushing injuries were more frequently encountered in older age groups. Conclusions: Hand trauma prevention strategies should be established considering the frequent trauma etiologies in specific age groups. An awareness of age-specific characteristics of pediatric hand trauma patients will be helpful to prevent hand trauma.

  • Pleural effusion is not a rare disease in Korea. The diagnosis of pleural effusion is very difficult, even though the patients often complain of typical symptoms indicating of pleural diseases. Pleural effusion is characterized by the pleural cavity filled with transudative or exudative pleural fluids, and it is developed by various etiologies. The presence of pleural effusion can be confirmed by radiological studies including simple chest radiography, ultrasonography, or computed tomography. Identifying the causes of pleural effusions by pleural fluid analysis is essential for proper treatments. This review article provides information on the diagnostic approaches of pleural effusions and further suggested ways to confirm their various etiologies, by using the most recent journals for references.

  • Facial palsy has a broad clinical presentation and the effects on psychosocial interaction and facial functions can be devastating. Pediatric facial palsy, in particular, introduces unique familial and technical considerations as anatomy, future growth potential, and patient participation influence treatment planning. Though some etiologies of pediatric facial palsy are self-limiting, congenital and long-standing facial palsies pose difficult challenges that require a combination of surgical, adjunctive, and rehabilitative techniques to achieve facial reanimation. Given the spectrum of ages and symptom severity, as well as the various surgical options available for facial palsy, a tailored approach needs to be developed for each child to restore facial balance and function. Here, we review the etiologies, workup, and treatment of pediatric facial palsy and present our novel algorithmic approach to treatment.
