본태성 진전 환자의 검지에서의 자세성 진전에 대한 감각자극 효과 (The Effect of Sensory Stimulation on Postural Tremor at Index Finger of Patients with Essential Tremor (ET))

  • The essential tremor is an involuntary oscillatory movement of body parts. Conventional treatments of essential tremor have little effects in some patients and also leads to significant side effects. Alternative to these treatments, sensory stimulation may have beneficial effects on the essential tremor. The purpose of this study was to analyze an effect of sensory stimulation on essential tremor. Ten patients with essential tremor ($67.4{\pm}8.82$ yrs, 5 men and 5 women) participated in this study. Three-axis gyro sensors were attached on index finger, hand and forearm of patients. Task of 'arms outstretched forward' was performed with and without sensory stimulation. Vectorsum of three dimensional angular velocities (pitch, roll, yaw) was calculated. Outcome measures included root-meansquare (RMS) mean of the vector-sum amplitude, total power, peak power and peak frequency. RMS amplitude, total power and peak power were reduced by sensory stimulation (p < 0.05). Peak frequency was not affected by sensory stimulation. The results indicate that the sensory stimulation is useful to suppress the essential tremor.

화병에 의해 악화된 본태성 진전증 환자 치험 1례 (A Case Report of a Essential Tremor Patient Aggravated by Hwa-byung)

  • 본 증례에서는 상기한 바와 같이 화병을 주로 한 심리상태 변화에 의해 악화된 진전증을 주소로 본원 한방신경정신과에 입원한 환자에게 한약치료와 침치료, 한방정신치료를 병행한 결과 제반증상 및 심리 상태 상 양호한 효과를 보여 이에 보고하는 바이다. 1. 18일간의 한약치료, 침치료, 정신요법 및 부항, 뜸의 기타치료를 통해 전신의 본태성진전증과 화병의 증세의 동반 호전을 경험하였다. 2. 화병 및 신체 상태를 심신불교(心腎不交)로 변증하고 황련아교탕(黃連阿膠湯) 처방을 기본 방제로서 증세변화에 따라 가감투여했으며, 정서적 안정과 기타 신체증상과 함께 진전증이 호전됨을 확인하였다.

억간산으로 호전 보인 본태성 떨림 환자 1례 (A Case Report of Essential Tremors Treated with Yeokhan-san)

  • Objective: This report describes the case of a patient with essential tremors of the mouth and hands, which were improved by Yeokhan-san treatment. Methods: To relieve the symptoms, Korean medicine treatments, including Yeokhan-san, Ohaeng-Hwa acupuncture, and electronic moxibustion, were performed. To evaluate the effects of the treatments, the visual analog scale and Fahn-Tolosa-Marin clinical rating scale for tremor were used. Results: After the treatment, the patient's clinical symptoms were improved. The visual analog scale score decreased from 10 to 2 for the hand tremor and from 10 to 1 for the mouth tremor. The Fahn-Tolosa-Marin tremor scores decreased from 3 to 1 for both the hand and mouth tremors. Conclusions: The present case suggests that Yeokhan-san and Ohaeng-Hwa acupuncture are effective for the treatment of essential tremor.

길입성 진전의 병태생리: 다증 증례 연구 (Pathophysiology of orthostatic tremor: a multiple case study)

  • Introduction : Orthostatic tremor develops in the legs while standing up with no weakness, pain or imbalance in the leg and the tremor is characteristically not observed when walking. However there have been some confusions about orthostatic tremor in several aspects. For the past ten years, we have observed 4 patients with orthostatic tremor. In each case tests were performed to investigate the following three important areas of inquiry about orthostatic tremor. Firstly, whether this disorder is an independent diagnostic entity or a variant of essential tremor. Secondly, whether the progress of this disorder is specifically related with standing posture. Lastly, the nature of the pathophysiologic mechanism behind the appearance of the tremor when standing after the lapse of a certain latent period and its disappearance upon the commencement of walking. Methods : Our 4 cases of orthostatic tremor were studied clinically, electrophysiologically, and pharmacologically. Electrophysiological tests included tremor spectrum test and electromyography. Results : We observed the presence of this tremor in several other tonic postures, as well as its absence, in a vertically lifted position from all our cases. Our cases registered a variable tremor frequency between 5 and 12 Hz according to the tremor spectrum test and EMG. Furthermore all our 4 cases demonstrated patterns of both synchronous EMG activity and alternating EMG activity at various times in homologous muscles of both legs. Orthostatic tremor was improved significantly with propranolol as well as clonazepam. Conclusions : From the results of our study we drew the following conclusions. It is probable that orthostatic tremor is simply a variant of essential tremor rather than being an independent diagnostic entity and that in most cases its development is specifically related with muscle contraction rather than merely with the act of standing. Furthermore we discovered a clue in the previously described neural control mechanism that the nuclear bag fibers in the muscle spindle have lag time of several seconds in their response to muscle strength and that their baseline does not reset fully in rapidly moving muscle. This neural control mechanism could offer sufficient explanation for the phenomena of tremor appearance when standing and disappearance when walking in orthostatic tremor.

전자궁적출술 후 발생한 본태성 떨림에 대한 형방지황탕 가감 투여: 증례보고 (Effect of Modified Hyeongbangjiwhang-tang for essential tremor after total vaginal hysterectomy : Case report)

  • Objectives This case reports the effect of Modified Hyeongbangjiwhang-tang for essential tremor after total vaginal hysterectomy. Methods The patient was treated with Modified Hyeongbangjiwhang-tang three times a day. To differentiate other diseases, we took her medical history. And, we used the Verbal Numerical Rating Scale (VNRS) and Gastrointestinal Symptom Rating Scale (GSRS) to assess tremor and gastrointestinal symptoms. Results Treatment with modified Hyeongbangjiwhang-tang resulted in a decrease in the patient's complaints of symptoms. Also, VNRS and GSRS scale were improved. Adverse effects were not observed. Conclusions Modified Hyeongbangjiwhang-tang may be effective for essential tremor after total vaginal hysterectomy.

복합 한의 치료를 시행한 교통사고 후 악화된 본태성떨림플러스 환자: 증례 보고 (Complex Korean Medicine Treatment for Essential Tremor Plus Patient Aggravated by Traffic Accident: A Case Report)

  • Objectives: The purpose of this study was to report the effect of complex Korean medicine treatments on Essential Tremor Plus (ET plus) patient aggravated by a traffic accident. Methods: We treated an ET plus patient with complex Korean medicine. The patient's resting tremor and kinetic tremor in both hands intensified after experiencing a traffic accident, with tremor in the left hand being worse than that in the right hand. Effect of complex Korean medicine treatment was evaluated using Fahn-Tolosa-Marin Tremor Rating Scale (FTMTRS), Numerical Rating Scale (NRS), and Beck Anxiety Inventory (BAI). Results: After 26 weeks of treatments (acupuncture, pharmacupuncture, cupping, Iigyeungbyunqi-therapy, and herbal medicine), the FTMTRS decreased from 38 to 15. NRS of Lt. upper limb pain decreased from NRS 9 to 0. BAI score also decreased from 31 to 17. Conclusions: Complex Korean medicine could be used to treat patients with ET plus aggravated by a traffic accident.

본태성 수전증으로 의심되는 환자에 대한 추나요법 병행치료 증례보고 (A Case Report of Suspected Essential Tremor Treated by Chuna Manual Therapy)

  • Objectives : The purpose of the this study is to report the improvement after Chuna manual therapy about patient with essential tremor. Methods : The patient of essential tremor was treated by Myofascial Release Technique(MRT) and Chuna manual therapy with acupuncture, cupping, Transcutaneous electrical nerve stimulation(TENS), herbal medicine therapy. Results : According to evaluation of Handwriting test and VAS, The patient has shown improvement aftert Chuna Manual Therapy. Conclusions : This study suggest that Chuna manual therapy with other treatment would be effective for patient of essential tremor.

본태성 진전에서 시각의 영향 (Visual Cues in Essential Tremor)

  • Purpose : The pathophysiology of essential tremor(ET) remains unknown. PET studies of ET showed some conflicting data. One study reported significant glucose hypermetabolism of the medulla and thalami, but other studies reported abnormal bilateral overactivity of cerebellar and red nuclear connections. The previous experimental studies suggested that each PET finding reflects a part of neural circuit which is responsible for ET. So it can be imagined that olivocerebellar oscillation may be transmitted by the way of cerebellar projections to the thalamus in ET. It has been reported that the cerebellar dentate nucleus neurons are involved in the generation and/or guidance of movement based on visual cues. The purpose of this study is to clarify the role of dentato-thalamic tract in ET. Methods : Tremor amplitudes were recorded as each patient perform two kinds of task, one involving sensory-guided movement and the other involving memory-guided movement. Each patient was asked to move his/her index finger following a smoothly moving target. He/She also was asked to perform the same movements with his/her eyes closed ET. Results : The results showed that average amplitudes of tremor were significantly higer during visually guided task than during memory guided task in ET patients. Conclusions : Our results led us to conclude that dentato-thalamic tract might be related to the control of tremor in ET.
