Regulatory Network of MicroRNAs, Host Genes, Target Genes and Transcription Factors in Human Esophageal Squamous Cell Carcinoma

  • Abnormally expressed microRNAs (miRNAs) and genes have been found to play key roles in esophageal squamous cell carcinoma (ESCC), but little is known about the underlying mechanisms. The aim of this paper was to assess inter-relationships and the regulatory mechanisms of ESCC through a network-based approach. We built three regulatory networks: an abnormally expressed network, a related network and a global network. Unlike previous examples, containing information only on genes or miRNAs, the prime focus was on relationships. It is worth noting that abnormally expressed network emerged as a fault map of ESCC. Theoretically, ESCC might be treated and prevented by correcting the included errors. In addition, the predicted transcription factors (TFs) obtained by the P-match method also warrant further study. Our results may further guide gene therapy researchers in the study of ESCC.

Prognostic Significance of Beta-Catenin Expression in Patients with Esophageal Carcinoma: a Meta-analysis

  • Many studies have reported ${\beta}$-catenin involvement in the development of esophageal carcinoma (EC), but its prognostic significance for EC patients remains controversial. Therefore, we conducted this meta-analysis to explore the issue in detail. After searching PubMed, EMBASE, Web of Science, and Chinese Biomedical Literature Database, we included a total of ten relevant studies. We pooled the overall survival (OS) data using RevMan 5.2 software. The results showed that aberrant expression of ${\beta}$-catenin was associated with a significant increase of mortality risk (hazard ratio 1.71, 95%CI 1.46-2.01; p<0.00001). Subgroup analyses further suggested that aberrant expression of ${\beta}$-catenin resulted in poor OS of EC patients regardless of histological type of EC, study location or criteria for aberrant expression of ${\beta}$-catenin, and the sensitivity analyses revealed that the result was robust. The meta-analysis revealed that aberrant expression of ${\beta}$-catenin could be a predicative factor of poor prognosis for EC patients.

Equivocal Association of RAD51 Polymorphisms with Risk of Esophageal Squamous Cell Carcinoma in a Chinese Population

  • Aim: To study the contribution of genetic variation in RAD51 to risk of esophageal squamous cell carcinoma (ESCC). Methods: Three single nucleotide polymorphisms (SNPs) in RAD51 (rs1801320, rs4144242 and rs4417527) were genotyped in 316 ESCC patients and 316 healthy controls in Anyang area of China using PCR-RFLP (polymerase chain reaction-restriction fragment length polymorphism). Demographic variables between cases and controls were statistically compared by T test and Chi-square test. Hardy-Weinberg equilibrium was evaluated by the Chi-square test. Odds ratios (ORs) and 95% confidence intervals (CIs) were calculated to measure any association with ESCC. Haplotype frequencies were estimated by Phase 2.1. Result: The genotype frequencies of rs1801320, rs4144242 and rs4417527 in patients with ESCC demonstrated no significant differences from those in control group (P>0.05). When the haplotypes of these three SNPs were constructed and their relationships with ESCC risk investigated, however, CGG was observed to increase the risk (P=0.020, OR=2.289). Conclusions: There was no association between the three SNPs of RAD51 and ESCC susceptibility in our Chinese population. However, the CGG haplotype might be a risk factor.

소아 흉부질환에 대한 임상적 고찰 [비심장혈관계 질환] (Clinical Analysis of Pediatric Thoracic Surgery: Non-Cardiovascular Disease)

  • The results of reconstruction of the lower esophagus with jejunum in a total of 24 cases of primary carcinoma of the lower third esophagus and gastroesophageal carcinoma were presented, and clinical values of substitution for the esophagus with jejunum were also discussed. They were operated in the department of thoracic and cardiovascular surgery, Hanyang University Hospital during the period of 9 years from 1972 to 1981. Surgical managements to lower esophageal reconstruction with jejunum were carded out with not the same procedure in all cases studied, but with three different procedure mentioned below/ In 13 cases of lower third esophagectomy with or without partial `8astrectomy of a total of 24 cases, interposition of jejunum between the esophagus and the stomach were performed after the fashion to esophagojejunostomy with mobilized jejunal loops and 8astro-JeJunostomy with side to side anastomosis. In 7 cases of lower third esophagectomy and total gastrectomy, the continuity of the esophagus were performed the fashion to esophagojejunostomy with mobilized jejunum. In 4 cases of unresectable gastro-esophageal carcinoma, bypass operation of the lower esophagus and the stomach were performed after the fashion to esophagojejunostomy with side to and anastomosis. After the bypass operation, it was observed that oral feeding to the patients was excellent. Following these consecutive series of 20 cases of radical operation for lower esophageal carcinomas and 4 cases of bypass operation for unresectable gastroesophageal carcinomas, no complication such as postoperative leakage and stenosis from anastomotic site or Infection In operating area and operative death were observed.

식도 편평세포암에시 Ivor-Lewis수술 성적 (Results of a Ivor-Lewis Operation for Esophageal Squamous Cell Carcinoma)

  • 배경: 흉부 식도암에서 Ivor-Lewis수술이 널리 이용되어 왔으나 최근 3구역 절제술로 보다 좋은 성적들이 보고되고 있다. 식도 편평세포암 환자에서 Ivor-Lewis수술의 유용성을 알아보고자 한다. 대상 및 방법: 1994년 9월부터 2004년 8월까지 식도암으로 절제술을 받은 273명의 환자 중 편평세포암으로 다른 원발성 암이 없고 Ivor-Lewis수술로 완전절제가 가능하였던 172명을 대상으로 술 후 합병증, 조기 및 장기성적, 재발양상을 후향적인 방법으로 분석하였다. 결과: 술 후 병기는 I기 40명(23%), IIA기 48명(27%), IIB기 18명 (10%), III기 55명(33%), IVA 5명(3%), IVB 6명(4%)이었다. 수술과 관련된 사망은 172명의 환자 중 7명(4%)에서 발생하였으며 술 후 합병증은 32명(18%)에서 발생하였고 55명(32%)에서 재발하였다. 전체 환자의 5년 생존율은 48%였는데, 병기별 5년 생존율은 I기 85.6%, IIA기 47.6%, IIB기 65%, III기 22.8%, IV기 0%였다. 종양의 위치에 따른 5년 생존율은 상흉부가 26.5%로 중흉부와 하흉부 52.4%에 비해 낮았으나 통계적 유의성은 없었다. 결론: 이상의 결과로 식도 편평세포암 환자에서 Ivor-Lewis수술의 적용은 비교적 만족할 만한 결과를 얻었으며, 상흉부 식도암인 경우에는 통계적 유의성은 없었으나 중 하 흉부식도암에 비해 낮은 5년 생존율을 보여 3구역 림프절절제술로의 전환도 고려해야 할 것으로 생각된다.

식도암의 임상적 고찰 [III] (Cancer of the Esophagus and Cardia [III]: A Clinical Review of 55 cases)

  • Treatment of patients with esophageal carcinoma remains disappointing. Fewer than 20% *of patients with esophageal carcinoma treated with curative intent by surgery and radiation therapy will be alive at 5 years. Fifty five patients underwent treatment for carcinoma of the esophagus and cardia which retrospective clinical study done between January 1983, and December, 1989 in Department of Thoracic and Cardiovascular Surgery, National Medical Center, Seoul, Korea. There were 48 men. 7 women and the mean age was 56.2 years[range, 38 to 78 years]. The most frequent preoperative findings included dysphagia[46 cases], weight loss[33 cases] and substernal pain[19 cases]. 19 cases[34.5%] were underwent resectional surgery, which was total esophagectomy with esophagogastrostomy[ 5 cases], with colon interposition[3 cases], lower esophagectomy with esophagogastrostomy [10 cases], with total gastrectomy and esophagojejunostomy [1 case]. The operative mortality was 15.8%[3/19] and causes of death were respiratory failure in 2 cases, and 1 case of sepsis. Palliative procedures, such as feeding gastrostomy and jejunostomy[14 cases], bypass surgery without resection[ 4 cases], radiation therapy[ 3 cases] were done.

부식성 식도협착 환자의 비개흉적 식도 적출술 - 2례 보고 - (Esophagectomy without thoracotomy in corrosive esophageal stricture - case report -)

  • The strictured esophagus is not removed usually in corrosive injury. But corrosion carcinoma and the late complications such as esophago-bronchial fistula, chronic mediastinal abscess, diverticulum can occur in case that strictured & inflamed esophagus be remained. Recently transhiatal esophagectomy is being done with low mortality and morbidity. So we are reporting 2 cases of esophageal stricture in young patients which were treated with transhiatal esophagectomy and esophagocologastrostomy. All of 2 cases were successfully treated and recovered. Postoperative esophageal function tests showed the mild reflux but it did not count clinically.

식도의 기저세포양 펀평세포암 -1례 보고- (Basaloid Squamous Carcinoma of the Esophagus - Report of a Case -)

  • 기저세포양 편평세포암(basaloid squamous carcinoma)은 상부 기도소화기계에 발생하는 드문, 악성도가 높은 암종으로 침습성 편평세포암(invasive squamous carcinoma)이나 상피내암(carcinoma in situ) 형태의 편평상피암종성분과 밀접하게 혼재되어 있는 기저세포앞종성분(basaloid component)으로 특징 지워질 수 있다. 이런 조직학적 특성에도 불구하고 식도의 선양 낭성암(adenoid cystic carcinoma)이나 선양 낭성 분화를 보이는 암종(carcinoma with adenoid cystic differentiation)과 혼동되어 왔으나, 순수 선양 낭성암은 악성도가 기저세포양 편평세포암보다 낮아감별 진단이 중요하다. 기저세포양편평세포암의 임상적 경과는 식도의 편평세포암과 유사하다. 본 병원에서는 60세 남자의 식도 중간 113 부위에 발생한 기저세포양 편굉세포암을 수술치험하였다.

내시경 점막 절제술 (Endoscopic Mucosal Resection)

  • Diagnosis of early esophageal cancer has become more frequent as a result of improved endoscopic technology, surveillance programmes, and increasing experience and awareness on the part of endoscopists. In early esophageal cancer, squamous cell carcinoma and early adenocarcinoma must be managed differently because they have different origins, pathogenesis. and clinical characteristics. The current treatment options vary widely, from extended resection with lymphadenectomy to endoscopic mucosal resection (EMR) or ablation. None of these treatment options can be recommended universally. Instead, an individualized strategy should be based on the depth of tumor infiltration into the mucosa or submucosa, the presence or absence of lymph node metastases, the multicentricity of tumor growth, the length of the segment of intestinal metaplasia, and comorbidities of the patient. EMR has become increasingly important, both as a diagnostic tool for the staging of esophageal carcinomas and as a method of carrying out definitive treatment when the cancer meets certain criteria in which the risk of lymph-node metastasis is negligible. EMR may be sufficient in a subset of patients who have m1 or m2 squamous cell carcinoma and in patients who have isolated foci of high-grade intraepithelial neoplasia or mucosal cancer.

Radiosensitivity Enhancement by Arsenic Trioxide in Conjunction with Hyperthermia in the EC-1 Esophageal Carcinoma Cell Line

  • Objective: To explore the effect on radiosensitivity of arsenic trioxide ($As_20_3$) in conjunction with hyperthermia on the esophageal carcinoma EC-1 cell line. Method: Inhibition of EC-1 cell proliferation at different concentrations of $As_20_3$ was assessed using the methyl thiazolyl blue colorimetric method (MTT method), with calculation of $IC_{50}$ value and choice of 20% of the $IC_{50}$ as the experimental drug concentration. Blank control, $As_20_3$, hyperthermia, radiotherapy group, $As_20_3$ + hyperthermia, $As_20_3$ + radiotherapy, hyperthermia + radiotherapy and $As_20_3$ + hyperthermia + radiotherapy groups were established, and the cell survival fraction (SF) was calculated from flat panel colony forming analysis, and fitted by the 'multitarget click mathematical model'. Flow cytometry (FCM) was used to detect changes in cell apoptosis and the cell cycle. Results: $As_20_3$ exerted inhibitory effects on proliferation of esophageal carcinoma EC-1 cells, with an $IC_{50}$ of 18.7 ${\mu}mol/L$. After joint therapy of $As_20_3$ + hyperthermia + radiotherapy, the results of FCM showed that cells could be arrested in the $G_2$/M phase, and as the ratio of cells in $G_0/G_1$ and S phases decreased, cell death became more pronounced. Conclusion: $As_20_3$ and hyperthermia exert radiosensitivity effects on esophageal carcinoma EC-1 cells, with synergy in combination. Mechanistically, $As_20_3$ and hyperthermia mainly influence the cell cycle distribution of EC-1 esophageal carcinoma cells, decreasing the repair of sublethal damage and inducing apoptosis, thereby enhancing the killing effects of radioactive rays.
