Purpose: The purpose of this study is to investigate the design optimization modeling of screening procedures based on the assessment of misclassification errors. Methods: Misclassification errors due to measurement variability are derived for normally distributed quality characteristics. Further, an optimization model for ensuring the level of outgoing quality is proposed and demonstrated through an illustrative example. Results: It is shown that two types of misclassification errors (i.e., false acceptance and false rejection) may be properly compromised through an analytical assessment of measurement errors and an optimization modeling. It is also discussed that a variety of optimization modeling may be enabled based on the derivation of measurement errors. Conclusion: It may be concluded that the design of screening inspection may further be facilitated by including the effect of measurement errors on the performance of screening inspection procedure.
The flight deck crew must manage complexity during daily flight operations. The Airline may obtain data regarding threats and errors through LOSA(Line Operations Safety Audits) on normal flights as predictive safety tool in Safety Management System of the Airline to actively improve the systems such as SOP(Standard Operation Procedure), training, evaluation and the TEM(Threat and Error Management) for the flight deck crew. The flight deck crew make errors when they fail managing threats. The crew mismanage around ten percent of threats and commit errors. The major mismanaged threats are aircraft malfunction, ATC(Air Traffic Communication), and wether threats. The effective countermeasures of TEM for manageing threats are leadership, workload management, monitor & cross check, Vigilance, communication environment and cooperation of the crew. It is important that organizations must monitor for the hazards of threats and improve system for the safer TEM environments.
LOSA is a flight safety program that analyses human errors in normal operations. Trained pilot observers monitor the normal flights at the observer seat. LOSA is a proactive non jeopardy data collection tool using threat and error management(TEM) as a framework. With the analysis of crew behaviors through LOSA with The LOSA collaborative(TLC), the airlines can identify the behaviors of the crew during normal operations. The major objective of LOSA is to measure how the crew manage threats, errors and undesired aircraft deviations in the cockpit on day to day operations. The airlines are able to set up effective TEM training with practical six generation Crew recourse management(CRM) with data of error from LOSA instead of theoretical CRM courses. The Airlines can use TEM as an integral part of a Safety Management System(SMS) and uses monitoring and cross-checking skills in the flight operations to manage threats and errors effectively when we know the errors we make in the cockpit on daily operation. The result of LOSA indicates that the error detection rate should be enhanced since around the half of the errors went undetected. The areas which should be focused for enhancing the error detection are monitor, cross-check, the management of workload, automation and taxiway/ runway to manage errors effectively.
Owing to the latest changes in the IT environment and the advancement of RFID (Radio Frequency Identification) technology, the RFID technology has been frequently applied to the field of logistics and distribution. Now it is possible to acquire information in real-time more accurately and promptly as compared to data collection in the past, through the application of the RFID technology. However, in terms of the application of the RFID technology, the range of the field of logistics and distribution is considerably widely distributed. The management system that is able to monitor the RFID system installed in logistics centers and stores distributed in environmentally many regions, in real-time in the center is insufficient. Therefore, this study proposes a management system which is capable of transmitting the report of the occurrence of errors according to the pre-defined error types at the time of the occurrence of errors in the RFID system installed at each strategic foothold, in real-time to SMS and to the integrated monitoring system, and of taking actions for those errors from a remote place by using a mobile device. The purpose of the error management system proposed in this study is to minimize a data loss in the supplying network by quickly coping with errors in the area where the RFID system is installed.
There is no universally agreed classification of human error, nor is there one in prospect. Thus, a taxonomy is usually made for a specific purpose. To seek the types of human errors in the environment of man-machine interface under the railway industry, we develop a cognitive information processing model incorporating the human's mental states. Using the model, this study investigates the types of human errors about the railway workers. Thus, a survey is conducted for railway safety personnel-locomotive engineers, station employees, and train commanders- in Korean railway company. Through the survey that is designed to investigate four types of human errors from the Questionnaires composed of thirty Questions, we analyze the types of human errors related to railway safety according to affiliated offices, operation shifts, age, and working years. Finally, from the insights of the results some guidelines for the railway safety management are presented.
Safety management paradigm which against human errors in aviation industry is now changing from the follow-up measures after accident in the past to systematic approach that a forecast the hazards and improve the working system of the group to prevent accidents. As human factors are based on the man's specific psychological traits, it takes much time and efforts to prepare the preventive measures. That's why aviation industry is interested in the accident-prevent measurements against human errors. In this thesis, therefore, we are going to introduce the efforts that aviation organizations have tried and recommend management systems and discuss the suggestive facts. At first, we discussed introduction of HFACS which is the systematic accidents-classification system related to human errors in the aviation organization and countermeasure in the aspects of management, technology/engineering, education training. We described about FOQA, LOSA, CRM/TEM, aviation safety information DB in the aspect of management, and explained safety technologies that prevent human errors or avoid technologically when emergency occurs in the aspect of technology/engineering. In the aspect of education training, we explained the application plan about safety programs(LOFT/Simulator use, CRM/TEM application etc).

On the basis of a simple small-sample low-noise a, pp.oximation, we propose a new method of comparing the mean squared errors(MSE) of the various competing estimators of the true regression parameters ((${\alpha}$ , ${\beta}$). Then we show that a class of estimators including the classical least squares estimator and the maximum likelihood estimator are consistent and first-order efficient within the class of all regular consistent estimators irrespective of type of measurement errors.
A spreadsheet is a office automation software which has been widely used in almost all areas in business. A spreadsheet is used for calculation, data analysis, model building and so on. As a result, spreadsheet is a very important tool for generating knowlege in a firm. Errors in a spreadsheet model can cause a serious financial damage in a corporation. However in Korea few researches for reducing errors have been performed. Neither serious efforts to reduce errors have been executed by corporations. In this research we provided summarized the results from a survey for spreadsheet usage, summarized researches on errors in spreadsheet up to now, and suggested a rules and structured development method for reducing errors in a spreadsheet. Errors in spreadsheets are prevalent. To solve this serious problem, we need much more attention on this problem in research area. Corporations must put more efforts on setting policies and rules. And also we need the government's concern and support.

The flight deck crew can reduce threats and errors which existing within the airlines by using threat and error management when the crew know these threats and errors. Construction errors can have significant effects on project performance. Yet, any attempt to tackle error should be driven by the ability to understand its archetypal nature. This study aims to analyze a variety of construction errors at the execution stage, in order to develop a comprehensive set of categories that shall provide insights about the effect of different error types on the industry. We investigate a database provided by an authoritative source, which includes a number of 256 construction errors that occurred in the year 2009 in Dubai construction industry. Results from this study reveal that the most common error arose from 'poor workmanship' which accounted for 21% of the total encountered faults. The next most common types arose from the 'usage of impaired materials', followed by the 'deviation from an intended dimension'. These observations infer that the majority of construction errors are driven by workers' lack of skill or competence. Moreover, it suggests that execution-oriented errors are the major cause of faults and accidents rather than design errors.
