제 4 새성 기형 1예 (A Case of Fourth Branchial Cleft Cyst)

  • The branchial anomaly is a lateral neck mass commonly seen by otolaryngologists. Depending on its anatomic location, branchial anomaly can be classified into first, second, third and fourth. The fourth branchial cleft anomaly is very rare entity and until now, only 35cases have been reported worldwide. It may present as neck cyst, recurrent neck abscess, thyroiditis. Combined with barium swallow esophagogram and computed tomography scan can aid in diagnosis of this rare disease entity. Complete excision of the entire epithelial tract combined with ipsilateral thyroid lobectomy remains the mainstay of treatment. Authors experienced a case of lateral neck mass which was anatomically presumed to be the fourth branchial cleft cyst. We report this case with the related literature.

  • Purpose: The odontogenic keratocysts demonstrated a high recurrence rate and a biologically aggressive nature. This might be due to unknown factors inherent in the epithelium or enzymatic activity in the fibrous wall. Bcl-2 protein is characterized by its ability to inhibit apoptosis. The aim of this study was to evaluate the expression and distribution of bcl-2 in the OKCs, its possible relationship with the tumorous characteristics, such as the aggressive nature and high recurrence rate, and its usefulness to differentiate OKCs from dentigerous cysts. Materials and Methods: Formalin fixed paraffin-embedded tissue sections of 53 OKCs, and 44 dentigerous cyst were immunohistochemically analyzed quantitatively for the immunoreactivity of the bcl-2 protein with i-solution. Results: More Bcl-2 expression was observed in the OKCs (mean34.387%) than dentigerous cyst (mean11.144%) with statistical significance (P<0.001). Seventeen and 15 of the 32 OKCs in this study showed positivity in the basal layer and basal/suprabasal layers, respectively. In dentigerous cyst, 2 of 3 showed positivity in the basal cell layer. Conclusion: Considering that bcl-2 over expression may lead to the increased survival of epithelial cells, this study demonstrated a possible relationship between the aggressive nature of OKC and the intrinsic growth potential of its lining epithelium. Furthermore, the basal/suprabasal distribution of bcl-2 positive cells was observed in some OKCs, which might have a significant impact on the behavior of cysts. The bcl-2 expression of OKCs can be useful for differentiating OKCs from dentigerous cysts.

식도(食道) 발생한 기관지성(氣管枝性) 낭종치험례(囊腫治驗例) (A Bronchogenic Cyst in the Wall of the Esophagus -Report of A Case-)

  • This is to report a case of bronchgenic cyst. While most of the bronchogenic cysts reported in the literature so far were located either in the lung parechym or in the mediastinum near the tracheal bifurcation or main bronchi. the cyst presenting in this study was originated in the wall of the esophagus and was reported to be very rare. The cystic tumor was found accidentally by X-ray fluoroscopic examination of the esophagus and stomach in the patient with gastric hemorrhage. X-ray study revealed that the cystic tumor was oval in shape and located in the left posterolateral wall of the esophagus in the thoracic lower third. Two surgical operations, gastrectomy for gastric hemorrhage and the resection of the cystic tumor, were carried out separately. Gastrectomy including the removal of prepyloric ulcer by the Billroth II type procedure was performed in regular fashion, and the cystic tumor was resected radically without any injury of the mucous membrane of the esophagus. The cyst removed appeared to be filled with mucinous material, and histological examination identified the tumor as a bronchogenic cyst with ciliated epithelial internal lining. Postperative course of the patient was uneventful.

기관지성 낭종(2례수술보고) (Bronchogenic Cyst (Two Case Report))

  • Bronchogenic cysts, though known as a relatively common malformation of the mediastinum, were rarely discussed in Korean literatures. Since the surgical removal of asymptomatic lesions was adopted as the reasonable therapeutic principle, the incidence of bronchogenic cysts were found to be higher than previously expected. Two cases of bronchogenic cysts operated on in The Dept. of Thoracic Surgery, Korea University Woo Sok Hospital were reported and related literatures were reviewed. Case 1. 5 year old boy with chief complaints of bulging cystic mass on left supraclavicular region during straining or coughing since his age of 2 was admitted, and cystogram with Lipiodol revealed hen-egg sized mass in the left antero-superior mediastinum without any communication with bronchus or esophagus. Cyst was successfully removed under general anesthesia thru left supraclavicular incision, and pathological examination of the cyst revealed thin cystic wall lined with stratified columnar epithelium and it`s content was milky white mucoid fluid devoid of any bacterial growth. Case 2. 15 year old school girl has been noted slowly growing walnut-sized mass on anterolateral side of the neck for 4 months without any subjective symptom except cosmetic problem... Mass was aspirated to find milky white mucoid fluid in some loculation and yellowish turbid fluid in other part due to chronic infection. Cystic tumor was removed under local anesthesia, which was loculated in between the trachea and esophagus without any communication, and pathological diagnosis of the cyst was bronchogenic cyst with columnar epithelial cell lining with moderate chronic inflammatory cell infiltrations. Postoperative conditions of the two cases were all excellent with normal life.

Ganglion Cyst of the Posterior Longitudinal Ligament Causing Lumbar Radiculopathy

  • Degenerated conditions such as herniated disc or spinal stenosis are common etiologies of lumbar radiculopathy. Less common etiologies include spinal extradural cyst such as synovial cysts and ganglion cysts. Ganglion cyst of the posterior longitudinal ligament (PLL) of the spine is a rare entity that can result in classical sciatica. Posterior longitudinal ligament cyst has no continuity with the facet joint and has no epithelial lining. Two young male patients presented with unilateral sciatica and were found to have intraspinal cystic lesions causing lumbar radiculopathy. Magnetic resonance imaging demonstrated rounded, cystic lesions (i.e., hypointense on T1-but hyperintense on T2-weighted images) adjacent to minimally dehydrated, nonherniated disc spaces in both cases. These patients underwent posterior decompression and cysts were excised, and their sciatic symptoms were completely resolved. Histological examination showed typical features of ganglion cysts in these cases.

Factors affecting complications after treatment of epidermal cyst

  • Background: Epidermal cysts are the most common benign epithelial tumors in humans. The curative treatment of epidermal cyst is surgical excision. However, only few studies have investigated the cause and mechanism of postoperative complications of epidermal cysts. Therefore, this study aimed to evaluate the factors affecting complications of epidermal cyst after surgical treatment. Methods: Patients with histologically diagnosed epidermal cysts were selected from among 98 consecutive patients with excised benign cystic tumors from March 2014 to August 2017. Sex, age, size, mobility, site of occurrence, history of infection, history of incision and drainage, complications, history of reoperation, and method of overcoming complications was obtained by analyzing medical records retrospectively. Results: Five of the 98 patients had wound dehiscence due to surgical infection. Three of them underwent wound healing with conservative treatment without a second operation. The other two patients underwent a second operation and showed signs of preoperative infection. None of the factors showed statistical significance in relation to the occurrence of complications. Conclusion: Postoperative complications occurred when the excision of the epidermal cyst was performed at preoperative infection sites or at sites with high tension, so attention should be paid to postoperative care.

  • Fibrous dysplasia is a benign fi bro-osseous lesion wherein normal bone is replaced with an excessive proliferation of cellular fibrous connective tissue intermixed with irregular bony trabeculae. Fibro-osseous lesions like fibrous dysplasia are often associated with non-epithelial cysts, such as simple bone cyst. The etiologic and pathogenic relationships between fi brous dysplasia and simple bone cyst have not been conclusively established. Nonetheless, the mechanism of cyst formation in fibro-osseous lesion associated with simple bone cyst can be said to differ from that of the typical simple bone cyst of the jaws. This article reports a case of bilateral lesions including fi brous dysplasia and simple bone cyst on each site and reviews the pathogenesis of cyst formation in the fibro-osseous lesion.

진성 비장 낭종 3예 (Three Cases of True Splenic Cyst)

  • Splenic cysts are uncommon and classified as either primary(true) or secondary(pseudo-) depending on the presence or absence of a true epithelial lining. True cysts (epidermoid cyst) of the spleen are very rare. Three cases of splenic cysts in childhood were treated at the Yeungnam University Hospital in the last eleven years(1989-1999). Two of patients were girls. The ages at diagnosis were 7, 12 and 15 years. Abdominal ultrasonography and computerized tomography were utilized for the diagnosis. Radionuclide scanning was performed in one patient. Surgical resection(one partial splenectomy and two total splenectomies) was performed. The sizes of cysts were 4, 6.5 and gem in maximum demension.

Tolvaptan: a possible preemptive treatment option in children with autosomal dominant polycystic kidney disease?

  • Tolvaptan is a highly selective vasopressin receptor 2 antagonist that regulates cyclic adenosine monophosphate levels to inhibit both epithelial cell proliferation and chloride ion excretion, two mechanisms known to induce cyst expansion in autosomal dominant polycystic kidney disease (ADPKD). Tolvaptan is currently the preferred treatment of rapidly progressive disease ADPKD in adult patients; however, since cyst formation in ADPKD begins early in life, (frequently in utero), and significant disease progression with cyst expansion occurs in the first decade, tolvaptan may be advantageous as a preemptive treatment in children with ADPKD. Tolvaptan has already been used to successfully treat refractory edema or hyponatremia in children; this literature review provides insight into the biochemical basis of its action to contextualize its use in the pediatric population.

치성낭종 상피세포의 증식, 분화 및 세포능동사망현상에 관한 면역조직화학적 연구 (AN IMMUNOHISTOCHEMICAL STUDY ON PROLIFERATION, DIFFERENTIATION, AND APOPTOSIS IN THE EPITHELIAL CELLS OF THE ODONTOGENIC CYSTS)

  • The epithelium of odontogenic cyst seems to be in a specific status of cellular proliferation and cytodifferentiation. With the identification of various genes, which play essential roles in the specific stages of cellular proliferation and differentiation, the cellular conditions of odontogenic cyst epithelium need to be reevaluated. This study aimed to estimate the degree of proliferating, differentiating and apoptotic activities of odontogenic cyst epithelium using antisera of PCNA, Ki-67, MPM-2, transglutaminase C, heat shock protein 70 and $ApopTag$^{(R)}$. method in 19 cases of odontogenic cysts. Cellular changes of the cyst epithelium were measured by intensity of each immunohistochemical staining. Results were as follows: 1. The proliferating activity of the cyst epithelium was slightly lower than that of normal oral mucosal epithelium, with the use of primary antibodies against PCNA, Ki-67, and MPM-2. And the proliferating activity of the epithelium in OKC group was even higher than that of the epithelium in non-OKC group. 2. The odontogenic cysts showed weakly positive reaction with transglutaminase C, but strongly positive reaction with HSP 70. 3. Occasionally, only a few apoptotic cell was observed in the superficial keratin layer of OKC. 4. The hyperplastic cyst epithelium infiltrated with mild inflammatory cells showed diffusely positive reaction with different proliferating factors. From the above results, we presumed that the endogenous proliferating and differentiating activity of the cyst epithelium was slightly lower than that of normal oral mucosal epithelium, and also supposed that the cyst epithelium could be reactivated for the further proliferation by the exogenous factors, such as inflammatory reaction and any chemicophysical irritations.
