Study on Epidemic Warm Diseases with dampness of "OnByeongJoByeon" ("온병조변(溫病條辨)" 습류온병(濕類溫病)에 대한 고찰)

  • Oriental Medicine always attach great importance to the damp diseases. Dampness is related with many organs and many clinical diseases. The cause and the location of the damp disease, nature of the symptoms, combination with other pathogenic factors are very diverse. This article analyzed the concept, cause of disease, pathogenesis, characteristic of symptoms, treatment method and prescriptions of Epidemic Warm Diseases of dampness syndrome and cases of dampness-heat diseases based on the theories of Epidemic Warm Diseases and found that theories of Epidemic Warm Diseases have very wide area of application. Dampness is classified into cold-dampness and dampness-heat by combination of heat or cold. The dampness syndrome is related with organs such as lung, spleen, kidney, triple energizers and bladder, and affects liver and heart. The basic treatment methods are dispelling dampness turbidity and diffusing qi movement. The detail treatment methods are spreading lung qi with lightness and resolving dampness and excreting turbidity in upper energizer, opening and dipping down with pungent-bitter and diffusing qi movement and strengthening the spleen and stomach in the middle energizer, draining dampness with bland in the lower energizer. Warming Yang is the main method of treatment for cold-dampness and clearing heat is for dampness-heat with the assistant methods such as resolving dampness and promoting the flow of qi. 5. Acute fever, virus diseases, epidemic diseases among modern diseases are much related with the dampness-heat syndrome.

  • The Office for Saving Lives (活人署) (OSL) was the office in charge of the treatment and relief of the poor in the Joseon Dynasty. This study disputes prior scholarship on the OSL by analyzing the use of a ready-made prescription and by focusing on the personality of the OSL's medical institutions. The work of the three government offices, the Office of Great Mercy (大悲院) (OGM), which was the formal office of OSL, the Office of Benefiting People (惠民署) and the Office of Aiding Life (濟生院), overlapped in the area of relief of the common people. But OGM was different from the other two in that it was not a purely medical office, had no educational function, and did not manage medicine. By analyzing a standing prescription, this article argues: 1) Heojun's influence on the composition of a standing prescription is absolute. 2) Epidemic warm disease (溫疫) was a major social problem in terms of emergency medical care at the time. 3) In the late Joseon Dynasty, the treatment of epidemic warm diseases became more sophisticated than the previous era.

A Study on the concept of makwon in onbyeonghak(溫病學) and its practical application (온병학(溫病學)의 막원(膜原) 개념과 그 실제적 활용에 대한 고찰)

  • Objective : Doctors' discussions on Makwon's warm-disease are studied in an effort to contemplate a way to use the outcome in practice. Method : Doctors' discussions on Makwon as found in researches and texts are categorized into physical and pathologica, and their symptoms and treatments are analyzed. Result & Conclusion : Many doctors of warm disease explained that, in terms of the physical aspect, Makwon is closely related to stomach. Some doctors also mentioned its relationship with triple engizers, defense and nutrient qi, and spleen. In terms of pathology, most of the doctors considered Makwon to be a hiding place of foul turbidity pathogen, and explained that a treatment has to be applied before the disease pattern worsens. Diverse syndroms of Makwonwere introduced. Their discussions mainly revolved around warm and epidemic or category of dampness-heat diseases. Alternating chills and fever was also mentioned often, and generalized pain, nausea, tongue of thick and slimy fur symptoms are shown. Ouguk's Dalwonneum is used as the basic treatment. The most effective prevention method of the incubation of pathogen are the symptoms that allows the analysis of the mechanism of disease. Therefore, a good utilization method of resolve turbidity with aroma or spices may contribute to the prevention of Makwon-disease in terms of cultural aspect.

A Study on the Development of the Understanding that the Cause of Warm Pathogen Lies in Upper Portion of Body ("온사상수(溫邪上受)" 개념의 형성요인에 대한 고찰)

  • Objectives : Contemporary researches suspect that, contrary to the past belief, the understanding that the cause of warm pathogen lies in the upper portion of human body is an understanding that had been well-established even before Yetianshi. This new understanding now requires us to contemplate the process of theoretical development which this understanding, termed Onsasangsu, had taken within the boundary of the theory of warm pathogen. This paper aims to shed light on this within the framework that this is the emergence of a new theory of warm pathogen caused by a new understanding of warm pathogen. Methods : First, the theories of warm pathogen as developed by historical doctors were studied, and elements that seem to be related to the understanding of Onsasangsu were selected and studied to understand their theoretical characteristics. Furthermore, the paper studied what academic significance do these theories have on the development of the theory of warm pathogen. Results & Conclusions : Provided that the underlying assumption of Onsasangsu is that febrile diseases are caused through moutn and nose, the study showed that this understanding arose before the period of Qing Dynsasty from the need by many doctors to differentiate the pathogens of various diseases such as the disease of heat, febrile disease, and epidemic. The reason that these discussions could not have much impact on the study of febrile disease during the Qing Dynasty could be because they were not passed on down to the future generations, or because commonly held perspective was unable to accept criticisms.

  • Onbyeong(溫病) is called an acute epidemic febrile disease caused by warm pathogen, a major symptom of Onbyeong is high fever. Doctrine of Onbyeong is a study of an occurrence, progress and treatments of an acute epidemic febrile disease. Doctrine of Onbyeong is valid in the Cheong Dynasty at China. Now, a theory of doctrine of Onbyeong at China and Korea is being applied in not only an acute febrile disease but also many other lifestyle diseases. Onbyeongjobyeon is a book written by Oguktong(吳鞠通). Oguktong was influenced by Jangjung-gyeong(張仲景) "Sanghanron(傷寒論)". Oguktong had organized Seopcheonsa(葉天士)'s medical thoughts and Oguktong's medical experiences. A Samcho(三焦) deteriorated case is divided into three groups - Upper, Middle and Lower-energizer - that is discussed of a vertical progress of Onbyeong. And a Wigiyeonghyeol(衛氣營血) deteriorated case is divided into four groups - Wi, Gi, Yeong and Hyeol - that is discussed of a horizontal progress of Onbyeong. In Korean medicine, there are four types of diagnosis which are watching, listening, asking and taking. Informations, got by four types diagnosis are synthesized and classified for medical treatments. A pulse diagnosis belongs to a method by taking a wrist among four diagnosis. A Korean Medicine doctor makes a conclusion of cause, region and condition of disease by taking a pulse. Because all organs in human body are connected by a meridian system. organs conditions are reflected in a meridian system. So by taking a pulse, a progress and a prognosis of disease is diagnosed In this thesis, by taking a pulse on "Onbyeongjobyeon(溫病條辨)", a location and a feature of disease's cause with weakness and strength of a vital force are examined, and a character of a pulse diagnosis of Onbyeong is examined.

A Study on Cold Damage(傷寒) in the Compendium of Prescription from the Countryside(鄕藥集成方) - Focusing on citation, medical theory, prescription, medicinal herbs - (조선 의서 『향약집성방』 중에 실린 상한(傷寒) 논의 연구 - 인용 문헌, 의론(醫論), 처방, 본초 등을 중심으로 -)

  • The purpose of this paper is to derive the features of cold damage clinical medicine during the early days of the Chosun(朝鮮) period by analyzing discussions on cold damage published in the official medical book of the Chosun period, Compendium of Prescription from the Countryside(鄕藥集成方, CPC). Cold damage was one of the typical diseases in East Asia where there was constant seeking of the utilization of prescriptions, ways of preparations, and awareness regarding cold damage as shown in Zhang, Zhongjing(張仲景)'s Treatise on Cold Damage Disease(傷寒論, TCDD) below. Traditional Korean medicine which possessed the medical universality of East Asia also was no exception and through an analysis of the part on cold damage in CPC, it is expected that medical features of cold damage in Korea passed down from the Koryo(高麗) Dynasty to the early Chosun period will be revealed. For this, first there needs to be an organization of past discussions on cold damage surrounding the existence of infection and after checking the issues, exploring which of the writings related to TCDD and editions are being utilized through an analysis on citing literature of Cold Damage Disease Literature(傷寒門) and Heat Pathogen Disease Literature(熱病門) which have developed discussions on cold damage in CPC. In addition, by comparing Peaceful Holy Benevolent Prescription(太平聖惠方, PHBP) and Complete Record of Sacred Benevolence(聖濟總錄, CRSB), known to have greatly influenced CPC and Cold Damage Literature and Heat Pathogen Disease Literature, features of form and content used by CPC were analyzed. Features of form were examined through pattern of organization and number of citing literature were examined and for features of content, cold damage infection, classification, syndrome differentiation method, and utilization of materia medica among prescriptions were examined. Discussions on cold damage as being uninfectious as stated in Treatise on the Pathogenesis and Manifestations of All Diseases(諸病源候論) unlike pestilence, epidemic pathogen(時氣), warm pathogen disease(溫病), and heat pathogen disease were excluded in PHBP. PHBP opened the possibility of cold damage infection and later writings, CRSB and CPC also follow this. As a result of analyzing citing literature of the part on cold damage in CPC, it is uncertain which edition of TCDD is being utilized; however, the most distinctive feature was that Classified Emergency Materia Medica(證類本草) and not writings specializing in cold damage are in use. In general, although CPC in terms of form is similar to CRSB, content creation predominantly depended on PHBP. More specifically; first, in terms of the existence of cold damage infection, arguments of PHBP and CRSB are maintained. Second, in terms of cold damage classification, although CRSB is followed, heat pathogen disease is classified separately developing PHBP as is. Third, in terms of method, as Book of Keep Healthy(南陽活人書) and CRSB compiled in later times are cited, it is deemed that arguments were raised to a certain extent regarding six-meridian syndrome differentiation(六經辨證). Fourth, although the majority of utilized materia medica among cold damage prescriptions utilize Materia Medica from the Countryside(鄕藥本草) in CPC and materia medica from Korean Peninsula, this is due to the desire for the compilation performance of CPC to be propagated to ordinary citizens and not the ruling class. CPC as the official medical book compiled in the early days of the Chosun period was greatly influenced by the Song(宋) Dynasty's medical books, PHBP and CRSB shows that cold damage medicine in the early Chosun Period indeed possesses the medical universality of East Asia. Furthermore, the features of published medical theory and prescriptions reveal the existence of the cold damage medical tradition of the Chosun period serving as clues for cold damage research tradition among Korea's medical history.
