청심포자음(淸心蒲子飮)의 구속 스트레스 및 절개 통증에 대한 항스트레스 작용과 진통효과 (Anti-stress and Anti-nociceptive Effects of the Aqueous Extracts of Chungsimyeonja-eum against Immobilization Stress and Incision Pain in Rats)

  • The aqueous extract of Chungsimyeonja-eum has been used to treat palpitation, anemia, chronic fatigue, hypertension, and stroke. It also possesses various pharmacological effects including hypotensive, blood circulating, sedative, anti-inflammatory, diuretic, and anti-stress activities. In the present results, the latency time of plantar test was decreased in the immobilization stress and incusion pain-induction group. However the paw withdrawal latency values were increased in the immobilization stress and incision pain-induction groups after treatment with the aqueous extract of Chungsimyeonja-eum. The c-Fos expression in the paraventricular nucleus (PVN), and 5-hydroxytryptamine (5-HT) and tryptophan hydroxylase (TPH) in the dorsal raphe were enhanced in the immobilization stress and incision pain-induction groups. However, the treatment with the aqueous extract of Chungsimyeonja-eum suppressed the immobilization stress and incision pain-induced increase of c-Fos, 5-HT, and TPH expressions. Here in this study, we have demonstrated the protective effects of Chungsimyeonja-eum on immobilization and incision pain-induced stress. The present study revealed that Chungsimyeonja-eum treatment diminishes immobilization and pain stress.

Propofol promotes osteoclastic bone resorption by increasing DC-STAMP expression

  • Background: Propofol is an intravenous anesthetic which has antioxidant effects due to its similarity in molecular structure to ${\alpha}$-tocopherol. It has been reported that ${\alpha}$-tocopherol increases osteoclast fusion and bone resorption. Here, we investigated the effects of propofol on signaling pathways of osteoclastogenic gene expression, as well as osteoclastogenesis and bone resorption using bone marrow-derived macrophages (BMMs). Methods: BMMs were cultured with macrophage colony-stimulating factor (M-CSF) alone or M-CSF plus receptor activator of nuclear factor kappa B ligand (RANKL) in the presence of propofol ($0-50{\mu}M$) for 4 days. Mature osteoclasts were stained for tartrate-resistant acid phosphatase (TRAP) and the numbers of TRAP-positive multinucleated osteoclasts were counted. To examine the resorption activities of osteoclasts, a bone resorption assay was performed. To identify the mechanism of action of propofol on the formation of multinucleated osteoclasts, we focused on dendritic cell-specific transmembrane protein (DC-STAMP), a protein essential for pre-osteoclastic cell fusion. Results: Propofol increased the formation of TRAP-positive multinucleated osteoclasts. In addition, the bone resorption assay revealed that propofol increased the bone resorption area on dentin discs. The mRNA expression of DC-STAMP was upregulated most strongly in the presence of both RANKL and propofol. However, SB203580, a p38 inhibitor, significantly suppressed the propofol/RANKL-induced increase in mRNA expression of DC-STAMP. Conclusion: We have demonstrated that propofol enhances osteoclast differentiation and maturation, and subsequently increases bone resorption. Additionally, we identified the regulatory pathway underlying osteoclast cell-cell fusion, which was enhanced by propofol through p38-mediated DC-STAMP expression.

Importance of Sacrotuberous Ligament in Transgluteal Approach for Sciatic Nerve Entrapment in the Greater Sciatic Notch (Piriformis Syndrome)

  • Objective : The efficacy of sciatic nerve decompression via transgluteal approach for entrapment of the sciatic nerve at the greater sciatic notch, called piriformis syndrome, and factors affecting the surgical outcome were analyzed. Methods : The outcome of pain reduction was analyzed in 81 patients with sciatic nerve entrapment who underwent decompression through a transgluteal approach. The patients were followed up for at least 6 months. The degree of pain reduction was analyzed using a numerical rating scale-11 (NRS-11) score and percent pain relief before and after last follow-up following surgery. Success was defined by at least 50% reduction in pain measured via NRS-11. To assess the degree of subjective satisfaction, a 10-point Likert scale was used. In addition, demographic characteristics, anatomical variations, and variations in surgical technique involving sacrotuberous ligamentectomy were analyzed as factors that affect the surgical outcome. Results : At a follow-up of 17.5±12.5 months, sciatic nerve decompression was successful in 50 of 81 patients (61.7%), and the pain relief rate was 43.9±34.17. Subjective improvement based on a 10-point Likert scale was 4.90±3.43. Among the factors that affect the surgical outcome, only additional division of the sacrotuberous ligament during piriformis muscle resection played a significant role. The success rate was higher in the scarotuberous ligementectomy group (79.4%) than in the non-resection group (42.6%), resulting in statistically significant difference based on average NRS-11 score, percent pain relief, and subjective improvement (p<0.05, independent t-test). Conclusion : Sciatic nerve decompression is effective in pain relief in chronic sciatica due to sciatic nerve entrapment at the greater sciatic notch. Its effect was further enhanced by circumferential dissection of the sciatic nerve based on the compartment formed by the piriformis muscle and the sacrotuberous ligament in the greater sciatic notch.

부인과 수술 후 한방 조기 회복 치료를 받은 입원 환자에 대한 호전도 및 만족도 조사 (A Study on Symptom Improvement and Satisfaction of Inpatients Received Traditional Korean Medicine for Enhanced Recovery after Surgery (ERAS) after Gynecological Surgery)

  • Objectives: The purpose of this study is to report the effectiveness and satisfaction of traditional Korean medicine (TKM) for enhanced recovery after surgery (ERAS) after gynecological surgery. Methods: The study is conducted by analyzing the medical records of hospitalized patients in a Korean medicine hospital. Sixty-five patients who received TKM for ERAS after gynecological surgery from May 2020 to July 2021 were included. We performed statistical analysis by using SPSS ver. 25.0. To verify the effect of TKM for ERAS, we analyzed improvement of symptoms. Also, we analyzed survey of patients' satisfaction to verify the satisfaction of TKM for ERAS. Results: Among the symptoms of pain, there was a statistically significant decrease in all types of pain on the questionnaire. Other systemic symptoms showed statistically significant improvement in all symptoms on the questionnaire. The satisfaction level questionnaires for TKM for ERAS showed a mean value of 4.37±0.63 out of 5-point scale. Especially, herbal medicine, acupuncture, and moxibustion showed high satisfaction without dissatisfaction. Conclusions: The result has shown that TKM for ERAS after gynecological surgery is effective and the satisfaction of the patients is high. Further study on TKM for ERAS for quality of life assessment is needed.

Prostaglandin D2 contributes to cisplatin-induced neuropathic pain in rats via DP2 receptor in the spinal cord

  • Background: Chemotherapy-induced peripheral neuropathy (CIPN) is a major reason for stopping or changing anticancer therapy. Among the proposed pathomechanisms underlying CIPN, proinflammatory processes have attracted increasing attention. Here we assessed the role of prostaglandin D2 (PGD2) signaling in cisplatin-induced neuropathic pain. Methods: CIPN was induced by intraperitoneal administration of cisplatin 2 mg/kg for 4 consecutive days using adult male Sprague-Dawley rats. PGD2 receptor DP1 and/or DP2 antagonists were administered intrathecally and the paw withdrawal thresholds were measured using von Frey filaments. Spinal expression of DP1, DP2, hematopoietic PGD synthase (H-PGDS), and lipocalin PGD synthase (L-PGDS) proteins were analyzed by western blotting. Results: The DP1 and DP2 antagonist AMG 853 and the selective DP2 antagonist CAY10471, but not the DP1 antagonist MK0524, significantly increased the paw withdrawal threshold compared to vehicle controls (P = 0.004 and P < 0.001, respectively). Western blotting analyses revealed comparable protein expression levels in DP1 and DP2 in the spinal cord. In the CIPN group the protein expression level of L-PGDS, but not of H-PGDS, was significantly increased compared to the control group (P < 0.001). Conclusions: The findings presented here indicate that enhanced PGD2 signaling, via upregulation of L-PGDS in the spinal cord, contributes to mechanical allodynia via DP2 receptors in a cisplatin-induced neuropathic pain model in rats, and that a blockade of DP2 receptor activation may present a novel therapeutic target for managing CIPN.

Efficacy of buccal piroxicam infiltration and inferior alveolar nerve block in patients with irreversible pulpitis: a prospective, double-blind, randomized clinical trial

  • Objectives: This randomized clinical trial aimed to assess the effectiveness of buccal infiltration with piroxicam on the anesthetic efficacy of inferior alveolar nerve block (IANB) with buccal infiltration in irreversible pulpitis, with pain assessed using the Heft-Parker visual analogue scale (HP-VAS). Materials and Methods: This study included 56 patients with irreversible pulpitis in mandibular molars, randomly distributed between 2 groups (n = 28). After evaluating the initial pain score with the HP-VAS, each patient received IANB followed by buccal infiltration of 2% lignocaine with adrenaline (1:80,000). Five minutes later, the patients in groups 1 and 2 were given buccal infiltration with 40 mg/2 mL of piroxicam or normal saline, respectively. An access opening procedure (AOP) was performed 15 minutes post-IANB once the individual showed signs of lip numbness as well as 2 negative responses to electric pulp testing. The HP-VAS was used to grade the patient's pain during caries removal (CR), AOP, and working length measurement (WLM). Successful anesthesia was identified either by the absence of pain or slight pain through CR, AOP, and WLM, with no requirement of a further anesthetic dose. A statistical analysis was done using the Shapiro-Wilk and Mann-Whitney U tests. Results: The piroxicam group presented a significantly lower (p < 0.05) mean pain score than the saline group during AOP. Conclusions: Buccal infiltration with piroxicam enhanced the efficacy of anesthesia with IANB and buccal infiltration with lignocaine in patients with irreversible pulpitis.

타액 단백질의 기능 및 구조 연구의 임상적 적용 (Clinical Appliance of Structure-Function Studies of Salivary Macromolecules)

  • Salivary research is at a critical crossroads regarding the clinical application of the basic knowledge. The purpose of this article is to introduce the current progress on salivary research to Korean dental scientists. The accumulated results based on advance technologies such as protein chemistry, molecular biology, and structural biology have showed that salivary macromolecules need structural requirements for proper function. Currently, several concepts or principles, which can be applied to salivary macromolecules, have been suggested. These include the role of molecules' conformation on biological activity, their multifunctional nature, their redundancy of function, their amphifunctional properties, and the potential importance of complexing between molecules. These concepts and the information available will help the development of saliva substitutes, the design of drug carriers and chimera molecules with enhanced function and the development of gene therapy protocols. These approaches will alleviate or restore lost salivary function and can be used to treat various kinds of oral and systemic diseases.

초음파 조영제 (Ultrasound Contrast Agent)

  • Ultrasound imaging in clinical practice is one of the widely used diagnostic methods because there is no radiation risk, more cost- effective compared to MRI or CT, and possible to perform an intervention through fast real-time imaging. In order to increase the diagnostic value, the studies of contrast-enhanced ultrasound (CEUS) using an ultrasound contrast agent have been actively conducted since about 50 years ago and are being used clinically in vascularity and microcirculation of internal organs. Although ultrasound is actively used for the diagnosis and treatment of various diseases in musculoskeletal disorders, there are some limitations in diagnosing mild or small lesions, inflammatory reactions, or abnormalities at the molecular level. In this review, the principles, types, and research, and clinical applications of ultrasound contrast agents have been summarized and introduced. If we understand the characteristics of the ultrasound contrast agents and anatomical knowledge, as well as molecular changes, the ultrasound contrast agents are widely applied in musculoskeletal disorders and have tremendous potential for diagnosis and treatment.

Standardized Protocols for Measuring Volatile Sulfur Compounds: Scientific Foundations and Methodologies

  • Halitosis is defined as a nasty odor emanating through the mouth and is primarily related to the enhanced concentration of volatile sulfur compounds (VSCs). VSC measurements have been commonly used for experimental comparison and clinical diagnosis. As quantitative methods for comparative analyses of oral malodor, gas chromatography devices have been most commonly used to quickly and easily determine the concentration of several gas components of VSCs, which are agents primarily responsible for halitosis. The concentrations of VSCs fluctuate dynamically depending on contributing factors, including various oral/systemic conditions, intake of medicine and food/drink, oral hygiene, and even routine daily activities. Therefore, the exact analysis of VSCs requires the appropriate standardization of not only exact measurement techniques but also participant conditioning with scientific considerations. Thus, this paper describes the experimental standardizations commonly recommended in previous literature and their scientific background.
