복잡 심기형에서의 동맥 전환술에 대한 연구 (Arterial Switch Operation in Complex Congenital Heart Diseaes (Application, procedure analysis,risk assessment,and results))

  • Between March 1989 and December 1994, one-stage repair was performed for correction of the intracardiac malformations associated with aortic coarctation in 34 patients or interrupted aortic arch in 8 patients via median sternotomy. There were 26 male and 16 female patients, and their body weight ranged from 1.8 to 8 kg [mean weight, 4.0 1.4 kg . The age at the operation ranged from 7 days to 18 months [mean age, 3.1 $\pm$ 3.8 months . The repair of aortic coarctation or interrupted aortic arch was performed using extended end-to-end anastomosis in most of the patients [86%, 36/42 , and six patients underwent ductal tissue excision and patch aortoplasty. Intracardiac defects were corrected concomitantly through the right atrium unless the anatomy dictated otherwise. Obstructive outlet septum was resected whenever necessary. There were seven early deaths [16.8 % , and three late deaths with a mean follow-up period of 25 months [range from 1 to 65 months . Three patients were reoperated upon residual subaortic stenosis, stenosis at the RPA origin, and subacute bacterial endocarditis respectively. None showed any significant residual or anastomotic stenosis postoperatively. One stage repair of the aortic coarctation and interrupted aortic arch associated with intracardiac defect leaves no native coarctation shelf tissue or residual hypoplasia in the repaired segment, has low incidence of recurrent or residual stenosis, minimizes reoperation and incisions, and manages arch hypoplasia easily. We concluded that surgical results of one-stage repair for the intracardiac malformation associated with aortic coarctation or interrupted aortic arch are reasonable.

Three dimensional finite element analysis of 4 inch smart flange on offshore pipeline

  • Smart flanges are used for pipeline and riser repair in subsea. In a typical case in the gas export pipeline project, the end cap bolts of a 4inch smart flange were broken during operation, and in turn leakage occurred. This work presents the detail of three dimensional finite element analysis of the smart flange to support the observed end cap bolts failure. From finite element analysis it turns out that in the presence of external bending moment, an uneven contact distribution is present between seal and end cap, which in turn changes the uniform load distribution on bolts and threaten the integrity of bolts. On the other hand, 3D finite element analysis of interaction between pipeline and seabed is presented by means of Abaqus to explore the distribution of bending moment along the pipeline route. It is found that lateral buckling occurs in the pipeline which introduces large bending moment.

  • Bonding strength of reinforcing material has been recognized to be the most important factor which determines the strengthening effect and the durability of repair work. The properties of bonding layers affects the stress distribution at the end of the plate, therefore the behavior of bonding layer has to be investigated. In this study, the stress distribution at the end of the bonded plate has been tested and compared with Roberts' analysis. Shear stress and vertical normal stress at the end of strengtening plate are analysized and the effedts of bonding layer thickness, plate thickness and plate length on the bonding behavior are tested. The test results showed that thickness is one of the most important factor, which is the thinner the thickness, the smaller the maximum stress.

아킬레스건 파열의 수술적치료 -단단 봉합술 및 건외막피복술- (Surgical Treatment of Ruptured Achilles Tendon - End-to-end suture & Paratendinous wrapping)

  • 목적: 스포츠와관련된급성아킬레스건파열의직접봉합및건외막피복술의결과및예후를평가하였다. 대상및방법: 1997년부터2001년까지수술후1년이상추시가능했던21예를대상으로하였다. 30대가$55\%$로가장많았으며축구후에생긴파열이5예로가장많았고, 양측이간격을두고파열된경우가2예로확인되었다. 수술은수상후1주이내에시행하였으며, 단단봉합술후주위건외막조직을충분히봉합하여혈액순환을최대한유지한상태로봉합부를피복하였다. 수술후슬관절을25도굴곡, 족관절은족저굴곡된상태로6주간장하지석고고정후2주간격으로단하지석고고정및족관절첨족을기능적위치로전환하여10주째90도족배굴곡상태로회복시켰다. 결과는Hooker의기준을이용하여수술후평균28개월에평가하였다. 결과: 환측이정상측에비해발뒤꿈치-지면거리가평균0.7cm 감소하였고, 20회체중부하족저굴곡후에는0.8 cm 감소하였으며, 하퇴둘레는0.3 cm 감소하였고, 능동적운동범위는족배굴곡과족저굴곡이각각3도와5도가감소하였다. 수술결과는1 6예에서우수, 5예에서양호를보였으며재파열이나상처부위괴사및감염의후유증은없었다. 결론: 아킬레스건파열의단단봉합술후주위건외막조직의충분한피복봉합은건의치유와수술후유착방지에, 그리고수술후주기적족관절족배교정은잔여첨족변형의예방에좋은치료방법이다

  • The concrete structure under fatigue load depresses in mechanical capability or breaks down finally because of the fatigue. So recently the study on strengthening methods using bonded steel plate is widely attended. Which could be constructed under use and without damaging the structure. The intention of this method is to increase the useability of the structure and to recover the strength. So application on fields are sharply increasing. In this study it is tried to develop strengthening methods using notches and anchor bolts which could supply the defect on falling off of the steel plate at the end.

  • This paper reports a manner to use a Bayesian approach to derive the optimal replacement policy. In order to produce a system with minimal repair warranty, a replacement model with the extended warranty is considered. Within the warranty period, the failed system is minimally repaired by the manufacturer at no cost to the end-user. The failure time is assumed to follow a Weibull distribution with unknown parameters. The expected cost rate per unit time, from the end-user's viewpoints, is induced by the Bayesian approach, and the optimal replacement policy to minimize the cost rate is proposed. Finally, a numerical example illustrating to derive the optimal replacement policy based on the Bayesian approach is described.

  • As malicious apps vary significantly across Android malware, it is challenging to prevent that the end-users download apps from unsecured app markets. In this paper, we propose an approach to classify the malicious methods based on permissions using Long Short Term Memory (LSTM) that is used to embed the semantics among Intent and permissions. Then the malicious method that is an unsecured method will be removed and re-uploaded to official market. This approach may induce that the end-users download apps from official market in order to reduce the risk of attacks.

혈관손상의 임상적 고찰 (clinical analysis of vascular trauma)

  • During the period from Mar. 1983 to Feb. 1986, 22 patients with vascular trauma were treated at Capital Armed General Hospital. 1] 11 patients had arterial injury alone, 6 patients had venous injury alone, and 5 patients had both arterial and venous injuries. 2] The mechanism of injury in these 22 patients was 7 penetrating non-gun shot wounds, 5 blast fragments injuries, 4 iatrogenic injuries, 4 blunt injuries, 2 gun shot wounds. 3] The method of 16 arterial repair was 5 autogenous saphenous vein graft, 8 synthetic vascular graft, 1 end to end anastomosis, 2 lateral suture. Midterm patency of 16 repairs was 100% within 1 months. 4] The method of 11 venous repair was 1 autogenous saphenous vein graft, 2 onlay vein patch, 2 ligation, 1 lateral suture. The thrombotic obstructions occurred in 4 repairs [36.4%], but they were resolved somewhat with heparinization. 5] Uncommon cases of false aneurysm of internal carotid artery and laceration of retrohepatic inferior vena cava were summarized.

  • Rupture of the main bronchus due to blunt chest trauma is very rare, especially In childhood although the incidence is increasing. Early diagnosis and primary repair not. only restore normal lung function but also avoid the difficulties and complications associated with delayed diagnosis and repair. We experienced 2 cases of right main bronchial rupture caused by traffic accidents. Patients suffered from progressively developing dyspnea and subcutaneous emphysema on the neck, anteriorchest,andanteriorabdominalwall. Emergency operations were performed through right posterolateral thoracotomy incision at the 4th intercostal space. Intraoperatively, the right main bronchus completely transsected and separated. Corrective bronchoplasty was performed with end-to-end anastomosis using interrupted suture with 3-0 Vicryle and the suture line was reinforced with azygos vein and parietal pleural flap. Postoperative courses were uneventful and patients discharged without any specific pro lems.

동일 승용차량에 대한 RCAR 신.구 충돌시험을 통한 차체 충돌특성에 관한 연구 (A Study on Vehicle Crash Characteristics with RCAR Crash Test in Compliance with the New Test Condition)

  • This research investigates vehicle structure acceleration and vehicle deformation with RCAR crash test. To investigate vehicle damage characteristics in an individual case, it is possible to RCAR low speed crash test. In this study, two tests were conducted to evaluate difference between RCAR new condition and RCAR old condition. A two large vehicles were subjected to a frontal crash test at a speed of 15km/h with an offset of 40% $10^{\circ}$ angle barrier and flat barrier. The results of the 15km/h with an offset of 40% $10^{\circ}$ angle barrier revealed high acceleration value on the vehicle structure and high repair cost compared to the RCAR 15km/h with an offset of 40% flat barrier. So in order to improve damage characteristics in low speed crash of vehicle structure and body component of the monocoque type passenger vehicles, the end of front side member and front back beam should be designed with optimum level and to supply the end of front side member as a partial condition approx 300mm.
