EFFECT OF FLUORIDE IN NATURAL POLYMER ON ENAMEL DEMINERALIZATION (천연고분자 화합물을 이용한 불소겔이 법랑질 탈회에 미치는 영향)

  • We have developed a sodium fluoride containing gelatin and methyl cellulose gel. Cariostatic abilities of those gel were investigated and compared with APF gel and fluoride varnish($Cavityshield^{TM}$). We prepared the bovine tooth samples and divided into two surface, control side and experimental side in same specimen for exclusion of difference between specimens. The experiment was consisted of 4 groups : (I) APF gel : (II) $Cavityshield^{TM}$ : (III) Gelatin F gel : (IV) Methyl cellulose F gel Decalcification were produced by placing each specimen into artificial acidic solution(pH 4.0) for 72 hours. Surface microhardness were measured and depth of demineralization lesion were measured by polarizing light microscope. The results were as follows: 1. The difference of VHN between control and experimental side is smallest in group I (p<0.05). 2. The largest difference was shown in group II (p<0.05). 3. There were no significant difference between group III and IV in microhardness test (p>0.05). 4. The difference of lesion depth is smallest in group I (p<0.05). 5. There were no significant difference between group II, III and IV in lesion depth (p>0.05). The result of the present study indicate that the fluoride containing gelatin and methyl cellulose gel is more effective than APF gel and is similar to fluoride varnish application for prevention of demineralization.

Comparison of Cariogenicity of Bovine Milk and Low-fat Milk on Streptococcus mutans Biofilm (국내 시판 우유와 저지방 우유의 Streptococcus mutans 세균막에 대한 우식원성 비교)

  • The purpose of this study was to compare the cariogenicity of commercially available bovine milk and low-fat milk in a biofilm model using the CDC Biofilm Reactor. Streptococcus mutans ATCC 25175 biofilms were formed on saliva coated bovine enamel slabs in a CDC Biofilm Reactor. Biofilms were exposed three times per day to one of the following materials: commercial whole milk (fat content: 3.4%), low-fat milk (fat content: 1%), or 0.9% NaCl. Medium pH was measured at different time points. After 5 days, biofilms were separated from slabs to evaluate the CFUs. The biofilm thickness was observed by confocal laser-scanning microscopy (CLSM). Enamel slab's demineralization was assessed by measuring surface microhardness before and after the experiment. For microhardness and CFUs assessment, no significant difference was found among the three groups. All groups showed similar pattern of medium pH change and biofilm thickness. Our results showed that there was no difference in the cariogenicity between whole milk and low-fat milk. Both milks were relatively non-cariogenic compared to the control group.

IN VITRO STUDY OF CARIOGENIC POTENTIAL OF INFANT FORMULAS (이유식의 우식유발능에 관한 생체외 연구)

  • The purpose of this study was to assess the cariogenic potential of five infant formulas in vitro. as compared with 10% sucrose solution and whole bovine milk. Buffering capacities were determined by the amount of 0.1 N lactic acid consumed to titrate the 50ml specimen solutions to pH 4.0. The pH of the specimen solution inoculated by Streptococcus mutans was measured by pH meter and the surface microhardness of primary tooth enamel immersed in the specimen solution was measured by the microhardness tester, before and after 48 hours incubation. Also, the solubility of calcium from synthetic hydroxyapatite was evaluated by atomic absorption spectrophotometer The buffering capacity of infant formulas was higher than that of sucrose solution and lower than that of milk, and there were significant differences among infant formulas(P<0.01). The average pH of infant formulas after 48 hours incubation was lower than that of sucrose solution and milk, and there was significant difference between infant formulas and milk(P<0.05). There were no signifiant differences among oops in the microhardness change of primary tooth enamel and in the amount of dissolved calcium ion from synthetic hydroxyapatite after incubation with Streptococcus mutans. In conclusion, infant formulas seemed to fulfill the basic requirments to cause dental caries in primary teeth, and there were significant differences of cariogenic potential among infant formulas. Cooperative efforts of dentistry and manufacturers to reduce the cariogenic potential of infant formulas would be necessary to prevent the early childhood caries in children.

RESIN INFILTRATION FOR THE ESTHETIC IMPROVEMENT OF ANTERIOR TEETH WITH DEVELOPMENTAL DEFECTS AND POST-ORTHODONTIC DECALCIFICATION (전치부의 발육 결함 및 교정 후 탈회 병소의 심미적 개선을 위한 resin infiltration)

  • The prevalence of developmental defects of enamel and lesios by post-orthodontic decalcification has been reported with increasing frequency. Even though there have been increasing interests and clinical challenges in esthetic improvement of these lesions, few of studies were reported for using non-invasive approach which is a very significant matter for child and young adults. This study was conducted to assess clinical effect on the improvement in color of these lesions via resin infiltration method developed as minimum invasive technique for white spot. For the 38 maxillary anterior teeth with calcification problem, the changes in color between before- and after- infiltration treatment, were evaluated and summarized as following. 1. A week after infiltration, 25% of developmental defects and 61% of decalcification lesions were improved in color as the value of ${\Delta}E $ below 3.7. 2. 40% of the developmental defects and 6% of decalcification lesions showed no significant change. 3. The developmental defects showed more remarkable changes in color 1 week after infiltration rather than immediately after the treatment. From our study results, it is considered that the amount of color improvement depended on the depth of lesion. In other words, for the lesion having more depth than the depth infiltrant resin can penetrate into, infiltration treatment showed no significant effect. Therefore, for clinical indication of resin infiltration treatment, further research on precise measurement technique of lesion depth is strongly required.

ANTICARIOGENIC EFFECT OF FLUORIDE VARNISHES AND CHLORHEXIDINE VARNISHES (불소 바니쉬와 클로르헥시딘 바니쉬의 항우식 효과)

  • Dental caries which is one of the most common chronic disease complexly developed by the action of oral bacteria, diet, and host factor. Various prevention program enhance resistance of demineralization and reduce the acidogenecity of oral bacteria have been introduced, representative material is fluoride and chlorhexidine. The purpose of the study was to evaluate and compare effectiveness of fluoride varnish and chlorhexidine varnish in vivo. Bovine tooth specimens were implanted in the lower space maintainers and applied with fluoride varnish and chlorhexidine varnish. After seven days in oral environment, metal mesh was covered to make similar condition of plaque accumulation and induce caries. All specimens were analysed by EPMA to evaluate quantitative change of Ca, P and by polarized microscope to identify histological changes. The results were as follows : After initial artificial caries induction in the mouth, there were remarkable enamel caries lesion in the control group under polarized light microscopy. The highest amount of mineral decrease were showed in control group. No statistically significant mineral decrease were showed in fluoride varnish group, while chlorhexidine varnish group showed only significant decrease of P(P<0.05). In conclusion both fluoride varnish and chlorhexidine varnish seemed to be effective for protecting enamel surface from caries activity, although fluoride varnish has more anticariogenic effect than chlorhexidine varnish.

Inhibitory Effects of Radix Pulsatillae Extract on Insoluble Glucan Synthesis and Adhesion of Streptococcus mutans (백두옹 추출물이 Streptococcus mutans의 비수용성 글루칸형성 및 부착억제에 미치는 영향)

  • Streptococcus mutans plays a virtal role in trigering dental caries establishment due to its ability to synthesize two significant factors. The two factors are organic acids and glucans. The former demineralized dental enamel and the latter mediates the attachment of bacteria to tooth surface. It is believed that demineralization of dental enamel and attachment of bacteria are the crucial events that indicate and develop dental caries. For this reason, we studied the effect of the ethanol extracts of Radix Pulsatillae on the growth and acid production of S. mutans. Ethanol extracts of the Radix Pulsatillae showed concentration dependent inhibitory activity against the growth and acid production of S. mutans, and produced significant inhibition compared to the control groups (p<0.05). The extracts inhibited S. mutans adherence to hydroxyapatite treated with saliva, and cell adherence was repressed by Radix Pulsatillae. the ethanol extract of Radix Pulsatillae showed remarkable inhibition of glucosyltransferase, which synthesizes water insoluble glucan form sucrose. Phytochemical analysis showed Radix Pulsatillae contained major components such as phenolic compounds, glycosides, steroids, terpenoid, and saponin. These results suggest that Radix Pulsatillae may have anti-cariogenic properties, which may be related with major components such as phenolic compounds, glycosides, steroids, terpenoid, and saponin.

Anti-Biofilm Effect of Egg Yolk Phosvitin by Inhibition of Biomass Production and Adherence Activity against Streptococcus mutans

  • The formation of biofilms on the enamel surface of teeth by Streptococcus mutans is an important step in dental plaque formation, demineralization, and early caries because the biofilm is where other bacteria involved in dental caries attach, grow, and proliferate. The objectives of this study were to determine the effect of phosvitin (PSV) on the biofilm formation, exopolysaccharides (EPS) production, adherence activity of S. mutans, and the expression of genes related to the compounds essential for biofilm formation (quorum-sensing inducers and components of biofilm matrix) by S. mutans. PSV significantly reduced the biofilm-forming activity of S. mutans and increased the degradation of preformed biofilms by S. mutans. PSV inhibited the adherence activity of S. mutans by 31.9%-33.6%, and the production of EPS by 62%-65% depending upon the strains and the amount of PSV added. The expressions of genes regulating the production of EPS and the quorum-sensing-inducers (gtfA, gtfD, ftf, relA, vicR, brpA, and comDE) in all S. mutans strains were down-regulated by PSV, but gtfB was down-regulated only in S. mutans KCTC 5316. Therefore, the anti-biofilm-forming activity of PSV was accomplished through the inhibition of biofilm formation, adherence activity, and the production of quorum-sensing inducers and EPS by S. mutans.

ORTHODONTIC BRACKET SHEAR BOND STRENGTH TO Nd:YAG LASER Er:YAG LASER IRRADIATED ENAMEL (Nd : YAG 및 Er : YAG 레이저로 치아표면 조사시 브라켓 전단접착강도에 관한 실험적 연구)

  • The purpose of this study was to evaluate the effectiveness of the Nd:YAG laser and the Er:YAAG laser on etching enamel for direct bonding of orthodontic bracket. The advantages of laser etching rather than conventional acid etching are to reduce the subsurface demineralization rate, to inhibit the spillage of acid onto uninvolved ""its of enamel, and to save the clinical manipulation time involving drying, trashing and drying again. 189 freshly extracted human premolars were prepared for this research. 165 out of them were divided into 11 groups of 15 teeth. One group was acid etching and the rest groups were irradiated with Nd:YAG laser by four different energy levels(100mj 10pps, 100mj 20pps, 150mj 20pps, 200mj 20pps) and with Er:YAG laser by six different energy levels(60mj 5pps, 60mj 10pps, 100mj 10pps. 200mj 10pps, 200mj l5pps, 400mj 10pps). Shear bond strength was tested with Instron after 24 hours, one week, and three weeks. Twenty-four out of 189 teeth were divided into twelve groups untreated control, acid etching, and ten laser irradiation subgroups. And the ultrastructural enamel surfaces of each group were observed with scanning electron microscope. The results were as follows; 1. The means and the standard deviations of shear bond strength of Nd:YAG and Er:YAU laser irradiation by different energy levels were obtained. 2. Shear bond strengths of Er:YAG laser irradiation groups were higher than those of Nd:YAG laser irradiation groups at the identical energy level. 3. Maximum bond strengths was achieved at the energy of I50mj, 20pps in Nd:YAG laser irradiation groups or 60mj, 10pps in Er:YAG laser irradiation groups. 4. It was acceptible for direct bonding to irradiate lb0mj 20pps with Nd:YAG laser or to irradiate 60mj 10pps with Er:YAG laser considering the results of shear bond strength tests and SEM obsesvation.

A Study on the Enamel Erosion Caused by Energy Drinks (시판되는 에너지음료의 법랑질 침식효과에 대한 연구)

  • To find out the effect of commercially available energy drinks on tooth enamel erosion, analyzed pH, buffering capacity, and the content of some of the inorganic components selecting 4 energy drinks that has high affinity of the products currently being sold. In addition, by observing the degree of erosion before and after immersion in energy drink by surface microhardness and scanning electron microscope (SEM) the results were as follows: Acidity of energy drink 'Burn Intense' was the lowest as $2.78{\pm}0.01$ highest on distilled water as $6.475{\pm}0.01$. 'Burn Intense' buffering capacity was $3.48{\pm}0.155$ at pH 5.5, $1.88{\pm}0.15$ at pH 7.0 which is the highest, and 'Hot6' was $1.71{\pm}0.37$, $1.23{\pm}0.35$ on each of it showing the lowest points. Ca content on energy drink was the highest at 'Volt Energy' as ($77.21{\pm}2.70mg/kg$), the lowest at 'Hot6' as ($0.98{\pm}0.05mg/kg$). P content was the highest on 'Hot6'($1.34{\pm}0.05mg/kg$) and detected at 'Red Bull'. Enamel surface hardness variation of the energy drinks before and after immersion showed rapid decrease at 'Red Bull' ($66.65{\pm}35.60$), and 'Volt Energy' ($61.96{\pm}31.42$), 'Burn Intense' ($58.53{\pm}24.84$), 'Hot6' ($53.99{\pm}60.26$) was in order. Distilled water, the control group, showed significant difference with the experimental group (p<0.05). But there was no significant difference between energy drinks. At SEM observation and analysis, 'Burn Intense' was the most severe demineralization, 'Volt Energy', 'Hot6', 'Red Bull', distilled water was in order. In the above results, taken together there were no statistically differences between energy drinks but significant difference in comparison with distilled water. In addition, at SEM observation and analysis all energy drink caused dental erosion, especially 'Burn Intense', has the lowest acidity, was serious. Thus, it is believed to provide a variety of oral health education and information about energy drinks that can affect the erosion of the teeth so public have the right to be recognized and reasonable dental care.

THE EFFECT OF FLUORIDE VARNISH AND ACIDULATED PHOSPHATE FLUORIDE GEL ON ARTIFICIAL CARIES LESION (Fluoride varnish와 acidulated phosphate fluoride gel이 인공우식 병소에 미치는 영향)

  • Fluoride has been used widely for the prevention of dental caries. Many dental professionals applicated acidulated phosphate fluoride(APF) gel for the purpose of prevention of dental caries in recent years. However, the topical application of fluoride varnish is growing and getting much recognition in many parts of Europe and Scandinavia. The main purpose of this study was to compare the effect of fluoride varnish and acidulated phosphate fluoride gel on artificial caries lesion. The artificial caries lesion was caused on the sound bovine enamel and divided 60 specimens into three groups each containing twenty for the purpose of study. No application was done on group 1, which acted as control group. Group 2 was treated with the topical application of fluoride varnish and removed after 1 hour Group 3 was treated with APF gel and removed after a minute. The cycle of remineralization/demineralization went on in vitro and the microhardness was measured for the each group after 5 and 15days. The following results were obtained : 1. According to the results after 5 days, the microhardness of groups 2 and 3 were significantly higher than group 1, the control group(p<0.05) Similar results was also noticed after 15 days(p<0.05) 2. In comparison of microhardness between groups 2 and 3 after 5days, there were no significant differences between them. The results after 15 days was also similar 3. Much difference in microhardness wasn't present in groups 2 and 3 after 5 and 15days. However, the microhardness of the group 1 dropped significantly in the result of 15 days and 5 days(p<0.05).
