Esthetic restoration of anterior dentition using Empress 2 system: A clinical report

  • Metal - ceramic restorations have been the standard of fixed oral rehabilitation for over 30 years, and while many advances have been made in materials and research, the esthetic challenges of the light reflective metal substructure will always put clinician in an esthetic dilemma, for only a very talented ceramist / dentist team can create esthetics that rival nature on a repeatable basis. All ceramic restorations have also been a choice in our restorative armamentarium, but in the past, the materials have also had their own limitation, number on being question-able strength to withstand occlusal forces generated during mastication. IPS Empress 2 offers dentistry a metal free alternative to traditional restorative techniques. This clinical report describes the treatment of patients with esthetic problem of anterior dentition using Empress 2 system.

MARGINAL FIT OF CELAY/IN-CERAM, CONVENTIONAL IN-CERAM AND EMPRESS 2 ALL-CERAMIC SINGLE CROWNS (Celay/In-Ceram, Conventional In-Ceram, Empress 2 전부도재관의 변연적합도에 관한 비교 연구)

  • There have been many studies about marginal discrepancy of single restorations made by various systems and materials. But many of statistical inferences are not definite because of sample size, measurement number, measuring instruments. etc. The purpose of this study was to compare the marginal adaptations of the anterior single restorations made by different systems and to consider more desirable statistical methods in analysing the marginal fit. The in vitro marginal discrepancies of three different all-ceramic crown systems (Celay In-Ceram. Conventional In-Ceram. IPS Empress 2 layering technique) and one control group (PFM) were evaluated and compared. The crowns were made from one extracted maxillary central incisor prepared with a 1mm shoulder margin and $6^{\circ}$ taper walls by milling machine. 10 crowns per each system were fabricated. Measurements or a crown were recorded at 50 points that were randomly selected for marginal gap evaluation. Non-parametric statistical analysis was performed for the results. Within the limits of this study, the following conclusions were drawn: 1 Mean gap dimensions and standard deviations at the marginal opening for the maxillary incisor crowns were $98.2{\pm}40.6{\mu}m$ for PFM, $83.5{\pm}18.7{\mu}m$ for Celay In-Ceram, $104.9{\pm}44.1{\mu}m$ for conventional In-Ceram, and $45.5{\pm}11.5{\mu}m$ for IPS Empress 2 layering technique. The IPS Empress 2 system showed the smallest marginal gap (P<0.05). The marginal openings of the other three groups were not significantly different (P<0.05). 2 The marginal discrepancies found in this study were all within clinically acceptable standards ($100\sim150{\mu}m$). 3. When the variable is so controlled that the system may be the only one, mean value is interpreted to be the marginal discrepancy of a restoration which is made by each system and standard deviation is to be technique-sensitivity of each one. 4. From the standard deviations. the copy-milling technique (Celay/In-Ceram) was not considered to be technique-sensitive in comparison with other methods. 5. Parametric analysis is more reliable than non-parametric one in interpretation of the mean and standard deviation. The sample size of each group has to be more than 30 to use parametric statistics. The level of clinically acceptable marginal fit has not been established. Further studies are needed.

  • This Study is to analyze characteristics of costume change of according to the status of character succeeding the investigation of the ceremonial costume change in historical TV dramas that The Empress Myungsung entered. For this study, the costume images among historical TV dramas, that Myungsung entered, '500 years of Chosun Dynastyv, Taewongun(1990, MBC)', 'The Glorious Dawn(1993, KBS1)', 'The Empress Myungsung(2001, KBS2)'were used: The characteristics of the costume change of main character are as follows. First, main character's costume has been increase gradually in number and item. Second, Especially in the last work,'The Empress Myungsung', the royal costume based on historical evidence in aspects of silhouette, but it didn't according to the historical costume in aspects of color and textile. Nevertheless the costume reveals the status of each person significantly. Third, the costumes of the recent historical drama appears as a optical symbol that represents historical view point and different interpretation of each drama. Therefore the costume according to the status of the character wrong from the historical point of view. Consequently, TV costume in historical drama Myungsung entered has been focused on the beauty of the screen and dramatic effect than historical viewpoint. So the costume have been to be an essential visual part by means of the symbol revealed the purpose.

Transtherapy for moderate to severe acne scar: a study of 2 cases

  • Objectives: The purpose of this study is to present effect of transtherapy which is treatment of Korean Medicine to acne scar patients. Methods: We used transtherapy to moderate and severe acne scar patients and evaluated by a Photo evaluation with 10-point score and Qualitative Global Acne Scarring Grading System. Results: After transtherapy treatment, mean of Qualitative Global Acne Scarring Grading System of patients changed from $3.5{\pm}0.71$ to $1.5{\pm}0.71$. Acne scar showed an improvement at least 8 up to 9 and mean of their acne scar improvement scores was $8.33{\pm}0.52$ on photo evaluation with 10-point score. Conclusions: This paper shows that transtherapy in the treatment of moderate to severe acne scars is beneficial for promoting skin regeneration.

A STUDY ON THE MICROSTRUCTURE OF IPS EMPRESS CERAMICS ACCORDING TO THE HEAT TREATMENT AND SPRUE TYPE (주입선 및 열처리에 따른 IPS Empress 도재의 미세구조에 관한 연구)

  • This study was undertaken to clarify the microstructure of the different IPS Empress ingots by etching and to observe the change of leucite crystal structure according to subsequent heat treatment and the crystal distribution according to sprue types(${\phi}2.8mm$, single sprue; ${\phi}1.8mm$, double sprue) by scanning electron microscopy. IPS Empress T1, O1 ingots used for staining technique, and Dentin(A2) ingots used for layering technique were selected for this study. To observe the microstructures of these ingots before pressing, the specimens were prepared in splinters($3{\times}3{\times}3mm$) taken from the original ingots. And to estimate crystal distribution and microstructural change by sprue type and subsequent heat treatment. the specimens($3{\times}3{\times}3mm$) were heat-pressed through the two types of sprues with different diameters and numbers, and all specimens were fired according to the recommended firing schedule. The observed surface was ground with waterproof papers($#800{\sim}#1800$) on the grind polisher and was cleaned ultrasonically. All specimen were etched with 0.5% hydrofluoric acid. After etching, the surface was treated by ion sputter coating for SEM observation at an accelerating voltage of 20kV. In all specimens, the central area of ground surface was observed because there was less difference in microstructure between the peripheral area and the central area. The results were as follows ; 1. In the microstructure according to the ingot type, there was a wide difference between the staining (T1,O1) and layering(Dentin A2) ingots, but there was not a considerable difference between the T1 ingot and the O1 ingot for staining technique. 2. In all specimens, the crystal dispersion of IPS Empress ceramic using double sprue was significantly more scattered than that of IPS Empress ceramic using single sprue. The degree of scattering was strongest in the Dentin(A2) specimen and weakest in the O1 ingot. 3. In the microstructural change according to the subsequent heat treatment, all of ingots had some microcracks in the inside of the leucite crystal and the glass matrix after pressing. The inner splinters of the leucite crystal became smaller, and more microcracks occurred in the glass matrix due to increasing heat treatment times. 4. The size of leucite crystals varied from $1{\mu}m\;to\;5{\mu}m$. The mean size of mature crystals was about $5{\mu}m$. The form of the crystal was similar to a circle when it was smaller and similar to an ellipse when it was larger.

A STUDY ON THE BOND STRENGTH OF RESIN CEMENTS TO EMPRESS 2 CERAMIC (Empress 2 도재와 레진시멘트의 결합강도에 관한 연구)

  • The objective of this study was to investigate the influence of resin cements and ceramic etching on shear bond strength of Empress 2 ceramic and observe the change of microstructure of ceramic according to etching time. Sixty-six square ceramic specimens($6{\times}6{\times}1.5mm$) were prepared. 6 specimens were etched with different etching times(0, 10, 20, 30, 40 and 60 seconds) and observed by means of a scanning electron microscope(SEM). Other sixty specimens were divided into 6 groups with 10 specimens in each group. 3 groups were etched with 4% hydrofluoric acid and each groups was bonded with 3 resin cements(Variolink II, Super-Bond C&B, Panavia F). Each specimen was subjected to a shear load in an Instron at a cross-head speed of 0.5mm/min and was observed with SEM after mechanical testing to establish modes of failure. The results were as follows : 1. Within etched groups, Variolink II and Super-Bond C&B exhibited significantly greater bonding strengths than Panavia F(p<0.05) 2. Bond strength of etching groups had three to five times greater than that of no-etching groups. 3. All of no-etching groups showed adhesive failure and etching groups mostly showed mixed failure. And, 20-second etching specimen showed the most distinct lithium disilicate crystal. so it is considered that 20-second etching is optimal time for bonding.

  • During the past several years, significant advances have occurred in the utilization of osseointegrated implants for the treatment of partially edentulous patients. One of the biggest purposes for treating of these patients is the high demand for improved esthetics, especially in the anterior region. For this esthetics, the new trend in dental implants is the immediate placement and immediate superstructure fabrication. The refined surgical technique, the skillful soft tissue management, and the proper prosthetic coordination are the main factors to achieve natural looking of implant supported prosthesis. The customized provisional restoration and the customized impression coping are recommended for the optimal peri-implant soft tissue contour. The basic concept of Frialit 2 system was the immediate replacement of a tooth with root-analog fixture after extraction. This system guarantees an ideal result in function and esthetics. The ceramic abutment system offers improved quality in the respect of esthetics, fitness, translucency, and biocompatibility. In this clinical report, the final restoration made with IPS Empress 2 crown on the CeraBase abutmen of Frialit 2 system allowed the reproduction of the natural vitality of tooth and adjacent gingiva.

A Reinterpretation of Sacheon Seonjin-ri Stone Monument's Chinese Characters of Empress Wu(則天文字) - On the Issue of the Introduction and Acceptance of Chinese Characters of Empress Wu to the Korean Peninsula - (사천 선진리비석 측천문자의 명문 재해석 - 측천문자의 한반도 전래 및 수용 문제에 대해 -)

  • Since the discovery of the inscriptions engraved with the "Chinese characters of Empress Wu(則天文字)" at 251 and 256-1, Seonjin-ri, Sacheon-si, Gyeongsangnam-do in 2003, researchers from the Gyeongnam Institute of Cultural Properties, Kwak Seung-Hoon, and Kim Chang-Gyeom have attempted to read and interpret the inscriptions. However, there are still different theories as to the origin of the "Chinese characters of Empress Wu" appearing in the inscriptions and the use of Idu(吏讀)-type notation. This study aims to clarify, this inscription was erected by the king of Silla in the late 7th and mid-8th centuries to commemorate his participation in the Sacheon and Jinju patrols and Buddhist activities, and it is revealed that the main character of the inscription is likely to be King Seongdeok who has visited southern countries. In addition, in the case of "上了言", which has been interpreted as a human name, it should be understood as an Idu-type notation or a 變體漢文 because of the use of the proposition 了, and it should be interpreted as a character that means the end of a specific act. Although it is not a religious text like 『Mugujeonggwangdaedaranigyeong (The Great Dharani Sutra)』, the reason why the "Chinese characters of Empress Wu(則天文字)" was used is that in the process of accepting the Huayan school, the royal family of Silla came into contact with the Buddhist scriptures of the period of the Empress Wu, which is closely related to the rise of the Huayan school in the Middle Ages, and it appears to have learned and used in this process.

Fracture strength of the IPS Empress crown :The effect of incisal reduction and axial inclination on upper central incisor (IPS Empress 도재관의 파절강도 : 상악 중절치에서 절단연 삭제량과 축면 경사도에 따른 영향)

  • The purpose of this study was to compare the fracture strength of the IPS Empress ceramic crown according to the incisal depth(2.0mm, 2.5mm, 3.0mm) and axial inclination($4^{\circ}$, $8^{\circ}$, $12^{\circ}$) of the upper central incisor. After 10 metal dies were made for each group, the IPS Empress ceramic crowns were fabricated and each crown was cemented on each metal die with resin cement. The cemented crowns mounted on the testing jig were inclined 30 degrees and a universal testing machine was used to measure the fracture strength. The results were : 1. The fracture strength of the ceramic crown with 2.5mm depth and $8^{\circ}$ inclination was the highest(965N). Crowns of 2.0mm depth and $4^{\circ}$ inclination had the lowest strength(713N). There were no significant differences of the fracture strength by axial inclination in same incisal depth group. 2. The fracture mode of the crowns was similar. Most of fracture lines began at the loading area and extended through proximal surface perpendicular to the margin irrespective of incisal depth. There had correlation between fracture strength and fractured surface area.
