권역응급의료센터에 내원한 노인의 제 특성에 따른 내원과정 비교 (A Comparative Study on the Process of Hospital Visits according to the Characteristics of the Elderly Visiting to Emergency Medical Centers)

  • The purpose of this study is to compare and analyze the process of hospital visits according to the characteristics of the elderly visiting to emergency medical centers. The subject of study was 571 patients over the age of 65 who agreed to participate in the study of emergency medical centers from May 1 to 31, 2010. The frequency, percentage and ${\chi}^2$ test of collected data were conducted with SPSS WIN 12.0. As a result, the frequency of the aged with or without urgency revisiting emergency rooms due to chronic degenerative diseases was high. In addition, there were many cases that the aged living only with a spouse or remaining single. It was obscure to classify them into a urgent or non-emergent group which made it longer time to visit a hospital. It may be necessary that the care for urgent elderly patients considering the characteristics of the family environment functions to care the elderly were weakened.

사고환자의 응급의료 서비스에 대한 만족도와 영향요인 분석 (Satisfaction regarding the usage of emergency treatment service)

  • This study was designed to acknowledge the satisfaction levels of patients who have been treated in the emergency center. Specifically, to acknowledge the factors influencing patient's satisfaction levels related in the usage of emergency centers. Data was collected through patients who have used anyone of four emergency centers of university hospitals holding over 550 hospital beds, and located in Seoul. Questionnaires consisted of 31 questions. Cronbach's alpha was 0.94. Data analysis was executed with the SAS program by t-test, ANOVA and stepwise multiple regression. The results were summarized as follows. (1) The total average score for the level of satisfaction was 3.3. Regarding the service of doctors and nurses, the score was high. While the score regarding reception process, payment and second hand environment were relatively low. (2) There was a differentiation in the level of satisfaction according to the type of injured. Patents of industrial calamities had the highest level of satisfaction while patient injured by traffic accidents had the lowest level of satisfaction. (3) The factors that affect the level of satisfaction were presence of medical insurance, lapse time until treatment and the education level of the patient. Based on this, we will provide the basic data of satisfaction levels according to the characteristics of patients and the types of injuries for improvement of emergency treatment centers.

119 구급대의 구급장비 감염 관리와 실태조사 (Infection management for emergency rescue equipment)

  • Purpose: This study investigated the use of infection management and the frequency of emergency equipment sterilization by emergency rescue crews to provide basic guidelines and suggestions for infection control. Methods: A self-reported questionnaire was completed by 160 emergency medical technician rescue crews in J area from May 7 to May 22 in 2015. The questionnaire consisted of 95 items, including the general characteristics of the subjects (8 items), the use of emergency equipment (33 items), the frequency of facility sterilization (33 items), infection control (12 items), vaccination (4 items), and the use of personal protective equipment (5 items). Results: In all, 97.3% of respondents were aware of the "Guidelines of Infection management for Rescue Operators' and 90.9% received training on infection control. A total of 72.7% and 47.3% of rescue centers were equipped with disinfection facilities and laundry rooms, respectively. The average frequency of sterilization was $3.17{\pm}0.75$, which significantly differed for teams with more clinical experience (p=.050) and teams with three members (p=.030). The average score for individual protective equipment supplies was $1.95{\pm}0.66$. Conclusion: For proper infection management of emergency equipment, our results suggest that the number of crew members should be increased for each ambulance, protective equipment and rescue products should be supplied, and the number of facilities for sterilization and laundry at rescue centers should be doubled.

대학병원 응급의료센터로 전원되는 중증 외상환자의 현황 및 문제점 (Problems with Transferring Major Trauma Patients to Emergency Medical Center of a University Hospital from Another Medical Center)

  • Purpose: The incidence of multiple trauma is increasing nowadays and is the leading cause of death among young adults. Initial treatment is well known to be crucial in multiple trauma victims. However, many indiscriminate transfers occur due to the lack of a well-organized trauma system in Korea. The objective of this study is to demonstrate the current serious state in which major trauma patients are transferred to the Emergency Medical Center of a university hospital from another medical center. Methods: From November 2009 to October 2010, we performed a retrospective study to analyze the characteristics of patients who visited the Ajou University Medical Center located in Gyeonggi-do. We evaluated the ISS (injury severity score), and a score over 15 point was identified as major trauma. The major trauma patients were separated into two groups according to the visit route, and the characteristics of each group were analyzed. Results: Among the 88,862 patients who visited to the Emergency Medical Center, trauma patients accounted for 19,950, and 343 of them were evaluated as major trauma patients. Among the 343 patients, 170 patients had been transferred from other medical centers. The proportion of males to females was 3.3:1, and the mean ISS was 22.7. The leading cause of trauma was motor vehicle accidents. Of the total 170 patients, 77.6% were admitted to the Intensive care unit and 36.3% underwent surgery. The 170 patients that had been transferred to our medical center, 78.8% were transferred from Gyeonggi-do, 15.3% were transferred from other regions, and 5.9% were miscellaneous. Conclusion: Almost half of the major trauma victims treat at our medical center had been transferred from other medical centers. Establishing a traumatic system, supported by well-organized trauma centers and emergency medical services, that can reduce inappropriate transfers among medical facilities is essential.

1급 응급구조사의 직종에 따른 직무만족도 및 업무스트레스 (Job Satisfaction and Work Stress of EMT-paramedic's According to type of Occupation)

  • 본 연구는 응급구조사가 배출된지 21년이 지난 시점에서 119와 병원에서 근무하는 응급구조사들을 근무 기관별로 업무스트레스와 직무만족도를 비교분석하여 119와 병원의 차이와 장단점을 파악함으로써 우리나라의 1급 응급구조사들의 근무여건을 개선하고 보다 나은 근무환경을 조성하는데 기초자료를 제공하는 것을 목적으로 수행하였다. 광주광역시와 전라남도, 전라북도의 28개 119안전센터와 14개 권역응급의료센터 및 지역응급의료센터에서 근무하는 1급 응급구조사 182명을 대상으로 설문조사를 실시하였다. 통계분석은 SPSS/WIN 20.0을 사용하였으며, 카이제곱검정, t-test, ANOVA, ANCOVA로 분석하였다. 업무스트레스는 119근무자(2.92), 병원근무자(3.01)으로 유의한 차이가 없었다. 직무만족도는 소방근무자(2.98), 병원근무자(2.54)으로 병원근무자들이 크게 유의하게 낮았다(p<0.001). 또한 병원근무자들의 보수 영역에서 가장 낮은 점수가 보였으며($2.07{\pm}0.62$), 월 평균 환자수가 적을수록(p<0.001), 월수입이 높을수록(p=0.047), 휴가가 많을수록(p<0.001) 업무스트레스가 낮고, 직무만족도가 높은 것으로 나타났다. 업무스트레스를 감소시키고 직무만족도를 높이기 위해 119와 병원 모두 근무복지여건의 개선이 필요하고, 119의 경우, 구급대원 폭행 방지 대책과 휴식공간이 필요하고 특히 병원의 경우는 급여와 연간 급여증가율, 고용형태의 안정을 개선해야 한다.

응급실로 내원하는 급성중독환자의 원인물질 분석을 위한 중독 분석실 이용 현황 및 이용 만족도 조사: 전국 권역 및 지역응급의료센터 설문조사 (A satisfaction survey of toxicological laboratory: Survey of regional and local emergency medical centers)

  • Purpose: The purpose of this study is to find out the current status of toxicology laboratory operated by six locations nationwide and to investigate the satisfaction of emergency medical professionals who working at local and regional emergency medical centers. Methods: This survey was conducted prospective. It was conducted on 665 emergency medical professionals working at regional and regional emergency medical centers across the South Korea. Among them, the analysis was conducted with data that 510 emergency medical professionals who respond to this survey. The questionnaire was conducted on an online basis for a month. To ensure statistical significance, consider a dropout rate of 10% based on a minimum response recovery rate of 70%. 506 people were selected for the survey. Results: According to a survey on the status of addiction analysis room usage, the average monthly usage of addiction test rooms among respondents were 406 cases.71.0 cases (17.4%) of toxicology laboratory in Seoul and 71 cases (17.4%) in Gwangju. 32 cases (7.8%), 118 cases (29.0%) requested by toxicology laboratory in Busan, and the toxicology laboratory in Daegu. Eighty two cases (20.1%), Daejeon area 25 cases (6.1%), Wonju area toxicology laboratory was 78 (19.6%). According to a survey on the satisfaction of the addiction analysis room,Seoul (4.9±2.71) and Gwangju (4.8±2.52) showed high satisfaction. Conclusion: Due to the limited operation time of the four addiction analysis rooms currently in operation, the satisfaction level of addiction analysis by emergency medical professionals in the area is low due to the delay until the result is notified.

응급의료기관에 근무하는 1급 응급구조사의 직업 불안정성 및 직무열의가 이직의도에 미치는 영향 (Effects of job insecurity and job engagement on turnover intention of paramedics in emergency medical institutions)

  • Purpose: This study aimed to identify the effects of job insecurity and job engagement on turnover intention of paramedics who work in emergency medical institutions. Methods: From October 14 to 28, 2014, data were collected by structured questionnaires from 171 paramedics who were working in emergency medical institutions. The data were analyzed by using SPSS/WIN 21.0. Results: Of the 171 subjects, 57.3% were temporary employees, of whom 87.5% were working in regional emergency medical centers. The mean scores were 3.19 for job insecurity, 4.58 for job engagement, and 3.28 for turnover intention. The correlation between the variables showed that the higher the job insecurity of the participants, the higher their turnover intention (r = .397, p <.001). Moreover, the higher their job engagement, the lower their turnover intention (r = -.354, p <.001). The variable that most significantly affected turnover intention was job insecurity. The coefficient of determination ($R^2$) of job insecurity and job engagement was 24.3%. Conclusion: A law should be enacted to involve paramedics as required personnel for emergency medical institutions in order to enhance the quality of emergency medical services and provide prompt and professional emergency medical services to emergency patients.

급성심근경색 환자의 증상 발현 후 골든타임내 응급의료센터 도착율 및 지연에 관련된 요인 (Hospital Arrival Rate within Golden Time and Factors Influencing Prehospital Delays among Patients with Acute Myocardial Infarction)

  • Purpose: This research was done to identify the hospital arrival rate and factors related to prehospital delay in arriving at an emergency medical center within the golden time after symptom onset in patients with acute myocardial infarction (AMI). Methods: Data used in the research was from the National Emergency Department Information System of the National Emergency Medical Center which reported that in 2014, 9,611 patients went to emergency medical centers for acute myocardial infarction. Prehospital time is the time from onset to arrival at an emergency medical center and is analyzed by subdividing arrival and delay based on golden time of 2 hour. Results: After onset of acute myocardial infarction, arrival rate to emergency medical centers within the golden time was 44.0%(4,233), and factors related to prehospital delay were gender, age, region of residence, symptoms, path to hospital visit, and method of transportation. Conclusion: Results of this study show that in 2014 more than half of AMI patients arrive at emergency medical centers after the golden time for proper treatment of AMI. In order to reduce prehospital delay, new policy that reflects factors influencing prehospital delay should be developed. Especially, public campaigns and education to provide information on AMI initial symptoms and to enhance utilizing EMS to get to the emergency medical center directly should be implemented for patients and/or caregivers.

일개 종합병원에 내원한 외상환자에 대한 1급 응급구조사의 처치 유무에 따른 만족도 분석 (Comparison of emergency medical service satisfaction among the trauma patients treated by paramedics or not)

  • Purpose & Method : This purpose of this paper was to show the importance of paramedics. The trauma patients receiving a temporary treatment from emergency medical technician filled out a self-administered questionnaire. The questionnaire consisted of the satisfaction for EMS. The data collection was from October 23 to November 12, 2006. The patients were in the Kyeonggi province hospital having more than 500 beds. At the time of discharge, the questionnaire forms were distributed. Results : Males showed higher satisfaction than females when they had a paramedics, and this indicated statistically significant difference(p<.05). The groups showing higher satisfaction for EMT 1 practice included patients over 40 years old, educated under high school, and income over 2 million won. Before the trauma patients were treated, they were satisfied with short waiting time less than 10 minutes. They showed higher satisfaction within 2 hours duration. Conclusion : Summing up the above result, it was found that the group treated by an emergency medical technician showed higher satisfaction than the group not treated by an emergency medical technician. Therefore, it is necessary to arrange paramedics in emergency medical centers to improve satisfaction with emergency medical services. And since it is a crucial factor that affects patient's satisfaction significantly, it is urgent to increase the roles of emergency medical technicians and lay the foundation for legal institutions.

고압산소 치료에 대한 일개 권역 응급센터의 최근 10년 경험 및 제언 (Recent 10-Year Experience of One Regional Emergency Center and Recommendation for Hyperbaric Oxygen Therapy (HBOT))

  • Purpose: The aim of this study is to investigate current status, indications, and complications of hyperbaric oxygen therapy. Methods: A retrospective investigation of patients who underwent hyperbaric oxygen therapy at a university medical center from September 2004 to August 2013 was conducted based on patients' medical records and results of an email survey for 99 emergency centers. Results: During the study period, a total of 233 patients underwent hyperbaric oxygen therapy. Indications for hyperbaric oxygen treatment of illness or injury were as follows: 1) 151 cases of acute carbon monoxide poisoning(65.4%), 2) flap wound management, including 42 cases(18.2%), 3) skin care transplanted, including 23 cases(10.4%), 4) Burger's disease, including 5 five cases(2.1%), respectively. Total application time$^*$ frequency was 1,088 and total time was 1,239 hours. Among 233 patients who underwent hyperbaric oxygen therapy, 32 patients(13.7%) had complications: 1) otalgia in 21 cases(9.0%), 2) mastoiditis?in six cases(2.6%), 3) hemotympanum in five cases(2.1%), respectively. There were only 8 emergency centers that currently had an operational hyperbaric oxygen chamber in 77 emergency centers(10.4%). Conclusion: Indications identified through this study showed difference from current indications worldwide. It seems necessary that physicians' perception regarding application of hyperbaric oxygen therapy for more indications be changed and improved. A hyperbaric chamber capable of providing respiratory assistance and intensive care is also needed. A good network for sharing treatment experiences and a specialized team for administration of hyperbaric oxygen therapy is also required.
