Conclusions and Suggestions on Low-Dose and Low-Dose Rate Radiation Risk Estimation Methodology

  • Background: For radiological protection and control, the International Commission on Radiological Protection (ICRP) provides the nominal risk coefficients related to radiation exposure, which can be extrapolated using the excess relative risk and excess absolute risk obtained from the Life Span Study of atomic bomb survivors in Hiroshima and Nagasaki with the dose and dose-rate effectiveness factor (DDREF). Materials and Methods: Since it is impossible to directly estimate the radiation risk at doses less than approximately 100 mSv only from epidemiological knowledge and data, support from radiation biology is absolutely imperative, and thus, several national and international bodies have advocated the importance of bridging knowledge between biology and epidemiology. Because of the accident at the Tokyo Electric Power Company (TEPCO)'s Fukushima Daiichi Nuclear Power Station in 2011, the exposure of the public to radiation has become a major concern and it was considered that the estimation of radiation risk should be more realistic to cope with the prevailing radiation exposure situation. Results and Discussion: To discuss the issues from wide aspects related to radiological protection, and to realize bridging knowledge between biology and epidemiology, we have established a research group to develop low-dose and low-dose-rate radiation risk estimation methodology, with the permission of the Japan Health Physics Society. Conclusion: The aim of the research group was to clarify the current situation and issues related to the risk estimation of low-dose and low-dose-rate radiation exposure from the viewpoints of different research fields, such as epidemiology, biology, modeling, and dosimetry, to identify a future strategy and roadmap to elucidate a more realistic estimation of risk against low-dose and low-dose-rate radiation exposure.

A Study on the Development of Intravenous Injection Management Application for EMR System Interworking (EMR 시스템 연동 정맥주사 관리 애플리케이션 개발에 대한 연구)

  • This paper is about developing an intravenous injection management system that can provide nurses with information related to intravenous injection in real-time to compensate for possible instability factors during intravenous injection. The intravenous injection management system consists of an app-based user S/W and a web-based administrator S/W. User S/W is implemented to provide users with the ability to identify patients who need intravenous injection through smartphones, tablet PCs, and nursing PDAs, recognize information codes given to patients, and enter and share treatment contents and treatment items after intravenous injection. As a result of intravenous injection treatment uploaded through the user app, the manager S/W can check the records of intravenous injection treatment items, perform user management functions, emergency notification registration and management functions, and data upload functions. The implemented system has not yet been tested on the EMR system used in the actual hospital. Therefore, through further research, S/W will be optimized and actual environmental application tests will be conducted through cooperation with hospitals.

Inhibitory Effect of Beet Extract on Cancer Cell Proliferation (비트 추출물의 암세포 증식 저해 효과)

  • The purpose of this study was to examine the inhibition of human cancer cell proliferation by using various concentrations of Beet Extract containing various bioactive ingredients. The six cancer cell lines used in the experiment were prostate cancer cells DU-145, lung cancer cells A549, breast cancer cells MCF-7, cervical cancer cells HeLa, liver cancer cells SNU-182, and biliary tract cancer cells SNU-1196. Human-derived cancer cell lines were used. The inhibition of cancer cell proliferation at various concentrations of Beet Extract was measured by the CCK-8 method. As a result of examining the inhibition of cancer cell proliferation, Beet Extract significantly and concentration-dependently inhibited DU145 of prostate cancer cells at all concentrations, and Lung cancer cells A549 and DU-145 of prostate cancer cells at 100ug/mL and 1000ug/mL, cervical cancer cells HeLa, and liver cancer cells SNU- 182, biliary tract cancer cell SNU-1196 showed significant proliferation inhibition at 1000ug/mL. Experiment result, the cancer cell proliferation inhibitory mechanisms of Beet Extract using various human-derived cancer cell lines can be considered to provide cancer prevention effects and the possibility of developing functional foods.

A Study on the difference in the sharpness of venous images between individual algorithms and combinations (개별 알고리즘과 조합 간 정맥 영상의 선명화 차이에 관한 연구)

  • Intravenous infusion therapy is a standard nursing procedure in medical institutions that provides patients with drugs, fluids, blood, and nutrients into the patient's mucus. It is mainly performed and managed by nurses. Additionally, it is an injection method that injects drugs directly into the blood vessels, and is used to achieve rapid results in emergency situations, and quick and accurate effects can be expected. Even experienced nurses through education and training often make mistakes, which can not only cause discomfort to patients but also cause various problems that threaten patient safety. Various studies are being conducted to reduce the pain caused by these mistakes. This paper acquired images of veins on the back of the hands of three subjects through an image detection device and conducted a study to derive an algorithm to provide clear vein images through image processing of the collected images. To sharpen the acquired vein images, existing algorithms Histogram Equalization, CLAHE, and Unsharp Masking were selected and combined. A histogram graph was used to compare images derived by applying individual algorithms and algorithm combinations to images. The histogram graph was checked by calculating the difference between the minimum and maximum values of distributed pixels and averaging them. The algorithm combination presented in this paper was 209.1, which was higher than the average values of individual algorithms of 138.7, 132.3, and 126.2, and it was confirmed that visibility was good even in actual images.

Correlation between the Preventive Behaviors on Middle East Respiratory Syndrome and the Knowledge, Attitude, and Compliance of Medically Inclined College Students (보건계열 대학생의 중동호흡기증후군에 대한 지식, 태도 및 예방행위 실천도와의 관계)

  • This study aimed to investigate the medically inclined college students' knowledge, attitude, and compliance on the preventive behaviors of the Middle East respiratory syndrome (MERS). A cross-sectional study was conducted on 251 medically inclined college students in Jeonju Kijeon College from June 8 to 13, 2015, using a scale on the knowledge and attitude toward MERS and a structured questionnaire. The t-test, analysis of variance, and Pearson correlation coefficient were used to analyze the data. The participants consisted of 46.6% college students majoring in dental hygiene, 30.7% in clinical pathology, and 22.7% in emergency rescue, and 69.7% of them had been educated on MERS prevention. This study revealed that students who received MERS prevention training (t=3.457, p=0.001) and female students (t=-2.945, p=0.005) had more knowledge on MERS, while students from the dental hygiene department (F=8.048, p<0.001) and those in their third year (F=3.978, p=0.020) showed a more positive attitude toward MERS. Regarding the correlation between knowledge, attitude, fear of infection and compliance on the preventive behaviors, students were more knowledgeable (r=0.133, p=0.036), presented a more positive attitude (r=0.158, p=0.012) and had more fear of infection (r=0.312, p<0.001), were more likely to comply with the preventive measures. For an effective prevention of MERS, a newly found infectious disease, we suggest that medically inclined college students must improve their knowledge of and have a positive attitude toward MERS infection management at a medical institution in compliance with the MERS infection preventive behaviors. Furthermore, this study shows that an infection management education program should be developed, so that which students could learn about the causes, dissemination routes, and preventive methods of infectious diseases, including the newly discovered ones, prior to their clinical training at a medical institution.

The Patient Recognition, Acceptability and Evaluation of Feasibility for Day Surgery (종합병원 일부 입원환자의 당일수술에 대한 태도와 당일수술 적용 가능성 평가)

  • Objectives . This study was conducted in order to investigate the degree of recognition, acceptability, and altitude towards day surgery of patients who were hospitalized with diseases that were candidates for day surgery; in order to analyze the average length of stay for treatment of the ailments; and to analyze the percentage of patients who could be discharged on the same day after the surgery ,using the post-anesthesia discharge scoring system. Methods : Data was collected between February 1 and March 31, 1999 from 353 patients who received surgery for cataract, adenoid hypertrophy, inguinal hernia, strabismus, ptosis, cholelithiasis, hemorrhoid, or anal fistula, at a general hospital in Daegu city. The patients were interviewed and surveyed by a post-anesthesia discharge scoring system(PADS) in order to collect data on patient condition such as vital signs, activity and mental status, pain, nausea and vomiting, surgical bleeding, intake and output after the surgery. Results : Among the 353 patients, 52.7% were after of the day surgery and 52.7% were interested in day surgery. Of the respondents, 43.1% said 'my ailment was not serious and the surgery was simple' and 30.4% said 'according to my condition rest at home was desirable' as the reasons for wanting day surgery Alternatively, 56.5% of those declining day surgery said the 'uncertainty of staying home' was the reason. The greatest concern in discharging within 24 hours after surgery was a post-op emergency situation. On the other hand, the shortened hospitalization was the largest advantage of day surgery with 39.1% responding this way, followed by the savings in hospitalization costs (25.8%) and emotional stability (13.7%). The majority of those surveyed (47.6%) believed that discharge should be determined within 1-2 days after the surgery. The average hospital stay was 3.1 days for dischargeable ailments. Pain (45.6%), nausea and vomiting (10.5%), and headache (7.9%) were the common symptoms following surgery. The percentage of patients who were able to be discharged within 24 hours after surgery revealed 95.2% were dischargeable after approximately 3 hours, 99.2% dischargeable after 12 hours, and 100% dischargeable after 24 hours. Conclusions : According to the PADS score, the cataract extract and strabismus correction patients were eligible for day surgery and the further evaluation concerning the reason for delayed recovery of the other diseases is needed.

An analysis of one-year experience of pediatric observation unit: The first report in Korea (소아 관찰병실 운영에 대한 분석)

  • Purpose : While pediatric observation units (POU) have become a common practice in hospitals throughout developed countries, there has been no report about POUs in Korea so far. The aims of this study were to analyze our one-year's experience of the POU and to decide which disease entities are suitable for the POU. Methods : All children admitted from March 2006 to February 2007 to the POU at the Department of Pediatrics in Our Lady of Mercy Hospital were included in this study. Data were collected from retrospective reviews of their medical records. Results : There were a total of 1,076 POU admissions. Median age of patients was 2.4 years and median length of hospital stay 14.0 hours. The most common diagnoses were gastroenteritis (42.7%), pharyngotonsillitis (19.1%), bronchiolitis (7.8%), pneumonia (5.5%) and febrile seizure (5.2%). Overall, 7.5% of the POU patients required subsequent inpatient admissions due to hospital stays of longer than 48 hours. The disease entities that were most likely to require inpatient admission were pneumonia (17.0%), febrile seizure (12.5%) and asthma (11.5%). Diseases that allowed successful discharge from the POU were gastroenteritis (4.6%), upper respiratory tract infection (5.8%), such as otitis media and pharygnotonsillitis and seizure disorder (6.4%). Compared with the previous year when the POU was not in operation, there was a statistically significant reduction in the average length of hospital stays (from 4.69 to 3.75 days), as well as a rise in the bed turnover rate (from 78.8 to 98.2 patients/bed). Conclusion : Our study shows that the POU is efficient for the management of children with certain acute illnesses. Based on this study, we suggest that the POU be used as a new modality which links between the outpatient, inpatient, and emergency departments in the field of pediatrics in Korea.

Expressional Analysis of Two Genes (Got1 andMat1) Up-regulated by Starvation Stress (영양고갈-스트레스에 의해서 상승 발현하는 유전자(Got1과 Mat1)의 분석)

  • Restricted supply of nutrients may affect genes at the molecular level as well as physiological functions. Understanding the cellular responses during starvation is necessary for developing strategies to reduce damage caused by starvation stress. After 1 h of starvation, Got1 gene expression was increased but its expression returned to the normal state after 24 h. Mat1 gene expression continuously increased with starvation from 1 h until 24 hr. Rats starved for 1-3 days showed significant changes in expression of the Got1 and Mat1 genes, which were significantly reduced in the cerebral cortex and cerebellum. In the lung, gene expression was increased by starvation for 1-2 days but decreased on the third day. No differences were observed in gene expression in the heart. Strong Got1 lung gene expression was seen in the starvation group one day after restoration of the food supply. Muscle mass was significantly reduced at the start of starvation and remained the same after two days of starvation and one day after the food supply was restored. The Mat1 gene expression did not change. The Got1 was induced by NaCl and showed strong expression in the lung and the thymus, but the apparent decrease of the remaining changes were not observed in male rats. The Mat1 gene was not as sensitive as the Got1 gene to induction by NaCl. However, differences in gene induction by NaCl were evident between males and females, indicating that diet control of gene expression is associated with hormones.

The effect of Type 2 diabetes management using a smartphone-based blood glucose management training program (모바일 자가혈당관리 교육프로그램을 이용한 2형 당뇨병 관리 효과 분석)

  • Background: Diabetes education is an important factor in blood glucose control. Reinforced education is necessary for effective diabetes education. However, it is difficult to provide reinforced diabetes education within Korea's medical environment. Therefore, we want to analyze the effect of continuous diabetes education using mobile health care that can effectively provide repeated education without having to face the patient. Methods: This study is a multicenter, randomized, controlled, pre-post design study conducted to analyze the effect of a continuous diabetes education method. A total of 109 people were registered at five hospitals in south Korea, and they were randomly assigned to the app group (34 people) who received real-time coaching and repetitive training, the logbook group (37 people) who received face-to-face training after writing a blood glucose logbook, and the general group (38 people) who received a one-time diabetes education. The study was conducted for a total of 24 weeks. Twenty-one patients withdrew their consent and failed to perform an HbA1c. A final 88 patients were analyzed. The difference in HbA1c, Self-management behavior, and Quality of life before and after education was analyzed. Results: The study involved 51 (58%) male subjects, mean age was 55.8 years and mean duration of diabetes was 7.6 years. After 24 weeks of intervention, there was no significant difference in self-care behavior and quality of life between the three groups, but the HbA1c of the app group significantly decreased after education compared to the logbook group and the general group (F=4.62, p=.013). Conclusion: It can be seen through the app group that receiving real-time education is more effective in improving blood glucose management and continuous diabetes education is important.
