Necessity of the Physical Distribution Cooperation to Enhance Competitive Capabilities of Healthcare SCM -Bigdata Business Model's Viewpoint- (의료 SCM 경쟁역량 강화를 위한 물류공동화 도입 필요성 -빅데이터 비즈니스 모델 관점-)

  • The purpose of this study is to develop business models for current situational scenarios reflecting customer needs emphasize the need for implementing a logistics cooperation system by analyzing big data to strengthen SCM competitiveness capacities. For healthcare SCM competitiveness needed for the logistics cooperation usage intent, they were divided into product quality, price leadership, hand-over speed, and process flexibility for examination. The wordcloud results that analyzed major considerations to realize work efficiency between medical institutes, words like unexpected situations, information sharing, delivery, real-time, delivery, convenience, etc. were mentioned frequently. It can be analyzed as expressing the need to construct a system that can immediately respond to emergency situations on the weekends. Furthermore, in addition to pursuing communication and convenience, the importance of real-time information sharing that can share to the efficiency of inventory management were evident. Accordingly, it is judged that it is necessary to aim for a business model that can enhance visibility of the logistics pipeline in real-time using big data analysis on site. By analyzing the effects of the adaptability of a supply chain network for healthcare SCM competitiveness, it was revealed that obtaining competitive capacities is possible through the implementation of logistics cooperation. Stronger partnerships such as logistics cooperation will lead to SCM competitive capacities. It will be necessary to strengthen SCM competitiveness by searching for a strategic approach among companies in a direction that can promote mutual partnerships among companies using the joint logistics system of medical institutes. In particular, it will be necessary to search for ways to utilize HCSM through big data analysis according to the construction of a logistics cooperation system.

Factors Influencing the Fear of Falling in Elderly in Rural Communities (일부농촌지역 재가 낙상경험노인의 낙상두려움 관련요인)

  • Objectives: The purpose of this study was to examine the factors associated with fear of falling among the elderly dwelling in rural communities. Methods: From February 1, 2010 to March 31, 2010, a questionnaire-based survey was sent to 2,628 persons 65 years or older dwelling in 1 Myeon and 1 Eup of Gyeongsangnam-do. Among these participants, 735 (27.9%) had fallen more than once in 2009. For data analysis, descriptive statistics and hierarchical multiple regression were performed using SPSS version 12.0. Results: Factors influencing the fear of falling in the elderly according to falls experienced were number of outpatient visits, number of falls, gender, admission, purchasing of health function foods, and activities of daily living. Overall, the study showed significant differences in the score of fear of falling according to gender, age, education, marital status, living arrangement, main means of mobilization, drinking, income, number of falls, admission, number of outpatient visits, experience with outpatient oriental medicine, purchasing of health function foods, medical assistance devices, season, fall location, fall cause, shoe type, injury type, limitation of activity, and activities of daily living. Conclusions: The results of this study suggest that programs should be developed specifically for elderly people who have experienced more than one fall because of increased fear due to multiple falls.

Correlation between Regular mealtime, Oral health knowledge and Oral health of college students (대학생의 식사규칙성과 구강건강지식 및 구강건강 간의 관계)

  • The purpose of this study was to investigate the correlation between regular mealtime, oral health knowledge and oral health to prevent dental impairment of college students. This research design is correlation study. Data of 903 were collected from June 1 to June 30, 2010. Data were analyzed using the SPSS PASW Statistics 18.0 Program. More than 50% of respondents were the irregular mealtime. Higher percentage of correct answer in the oral health knowledge showed periodontal disease prevention(82.6%). There were a statistical significant in grade, major, smoking, drinking, and wishing for dental care between general characteristics and regular mealtime respectively(p<.05). Between general characteristics and oral health knowledge were a statistical significant in gender, age, grade, smoking, and dental checkup respectively(p<.05). On the other hand, there was a statistical significant wishing for dental care between general characteristics and oral health(p<.001). Regular mealtime was negatively associated with oral health(p<.01). Education for oral health should include regular mealtime, prohibit of smoking and alcohol, right toothbrushing and so on.

Hand Hygiene Compliance of Healthcare Workers in a Children's Hospital (소아병원 종사자의 손 위생 수행)

  • Purpose: The aim of study was to estimate the hand hygiene (HH) compliance of healthcare workers (HCWs) in a children's hospital. Methods: This study was conducted in a hospital which is a tertiary and educational children's hospital with 313 beds and 533 HCWs. Data were collected by direct observation methods from November 1, 2010 to December 31, 2010. Results: A total of 2,999 opportunities for HH were observed, and the overall HH rate was 95.3%. HH rate of the registered nurse, physicians and transferer was 97.7%, 89.2%, and 72.1%, respectively (P<0.001). Among physicians, HH rate of the fellows, professors, residents and interns was 97.5%, 93.9%, 89.7%, and 80.9%, respectively (P<0.001). HH rate in the emergency room, operation room, outpatient department (OPD), and the intensive care unit (ICU) was 97.2%, 97.2%, 95.4%, and 92.5%, respectively (P<0.001). Hand rubbing was the most frequently used (81.1%), and hand washing was frequently used in the case of 'after body fluids exposure risk' (37.7%) and 'after touching patient surroundings' (28.5%). HH methods were not statistically different from each departments (P =0.083), however, they were significantly different according to the World Health Organization (WHO) 5 Moments (P<0.001). Distributions in WHO 5 Moments by the job titles were significantly different (P<0.001). The odds ratio of physicians, ICU and OPD was 0.353 (95% CI, 0.241-0.519), 0.291 (95% CI, 0.174-0.487), and 0.484 (95% CI, 0.281-0.834), respectively. Conclusions: Compliance of HH was different by the job titles and departments. Effective custom-tailored HH programs for each job title and department need to be developed.

Impact on Job Satisfaction to Job Stress of Physician Assistant Nurses (Physician Assistant(PA)간호사의 직무스트레스가 직무만족도에 미치는 영향)

  • This study aims to investigate the factors influencing job satisfaction to job stress of physician assistant(PA) nurses. This research design was a descriptive study. The subjects in this study were 104 PA nurses. Data were collected from September 1, 2010 to January 10, 2011, and analyzed by SPSS PASW statistics 18.0 program. The mean score of men on the job stress was 43.47(${\pm}13.77$), The mean score of women was 45.07(${\pm}9.78$). The mean score of job satisfaction was 2.72(${\pm}0.30$). There was no statistical difference between general characteristics and job satisfaction. There were statistical difference between unmarried subjects and organizing system(p<.05), inadequate compensation(p<.01). In the results of multivariate regression analysis, adjusted $R^2$ value was 0.567. The model fit was 56.7%. Job satisfaction was low in the insufficient job control and inadequate organizing system. It is hospital policies for the reduction of job stress(particularly, insufficient job control, and organizing system stress).

Correlation between Oral dryness and Stress level of college students (대학생의 구강건조감과 스트레스)

  • The purpose of this study was to investigate the correlation between oral dryness and stress and to collect baseline data for health promotion plan of college students. This research design is correlation study. Data of 835 were collected from May 2 to June 17, 2011, and analyzed using the SPSS PASW Statistics 18.0 Program. There was a statistical significant between oral dryness and perceived health status(p<.001). there were statistical significant in gender(p<.001), age(p<.001), grade(p<.01), major(p<.01), perceived health status(p<.001), exercise(p<.001), smoking(p<.001), drinking(p<.01) between general characteristics and stress. The mean score of oral dryness level was $12.89{\pm}10.15$ from 0 to 60 score range. Higher percentage in oral dryness action was "When I swallowing dry food, drink water or beverage"(48.7%). The mean score of stress was $7.17{\pm}4.78$ from 0 to 20 score range. Oral dryness level was positively related to stress(p<.01) and oral dryness action(p<.001). It is necessary to develop the educational program for health promotion of college students.

A Study on the Perception and Improvement Plans of Performance Based Pay System of Elementary School Teachers and Elementary School Health Teachers (교원 성과급 제도에 대한 초등교사와 초등보건교사의 인식과 개선방안)

  • Purpose: The purpose is to provide basic information for establishing improvements on performance-based pay's evaluation method of health teachers. Methods: For subjects, 200 teachers at public elementary schools and 200 health teachers at public elementary schools in Gyeonggi-do were conveniently sampled, and then surveyed through a questionnaire. The questionnaire was to recognize recognition of teachers working under the teacher's performance-based pay system, which was quoted in the questionnaire of Choi ji-hye (2005) and Lee mi-gyeong (2008). Inquiry for the recognition of teachers on evaluation of health teacher's performance-based pay system and improvements on the performance-based pay's evaluation method of health teachers were used after consultation with five incumbent health teachers and a review with the thesis director. Results: The performance-based pay's evaluation method of health teachers has the same method with general teachers in schools, so it is not fair to evaluate the performance of health teachers. The ways to improve the performance-based pay evaluation method of health teachers is as follows: first, the approval on improving 'the number of class hours' which is a detailed item of a teacher's performance evaluation criterion to 'the number of health lesson hours per week and the number of students visiting the school infirmary per week'; second, improving 'life guidance' into 'counseling results of medically-treated students, parents of students and personal hygiene guidance'; third, improving 'a teacher in charge' into 'awarding of points by being recognized as a health teacher in charge of all students and considering the economic situation of the region,; forth, improving 'difficulty of position' into 'the number of health-teacher's annual promotion task items and the treatment number of issuing and receiving of official documents', and improving the 'task difficulty' into 'importance and urgency of emergency patient management, risk level and urgency of infectious diseases and avoiding work in charge' appeared to be more than 90% respectively. Conclusion: The performance-based pay system of teachers being carried out every year should be executed by preparing fair evaluation criteria suitable for task properties and the role of health teachers with different evaluation criteria compared to general teachers.

Factors Associated with Ruptured Intracranial Aneurysm in a Hospital (일개 병원의 뇌동맥류 파열의 위험요인)

  • This study was conducted to identify the factors associated with the ruptured intracranial aneurysm (RIA) among demographic, admission to hospital, clinical, aneurysym itself and lifestyle characteristics. Medical records of RIA patients and un-RIA patients which had been admitted to undergo treatment including surgery (From January to December 2016) were included into data analysis. Multiple logistic regression showed that two and more than warning signs (14.14 (CI: 1.25-159.40)) indicated the greatest odds ratio with RIA, was followed by headache more than 3 scores (13.95 (CI: 3.68-52.83)), the admission via emergency room (13.62 (CI: 4.85-38.26)), single marital status (9.72 (CI: 2.22-42.49)), 1 mmHg increased systolic blood pressure (1.04 (CI: 1.01-1.08)), 1 score increased GCS (0.58 (CI: 0.37-0.90)), arrythmia finding in electrocardiogram (3.70 (CI: 1.22-11.22)) and increased age (0.95 (CI: 0.91-0.99)). The risk groups having factors associated with RIA were identified. Preventive activities including routine assessment should be done before developing the rupture and urgent care should be needed after developing the rupture for risk groups.

Protective Effect of Agrimonia pilosa var. Extract on Cultured NIH3T3 Fibroblasts Damaged by Potassium Dichromate (크롬염으로 손상된 배양 NIH3T3 섬유모세포에 대한 짚신나물 추출물의 보호 효과)

  • The protective effect of Agrimonia pilosa var. (AP) extract on potassium dichromate ($K_2Cr_2O_7$)-induced cytotoxicity in cultured NIH3T3 fibroblasts, was examined by performing an XTT assay for the cell viability and antioxidative effects, such as lactate dehydrogenase (LDH) activity and superoxide anion-radical (SAR) scavenging activity. In this study, $K_2Cr_2O_7$ decreased the cell viability significantly in a dose-dependent manner, and the $XTT_{50}$ value was determined to be $37.5{\mu}M$, which was highly-toxic according to the Borenfreund and Puerner' toxic criteria. The antioxidant, butylated hydroxytoluene (BHT), increased remarkably the cell viability damaged by $K_2Cr_2O_7$-induced cytotoxicity in these cultures. With regard to the protective effect of the AP extract on $K_2Cr_2O_7$-induced cytotoxicity, AP extract produced a significant increase in cell viability and antioxidative effects as the inhibitory ability LDH and SAR scavenging ability. These findings suggest that oxidative stress is involved in the cytotoxicity of $K_2Cr_2O_7$, and the AP extract effectively protected the cells from $K_2Cr_2O_7$-induced cytotoxicity by antioxidative effects. These results suggest that natural resources, such as AP extract, may be a putative therapeutic agent for the diminution or treatment of cytotoxicity induced by heavy metallic bases, such as $K_2Cr_2O_7$ correlated with oxidative stress.

Risk Factors of Predicting Intensive Care unit Transfer in Deteriorating Ward Patients (병동 급성악화 환자의 중환자실 전동 위험요인 분석)

  • Purpose: When a patient with acute deterioration occurs in a ward, the decision to transfer to intensive care unit (ICU) is critical to improve the patient's outcomes. However, when available ICU resources limited, it is difficult to determine which of the deteriorating ward patients to transfer to the ICU. Therefore the purpose of this study was to identify risk factors in predicting deteriorating ward patients transferred to intensive care unit (ICU). Methods: We reviewed retrospectively clinical data of 2,945 deteriorating ward patients who referred medical emergency team. Data were analyzed with multivariate logistic regression. Results: The solid cancer that diagnosed at hospitalization (odds ratio[OR] 0.39; 95% confidence interval [CI] 0.32-0.47), when the cause of deterioration was respiratory problem (1.51; 95% CI 1.17-1.95), high MEWS (1.22; 1.17-1.28) and SpO2/FiO2 score (2.41; 2.23-2.60) were predictive of ICU transfer. Conclusion: These findings suggest that early prediction and treatment of patients with high risk of ICU transfer may improve the prognosis of patients.
